"One of the Proudest Achievements:" Organized Birth Control in Indiana, 1870s to 1950s

Indiana University-Purdue University Indianapolis (IUPUI) / Local birth control advocates often found themselves attempting to open birth control clinics in restrictive climates that did not welcome open discussions of sexuality. To accomplish this, the organizers chose their tactics carefully and followed common strategies. After a few stumbles, organized birth control in many states began in earnest in the 1930s. By 1943, just ten years after its opening, the Indianapolis birth control clinic could boast that it had served 4,531 women and openly solicit funds from the community. The Maternal Health League’s plea on its fundraising brochure assured citizens of their patriotic duty to help. The league reminded Hoosiers, “In War and In Peace a Nation is as Strong as the Health of Its Mothers and Children.” Arguments such as these allowed the clinic to open and assured its existence for many years to come. Indiana birth control organizers relied on specific tactics that allowed them to accomplish their goals in a restrictive Midwestern state. Indiana’s birth control story offers modern-day readers a very different picture than that offered by studies of the national movement of birth control. Indiana offers an account that relies more on cooperation and less on confrontation.

Planläggningen av de empiriska och tidigare forskningar som forskats i denna studie sammanfattar, sociologiska problemet ungdomskriminalitet bland unga män i Sverige. Studien använder en kvalitativ metod med ett semistrukturerad intervjuval. Urvalet bestod av sex intervjudeltagande med erfarenhet av att arbeta inom kriminella nätverk bland ungdomar. Varav tre av samtliga deltagande är från Unga Kris och tre från Statens institutionsstyrelse, SiS. Syftet med denna undersökning är att bygga förståelse för helheten av ungdomskriminalitets beteendemönster. Vilka är de bakomliggande orsakerna till ungdomsbrott, vad har familjen för betydelse och hur kan samhället bidra till att minska riskfaktorerna. / The planning of the empirical and previous researches researched in this study summarizes, the sociological problem of juvenile delinquency among young men in Sweden. The study uses a qualitative method with a semi-structured interview choice. The sample consisted of six interview participants with experience of working in criminal networks among young people. Of which three of all participants are from Unga Kris and three from the Swedish National Board of Education, SiS. The purpose of this study is to build an understanding of the totality of juvenile delinquency behavior patterns. What are the underlying causes of juvenile delinquency, what is the significance of the family and how can society contribute to reducing the risk factors.

The attention-emotion interaction in healthy female participants on oral contraceptives during 1-week escitalopram intake

Previous findings in healthy humans suggest that selective serotonin reuptake inhibitors (SSRIs) modulate emotional processing via earlier changes in attention. However, many previous studies have provided inconsistent findings. One possible reason for such inconsistencies is that these studies did not control for the influence of either sex or sex hormone fluctuations. To address this inconsistency, we administered 20 mg escitalopram or placebo for seven consecutive days in a randomized, double-blind, placebocontrolled design to sixty healthy female participants with a minimum of 3 months oral contraceptive (OC) intake. Participants performed a modified version of an emotional flanker task before drug administration, after a single dose, after 1 week of SSRI intake, and after a 1-month wash-out period. Supported by Bayesian analyses, our results do not suggest a modulatory effect of escitalopram on behavioral measures of early attentional-emotional interaction in female individuals with regular OC use. While the specific conditions of our task may be a contributing factor, it is also possible that a practice effect in a healthy sample may mask the effects of escitalopram on the attentional-emotional interplay. Consequently, 1 week of escitalopram administration may not modulate attention toward negative emotional distractors outside the focus of attention in healthy female participants taking OCs. While further research in naturally cycling females and patient samples is needed, our results represent a valuable contribution toward the preclinical investigation of antidepressant treatment.

Factors affecting contraceptive use among women of reproductive age in northern Jordan : a framework for health policy action

Jordan has a higher fertility rate (3.8) than the averages of countries similar in income to Jordan (2.2) and compared to the Middle East and North Africa region as a whole (2.8) (WHO, WB, UNICEF, & DHS, 2011). The findings of the 2009 Jordanian Population and Family Health Survey demonstrated that the total fertility rate (TFR) has stopped declining in the country since 2002 (DOS, 2010b; USAID, 2010). The prevalence of contraceptive use has also shown little change in Jordan over the last decade (DOS, 2010b; USAID, 2010). Given that contraception is one of the proximate determinants of fertility (Rahayu et al., 2009), the main purpose of this study was to investigate which factors are contributing to women's current contraceptive behavior and intention for future contraceptive use. Research questions were developed in a comprehensive framework that considers women's intention and actual behavior as outcomes of various interactive factors within a socio-cultural context. In particular, the study's framework was directed by a theoretical basis adapted from Ajzen and Fishbein's Theory of Reasoned Action (TRA) and an extensive review of the available literature in the research area. Obviously, the social set-up and cultural norms in the study setting, together with attitudes toward children and family, represent a traditional scenario that could help explain the consistency of fertility and contraceptive use in the country. Further, the influences of background characteristics on women's contraceptive behaviors and intentions provide another scenario that could help assess the current situation of family planning (FP) in Jordan. In this study, demographic factors, spousal communication variables and healthcare system-related factors are all defined as background characteristics. Attitudes and social norms reflect the women's behavioral determinants and represent the main constructs of the TRA. In fact, involving a set of factors related to women's beliefs and social norms in the study's framework provided an opportunity to explore how these factors might promote or inhibit a woman's intentions and behaviors in respect to contraceptive use. In a three-manuscript format, this research was designed to achieve a number of objectives. The first manuscript aimed at identifying the major factors associated with the current use of contraception among women of childbearing age in northern Jordan. The second manuscript focused on investigating the main factors that are associated with women's contraceptive method preference (e.g. the choice of modern contraceptives as effective methods in preventing pregnancy versus the choice of traditional contraceptives as methods with high failure rates). The third manuscript attempted to explore the key factors associated with women's intention for future contraceptive use since the existence of such an intention would consequently translate into an actual behavior later. In 2010, original cross-section data were collected by means of a face-to-face interview using a structured pre-tested survey. The study sample included women who were currently married and were between 18 and 49 years old. Applying a systematic random sampling procedure, all respondents were recruited from the waiting rooms of five randomly selected Maternal and Child Health (MCH) centers in the Governorate of Irbid, northern Jordan. Using a list provided by the Ministry of Health, all centers in the Governorate were stratified according to the region (urban vs. rural) and randomly selected in proportion to their number in each region. The final sample size for this research consisted of 536 women surveyed, giving a response rate of 92.4 percent. Utilizing logistic regression analyses, the results of the dissertation manuscripts indicate that women's behaviors and intentions toward the use of contraception are affected by a number of factors at the individual, familial and institutional levels. The findings that emerged from the three manuscripts provide health professionals and policy makers with important information to assist in the design of FP programs and campaigns aimed at increasing current contraceptive use, enhancing the adoption of modern contraception and motivating the intention for future contraceptive use. This research strongly suggests that health professionals develop health policies that both expand the availability of MCH centers and strengthen the role of healthcare providers to dispel the numerous rumors and misconceptions surrounding the use of contraceptives, particularly modern ones. Health workers at the MCH centers need to ensure that women have sufficient information about the benefits and side effects of different types of contraception by offering proper FP counseling. The messages that religious leaders can use in advocating for FP would also help make contraceptive use socially acceptable since their opinions are often followed by the majority. This would be a key step toward removing the barriers to contraceptive use. Moreover, to design effective FP interventions, planners should take into account women's attitudes toward the use of contraceptive methods and the components of those attitudes (e.g. women's approval of contraceptive use for birth spacing and perceptions regarding the value of large family sizes and the importance of having male children in Jordanian families). / Graduation date: 2012 / Access restricted to the OSU Community at author's request from May 9, 2012 - May 9, 2013

Knowledge, perceptions and attitudes regarding contraception among secondary school learners in the Limpopo Province

This study explored knowledge, perceptions and attitudes regarding contraception and contraceptive practices among secondary school learners in the Limpopo Province. Self-administered questionnaires were completed by 612 Grade 8 and 332 Grade 12 learners from 24 randomly selected secondary schools. The study found that permissive attitudes prevailed towards sex, characterised by casual sexual activities commencing at 12 years of age. The availability of contraceptive and termination of pregnancy (TOP) services did not enable learners to utilise them, because of social, cultural, financial and service barriers. Most learners were sexually active without being knowledgeable about contraceptives, emergency contraceptives and TOP services. Two workshops conducted with learners produced similar results to those obtained from the completed questionnaires. Semi-structured interviews conducted with nurses, providing contraceptive and TOP services in the Limpopo Province, also substantiated the findings from the questionnaires. Secondary school learners in the Limpopo Province require more knowledge about and ready access to contraceptives to enable them to delay child bearing until they are emotionally, financially and physically ready for these responsibilities. Nurses and teachers in this province can enhance the learners' contraceptive knowledge and utilisation to help learners make better informed decisions about their own and their future children's lives. / Health Studies / D. Emergency contraception in Addis Ababa : practice of service providers

A quantitative, descriptive, explorative, contextual study was conducted to determine pharmacists and drug vendors' level of knowledge, attitude towards and practice on Emergency Contraceptive (ECl in Addis Ababa. Forty licensed service providers in Addis Ababa were randomly selected during 2008 and interviewed using a structured interview schedule. Data were analysed using a computer software package. The findings revealed that although these service providers were knowledgeable on the purpose and dOSing schedule of EC, they lacked knowledge on side-effects, contra-indications, and types of ECs. Most respondents portrayed a subjective attitude towards easy EC access of especially adolescent girls, since they believed that it will encourage promiscuity and unprotected intercourse. Healthcare service provider’s knowledge and skills for provision of intra uterine contraceptive device services were low. Community members lack awareness and knowledge of the benefit and side effects of the device. Therefore, community members need education to promote adherence and effective use of IUD. Σκοπός: η μελέτη της επίδρασης εξάμηνης θεραπείας με από του στόματος αντισυλληπτικό δισκίο περιέχον 35μg αιθινυλ-οιστραδιόλης και 2mg οξικής κυπροτερόνης στη γλοιότητα πλάσματος νέων γυναικών με σύνδρομο πολυκυστικών ωοθηκών. Σχεδίαση: Η γλοιότητα πλάσματος μετρήθηκε σε ασθενείς με σύνδρομο πολυκυστικών ωοθηκών πριν και 6 μήνες μετά από τη χορήγηση αντισυλληπτικού δισκίου περιέχοντος 35μg αιθινυλ-οιστραδιόλης και 2mg οξικής κυπροτερόνης. Η μέτρηση της γλοιότητας έγινε σε ιξωδόμετρο τύπου 53610/I SCHOTT-Instruments, Mainz στους 37ο C. Ασθενείς: Οι ασθενείς στρατολογήθηκαν από το τμήμα Αναπαραγωγικής Ενδοκρινολογίας της Μαιευτικής - Γυναικολογικής Κλινικής του Πανεπιστημιακού Νοσοκομείου Πατρών Ελλάδας. Στη μελέτη περιλήφθηκαν 66 νέες γυναίκες με σύνδρομο πολυκυστικών ωοθηκών. Βασικοί προσδιορισμοί: Γλοιότητα πλάσματος Αποτελέσματα: Στις ασθενείς ως σύνολο, η γλοιότητα πλάσματος ήταν 1.249±0.049 mm2/s (n=66). Μετά από 6 μήνες θεραπείας με από του στόματος αντισυλληπτικό δισκίο περιέχον 35μg αιθινυλ-οιστραδιόλης και 2mg οξικής κυπροτερόνης, η γλοιότητα πλάσματος αυξήθηκε σε 1.268±0.065 mm2/s (p=0.038). Design: PCOS patients were assessed for plasma viscosity before and after 6 months of treatment with an OC containing 35μg ethinyl estradiol and 2mg cyproterone acetate. Plasma viscosity was determined by a viscometer Type 53610/I SCHOTT-Instruments, Mainz at 37o C. Settings: Subjects were recruited from the Department of Obstetrics and Gynaecology, Division of Reproductive Endocrinology at the University Hospital of Patras, Greece. Patients: The study included 66 young PCOS women. Main Outcome measures: Plasma viscosity. Results: In PCOS women as a whole, plasma viscosity at baseline was 1.249±0.049 mm2/s (n=66). After 6 months of treatment with an oral contraceptive containing 35μg ethinyl estradiol and 2mg cyproterone acetate, plasma viscosity increased to 1.268±0.065 mm2/s (p=0.038). The difference between plasma viscosity before and after 6 months of treatment with an oral contraceptive containing 35μg ethinyl estradiol and 2mg cyproterone acetate (Δviscosity) was 0,01864±,071452 mm2/s. Δviscosity was related to Δfibrinogen (r=0.270, p=0.046), to Δhaematocrit (r=0.514, p=0.09) and to Δtriglycerides (r=0.292, p=0.021). Conclusion: Young PCOS women presented an increased plasma viscosity under OC treatment, which therefore should be used with caution.
