Efeitos do uso de estrógenos orais e transdêmicos sobre IGF-1, IGFBP-3, IGFBP-1, lipídios e metabolismo da glicose em pacientes com hipopituitarismo : um estudo ramdomizado

O tratamento do hipogonadismo hipogonadotrófico na mulher adulta com hipopituitarismo inclui diversas alternativas terapêuticas de estrógenos e progestágenos, sendo a via oral a de menor custo e a de maior comodidade à paciente. A rota estrogênica oral, entretanto, exerce marcada influência sobre o eixo hormônio de crescimento/fator de crescimento insulinasímile número 1 (GH/IGF-1) nessas mulheres. O tratamento com estrógenos orais, concomitante ao uso de GH em pacientes com hipopituitarismo, antagoniza as ações biológicas do GH e agrava as anormalidades de composição corporal e o metabolismo em geral. Presume-se que o estrógeno oral iniba a secreção/produção de IGF-1 através de um efeito de primeira passagem hepática, causando um aumento da secreção de GH através de inibição do feedback negativo de IGF-1 em mulheres normais. Isso é demonstrado clinicamente por redução da massa magra, aumento da massa gorda, perfil lipídico aterogênico e prejuízo do bem-estar psicológico. Alguns estudos apontam que os progestágenos com ação androgênica revertem o efeito de diminuição dos níveis séricos de IGF-1 induzida pelos estrógenos orais. Progestágenos neutros não apresentam esse efeito, porém, quanto maior a potência androgênica, maior será a reversão do efeito de diminuição de IGF-1. Na presente revisão da literatura, serão abordados os aspectos clínicos da reposição com estrógenos e progestágenos nas mulheres com hipopituitarismo, suas interações nas outras deficiências hormonais, bem como o impacto do uso de estrógenos sobre as ações metabólicas do GH. / Treatment of hypogonadotropic hypogonadism in adult woman with hypopituitarism can include a wide range of estrogen and progestogen treatment alternatives, and oral administration is the route of least cost and greatest patient comfort. The oral estrogen route has a major impact on the growth hormone-insulin-like growth factor I (GH-IGF-I) axis. Oral estrogen therapy, when given concurrently with GH to patients with hypopituitarism, antagonizes the biological effects of GH treatment and aggravates the abnormalities of body composition and the metabolism in general. It is presumed that oral estrogen suppresses the secretion/production of IGF-1 by a hepatic first-pass mechanism, resulting in increased GH secretion by means of suppressing the IGF-I negative feedback that is present in healthy women. This is manifest clinically in reduced lean body mass, increased fat mass, an atherogenic lipid profile and damage to psychological well-being. Some studies have indicated that progestogens with androgenic actions reverse the effect of reduced serum IGF-1 levels that is induced by the oral estrogens. Neutral progestogens do not exert this effect, however the stronger the androgenic potential, the more the effect of reduced IGF-1 will be reversed. This bibliographical review will deal with the clinical aspects of estrogen and progestogen replacement in women with hypopituitarism, their interactions with other hormone deficiencies and the impact of estrogen treatment on the metabolic actions of GH.

Efeitos do estrogênio, raloxifeno e extrato de soja rico em genisteína sobre o osso de ratas adultas ovariectomizadas previamente androgenizadas / Effects of estrogen, raloxifene and genistein-rich soy extract on bone of ovariectomized adult female rats and previously androgenized

INTRODUÇÃO: O hipoestrogenismo pode determinar perda da massa mineral óssea, diminuindo a qualidade do osso. Assim, vários fármacos são ministrados para evitar esta perda, porém, podem determinar efeitos colaterais importantes. Portanto, questiona-se se o emprego do estrogênio associado a estas substâncias poderia minimizar os efeitos adversos e manteria a massa mineral óssea. Contudo, há poucas informações sobre os efeitos destas combinações. Esta pesquisa tem como objetivo avaliar a ação do estrogênio, raloxifeno e do extrato de soja rico em gensteína, isolado ou combinado no osso de ratas ovariectomizadas. MATERIAIS E MÉTODOS: No nono dia de nascimento, todas as ratas receberam propionato de testosterona (0,1 g/g). No sexto mês de idade, os animais do controle fisiológico foram identificados como GI e receberam apenas o veículo (propilenoglicol em 0,5 ml/dia) durante o experimento e os outros que receberam testosterona foram ovariectomizados e divididos aleatoriamente em seis grupos: GII veículo (controle castrado, n=6); GIII - estrogênio conjugados eqüinos (ECE, (50 g/Kg/dia, n=8); GIV raloxifeno (RAL, 0,75 mg/kg/dia, n=8); GV extrato de soja enriquecido com genisteína (ESG, 300 mg/kg/dia, n=7); GVI ECE + ESG (50 g/Kg/dia + 300 mg/kg/dia, n=7); GVII - ECE+RAL (50 g/Kg/dia + 0,75mg/kg/dia, n=6). Após três meses da cirurgia, os fármacos foram ministrados por 120 dias consecutivos. Posteriormente, os animais foram sacrificados sob anestesia, sendo retirada a tíbia esquerda para rotina histológica. Os cortes histológicos foram corados pela hematoxilina-eosina para avaliar a microarquitetura óssea. Foram feitos procedimentos imunoistoquímicos, de imunofluorescência e PCR para quantificar as principais proteínas ósseas estruturais (colágeno tipo I, osteocalcina, osteopontina e osteoprotegerina), bem como de seus respectivos RNA mensageiros. Os dados foram analisados pelos testes de ANOVA e Tukey. RESULTADOS: Todos os tratamentos determinaram aumento da quantidade de osso trabecular (p<0,05). As fibras totais de colágeno apresentaram-se aumentadas em todos os grupos tratados, exceto com o raloxifeno. Já as fibras finas de colágeno diminuíram apenas no grupo tratado com estrogênio. As frações de colágeno tipo I, mostraram-se aumentadas nos grupos tratados com estrogênio e sua asssociação com o raloxifeno. O colágeno tipo III esteve aumentado no grupo tratado com estrogênio em associação com extrato de soja rico em genisteína. Em relação às proteínas não colagenosas, a osteoprotegerina apresentou-se aumentada nos grupos tratados com estrogênio, suas associações e com o extrato de soja rico em genisteína. A osteopontina esteve diminuída em todos os grupos tratados e a osteocalcina mostrou-se aumentada apenas no grupo tratado com ralolxifeno, em comparação ao grupo castrado (p<0,05). Não houve diferença estatística significante do PCR em tempo real na análise dos transcritos entre os grupos estudados. CONCLUSÃO: A combinação de estrogênio com raloxifeno ou extrato de soja rico em genisteína não trouxe benefícios adicionais na qualidade do tecido ósseo, como ocorreu com esses fármacos isoladamente / INTRODUCTION: Hypoestrogenism can determine bone mineral loss, resulting in decreased bone quality. To prevent that process, several drugs are administered, which can lead, however, to important side effects. Therefore, it is questionable whether the use of estrogen associated with those substances could minimize the adverse effects and maintain bone mineral mass. There is little information on the effects of those compounds. This research aims to evaluate the action of unopposed estrogen or combined with raloxifene and genistein-rich soy extract on ovariectomized adult female rats. MATERIALS AND METHODS: On the ninth day of birth, rats received, testosterone propionate (0.1 mg / g). On the sixth month, animals in the physiological control were identified as GI and received only the vehicle (propylene glycol at 0.5 ml / day) during the experiment and the other which was administered testosterone underwent ovariectomy and divided randomly into six groups: GII - vehicle (control castrated, n = 6); GIII - conjugated equine estrogen (CEE, 50 mg / kg / day, n = 8); GIV - raloxifene (RAL, 0.75 mg / kg / day, n = 8) ; GV - soy extract enriched with genistein (ESG, 300 mg / kg / day, n = 7), GVI - ECE + ESG (50 mg / kg / day + 300 mg / kg / day, n = 7); GVII - ECE + RAL (50 mg / kg / day + 0.75mg/kg/day, n = 6).Three months after the surgery, drugs were consecutively administered for 120 days. Subsequently, the animals were sacrificed on anesthesia and their left tibiae were removed for routine histology. The histological sections were stained by hematoxylin-eosin to evaluate bone microarchitecture. Immunohistochemical, immunofluorescence and PCR procedures were performed to quantify the main structural bone proteins (type I collagen, osteocalcin, osteopontin, and osteoprotegerin) as well as their mRNA. The data were analyzed by ANOVA and Tukey test. RESULTS: All treatments led to increased amounts of trabecular bone (p <0.05). The total collagen fibers had to be enlarged in all treated groups, except with raloxifene. Already thin collagen fibers decreased only in the group treated with estrogen. The fractions of type I collagen, were increased in groups treated with estrogen and its asssociação with raloxifene. Type III collagen was increased in the group treated with estrogen in combination with soybean extract rich in genistein. Regarding the non-collagenous proteins, the increased osteoprotegerin presented in groups treated with estrogen, and their associations with soy extract rich in genistein. The osteopontin was decreased in all treated groups and osteocalcin was increased only in the treated group ralolxifeno, compared to the castrated group (p <0.05). There was no statistically significant difference from the real-time PCR analysis of transcribed between the groups. CONCLUSION: The combination of estrogen with raloxifene or genistein-rich soy extract was uncapable of bringing additional benefits to the quality of bone tissue as observed with those drugs alone

Estudo endócrino não invasivo e comportamental da gestação, parto e lactação da jaguatirica (Leopardus pardalis) em cativeiro / Noninvasive endocrine and behavioral study of the pregnancy, birth and lactation of ocelots (Leopardus pardalis) in captivity

Os perfis longitudinais dos metabólitos dos esteróides sexuais, progesterona e estrógenos, e dos glicocorticóides foram analisados para 22 eventos (gestação, parto e lactação) de 8 jaguatiricas mantidas em cativeiro. Três eventos foram oriundos da Transferência de Embrião (TE) e foram comparados àquelas que gestaram por fertilização natural. As análises estatísticas demonstraram haver diferenças altamente significativas para metabólitos de progesterona na fase inicial (P<0001) e média (P< 0,007) da gestação, bem como, no período da lactação (P<0,0015). As análises descritivas sugeriram uma atividade ovariana para a espécie no período de lactação, uma vez detectado picos de metabólitos de estrógenos para alguns eventos. Foi realizado ainda, um estudo do comportamento materno de 3 fêmeas através do sistema de vídeo-monitoramento dos primeiros meses da lactação (n=3989,5 horas observadas). A espécie em cativeiro despendeu mais tempo cuidando do seu filhote e descansando durante o dia, entrando na caixa de abrigo entre 5:00h e 8:00h (61,3%) e nela permanecendo por um tempo médio de 8 horas seguidas. Outras análises comportamentais sugerem que o estresse é um fator preponderante na determinação do sucesso da criação do filhote. A variável comportamental executada pela fêmea de entrar e sair da caixa por repetidas vezes foi associada ao estresse, sendo possível avaliar a resposta da adrenal pela secreção dos glicocorticóides e realizar sua validação fisiologicamente. Espera-se desta forma, contribuir para uma mudança no quadro bastante crítico da população em cativeiro, considerando a baixa taxa de natalidade e a alta taxa de mortalidade dos filhotes, principalmente, no seu primeiro mês de vida. / The longitudinal profiles of the sexual steroid\'s metabolites, progesterone and estrogen, and glucocorticoids were analyzed for 22 events (pregnancy, birth and breastfeeding) of 8 ocelots kept in captivity. Three events came from the Embryo Transfer (TE) and were compared against the ones that were gestated by natural fertilization. The statistical analysis demonstrated that the differences were highly significative during the initial (P<0001) and middle stages (P< 0,007) of pregnancy and lactation (P<0,0015) periods for the progesterone metabolites. The descriptive analysis suggests a ovarian activity for the species during the lactation period, once detected the estrogen metabolites peaks during some events. A study was also performed, concerning the maternal behavior of 3 female through video-monitoring of the first months of lactation (n=3989,5 hours observed). The species that were kept in captivity spent more time resting and caring the puppy, entering it\'s shelter between 5:00h and 8:00h (61,3%) and there staying during 8 hours average. Other behavior analysis suggests that the stress level is a predominant fact in the success of the establishment of the puppy. A behavior variable performed by the female, by entering the shelter for a repeated number of times was linked to the stress level, being possible to assess the adrenal response by the secretion of glucorticoids and perform it\'s phisiological validation. It is expected by this means, a contribution for a change in the critical scene of the population that is kept in captivity, considering the puppies\' low birth rate and high mortality rate, specially during it\'s first months.

The thesis focuses on the use of passive sampling during an analysis of hormones in drinkable water. In the theoretical part there are described selected passive samplers and the sample of the type of POCIS is described at fuller length. The next chapter engages with steroid hormones and an input of estrogenic hormone into the environment. The thesis contains chapters dealing with possibilities of the determination of hormones in water. It is specialized on the analysis of drinkable and surface water. In the last chapter of the theoretical part there are introduced water works where samples for an experimental assessment were collected from. The experimental part of the thesis presents the specification of the extraction of analytes and an adjustment of used device. In the thesis there are specific calibrations for individual assessment of hormones. The results of the assessment for each collection point are ordered to a well-arranged table. Increased levels of targeted estrogens were not approved in most of sampling points.

Characterisation of oestrogenic properties of Isoflavones derived from Millettia griffoniana Baill.: - Molecular mode of action and tissue selectivity

Six isoflavones derived from Millettia griffoniana namely, 4’-methoxy-7-O-[(E)-3-methyl-7hydroxymethyl-2,6 octadienyl]isoflavone (7-O-DHF), Griffonianone C (Griff C), 7-O-geranylformononetin (7-O-GF), 3’,4’-dihydroxy-7-O-[(E)-3,7-dimethyl-2,6-octadienyl]isoflavone (7-O-GISO), Griffonianone E (Griff E), 4’-O-geranylisoliquiritigenin (4-O-GIQ) were tested for potential oestrogenic activities in three different oestrogen receptor alpha (ERα) dependent assays, namely a recombinant yeast assay, a reporter gene assay based on stably transfected MCF-7 cells (MVLN cells) and the induction of alkaline phosphatase in Ishikawa cells. The oestrogenic activities of isoflavones from Millettia griffoniana could be completely suppressed by the pure oestrogen antagonist, fulvestrant. The expression of Ki-67, proliferating cell nuclear antigen (PCNA) and cyclin D1 (CD1) mRNA used as indicator of cell proliferation in MCF-7 cells was assayed. Based on these in vitro results, Griff C was further tested in vivo. The main objective of this part of the work was to study the mechanistic basis of the oestrogenicity Three different doses of Griff C (2, 10, or 20 mg/kg BW) of Griff C in ovariectomised Wistar rats. 17β-oestradiol (E2: 10 µg/kg BW) was used as positive control. They were treated daily for three consecutive days and sacrificed 24 hours after receiving the last dose. The whole uterus was removed and weight. Liver and vena cava fragments were also collected and stored together with uteri in liquid nitrogen for subsequent real-time PCR to evaluate the effects of Griff C on the regulation of some relevant oestrogen–responsive genes in the uterus, the liver and the vena cava. The role of Griff C in apoptosis or in cell survival, through mediation of the phosphatidylinositol 3 kinase-Akt (PI3K-Akt) signaling pathway, was also investigated. Western blot analysis revealed that Griff C slightly increased the phosphorylation of Akt at its serine 473 residue. In this work, oestrogenic properties of the isoflavones derived from Millettia griffoniana are described using reporter gene assays and the oestrogen-inducible alkaline phosphatase Ishikawa model for the first time. These in vitro data were verified in vivo showing the regulation of the expression of various relevant oestrogen-responsive genes by Griff C. The spectrum of its activity was clearly similar to that of 17β-oestradiol on uterine hepatic and vena cava tissues of ovariectomised rats except for the proliferative response. However Griff C remained 100 to 1000 times less effective than oestradiol. These findings confirmed that some of the biological effects attributed to Millettia griffoniana are closely related to oestrogen-mediated action.

Terapia hormonal e sexualidade em mulheres na pós-menopausa / Hormone therapy and sexuality in postmenopausal women

Verificar os efeitos da terapia hormonal com derivados estrogênicos e progestogênicos, isolados ou associados à metiltestosterona, na sexualidade e nos sintomas climatéricos em mulheres na pós-menopausa e comparar os dois tipos de terapia hormonal. CASUÍSTICA. Selecionaram-se sessenta mulheres sexualmente ativas, com queixas sexuais, com relacionamento estável com parceiro capacitado para o coito, com idade de 42 a sessenta anos (média etária 52,1 + 4 anos) e tempo de menopausa de um a 28 anos (média 5,6 anos). Excluíram-se mulheres com doenças sistêmicas, doenças psiquiátricas, endócrinas, distopias genitais, tabagistas e usuárias de terapia hormonal ou de medicamentos que apresentavam interferência na sexualidade. METODOLOGIA. Realizou-se estudo de coorte progressiva, duplo-cego, randomizado, com duração de 12 meses. As mulheres foram divididas em dois grupos: EP (n=29), medicadas com estrogênios conjugados (EEC) 0,625 mg + acetato de medroxiprogesterona (AMP) 2,5 mg + placebo, e grupo EP+A (n=31), medicadas com EEC 0,625 mg + AMP 2,5 mg + metiltestosterona 2,0 mg. Para estudar a sexualidade, utilizou-se o Questionário Sexual do Hospital das Clínicas e foram avaliados o desejo sexual, a excitação e a capacidade orgástica nas atividades desenvolvidas com o parceiro, o interesse sexual não vinculado, exclusivamente, às atividades desenvolvidas com o parceiro, a dispareunia, a secura vaginal e a freqüência sexual. Para as análises estatísticas, utilizaram-se Modelos Lineares Gerais para Medidas Repetidas, Análise de Variância (ANOVA), Modelos de Regressão Logística Multinomial e Qui-quadrado de Pearson. O nível de significância foi de 5%. RESULTADOS. Nos grupos EP e EP+A, houve aumento no escore de desejo sexual vinculado, exclusivamente, às atividades desenvolvidas com o parceiro (F=18,334; p<0,001), no escore de excitação sexual (F=14,022; p < 0,001), na capacidade orgástica (F=34,650; p < 0,001) e na freqüência sexual (F=7,687; p=0,008), bem como redução da secura vaginal (?2=44,153; p<0,001), da dispareunia (?2=34,447; p < 0,001) e do índice menopausal de Kupperman (F=158,460; p < 0,001). A análise comparativa entre os grupos EP e EP+A mostrou maior interesse sexual não vinculado, exclusivamente, às atividades com o parceiro (?2=11,551; p=0,021) e mais altos índices de Castelli I (F=8,542; p < 0,001) e índices de Castelli II (F=11,500; p < 0,001) no grupo EP+A. Não se observaram hirsutismo nem alopécia em nenhum dos grupos; acne grau I foi observada em duas mulheres do grupo EP e em 13 do grupo EP+A. CONCLUSÕES. As terapias hormonais com derivados estrogênicos e progestogênicos, isolados ou associados à metiltestosterona, causaram impacto positivo em todos os parâmetros sexuais e nos sintomas climatéricos analisados. A associação de metiltestosterona ao tratamento estro-progestacional aumentou o interesse sexual não vinculado, exclusivamente, às atividades com o parceiro e os índices de Castelli I e II. Nos demais parâmetros estudados, não houve diferença entre os dois grupos / Objectives: To verify the effects of hormone therapy with estrogen and progesterone derivatives when used singly or combined with methyltestosterone, on sexuality and on climacteric symptoms in postmenopausal women. Subjects: The series included sixty-sexually active women, with sexual complaints, in a stable relationship with a partner capable of intercourse, ages ranging from 42 to sixty years (average 52,1 + 4) and menopause time from one to 28 years (average 5,6). Excluded were women with systemic diseases, psychiatric and endocrine disorders, genital dystopias, smokers and those on hormone therapy or medications that affect sexuality. Method: A double blind, randomized, progressive cohort study was performed over a twelve month period. The women were divided into two groups: EP (n=29), medicated with conjugated estrogens (EEC) 0,625 mg + medroxyprogesterone acetate (AMP) 2,5 mg + placebo, and group EP+A (n=31), medicated with EEC 0,625 mg + AMP 2,5 mg + methyltestosterone 2,0 mg. For the study of sexuality, the Hospital das Clínicas Sex Questionnaire was utilized, assessing sexual desire linked exclusively to activities developed with the partner; excitation and orgasmic capacity in activities with the partner; sexual interest not linked exclusively to activities developed with the partner; dyspareunia, vaginal dryness and sexual frequency. For statistical analysis, the General Linear Models for Repeated Measures, Analysis of Variance (ANOVA), Multinomial Logistic Regression Models and Pearson Chi square were employed. A 5% significance level was adopted. Results: In groups EP and EP+A, was observed an increase in the sexual desire score linked exclusively to activities developed with the partner (F=18,334; p<0,001), sexual excitation (F=14,002; p<0,001), orgasmic capacity (F=34,650; p < 0,001) and in sexual frequency (F=7,687; p=0,008), as well as an reduction in vaginal dryness (x2=44,153; p < 0,001), dyspareunia (x2=34,447; p < 0,001) and in the Kupperman menopausal index (F=158,460; p < 0,001). Comparative analysis between groups EP and EP+A revealed a greater sexual interest not linked exclusively to activities with the partner (x2=11,551; p=0,021) and higher Castelli I index (F=8,542; p < 0,001) and Castelli II index (F=11,500; p<0,001) in group EP+A. Neither hirsutism nor alopecia were noticed in either group; Class I acne was observed in two women of group EP and in 13 of group EP+A. Conclusion: Hormone therapy with estrogen and progesterone derivatives used singly or together with methyltestosterone had a positive result on all sexual parameters and on climacteric symptoms analyzed. Association of methyltestosterone to estrogen-progesterone treatment increased sexual interest not linked exclusively to activities with the partner and Castelli I and II indexes. No difference between the two groups in the other parameters studied was demonstrated

Para estudar a sexualidade, utilizou-se o Questionário Sexual do Hospital das Clínicas e foram avaliados o desejo sexual, a excitação e a capacidade orgástica nas atividades desenvolvidas com o parceiro, o interesse sexual não vinculado, exclusivamente, às atividades desenvolvidas com o parceiro, a dispareunia, a secura vaginal e a freqüência sexual. Para as análises estatísticas, utilizaram-se Modelos Lineares Gerais para Medidas Repetidas, Análise de Variância (ANOVA), Modelos de Regressão Logística Multinomial e Qui-quadrado de Pearson. O nível de significância foi de 5%. RESULTADOS. Nos grupos EP e EP+A, houve aumento no escore de desejo sexual vinculado, exclusivamente, às atividades desenvolvidas com o parceiro (F=18,334; p<0,001), no escore de excitação sexual (F=14,022; p < 0,001), na capacidade orgástica (F=34,650; p < 0,001) e na freqüência sexual (F=7,687; p=0,008), bem como redução da secura vaginal (?2=44,153; p<0,001), da dispareunia (?2=34,447; p < 0,001) e do índice menopausal de Kupperman (F=158,460; p < 0,001). A análise comparativa entre os grupos EP e EP+A mostrou maior interesse sexual não vinculado, exclusivamente, às atividades com o parceiro (?2=11,551; p=0,021) e mais altos índices de Castelli I (F=8,542; p < 0,001) e índices de Castelli II (F=11,500; p < 0,001) no grupo EP+A. Não se observaram hirsutismo nem alopécia em nenhum dos grupos; acne grau I foi observada em duas mulheres do grupo EP e em 13 do grupo EP+A. CONCLUSÕES. As terapias hormonais com derivados estrogênicos e progestogênicos, isolados ou associados à metiltestosterona, causaram impacto positivo em todos os parâmetros sexuais e nos sintomas climatéricos analisados. A associação de metiltestosterona ao tratamento estro-progestacional aumentou o interesse sexual não vinculado, exclusivamente, às atividades com o parceiro e os índices de Castelli I e II. Nos demais parâmetros estudados, não houve diferença entre os dois grupos / Objectives: To verify the effects of hormone therapy with estrogen and progesterone derivatives when used singly or combined with methyltestosterone, on sexuality and on climacteric symptoms in postmenopausal women. Subjects: The series included sixty-sexually active women, with sexual complaints, in a stable relationship with a partner capable of intercourse, ages ranging from 42 to sixty years (average 52,1 + 4) and menopause time from one to 28 years (average 5,6). Excluded were women with systemic diseases, psychiatric and endocrine disorders, genital dystopias, smokers and those on hormone therapy or medications that affect sexuality. Method: A double blind, randomized, progressive cohort study was performed over a twelve month period. The women were divided into two groups: EP (n=29), medicated with conjugated estrogens (EEC) 0,625 mg + medroxyprogesterone acetate (AMP) 2,5 mg + placebo, and group EP+A (n=31), medicated with EEC 0,625 mg + AMP 2,5 mg + methyltestosterone 2,0 mg. For the study of sexuality, the Hospital das Clínicas Sex Questionnaire was utilized, assessing sexual desire linked exclusively to activities developed with the partner; excitation and orgasmic capacity in activities with the partner; sexual interest not linked exclusively to activities developed with the partner; dyspareunia, vaginal dryness and sexual frequency. For statistical analysis, the General Linear Models for Repeated Measures, Analysis of Variance (ANOVA), Multinomial Logistic Regression Models and Pearson Chi square were employed. A 5% significance level was adopted. Results: In groups EP and EP+A, was observed an increase in the sexual desire score linked exclusively to activities developed with the partner (F=18,334; p<0,001), sexual excitation (F=14,002; p<0,001), orgasmic capacity (F=34,650; p < 0,001) and in sexual frequency (F=7,687; p=0,008), as well as an reduction in vaginal dryness (x2=44,153; p < 0,001), dyspareunia (x2=34,447; p < 0,001) and in the Kupperman menopausal index (F=158,460; p < 0,001). Comparative analysis between groups EP and EP+A revealed a greater sexual interest not linked exclusively to activities with the partner (x2=11,551; p=0,021) and higher Castelli I index (F=8,542; p < 0,001) and Castelli II index (F=11,500; p<0,001) in group EP+A. Neither hirsutism nor alopecia were noticed in either group; Class I acne was observed in two women of group EP and in 13 of group EP+A. Conclusion: Hormone therapy with estrogen and progesterone derivatives used singly or together with methyltestosterone had a positive result on all sexual parameters and on climacteric symptoms analyzed. Association of methyltestosterone to estrogen-progesterone treatment increased sexual interest not linked exclusively to activities with the partner and Castelli I and II indexes. No difference between the two groups in the other parameters studied was demonstrated

Role of the <em>RNF8</em>, <em>UBC13</em>, <em>MMS2</em> and <em>RAD51C</em> DNA damage response genes and rare copy number variants in hereditary predisposition to breast cancer

Abstract Mutations in the currently known breast cancer susceptibility genes account for only 25–30% of all familial cases. Novel susceptibility genes can be identified by several methods, including candidate gene re-sequencing and genome-wide microarrays. We have applied microarrays for the detection of a new genomic variation class, copy number variants (CNVs), which potentially could disrupt genes in multiple pathways related to breast cancer susceptibility. The aim of the current study was to evaluate the role of the RNF8, UBC13, MMS2 and RAD51C DNA damage response genes in breast cancer susceptibility as well as to study if rare CNVs are associated with the predisposition to this disease. The analysis of 123 familial breast cancer cases revealed altogether nine different changes in the RNF8 and UBC13 candidate genes. However, none of the observed alterations were considered pathogenic. No alterations were observed in MMS2. The obtained results suggest that breast cancer predisposing alterations in RNF8, UBC13 and MMS2 are rare, or even absent. The RAD51C mutation screening of 147 familial breast cancer cases and 232 unselected ovarian cancer cases revealed two deleterious mutations: c.-13_14del27 was observed in a breast cancer case with familial history of ovarian cancer and c.774delT in an ovarian cancer case. Both mutations were absent in the control cohort. The results of the study support the hypothesis that rare variants of RAD51C predispose predominantly to ovarian cancer. A genome-wide scan of CNVs was performed for 103 familial breast cancer cases and 128 controls. The biological networks of the genes disrupted by CNVs were different between the two groups. In familial breast cancer cases, the observed mutations disrupted genes, which were significantly overrepresented in cellular functions related to maintenance of genomic integrity (P=0.0211). Biological network analysis showed that the disrupted genes were closely related to estrogen signaling and TP53-centered tumor suppressor network, and this result was confirmed by the analysis of an independent young breast cancer cohort of 75 cases. These results suggest that rare CNVs represent an alternative source of genetic variation contributing to hereditary risk for breast cancer. / Tiivistelmä Tunnetut rintasyöpäalttiusgeenien mutaatiot selittävät vain 25–30 prosenttia kaikista perinnöllisistä rintasyöpätapauksista. Uusia alttiusgeenejä voidaan tunnistaa useilla eri menetelmillä, kuten kandidaattigeenien mutaatiokartoituksella ja genomin-laajuisilla mikrosirutekniikoilla. Tässä tutkimuksessa sovelsimme mikrosirutekniikkaa uuden geneettisen variaatioluokan, kopiolukuvariaation (CNV), tutkimiseen. CNV:t voivat vaurioittaa lukuisia rintasyöpäalttiuteen liittyviä biokemiallisia reittejä. Tämän tutkimuksen tarkoitus oli arvioida RNF8-, UBC13-, MMS2- ja RAD51C -DNA- vauriovastegeenien sekä harvinaisten CNV:iden yhteyttä rintasyöpä-alttiuteen. 123 familiaalisen rintasyöpätapauksen analyysissä löytyi yhteensä yhdeksän muutosta RNF8- ja UBC13-geeneistä, joista yksikään ei osoittautunut patogeeniseksi. RAD51C-geenin mutaatiokartoitus 147 familiaalisesta rintasyöpätapauksesta sekä 232 valikoimattomasta munasarjasyöpätapauksesta paljasti kaksi haitallista mutaatiota. c.-13_14del27 havaittiin rintasyöpäpotilaalla, jonka suvussa esiintyi munasarjasyöpää, ja c.774delT todettiin munasarjasyöpäpotilaalta. Kumpaakaan mutaatiota ei havaittu verrokkiaineistossa. Tulokset vahvistavat hypoteesia RAD51C-geenin harvinaisten varianttien yhteydestä pääasiassa munasarjasyöpäriskiin. CNV:iden genomin-laajuinen skannaaminen suoritettiin 103 familiaaliselle rintasyöpätapaukselle ja 128 verrokille. CNV:iden häiritsemien geenien muodostamat biologiset verkostot olivat erilaiset näiden kahden ryhmän välillä. Familiaalisilla rintasyöpätapauksilla havaitut CNV:t vaikuttivat geeneihin, jotka olivat voimakkaasti korostuneita genomin eheyttä ylläpitävissä tehtävissä (P=0.0211). Biologisten verkostojen analyysi paljasti, että CNV:iden vahingoittamat geenit liittyivät läheisesti estrogeenisignalointiin sekä TP53-tuumorisupressoriverkostoon, ja tämä tulos vahvistettiin analysoimalla riippumatonta nuorista rintasyöpäpotilaista koostuvaa kohorttia (N=75). Tutkimuksen tulosten mukaan harvinaiset CNV:t ovat vaihtoehtoinen geneettisen variaation lähde perinnölliseen rintasyöpäalttiuteen.
