An Address, Not a Room Number: An Assisted Living Community within a Community

As the son of parents of the "baby boomer" generation I foresee the immanent increased demand for assisted living for the elderly. By 2030 an estimated 20 percent - one in five Americans - will be 65 or older. Unless attention is paid now to advancing the designs and opportunities for assisted-living, there will either be a distinct lack of available options, or worse, the continuation of out-dated designs that simply will not be a marketable. Contemporary thinking with regard to the design of places of residence for the elderly has changed dramatically from the days of outdated structures and philosophies. In my research, I have seen a shift toward the concept of community rather than institution. Owing to cultural change that will surely be the future of assisted living design strategy. Yet, the shift is still in the rut of scaling designs to accommodate up to only a few dozen individuals. While I understand the rationale of such designs, I foresee a shortcoming to that approach. If the design idea is the drive toward the feeling of "home" then I feel it to be conceivable for an expansion of the current model to include multiple neighborhoods of residences, subsidiary structures, and more connection to a feeling of neighborhood and social interaction; the attributes of a true "community." What are missing are the environments that appeal to younger citizens. Baby boomers, easily described as much more educated, well-informed, well traveled, financially successful and much more discerning will not find the traditional options appealing or acceptable. My primary method of research for such an endeavor falls under the classification of Qualitative Research; the category that focuses on personal and informal social science. My design intention is not to separate and isolate the population of those seeking assisted living. Instead, offer more than just a place to live. I intend to discover validity for the concept of a larger networked residential community with more opportunity for social interaction and active lifestyle extending beyond what currently exists.

<p>Syftet med denna studie var att beskriva hur vårdare inom äldrevården upplever sin arbetssituation. Metoden som använts var en beskrivande litteraturstudie. Data samlades in via databaserna Medline via Pubmed och Cinahl. Sökorden nursing homes, elderly care och geriatric care kombinerades med experience och personnel för att ett så stort område som möjligt av äldrevården skulle ingå i sökningen. Sökordet Scandinavia valdes för att författarna ville fokusera studien på skandinavisk äldrevård. Resultatet visade att vårdarna ansåg tidsbristen vara det som påverkade den goda omvårdnaden negativt, det vill säga att det inte fanns tid att ge den omvårdnad som var önskvärd. Vårdtagarnas anhöriga sågs som en tillgång för de äldres välmående men att de ibland upplevdes som krävande och tålamodsprövande. En dåligt fungerande organisation samt ett dåligt ledarskap var orsak till att de inte kunde se till att de äldres behov av omvårdnad uppfylldes. Den goda kamratskapen och lagandan inom arbetsgruppen var viktig för att kunna ge god vård till de äldre. Positivt ansågs också utbildning och handledning för att kunna reflektera över sitt arbete och göra prioriteringar i arbetet. Arbetstillfredsställelse fick vårdarna genom den glädje som uppstod genom den ömsesidiga närheten mellan vårdare och de äldre. Lex Sarah inom kommunal äldreomsorg : En kvalitativ studie utifrån enhetschefers perspektiv på anmälningar vid missförhållanden

The purpose of this study has been to examine the unit manager’s perspective on making Lex Sarah-notifications. The study has been carries out by six semi structured interviews with persons in leading positions of the elderly from three different municipalities. They were asked about different circumstances that affects the problems with Lex Sarah. The results indicates that the unit manager’s act on elder abuse from the complex mechanisms that exist within the organization. The study results shows that the unit manager’s in the daily tasks are doing their work and solve problems regarding the risk of elder abuse that occur over time, but circumstances will determine if its documented prescribed by a Lex Sarah. The conclusion is that the unit manager’s of the elderly is limited by the circumstances and processes within the organization and that it affects them to lift up and report abuse among the elderly.

Konflikthantering : Ett verktyg i det dagliga arbetet / Conflict Management : A tool in the daily work at the workplace

Syftet med denna studie är att undersöka faktorer som kan vara av betydelse för att möjliggöra konflikthantering i det dagliga arbetet. Vi menar att om möjligheter skapas och initiativ tas från ledningens sida så kan organisationen utvecklas i en positiv riktning. Studien grundar sig delvis på teorier om vikten av att ha en tydlig struktur som uppfattas av medarbetarna som rättvis och att konflikter kan få konsekvenser för hela organisationen. Studien är av kvalitativ design och är utförd inom äldreomsorgen i en kommun i södra Sverige. Analysen visar att faktorer som: tydlig arbetsstruktur, gemensam vision och målsättning, utbildning inom konflikthantering, tydlig och öppen kommunikation, att ta sig tid till konflikthantering, att vara professionell och neutral och att ta ansvar och visa respekt har en betydande inverkan vid konflikthantering. Dessa faktorer kan vara till hjälp för organisationen att utveckla konflikthantering som ett verktyg på arbetsplatsen. Genom att använda sig av en konflikthanteringsmetod utifrån den ledarskapsstil man väljer att använda som chef, kan en rutin utvecklas kring konflikthantering på arbetsplatsen som integrerar medarbetarna. På detta sätt minimeras risken för att konflikter ska eskalera på arbetsplatsen och få förödande konsekvenser. / The aim of this study is to investigate factors which may be of importance to enable conflict management in daily work. We believe that if opportunities are created and initiatives taken by management, the organization will be able to develop in a positive direction. The study is partly based on theories about the importance of clear structure, seen by employees as fair and that conflicts can affect the entire organization. The study is made by a qualitative design and carried out in elderly care in a municipality in southern Sweden. The analysis shows that factors such as: clear working structure, shared vision and goal, education in conflict management, clear and open communication, make time for conflict management, be professional and neutral, and to take responsibility and show respect have a significant impact on conflict management. These factors can be of help for the organization to develop their conflict management as a tool in the workplace. By using a conflict management method based on the leadership style one chooses to use as a manager, then a routine can be developed around conflict management in the workplace that integrates employees. In this way minimizes the risk that conflicts will escalate in the workplace and have devastating consequences.

Background: Malnutrition is a nursing issue that causes both physical and psychological suffering of those who are affected. In order to achieve care that is safe and has a good quality, it must be based on the assessment and actions of the state. District nurses play a central role in this work. Aim: The aim of this study was to describe the district nurses' work with identifying and addressing malnutrition in older people in nursing Home Care of the Community. Method: Six interviews with district nurses were conducted. The data were analyzed using qualitative content analysis with an inductive approach Results: From the results of the analysis two categories emerged: observations for finding signs of malnutrition, treatment and following up by malnutrition. Conclusion: Cooperation in the form of communication and information within the care team, together with the patient and their relatives, were found to be important for the patient's nutritional needs and should be identified and addressed in order to avoid the risk of malnutrition. The results that emerge can contribute to new knowledge that can help to identify older persons in order to get early and appropriate interventions and thus reducing the suffering that is caused by malnutrition. / Bakgrund: Malnutrition är ett omvårdnadsproblem som medför både fysiskt och psykiskt lidande för den som drabbas. För att uppnå en vård som är säker och har god kvalitet, måste den baseras på bedömning samt åtgärder av nutritionsproblemet. Distriktssköterskan har en central roll i detta arbete. Syfte: Syftet med studien var att beskriva distriktssköterskors arbete med att identifiera och åtgärda malnutrition hos äldre personer inom kommunal hemsjukvård. Metod: Sex intervjuer med distriktssköterskor genomfördes. Datamaterialet analyserades med kvalitativ innehållsanalys med induktiv ansats. Resultat: Ur analysen av datamaterialet framträdde två kategorier; Observationer för att finna tecken på malnutrition, behandling och uppföljning av malnutrition. Konklusion: Samarbete i form av kommunikation och information inom vårdteamet och tillsammans med patienten och dess anhöriga, visade sig vara av vikt för att patientens nutritionsbehov ska kunna identifieras och tillgodoses för att inte riskera malnutrition. Resultatet som framkom kan bidra med kunskap som kan underlätta att identifiera äldre personer tidigt och med adekvata insatser förmå att minska lidandet som malnutrition medför.

The aim of this essay was to describe and interpret the managements and organizations impact regarding the caregivers working conditions. To examine this we used a qualitative method. We completed two group interviews and five individual interviews with a total of fifteen enrolled nurses that worked within elderly care. In our interviews we found that the enrolled nurses felt that the organizations economic interests collided with their personal values regarding good quality elderly care. We also found that the caregivers experienced that they had an impact on their working conditions and that their employer was the main reason for making it possible.

Empowerment i hemtjänsten : Fallstudie inom hemtjänsten i Reykjavík

I många år har det stått klart att på grund av ökning av antalet äldre kommer det blir svårt med framtida bemanning i hemtjänsten. Arbeten anses vara låg status och underbetalda. Om arbeten i hemtjänsten anses ge anställda upplevelser av empowerment är det möjligt att flera söker sig till dessa arbeten och att befintlig personal stannar kvar. Syftet med denna uppsats är att få ökad förståelse för hur personal i hemtjänst upplever empowerment i sitt arbete samt att studera på vilket sätt ledare inom hemtjänsten kan arbeta med att stödja upplevelser av empowerment hos sina anställda.   Undersökningen är genomförd som en kvalitativ fallstudie i hemtjänsten i Reykjavík. Med semistrukturerade intervjuer har sex anställda och tre ledare intervjuats och materialet har analyserats med en tematisk analys. Det visar sig att de anställda i hemtjänsten i Reykjavík upplever delvis strukturell empowerment i sina arbeten samt pyskologisk empowerment genom alla fyra kognitionerna; mening, kompetens, självbestämmande och påverkan. Ledarna stödjer personalens upplevelser av empowerment genom tillit, respekt och värdighet samt tillåtelse att göra misstag. / For years it has been foreseen that the increase in number of the elderly living at home will cause difficulties when it comes to providing homecare. The cause of those difficulties are mainly because of difficulties in recruiting new employees and keeping those who are working in the home care. Recruiting problems are mainly because employment in the elderly care is considered low status and underpaid. If the employees are empowered in their work there is a possibility that employment would be considered attractive and would therefore attract more young people.  The purpose of this essay is to gain increased understanding of experiences of empowerment by the employees in home care. Another purpose is to see in what way the leaders in home care can encourage empowerment of their employees.  The survey was conducted as a qualitative case study in home care in Reykjavík. With semi-structured interviews six employees and three managers were interviewed and the material has been analyzed with a thematic analysis.  It turns out that the employees of home care services in Reykjavík experience to a certain degree structural empowerment in their jobs and to a higher degree psychological empowerment. Sjuksköterskors upplevelser av interaktion med närstående i kommunal äldrevård / Nurses experiences of interaction with related parties in municipal elderly care

Background: In pace with the increasingly aging population will the number of older people in nursing homes and with care in home grow. This means that the nurse will face related to the older more frequently and more widely. Nurses has overall responsibility over both the care of the elderly as a responsibility of maintaining good relationships with related parties. Related parties are of great importance for the older individual, it is therefore relevant for the nurse to be able to interact in a respectful and sensitive manner to involve the relatives in care. Aim: The aim with this study was to highlight nurse's experiences of interaction with related parties to elderly in community health care. Method: A literature review based on nine qualitative articles was conducted. Friberg design was used with the inspiration of Lundman and Hällgren Graneheim regarding the analysis of Articles. Results: The study resulted in four themes. They included information about nurse's experiences of interaction with related parties. These themes describes the importance of communication in the interaction, the attitudes various expressions, cooperation with related parties and experiences with related parties' participation in care. Conclusion: An important conclusion is that the nurse should take advantage of related parties skills and resources in health care around the elderly. The nurses sometimes has difficulties interacting with related parties. Nurses probably need to see the difficulties from different perspectives and seek help of colleagues.
