Avaliação dos mecanismos de ruminação em pacientes com disforia de gênero

A Ruminação tem sido um importante campo de investigação para estudar os mecanismos cognitivos e alteração dos estados emocionais associados ao processo do desenvolvimento de condições de saúde mental. A presente dissertação, cujo relatório de pesquisa deriva nessa produção, busca elucidar as relações entre ruminação e disforia de gênero. Dessa forma, a dissertação apresentará dois estudos de delineamento transversal que, no entanto, sofrem adaptações na disposição dos indivíduos da amostra para testar hipóteses específicas. Em ambos os estudos foram recrutadas 39 mulheres transexuais, no período de 2014 a 2015, que atenderam aos critérios do DSM 5 para diagnóstico de Disforia de Gênero (DG). O primeiro estudo teve como objetivo analisar os níveis de ruminação em pacientes com disforia de gênero antes e depois da Cirurgia de Redesignação Sexual (CRS), o que contribui com a literatura que investiga marcadores de desfecho positivo para CRS. O primeiro subgrupo (T0) foi constituído por participantes com diagnósticos confirmados para DG e com, no máximo, um ano de acompanhamento em grupoterapia. O T1, por pacientes que atenderam os pré-requisitos em relação à frequência aos atendimentos dos grupos psicoterapêuticos de, no mínimo, um ano, e no máximo dois anos, sem contra-indicação para realização da CRS. E o T2, constituído por pacientes pós-cirúrgicos em um período superior a seis meses. O segundo estudo teve como objetivo tanto analisar a relação entre abuso emocional na infância e ruminação em mulheres transexuais quanto identificar, entre os indivíduos da amostra, quais ruminam em nível p[baixo] = (33) ≤18 e p[alto] = (67) ≥23. Essa classificação entre dois subtipos do pensamento ruminativo pode elucidar diferenças entre os fatores protetivos e os desfechos nocivos na saúde mental e física dessa população. No primeiro estudo, a ruminação diminuiu entre os pacientes no T2 e os escores em ruminação foram ainda gradualmente menores a cada procedimento realizado nas características sexuais secundárias por pacientes deste grupo. Esse achado contribui para pesquisas do paradigma Research Domain Criteria (RDoC), que almeja demonstrar marcadores psicológicos para condições de saúde mental. Nesse caso, a ruminação parece se mostrar como um marcador importante para o desfecho positivo em pacientes com disforia de gênero pós-CRS. No segundo estudo, nosso principal achado reside na prevalência em até 15 vezes (IC95%: 2,25-99,63) de maior chance para engajamento em ruminação elevada ou comportamento disfórico entre os indivíduos que foram expostos ao abuso emocional, sobretudo, entre aqueles que o vivenciaram no intervalo da tipologia que variou de moderado – grave a grave – extremo se comparado aos indivíduos com gravidade entre mínimo a moderado na escala Childhood Trauma Questionnaire (CTQ). O abuso emocional está relacionado com engajamento em ruminação, sobretudo, ao subtipo elevada, o que contribui para aumento em comportamentos disfóricos, e ao estar associado à desregulação da emoção, que contribui para os desfechos nocivos na saúde física e mental desta população. / The Rumination has recently become an important field of study to better understand cognitive mechanisms and emotional status associated to the development of mental health conditions. The current work aims to shine a light upon the relation between rumination and gender dysphoria (GD). In this sense, this work presents two cross-sectional studies, each testing two specific hypotheses. Therefore, they have suffered adaptations in the disposition of the sample. In both studies, 39 transexual women, who fulfilled the DSM 5 diagnostic criteria for GD, were recruited within the years of 2014 and 2015. The first study aimed to analyse how much GD patients have rumination processes before and after sex reassignment surgery (SRS), which contributes to the body of literature seeking for positive outcome markers for SRS. The first group (T0) was made of participants who had a confirmed DG diagnostic and participated of group therapy for up to one year (between 0 and 12 months). The second group (T1) was composed by participants who had a confirmed DG diagnostic and attended from one up to two years of group therapy and had no contraindications for SRS. The third group (T2) was comprised of patients who have gone through SRS surgery at least six months prior to the data collection. The second study aimed to analyse the relation between emotional abuse during childhood and rumination in transexual women and to identify, amongst sample individuals, those who ruminate at a low or high levels. This classification into two subsets of ruminative thinking might clarify diferences between protective factors and poor mental and physical health outcomes in this specific population. In the first study, patients of the T2 group ruminate significantly less than their counterparts; moreover, scores of the ruminative thinking process seems to fall gradually with each alteration in the secondary sex characteristics. This finding contributes to the researches of the Research Domain Criteria (RDoC) paradigm, which seeks to find physiological markers for mental health conditions. In this case, rumination seems to be an important marker for positive outcomes in post-SRS GD patients. In the second study, the main result is the higher (up to 15 fold) prevalence for likeliness to engane in high level of ruminative thinking or dysphoric behaviour amongst the individuals who were exposed to emotional abuse. Above all, the engagement in ruminative thinking process is even higher amongst those who lived through emotional abuse in the moderated - severe and the severe - extreme categories of the Childhood Trauma Questionnaire (CTQ) scale than those in the light - moderate category. Emotional abuse is related to ruminative engagement, specially to the high level subtype of rumination. This contributes to the increase in dysphoric behaviours and to the bad health and mental outcomes in this population, probably associated with emotional dysregulation.

Emotional Neglect in Childhood and Attachment Anxiety in Adult Relationships as Predictors of Social Networking Addiction

This study investigated the relationship between childhood maltreatment and social networking addiction to determine if it was mediated by anxious-avoidant attachment. It was hypothesized that the presence of an anxious-avoidant attachment pattern developed as a result of childhood maltreatment would mediate the correlation between anxious-avoidant attachment and social networking addiction. This would mean that participants who developed an anxious-avoidant attachment style due to childhood maltreatment would use social media in unhealthy ways. To measure childhood maltreatment, the Adverse Childhood Experiences scale (ACE) was used. The Experiences in Close Relationships-Revised scale (ECR-R) was used to measure anxious-avoidant attachment. Finally, social networking addiction was measured by the Social Networking Addiction Scale (SNAS). The sample was relatively young (M = 19.88, SD = 2.62), (n = 82). Three bivariate correlations and multiple linear regression were conducted to determine if an anxious-avoidant attachment pattern mediated the connection between childhood maltreatment and social networking addiction. A significant correlation was found between childhood maltreatment and anxious-avoidant attachment, as well as between anxious-avoidant attachment and social networking addiction. However, no correlation was found between childhood maltreatment and social networking addiction. It was concluded that no mediation was present.

Psychological violence is mistreatment that is severe enough to influence the target person's health, to destroy his work or total career and overstrain his relationship with his family or friends. The impact of psychological violence has compelled many a company and organization to introduce policy to stop the serious consequences thereof. It seems not to be the case in the South African education. The mixed method research design was used in this study to investigate the nature and impact of psychological violence on the health of secondary school educators in the Free State. Strategies were proposed to reduce educators' experiences of psychological violence and the impact thereof on their health. Two measuring instruments were used in this study to collect the quantitative data, namely the Revised Negative Acts Questionnaire (NAQ-R) and the Symptoms Checklist. Both these measuring instruments were distributed amongst 727 secondary school educators at 82 schools in the five education districts of the Free State. The qualitative data collection was done with 32 educators. The sources of psychological violence, likewise in the literature, are management, learners and parents. Psychological violence often causes enormous stress and it leads to a situation where educators feel depressed and traumatised. The general aim of this study is to invsetigate the nature and impact of psychological violence on secondary school educators in the Free State. Some of the findings in this study are that there are not differences between men and women with regard to exposure to psychological violence; older educators experience more exposure to psychological violence; teaching experience, qualifications and ethnicity are indeed determining factors with regard to exposure. It is also possible, as appears from the findings, that here a minority group is exposed more to psychological violence. The results of both data sets, quantitative and qualitative, are mutually confirming. It was found in this study that secondary school educators indeed experience psychological violence. This participatory research journey looked at the lives of adult children of alcoholics, with particular reference to how identity is affected by growing up in a home where one or both parents are struggling with alcohol abuse, and how this struggle with identity carries through into adulthood. A study was made of the particular discourses which impacted negatively upon the child from this home and the manner in which the discourses had a negative impact later on as an adult. Through narrative pastoral conversations the possibilities for healing, as well as the barriers for healing were explored. Through this exploration alternative stories were created. A vital part of the study was focused upon the witnessing of the stories of the participants` lives and the acknowledgement of some of the events from their childhood that formed their identity and contributed to who they are today. / Practical Theology / M.Th. Peer on peer abuse : safeguarding implications of contextualising abuse between young people within social fields

An existing body of research indicates that peer-on-peer abuse, involving the physical, sexual and/or emotional abuse of young people by their peers, is an issue of serious concern within the UK. Whilst a range of studies have explored the individual and familial vulnerabilities associated with this phenomenon, there is an increasing recognition of the need to also consider the relationship between young people‟s peer groups, and other pertinent social fields, to their experiences of such abuse. This thesis offers an original contribution to the field by explicitly seeking to develop this contextual approach. It applies an age-specific and gendered interpretation of Bourdieu‟s constructivist structuralism (and specifically the concepts of field, habitus and symbolic violence) to the analysis of nine cases where young people raped or murdered their peers. In doing so, it offers a unique, in-depth, exploration of the interaction between individuals and the social fields that they navigate, in the context of nine abusive incidents. This methodological approach demonstrates how harmful norms underpinning these incidents are informed by a multi-way interplay between various social fields and young people‟s reflexive engagement with this process. It is through this interplay that motives and power hierarchies are established, and gender, age, consent, culpability, vulnerability and ultimately safety, are socially constructed and experienced.

Abuso emocional : suas relações com autoestima, bem-estar subjetivo e estilos parentais em universitários

A revisão da literatura mostrou que há uma carência de estudos específicos para a avaliação do abuso emocional e de suas repercussões especialmente no Brasil. Esta dissertação consistiu em dois estudos: o primeiro foi uma revisão da literatura sobre as relações entre abuso emocional, autoestima, bem-estar subjetivo e estilos parentais percebidos. O segundo, teve como objetivo analisar as relações entre a memória de abuso emocional, autoestima, bem-estar subjetivo e verificar diferenças na incidência de memória de abuso emocional nos diferentes estilos parentais percebidos. Participaram 305 universitários (64,7% mulheres), de 17 a 62 anos (M= 21,6) de Porto Alegre. Os sujeitos responderam questionários para investigar: dados sócio-demograficos, abuso emocional, autoestima, afeto positivo e negativo, satisfação de vida e estilos parentais. Foram encontradas correlações negativas entre abuso emocional, autoestima e afeto positivo e satisfação de vida e correlações positivas entre abuso emocional, idade e afeto negativo. Os resultados também mostraram diferenças significativas entre as médias de memória de abuso emocional, autoestima e bem-estar subjetivo entre os diferentes estilos parentais. Os resultados do segundo estudo sugerem que os estilos parentais autoritário e negligente são prejudiciais ao desenvolvimento da autoestima, do bem-estar subjetivo e estão relacionados com memórias de abuso emocional. / The revision of literature showed a lack of specific instruments for the assessment of emotional abuse especially in Brazil. The present study consisted of two studies: the first one, a theoretical research about the relations between emotional abuse, self-esteem, subjective well-being and perceived parenting styles. The second study aimed to examine empirically the links between the memory of emotional abuse and self-esteem, subjective well-being and to verify the differences in the incidence of the memory of emotional abuse in the different perceived parenting styles. The participants consisted of 305 university students (64.7% women and 35.3% men), ranging from 17 to 62 years of age (M=21.6 years; SD=5.4), of Porto Alegre, Brazil. Subjects answered self-report questionnaires comprising demographics, emotional abuse in childhood, self-esteem, subjective well-being, and parenting styles. Negative correlations were found between emotional abuse and self-esteem, positive affect and life satisfaction. Positive correlations were found between emotional abuse, age and negative affect. Significant differences were found between the means of the memories of emotional abuse, self-esteem and subjective well-being between the different the parenting styles. The results shown in the second study suggest that the authoritarian and negligent parenting styles are harmful to the development of self-esteem, subjective well-being and are related with memories of emotional abuse.
