Induction and characterization of endotoxin tolerance in equine peripheral blood mononuclear cells in vitroFrellstedt, Linda 10 September 2010 (has links)
Endotoxemia is responsible for severe illness in horses. Individuals can become unresponsive to the endotoxin molecule after an initial exposure; this phenomenon has been called developing a state of "endotoxin tolerance" (ET). ET has been induced in horses in vivo; however, cytokine expression associated with ET has not been investigated. The purpose of this study was to develop and validate a method for inducing ET in equine peripheral blood mononuclear cells (PBMCs) in vitro, and to describe the cytokine profile which is associated with the ET.
Blood was collected from 6 healthy horses and PBMCs were isolated. ET was induced by culturing cells with three concentrations of endotoxin given to induce ET, and evaluated after a second dose of endotoxin given to challenge the cells. The relative mRNA expression of IL-10 and IL-12 was measured by use of quantitative PCR.
ET was induced in all cells (n=6) exposed to the 2-step endotoxin challenge. In PBMCs treated with 1.0 ng/ml of endotoxin followed by challenge with 10 ng/ml of endotoxin, the relative mRNA expression of IL-10 in tolerized cells was not different from positive control cells. In contrast, the relative mRNA expression of IL-12 in tolerized cells was decreased by 15-fold after the second endotoxin challenge compared with positive control cells.
This experiment demonstrated a reliable method for the ex vivo induction of ET in equine PBMCs. A marked suppression of IL-12 production is associated with ET. The production of IL-10 was not altered in ET in our model. / Master of Science

TNF is an inflammatory cytokine that plays a critical role in the acute phase response to infection, and its dysregulation has been implicated in the pathology of several inflammatory and autoimmune disorders. TNF gene expression is regulated in a cell type- and inducer-specific manner that involves chromatin alterations at both the TNF promoter and distal DNase I hypersensitive (DH) sites within the TNF/LT locus. While the mechanisms underlying TNF gene activation in monocytes/macrophages and T cells have been studied intensively, the mechanisms of enhanced, repressed, and restored TNF gene expression in the context of classical macrophage activation and endotoxin tolerance remain largely unknown. We set out to understand how TNF gene expression is modulated during these biological processes by characterizing the chromatin environment of the TNF/LT locus.

Within hours after its initiation, the severe systemic inflammatory response of sepsis shifts to an adaptive anti-inflammatory state with coincident immunosuppression. This anti-inflammatory phenotype is characterized by diminished proinflammatory cytokine gene expression in response to toll-like receptor (TLR) stimulation with bacterial endotoxin/lipopolysaccharide (LPS), also known as endotoxin tolerance/adaptation. Our and other studies have established that gene-specific reprogramming following TLR4 responses independently represses transcription and translation of proinflammatory genes such as tumor necrosis factor alpha (TNFα). We also previously demonstrated that TNFα and interleukin (IL)-6 mRNA translation is repressed in endotoxin-adapted THP-1 human monocytes by an miRNA-based mechanism involving the argonaute family protein argonaute 2 (Ago2). Here, we further define the molecular nature of reprogramming translation by showing that TLR4-induced microRNA-146 promotes a feed-forward loop that modifies the subcellular localization of the RNA-binding protein RBM4 (RNA-binding motif protein 4) and promotes its interaction with Ago2. This interaction results in the assembly of a translation-repressor complex that disrupts TNFα and IL-6 cytokine synthesis in endotoxin-adapted THP-1 monocytes. This novel molecular path prevents the phosphorylation of RBM4 on serine-309 by p38 MAPK (mitogen-activated protein kinase), which leads to RBM4 accumulation in the cytosol and interaction with Ago2. We further find that microRNA-146a knockdown by antagomirs or protein phosphatase inhibition by okadaic acid increases p38 MAPK phosphorylation and results in RBM4 serine-309 phosphorylation and nuclear relocalization, which disrupts RBM4 and Ago2 interactions and restores TLR4-dependent synthesis of TNFα and IL-6. We conclude that miR-146a has a diverse and critical role in limiting an excessive acute inflammatory reaction.

An anti-inflammatory phenotype with pronounced immunosuppression develops during sepsis, during which time neutrophils and monocytes/macrophages limit their Toll-like receptor 4 responses to bacterial lipopolysaccharide (LPS/endotoxin). We previously reported that during this endotoxin-tolerant state, distinct signaling pathways differentially repress transcription and translation of proinflammatory cytokines such as TNFα and IL-6. Sustained endotoxin tolerance contributes to sepsis mortality. While transcription repression requires chromatin modifications, a translational repressor complex of Argonaute 2 (Ago2) and RNA-binding motif protein 4 (RBM4), which bind the 3′-UTR of TNFα and IL-6 mRNA, limits protein synthesis. Here, we show that Dcp1 supports the assembly of the Ago2 and RBM4 repressor complex into cytoplasmic processing bodies (p-bodies) in endotoxin-tolerant THP-1 human monocytes following stimulation with LPS, resulting in translational repression and limiting protein synthesis. Importantly, this translocation process is reversed by Dcp1 knockdown, which restores TNFα and IL-6 protein levels. We also find this translational repression mechanism in primary macrophages of septic mice. Because p-body formation is a critical step in mRNA translation repression, we conclude that Dcp1 is a major component of the translational repression machinery of endotoxin tolerance and may contribute to sepsis outcome.

Endotoxin-tolerante Zellen zeigen über mehrere Tage eine verminderte Produktion pro-inflammatorischer Zytokine, sodass epigenetische Veränderungen ein Grund für die Endotoxintoleranz sein könnte. Im 1. Teil wurden epigenetische Veränderungen an gezielten LPS-tolerisierbaren Genen mithilfe eines in-vitro-Modells mit humanen Monozyten untersucht. Die Gene kodierend für TNF und CXCL10 zeigten eine Reduktion der transkriptionsaktivierenden Histonmarker H3K27ac und H4ac, die durch eine stark reduzierte Genexpression in toleranten Monozyten begleitet wurde. Demgegenüber wiesen Gene wie IL6 und IL1B eine Zunahme an H4ac und H3K27ac auf, während ihre Genexpression in widersprüchlicher Weise reduziert war. Repressive epigenetische Marker (H3K9me2, H3K27me3, H4K20me3, DNA-Methylierung) konnten in den untersuchten Genen nicht nachgewiesen werden. Zudem war die IL6-Genexpression verstärkt abhängig von der Signaltransduktion toleranter Monozyten, was auf unterschiedliche Repressionsmechanismen schließen lässt. Im 2. Teil konnte gezeigt werden, dass die genomweite transkriptionelle Reprogrammierung durch eine globale Verschiebung von aktiven H3K27ac und H4ac in naiven Monozyten zu repressiven H3K9me2, H3K27me3 und H4K20me3 in toleranten, restimulierten Zellen einherging. Mehr als 10000 Genombereiche wiesen Veränderungen an Histonmarkern auf, obwohl nur 3638 Gene unterschiedlich exprimiert waren. Circa 27% der differentiell exprimierten Gene zeigten ein Expressionsmuster, welches mit Veränderungen an aktiven und/oder repressiven Markern innerhalb der Promoterregion korrelierte. Zudem zeigten intergenische Regionen einen verstärkten Anstieg an repressiven Histonmarkern, was auf eine mögliche regulatorische Funktion dieser Bereiche in der Endotoxintoleranz schließen lässt. Die Studie zeigt, dass die Epigenetik der Monozyten stark von der Endotoxintoleranzinduktion betroffen ist, wenn auch nicht alle Veränderungen dem beobachteten Genexpressionsmuster zugeordnet werden konnten. / Endotoxin-tolerant cells show a reduced ability to produce pro-inflammatory cytokines for several days, which assumes an impact of epigenetic changes in endotoxin tolerance induction. Using an in vitro model with human monocytes, the first part focused on the analysis of epigenetic changes in specific LPS-tolerizable genes. The genes encoding for TNF and CXCL10 showed a reduction in the transcription-activating histone marks H3K27ac and H4ac in tolerant monocytes, which was accompanied by a strongly reduced gene expression. In contrast, the IL6 and IL1B genes showed an increase in activating histone modifications, while their gene expressions were moderately reduced. Particularly the IL6 gene expression was more susceptible to the signaling strength in tolerant monocytes implying distinct mechanisms in the repression of the genes analyzed. Within the second part, genome-wide reprogramming of tolerant monocytes was accompanied by a global shift from activating H3K27ac and H4ac in naive monocytes to repressive H3K9me2, H3K27me3 and H4K20me3 in tolerant cells treated with LPS. More than 10000 genomic regions were distinctly regulated by histone marks, though only 3638 genes were differentially expressed. Correlation analyses identified 27 % of the differentially expressed genes that showed a transcriptional level consistent with changes in activating and/or repressive histone marks within their promoter regions. Intergenic regions were highly enriched for repressive histone marks in LPS-tolerant monocytes implying a regulatory function in endotoxin tolerance. The data indicate that the epigenetic environment of monocytes is highly affected by endotoxin tolerance induction, though not all changes are directly linked to the gene expression pattern observed.

Le sepsis est défini par l’apparition de dysfonctions d’organes, multiples et mortelles, causées par une réponse de l’hôte dérégulée suite à une infection. L’hétérogénéité de la maladie représente un défi clinique majeur au regard de la prise en charge thérapeutique, et à ce jour les marqueurs proposés ne suffisent pas à stratifier les patients. Les rétrovirus endogènes humains (HERV) pourraient être des marqueurs pertinents,compte tenu des propriétés immunosuppressives de leurs enveloppes et de leur expression dans des maladies inflammatoires et auto-immunes. Cette thèse a pour objectif de savoir dans quelle mesure les HERV sont exprimés et modulés, dans des conditions d’inflammation et d’immunosuppression. Pour cela,nous avons utilisé une puce à ADN haute densité permettant (i) l’analyse de la transcription de 363 689HERV et 1500 gènes, et (ii) une lecture fonctionnelle de l’activité des LTR. L’expression des HERV a été objectivée (i) dans un modèle ex-vivo de tolérance à l’endotoxine sur des cellules mononuclées du sang périphérique (PBMC) d’individus sains et (ii) sur sang total provenant d’individus sains et de patients en choc septique, stratifiés ou non en fonction du statut immunitaire. (1) De 5,6% à 6,9% des HERV sont exprimés dans le compartiment sanguin et environ 20% des LTR possèdent une fonction promotrice ou polyA, les deux fonctions étant mutuellement exclusives. (2) Le contenu du transcriptome HERV est modulé ex vivo dans le contexte de tolérance à l’endotoxine laissant apparaitre deux grands phénotypes transcriptionnels. L’expression de certains loci HERV est corrélée au statut immunitaire de patient septique.L’évaluation d’une signature moléculaire complexe sur une cohorte de validation, permet la séparation en deux groupes présentant des critères de sévérité distincts, suggérant les HERV/MaLR comme biomarqueurs de stratification. Le choc septique reste une pathologie grave, associée à des taux de mortalité et d'infections nosocomiales (IN) secondaires élevés. La prédiction du pronostic est de la plus haute importance pour sélectionner les patients qui pourraient bénéficier de traitements visant à moduler la réponse immunitaire. Le système immunitaire, classiquement active par des agents externes, peut également être activé par des médiateurs endogènes exprimés à la suite d'une agression d'origine septique ou non. Les protéines S100 font partie de ces signaux de danger endogènes (ou alarmines). Le but de ce travail est d'évaluer la capacité de l'ARNm de S100A9 mesuré dans le sang total de patients en choc septique à prédire la survie et la survenue d'IN. Nous avons également étudié la régulation de l'expression des ARN messagers de S100A8 et S100A9 dans un modèle ex vivo de tolérance à l'endotoxine qui reproduit partiellement les dysfonctions de l'immunité innée induites par le sepsis. L'ARNm de S100A9 est surexprimé dans le sang des patients en choc septique. Un taux élevé entre le 7eme et le 10eme jour du début du choc septique est associé à la survenue d'IN secondaires. Ex vivo, l'expression des ARNm de S100A8 et S100A9 est augmentée durant le phénomène de tolérance à l'endotoxine. Le blocage de l IL-10 et l'administration d'IFN-γ réduisent l'augmentation de ces ARNm dans ce modèle. Apres confirmation dans des études cliniques, ces résultats préliminaires suggèrent que l'expression des ARNm de S100A8 et S100A9 puisse être utilisée comme marqueur du phénomène de tolérance à l'endotoxine et comme outils pour évaluer la dysfonction immunitaire des patients en choc septique. Ces patients pourraient alors bénéficier de thérapies visant à restaurer leurs fonctions immunitaires / Septic shock remains a serious disease with high mortality and increased risk of hospital-acquired infection. The prediction of outcome is of the utmost importance for selecting patients for therapeutic strategies aiming to modify the immune response. Immune system, typically activated by external agents, can also be activated by endogenous mediators induced by various types of stress (trauma, infection, burns). S100 proteins are part of the alarmins family. The aim of this study was to assess the capability of S100A9 messenger RNA in whole blood from patients with septic shock to predict survival and the occurrence of hospital-acquired infection. We also investigate the regulation of S100A8 and S100A9 mRNA expressions in an ex vivo model of endotoxin tolerance which partially reproduces sepsis-induced innate immune alterations. S100A9 messenger RNA is increased in septic shock and its delayed overexpression is associated with the occurrence of secondary hospital-acquired infection. Ex vivo, S100A8 and S100A9 mRNA expressions are increased during endotoxin tolerance. IL-10 blockade and rIFN-γ treatment partially abrogated S100A8/A9 mRNA increases in this model. Pending confirmation in larger, independent clinical studies, these preliminary results suggest that S100A8 and S100A9 mRNA levels might be used as surrogate markers of endotoxin tolerance and as evaluation tools for immune dysfunctions in septic shock patients. These patients could be selected for therapeutic aiming to restore immune functions
