Caracterização epidemiológica da brucelose bovina no Estado do Espírito Santo / Epidemiological characterization of bovine brucellosis in Espírito Santo State, Southeast region of Brazil

Foi conduzido um inquérito soroepidemiológico da brucelose bovina no Estado do Espírito Santo através de parceria firmada entre o Ministério da Agricultura, Pecuária e Abastecimento (MAPA), o Instituto de Defesa Agropecuária e Florestal do Estado do Espírito Santo (IDAF-ES) e o Departamento de Medicina Veterinária Preventiva e Saúde Animal (VPS) da Faculdade de Medicina Veterinária e Zootecnia (FMVZ) da Universidade de São Paulo (USP). O Estado foi dividido em dois estratos amostrais, conforme o tipo de exploração predominante e a capacidade operacional do IDAF-ES para a condução do trabalho de campo. A amostragem foi delineada para a determinação da prevalência de propriedades positivas (focos) e de animais soropositivos para a brucelose bovina por estrato amostral. Foi realizada uma seleção aleatória de 300 propriedades (unidades primárias) por estrato amostral, e dentro das unidades primárias, foram amostradas, aleatoriamente, 10 fêmeas bovinas com idade igual ou superior a 24 meses (unidades secundárias) quando o rebanho foi constituído por até 99 fêmeas da mesma faixa etária, ou todas as fêmeas existentes nessa faixa etária se não totalizassem 10 animais; quando o rebanho foi constituído por mais de 99 fêmeas com idade igual ou superior a 24 meses, foram amostradas 15 fêmeas da mesma faixa etária. Ao todo foi colhido sangue de 5370 fêmeas bovinas provenientes de 636 propriedades distribuídas nos dois estratos amostrais. Na ocasião da colheita, foi aplicado um questionário epidemiológico por propriedade e as coordenadas geográficas foram obtidas com um aparelho de GPS. Para o diagnóstico sorológico da brucelose bovina, foi utilizado o teste do Antígeno Acidificado Tamponado (AAT) como prova de triagem e o teste do 2-mercaptoetanol como prova confirmatória. Uma propriedade foi considerada foco quando apresentou pelo menos um animal soropositivo. As análises realizadas foram: (a) determinação das prevalências de focos e de animais soropositivos; (b) identificação de fatores de risco para a brucelose bovina; e (c) análise de agrupamentos espaciais de focos. As prevalências de focos de brucelose bovina e de animais soropositivos no Estado do Espírito Santo foram de 9,00% [6,97% - 11,55%] e 3,53% [1,93% - 6,37%], respectivamente. No estrato amostral 1, as prevalências de focos e de animais soropositivos foram de 6,80% [4,47% - 10,21%] e 3,43% [1,33% - 8,57%], respectivamente. No estrato amostral 2, a prevalência de focos foi de 10,86% [7,86% - 14,84%] e a prevalência de animais soropositivos foi de 3,69% [2,13% - 6,33%]. Os fatores de risco para a brucelose bovina no Estado foram utilização de inseminação artificial (OR = 7,05) e confinamento/semi-confinamento dos animais (OR = 2,98). No estrato amostral 1, as propriedades com mais de 15 fêmeas com idade ≥ 24 meses (OR = 5,18), que alugam pasto (OR = 2,85) e que utilizam inseminação artificial (OR = 6,76) possuíram maior chance de serem focos. No estrato amostral 2, a existência de mais de 42 bovinos no rebanho (OR = 5,31), utilização de inseminação artificial (OR = 11,72), compra de reprodutores em exposição (OR = 16,7) e confinamento/semi-confinamento dos animais (OR = 2,99) foram os fatores mais associados com a presença da doença. A vacinação de fêmeas entre três e oito meses de idade foi um fator protetor contra a doença nos dois estratos amostrais e em todo o Estado. Não houve tendência de formação de agrupamentos espaciais de focos em relação às propriedades livres. / As result of a partnership joining the Ministry of Agriculture, Livestock and Food Supply (MAPA), the Animal and Plant Health Institute of the State of Espírito Santo (IDAF-ES) and the Department of Preventive Veterinary Medicine and Animal Health (VPS) of the Faculty of Veterinary Medicine and Zootechny (FMVZ) of the University of São Paulo (USP), a seroepidemiological survey of bovine brucellosis was conducted in State of Espírito Santo, Southeast region of Brazil. The State was stratified in two sampling strata according to the predominant farm enterprise and the operational capacity of the IDAF-ES to conducting the field work. The sampling was delineated for the determination of the prevalence of positive herds and seropositive animals for bovine brucellosis per sampling stratum. It was made a random selection of 300 herds (primary units) per sampling stratum, and inside the primary units, if the herd was constituted by up to 99 cows over 24 months of age, 10 cows of the same age (secondary units) were randomly sampled, or all existing cows if they did not totalize 10 cows; if the herd was constituted by more than 99 cows over 24 months of age, 15 cows of the same age were randomly sampled. A total of 5370 blood samples of cows from 636 herds distributed in the sampling strata were collected. On the occasion of the blood collection, an epidemiological questionnaire was applied in each herd and the geographical coordinates were obtained with a GPS. For the serological diagnosis, the Rose Bengal Test was applied as a screening method and the 2-mercaptoetanol test as a confirmatory method. Herds with at least one test-positive animal were considered positive. The following analyses were carried out: (a) determination of the prevalence of positive herds and eropositive animals; (b) identification of risk factors for bovine brucellosis; and (c) spatial clustering analysis of positive herds. The prevalence of positive herds and seropositive animals in the State of Espírito Santo were 9.00% [6.97% - 11.55%] and 3.53% [1.93% - 6.37%], respectively. In sampling stratum 1, the prevalence of positive herds and seropositive animals were 6.80% [4.47% - 10.21%] and 3.43% [1.33% - 8.57%], respectively. In sampling stratum 2, the prevalence of positive herds was 10.86% [7.86% - 14.84%] and the prevalence of seropositive animals was 3.69% [2.13% - 6.33%]. The risk factors for bovine brucellosis in State were utilization of artificial insemination (OR = 7.05) and intensive/semi-intensive management systems (OR = 2.98). In sampling stratum 1, herds with more than 15 cows over 24 months of age (OR = 5.18), which rent pasturage (OR = 2.85) and that use artificial insemination (OR = 6.76) presented larger chance of being positive. The risk factors for bovine brucellosis in sampling stratum 2 were: existence of more than 42 bovine in the herd (OR = 5.31), utilization of artificial insemination (OR = 11.72), purchase of bovine reproducers in exhibitions (OR = 16.7) and intensive/semi-intensive management systems (OR = 2.99). The vaccination of heifers between three and eight months of age was a protective factor against the disease in the sampling strata and in State. There was no tendency for spatial clustering of positive herds.

O presente trabalho é resultante de uma parceria estabelecida entre o Ministério da Agricultura, Pecuária e Abastecimento (MAPA), a Coordenadoria de Defesa Agropecuária do Estado de São Paulo (CDA) e o Departamento de Medicina Veterinária Preventiva e Saúde Animal da Faculdade de Medicina Veterinária e Zootecnia da Universidade de São Paulo (VPS-FMVZ-USP). O Estado de São Paulo foi estratificado em sete circuitos produtores de bovinos e 150 rebanhos com atividade reprodutiva foram aleatóriamente selecionados em cada um deles. Em cada rebanho foi aplicado um questionário epidemiológico, foram aletoriamente coletadas amostras de soro de 10 ou 15 fêmeas com idade maior ou igual a 24 meses (em rebanhos com < 100 fêmeas, e rebanhos &ge; 100 fêmeas, respectivamente) e as coordenadas geográficas foram obtidas com um aparelho GPS. Os soros foram submetidos a um protocolo de testes em série, tendo o Rosa Bengala como método de triagem e a fixação de complemento como método confirmatório. A prevalência estimada de rebanhos com, ao menos, um animal soropositivo foi de 9,7% [7,8%; 11,6%], enquanto que a prevalência estimada de animais soropositivos foi 3,8% [0,7%; 6,9%], no Estado de São Paulo. Foi elaborado um mapa temático georreferenciado do Estado de São Paulo, no qual evidenciou-se a ausência de agrupamentos espaciais de rebanhos infectados. A análise também procurou medir o grau de associação das características de criação com a presença da doença, sendo que propriedades com 87 ou mais bovinos (OR = 2,25) e compra de reprodutores (OR = 1,56) foram as mais associadas. O modelo matemático proposto nesse trabalho mostrou que deverá ser intensificado o esforço para a vacinação de fêmeas e que, mesmo assim, os benefícios advindos da adoção desta medida, em termos de queda da prevalência da doença e diminuição do número de abortos, só se darão em prazos que podem alcançar décadas. / The present work is the result of a partnership established by the Ministry of Agriculture, Livestock and Food Supply (MAPA), the Animal and Plant Health Service of the State of São Paulo (CDA) and the Department of Preventive Veterinary Medicine and Animal Health of the University of São Paulo (VPS-FMVZ-USP). The State of São Paulo was stratified in seven cattle production regions and 150 herds with reproductive activity were randomly sampled within each one. In each herd was applied an epidemiological questionnaire, 10 or 15 cows over 24 months of age (on herds with < 100 cows, and herds &ge; 100 cows, respectively) were bled at random and the geographical coordinates were recorded with a GPS. Sera were tested using a serial test-ing procedure with the Rose Bengal Test as the screening method and the Complement Fixation as the con-firmatory method. The estimated prevalence of herds with at least one test-positive animal was 9.7% [7.8%; 11.6%], whereas the estimated prevalence of test-positive cattle was 3.8% [0.7%; 6.9%]. It was developed a georeferenced thematic map of the state of São Paulo, which revealed the absence of spatial clusters of infected herds. The analysis looked into the likely association of herd characteristics with the presence of the infection. The most associated characteristics were farms with more than 87 bovines (OR = 2.25) and reproduction animals reposition (OR = 1.56). A mathematical model was proposed in the present work, in wich the female vaccination effort was proposed to be intensificated, but according to the same model, the benefits of this measure will only be noticed, in terms of brucellosis prevalence and number of abortions decrease, in a matter of decades.

Epidemiology and control of human African trypanosomiasis in Uganda

Poverty and disease are bound together in rural communities of sub-Saharan Africa (SSA) exacerbated by weak social services and conflict. The infectious disease burden in SSA combines the neglected tropical diseases (NTDs) and the 'big three' (malaria, HIV/AIDS and tuberculosis), so-called because they attract more global attention and hence funding. NTDs include human African trypanosomiasis (HAT or sleeping sickness), first noticed by the outside world during the slave trade era and later in the 2-th century by widespread epidemics of disease across the tsetse fly belt. HAT describes two diseases: i) Gambian HAT caused by Trypanosoma brucei gambiense is characteristically chronic with an infectious period lasting up to three years and ii) Rhodesian HAT caused by T.b. rhodesiense is an acute disease, killing its victim within weeks of infection. The two diseases are frequently considered together as both are transmitted by tsetse flies, the parasites are morphologically indistinguishable and the associated diseases are both fatal if left untreated. However, the two diseases are clinical, epidemiologically and geographical distinct, each requiring different control strategies. Under field conditions, where microscopy is the basic diagnostic tool, differentiation is simply by geographical location of the patient; the Great Rift Valley separates the Gambian disease present in West and Central Africa, from East and southern Africa's Rhodesian disease. Control strategies are also distinct; while the Belgian and French colonial strategies to control the disease were patient-centred, the British colonial powers in East Africa were motivated by the effect of tsetse borne diseases on animal health. Towards the end of the colonial ear, both types of disease were heading for elimination but during the immediate post-colonial era in the 1960s, political instability compromised the rigid HAT control programs that had been put in place. For zoonotic Rhodesian sleeping sickness, complex tsetse control programmes proved difficult to maintain and to justify economically; for Gambian sleeping sickness the generalised breakdown of medical services allowed the disease to return, sometimes to devastating levels. The millennium development goals (MDGs) set out in 2000, highlighted specific challenges and opportunities for national and global development. HAT impacts national health goals of national development plans and MDGs and impedes rural development of SSA. NTDs were not addressed directly by MDGs but the World Health Organization (WHO) has reaffirmed its commitment not only to control of HAT but also to eliminate it as a public health problem by 2020. Currently there are 25 countries reporting HAT to WHO, and while the overall prevalence of HAT across Africa continues to fall, epidemics have been recorded, particularly from central Africa, South Sudan and Uganda. Uganda is uniquely, the only country affected by both T.b. gambiense and T.b. rhodesiense and until the present study, there was no evidence to suggest that the two parasite species co-existed in Uganda. The development of a new control paradigm for T.b. rhodesiese in South East Uganda has lowered the incidence of human infections and, more importantly, halted the northerly spread of this parasite. However, recurring epidemics in several established and new disease foci in central Uganda highlight the difficulties involved in eliminating this disease. The present study assesses past and present HAT control strategies centred on Dokolo, Kaberamaido and Soroti Districts located at the centre of Uganda. These districts are highly endemic for T.b. rodesiense, they represent the region of concern for overlap with T.b. gambiense foci in central Uganda, and are the current focus of the Stamp out Sleeping sickness control initiative. The point prevalence of T. brucei s.1 in cattle reservoir from villages with (out) reported human disease located at specific distances to Otuboi, Chagwere and Ochero cattle markets, was evaluated before and six months after trypanocidal treatment, to assess the transferrable impact of zoonotic T.b. rhodesiense to the human population. Overall, the proportion of T. brucei s.1 in cattle dropped significantly from 22% at baseline to 9% six months after trypanocide treatment (P < 0.05, Chi-square + 17.92, 95% C.I. + 1.71 to 4.49). All villages located in sub-counties that received at least 80% treatment coverage had a drop in T. brucei s.1 prevalence from 30.4% (95%, C.I + 22.8 to 38.0) before treatment was done, to 12.9% (95%, C.I. + 7.4 to 18.4) six months after treatment. More specifically, impact on human infective T.b. rhodesiense was also halved. In fact only three cattle were detected with the parasite six months after treatment compared with six from those sampled as baseline. This study also utilises documented cases between 2009 and 2012 to assess the current HAT reporting system for monitoring and evaluating transmission dynamics of the disease. Using a questionnaire, capacity and preparedness of healthcare professionals to respond to disease epidemics was assessed. The point prevalence of sleeping sickness in the three districts in 2009 was determined by screening volunteers. Microscopic examinations detected trypanosomes in four volunteers (4/5311 or 0.075 %) while PCR detected significantly more infections (24, p < 0.001). Multiplex PCR showed that ten of the Trypanozoon infections were T.b. rhodesiense while nested PCR identified four infections as T.b. gamiense, indicating that the distribution of the two forms of sleeping sickness overlaps in Uganda. Second phase investigations followed up the PCR positive cases; these people were screened again, together with members of their homestead and the inhabitants of three neighbouring homes. Besides microscopy and PCR, study subjects were examined clinically for sleeping sickness and completed a questionnaire to assess community recognition of the disease. This extended screen revealed no new cases underlining the importance of stringent early screening that PCR techniques can provide. At local healthcare centres, 54% of reported sleeping sickness cases were diagnosed only at the late stage, indicating a weakness in early diagnosis and hence early reporting. Interviews with local health workers also revealed weaknesses in recognition of clinical signs and a gap in diagnostic capacity. While records at treating hospitals remain a useful indicator for targeting active foci of infection, improvement in capacity to diagnose HAT at an early stage should contribute both to rural health and disease control strategies and also towards WHO's 2020 target of elimination of HAT.

O modelo matemático proposto nesse trabalho mostrou que deverá ser intensificado o esforço para a vacinação de fêmeas e que, mesmo assim, os benefícios advindos da adoção desta medida, em termos de queda da prevalência da doença e diminuição do número de abortos, só se darão em prazos que podem alcançar décadas. / The present work is the result of a partnership established by the Ministry of Agriculture, Livestock and Food Supply (MAPA), the Animal and Plant Health Service of the State of São Paulo (CDA) and the Department of Preventive Veterinary Medicine and Animal Health of the University of São Paulo (VPS-FMVZ-USP). The State of São Paulo was stratified in seven cattle production regions and 150 herds with reproductive activity were randomly sampled within each one. In each herd was applied an epidemiological questionnaire, 10 or 15 cows over 24 months of age (on herds with < 100 cows, and herds &ge; 100 cows, respectively) were bled at random and the geographical coordinates were recorded with a GPS. Sera were tested using a serial test-ing procedure with the Rose Bengal Test as the screening method and the Complement Fixation as the con-firmatory method. The estimated prevalence of herds with at least one test-positive animal was 9.7% [7.8%; 11.6%], whereas the estimated prevalence of test-positive cattle was 3.8% [0.7%; 6.9%]. It was developed a georeferenced thematic map of the state of São Paulo, which revealed the absence of spatial clusters of infected herds. The analysis looked into the likely association of herd characteristics with the presence of the infection. The most associated characteristics were farms with more than 87 bovines (OR = 2.25) and reproduction animals reposition (OR = 1.56). A mathematical model was proposed in the present work, in wich the female vaccination effort was proposed to be intensificated, but according to the same model, the benefits of this measure will only be noticed, in terms of brucellosis prevalence and number of abortions decrease, in a matter of decades.

Nessa região, onde são freqüentes os contatos dos habitantes com coleções de água poluídas com fezes humanas e colonizadas por Biomphalaria tenagophila, notificam-se casos de esquistossomose desde meados do século passado. O controle da transmissão de Schistosoma mansoni se intensificou após o desenvolvimento de um plano de controle específico para Bananal, que durou de 1998 a 2002, projetado para reduzir a prevalência da doença a 1 %. Na vigência deste, observou-se perda de eficácia dos exames de fezes na busca ativa de casos para tratamento. Para a redução dos riscos resultantes da permanência do número de casos dados como falsos negativos e obtenção de dados epidemiológicos mais detalhados, buscou-se o aprimoramento dos resultados de laboratório com a realização de um inquérito soro lógico com a reação de imunofluorescência indireta (RIFI-IgM) quantitativa. O trabalho permitiu a confirmação de casos não detectados no inquérito coproscópico precedente, a partir da triagem dos resultados soro-reagentes encontrados no Bairro Palha. A curva bimodal da distribuição dos títulos soro lógicos caracterizou a baixa endemicidade. Na primeira fase do estudo, 452 exames soro lógicos revelaram129 soro-reagentes (28,5%). Destes, colheram-se 97 amostras de fezes que permitiram a identificação de 11 casos (11,3%) não detectados nos inquéritos coproscópicos. Na segunda fase do trabalho, de um total de 674 casos do município notificados e tratados no período de 1994 a 2002, foram examinadas 230 amostras de sangue (34,13%), dentre as quais 201 resultaram positivas. Determinaram-se os parâmetros da técnica RIFI-IgM utilizada: sensibilidade, 87,4%; especificidade, 98,7%; valores de predição positivo e negativo, respectivamente 98,5% e 88,7% ; título de corte igual a 8, avaliado em confronto com amostras de área controle negativas para esquistossomose, pareadas por idade e sexo. Nos grupos de até cinco anos observaram-se os mais altos valores de médias geométricas de títulos: 446; entre seis e dez, 142; nos maiores de 49 anos, 81. Não houve associação entre títulos sorológicos e intensidades de infecção estimadas em termos de ovos por grama de fezes (opg). O levantamento epidemiológico indicou redução da prevalência e da intensidade da infecção, coincidentes com a ampliação da cobertura dos inquéritos coproscópicos e melhoria das condições de saneamento básico. A RIFI-IgM mostrou-se adequada ao aprimoramento do controle da esquistossomose em Bananal e para o uso em outras áreas de baixa prevalência, o que contribui na redução dos níveis de contaminação ambiental por ovos do parasito e dos riscos de contato da população com as coleções hídricas / Abstract: The municipality ofBananal is located in the area ofthe Valley ofthe Paraíba do Sul river where schistosomiasis mansoni is endemic. Since the middle seventies of the past century, the local public health authorities have been notified of the occurrence of Schistosoma mansoni infection in this region, where people come tTequent1y in contact with bodies of tTesh water polluted with human feces and infested with Biomphalaria tenagophila. During the period between the years 1998 and 2002, a new plan was put into effect in Bananal with the purpose of eventually reducing the prevalence of schistosomiasis to 1 %. As the action proceeded, the use of parasitological examinations to detect new cases of schistosomiasis to be treated was obviously not efficient enough. Aiming at reducing the proportion of false negative results, the indirect fluorescent antibody test (IF AT -IgM) was then adopted as a subsidiary to egg identification techniques and as. a source of seroepidemiological data. A bimodal distribution curve of serological titers, characteristic of low endemicity, was found. Seropositive cases were matched with parasitological examination results from previous yearly surveys. During the first stage of investigation, from 452 serological examinations, 129 (28.5%) seropositive cases carne out. Fecal samples tTom 97 ofthese subjects inc1uded 11 parasitologically positive cases (11.3%) which had escaped identification during the previous yearly surveys. During the second stage of our investigation, tTom a total of 674 cases previously notified and treated in the 1994 - 2002 period, we processed 230 blood samples (34,1%), ofwhich 201 seropositives carne out. The IF AT -IgM parameters calculated were: sensitivity, 87.4%; specificity, 98.7 %; Positive and negative predicted values, 98.5% and 88.7% respectively; estimated cut off leveI equal to 8, obtained by matching samples from negative control areas and pairing them according to age and sexo Subjects aged five years or less had the highest mean titers: 446; subjects in the 6-10 age group: 142; subjects aged more than 49 years: 81. No association was observed between serological titers and intensity of infection as estimated in terms of eggs per gram offeces (epg). Epidemiological investigation point to a decrease in both prevalence and intensity of infection, coincident with comprehensive parasitological surveys and the inception of basic sanitation measures. IFAT-IgM was found to have been adequate to produce epidemiological data pertinent to the control of schistosomiasis in such low prevalence conditions as those found in Bananal / Doutorado / Doutor em Parasitologia

Um inquérito soroepidemiológico foi conduzido para a pesquisa de anticorpos anti-Chlamydophila abortus, anti-Toxoplasma gondii, anti-Neospora caninum e anti-Brucella abortus em caprinos leiteiros do semiárido da Paraíba. A amostragem foi delineada para a determinação da prevalência de propriedades positivas (focos) e de animais soropositivos. Foi realizada uma seleção aleatória de propriedades (unidades primárias) e, dentro dessas, foi escolhido, de forma aleatória, um número pré-estabelecido de fêmeas caprinas (unidades secundárias). Ao todo foi colhido sangue de 974 fêmeas caprinas provenientes de 110 propriedades. Na ocasião da colheita, foi aplicado um questionário epidemiológico por propriedade e as coordenadas geográficas foram obtidas com um aparelho de GPS. Para o diagnóstico sorológico das infecções por C. abortus e B. abortus, foram utilizados os testes de Fixação de Complemento e do Antígeno Acidificado Tamponado (AAT), respectivamente. Para o diagnóstico das infecções por T. gondii e N. caninum foram utilizadas reações de imunofluorescência indireta (RIFI). Uma propriedade foi considerada foco quando apresentou pelo menos um animal soropositivo. As análises realizadas foram: determinação das prevalências de focos e de animais soropositivos e identificação de fatores de risco. As prevalências de focos de C. abortus, T. gondii, N. caninum e B. abortus foram de 50,0% [40,3% - 59,7%], 70,0% [60,5% - 78,4%], 16,4% [10,0% - 24,6%] e 0,0% [0,0% - 3,3%], respectivamente. As prevalências de animais soropositivos para C. abortus, T. gondii, N. caninum e B. abortus foram de 11,9% [8,3% - 16,8%], 18,1% [14,3% - 22,6%], 1,9% [1,0% - 3,4%] e 0,0% [0,0% - 3,8%], respectivamente. Os fatores de risco para a infecção por C. abortus foram: compartilhar reprodutores e/ou sêmen (odds ratio = 2,35) e histórico de abortamentos (odds ratio = 3,06). Presença de plantas tóxicas na propriedade (odds ratio = 5,11) e a caprinocultura não ser a principal atividade (odds ratio = 3,34) foram apontadas como fatores de risco para a ocorrência de focos de infecção por T. gondii. Para a infecção por N. caninum, os fatores de risco associados à ocorrência de focos foram: alugar pastos (odds ratio = 10,34), não usar seringas descartáveis (odds ratio = 3,78) e possuir 25 ou mais caprinos (odds ratio = 6,51). Sugere-se que esforços sejam concentrados na condução de medidas de prevenção e controle das infecções por C. abortus, T. gondii e N. caninum em caprinos leiteiros do semiárido da Paraíba, especialmente aquelas relacionadas à correção de fatores de risco, visando a redução da prevalência e da disseminação desses agentes nesses animais e a possível exposição de seres humanos. / A seroepidemiological study for anti-Chlamydophila abortus, anti-Toxoplasma gondii, anti-Neospora caninum and anti-Brucella abortus antibody research was carried out in dairy goats from the semiarid of Paraíba state. A total of 975 serum samples from 110 flocks were collected. On the occasion of the blood collection, an epidemiological questionnaire was applied in each flock and the geographical coordinates were obtained with a GPS. For the serological diagnosis of C. abortus and B. abortus infections complement fixation test and Rose Bengal test (RBT) were used, respectively. Indirect fluorescent antibody tests (IFAT) were carried out for detecting anti-T. gondii and anti-N. caninum antibodies. A flock was considered focus when presented at least one seropositive animal. The following analyses were performed: determination of foci and seropositive animals prevalences and identification of risk factors. Foci prevalences of C. abortus, T. gondii, N. caninum and B. abortus were 50.0% [40.3% - 59.7%], 70.0% [60.5% - 78.4%], 16.4% [10.0% - 24.6%] and 0.0% [0.0% - 3.3%], respectively. Seropositive animals prevalences to C. abortus, T. gondii, N. caninum and B. abortus were 11.9% [8.3% - 16.8%], 18.1% [14.3% - 22.6%], 1.9% [1.0% - 3.4%] and 0.0% [0.0% - 3.8%], respectively. Risk factors associated with C. abortus infection were: lend buck and/or semen (odds ratio = 2.35) and history of abortions (odds ratio = 3.06). Presence of toxic plants in the property (odds ratio = 5.11) and goat production do not be the main activity in the property (odds ratio = 3.34) were identified as risk factors for the occurrence of foci of T. gondii infection. For the N. caninum infection risk factors associated with occurrence of foci were: share pasture (odds ratio = 10.34), do not use disposable syringes (odds ratio = 3.78) and flock size 25 goats (odds ratio = 6.51). It was suggested that efforts be concentrated on the application of control and prevention measures for C. abortus, T. gondii and N. caninum infections in dairy goats from the semiarid of the Paraíba state, especially those related to the correction of risk factors, aiming to reduce the prevalence and spread of these agents and possible exposure of humans.

Prevenção de incapacidades em hanseniase, no ambulatorio regional de especialidades de Taubate de 1990 a 1999

No. of bitstreams: 1 Vieira_CarmenSilviadeCamposAlmeida_M.pdf: 39334524 bytes, checksum: 444cb8675ea2176af0bb043ff9c91960 (MD5) Previous issue date: 2001 / Resumo: Trata-se de análise retrospectiva do atendimento no Programa de Controle da Hanseníase, com ênfase na Prevenção de Incapacidades (PI), na década de 90 aprofundando conhecimentos sobre a história da atenção à hanseníase e a organização dos serviços, com base nos indicadores epidemiológicos e operacionais, em algumas ações de Controle do Programa, na caracterização sociodemográfica da população, bem como na avaliação do grau de incapacidades, conforme padronização do Programa de Controle da Hanseníase no diagnóstico e na alta do doente. Desenvolveu-se no Ambulatório Regional de Especialidades de Taubaté (ARE), da Secretaria de Estado da Saúde de São Paulo, levantamento e análise de 408 prontuários, fichas epidemiológicas e de PI, correspondendo a 100% dos casos novos inscritos no Programa e em Poliquimioterapia. Utilizaram-se Tabelas e Figuras para descrever o perfil do grupo estudado. Os dados epidemiológicos no ARE Taubaté, na década de 90, demonstraram coeficientes de prevalência de 3,1% por 10.000 habitantes e de detecção de 0,9% por 10.000 habitantes em 1999; portanto, a endemia está presente, com muitos diagnósticos de casos novos. Os dados operacionais mostraram que se consegue avaliar as incapacidades físicas na maioria dos casos novos. A maior ocorrência foi das formas polarizadas: Virchoviana (43,5%), Tuberculóide (25,5%) e Dimorfa (23,3%). Na caracterização da população estudada, conclui-se que a hanseníase ocorreu, com maior freqüência, na faixa etária de 30 a 49 anos (48,2%), no sexo masculino (60%), e em moradores da região baixa do município (48,6%). Quanto à escolaridade, predominou o ensino fundamental incompleto (57,7%). O analfabetismo está mais presente entre as formas avançadas (15,2%) e, à medida que aumenta a escolaridade (nível superior), a forma indeterminada é mais freqüente. Observa-se que as reações hansênicas ocorrem com maior incidência entre as mulheres (63%). A avaliação do grau de incapacidades no diagnóstico apresentou maior percentual na forma Indeterminada, sem incapacidades instalada (63,3%), enquanto nos graus 2 e 3 o percentual maior é na forma Virchoviana (34%). Constata-se que o maior percentual de incapacidades graus 2 e 3 ocorreram nos pés (14,8%), seguido das mãos (13,2%) e olhos (7,6%), o que sinaliza a importância de valorizar e implementar as ações de PI nos membros inferiores e superiores. Quanto à recuperação dos doentes com grau 1 (anestesia) no diagnóstico, (46,3%) recuperaram para grau O e (48,4%) mantiveram-se no grau 1, sem evoluir para incapacidades mais severas (deformidades). Nos doentes com grau 2 e 3 de incapacidades no diagnóstico, verifica-se percentual maior de manutenção nesses graus (62,8% e 40% respectivamente), o que demonstra a dificuldade de recuperar as incapacidades mais avançadas. Quanto ao percentual de piora, para os doentes diagnosticados no grau 1, e no 2, foi baixa, com um percentual de 7,2%. Portanto, não são todos os doentes que conseguiram recuperar-se, o que demonstra a complexidade das Ações da Prevenção de Incapacidades, pois a mesma é fruto de vários fatores, tais como: interação entre o doente e o profissional, incorporação do autocuidado, maior escolarização, efetivação das Ações de Controle da Hanseníase de forma global, com descentralização das ações de PI e avaliação continuada / Abstract: The current study aims at a retrospective analysis of the assistance in the Leprosy Control Program, emphasising the Prevention of Disabilities(DI) in the 90's, searching deeply the knowledge of history with attention to leprosy and organisation of the service, based on the epidemiological and operational indicators, on the actions of the Control Programme, on the socio-demographic characterisation of the population as well as on the evaluation of the degree of disabilities according to the Standardisation of the Leprosy Program, in the diagnosis and discharge of the patient. The research has been carried out in the Outpatient Department of Specialisation of Taubaté (ODS), of the State Health Office of São Paulo State, by means of survey and analysis of 408 handbooks, epidemiological and DI cards, corresponding to 100% new cases enrolled in the Programme and in Polichemiotherapy. Tables and figures have been used to describe the profile ofthe group studied. The epidemiological data in the ODS Taubaté during the 90's, showed coefficients of prevalence of 3,1 per 10,000 inhabitants and of detection of 0,9 per 10,000 inhabitants, therefore, the endemic condition is present, with many diagnoses of new cases. The operational data showed that it is possible to assess the physical disabilities in the majority of the new cases. The biggest incidence was of the polarised forms: Virchoviana (43,5%), Tuberculóide (25,5%) and Dimorfa (23,3%). While characterising the population studied, it has be concluded that leprosy occurred more often in the age group between 30 to 39 years old (48,2%) in males (60%) , belonging the lower region of the town (48,6%). In what concerns schooling, the incomplete secondary level prevailed (57,7%). Illiteracy is mainly present in the advanced forms (15,2%), and as the schooling level improves (college); the indeterminate form is more frequent. Leprosy reactions have been found mainly in women (63%). The assessment of the degree of disability in the diagnosis showed higher percentage in the Indeterminate form with no disability installed (63,3%), whereas in the levels 2 and 3 the higher percentage is in the Virchoviana form (34%). It has been seen that the highest percentage of disabilities of levels 2 and 3 occurred in feet (14,8%), followed by hand (13,2%) and eyes (76%), thus showing the importance of valuing and implementing the actions of DI in lower and higher limbs. Concerning the recovery of patients with level 1 disabilities (anaesthesia) in the diagnosis, (46,3%) recovered to leveI O and (48,4%) stayed in level 1, without developing more severe disabilities (deformities). With regard to patients with levels 1 and 2 in the diagnosis, a higher percentage of maintenance in these levels, (62,8% and 40% respectively) was observed, what shows the difficulty of recovery in the more advanced forms. For patients diagnosed with levels 1 and 2, the percentage of worsening was low (7,2%). Therefore not all patients were able to recover.
