Etablierung eines isoliert druckkonstant perfundierten Ex-vivo-Modells des equinen Larynx

Der Einsatz von Ex-vivo-Modellen ermöglicht die experimentelle Untersuchung isolierter Organe, die vom Gesamtorganismus unbeeinflusst sind. Für viele Spezies und Organe sind solche Modelle bereits beschrieben. Wird das Organ dabei perfundiert, stehen flusskonstante und druckkonstante Perfusion zur Auswahl. Für den Larynx des Pferdes existiert in der Literatur bisher nur ein einziges flusskonstant perfundiertes Modell. Im Bereich der Pferdemedizin können solche Modelle helfen, neue Behandlungsoptionen der sogenannten recurrent laryngeal neuropathy (RLN) zu entwickeln, bei der es zu einer Atrophie des Musculus cricoarytaenoideus dorsalis (CAD) kommt. Die Studie hatte zum Ziel, ein druckkonstant perfundiertes Ex-vivo-Modell des equinen Larynx zu etablieren, in dessen Fokus insbesondere die Funktionalität des CAD stand. Hierzu wurden in zwei Vorversuchsphasen verschiedene Parameter auf ihre Eignung als Marker für die Vitalität des entnommenen Kehlkopfes getestet. Die ermittelten Parameter wurden dann in den Hauptversuchen verwendet, um die Vitalität des perfundierten Kehlkopfes zu Beginn und am Ende der Perfusion zu untersuchen. Für die Untersuchungen wurden Kehlköpfe von 16 Pferden verwendet. Diese wurden im direkten Anschluss an die Euthanasie frisch entnommen und im Labor an einen Perfusionskreislauf angeschlossen. Hierzu wurde eine modifizierte Tyrode-Lösung beidseits über die Arteria thyroidea cranialis perfundiert. Die Lösung wurde über den gesamten Versuchszeitraum mit Carbogen begast und somit ein pH-Wert von 7,35 bis 7,45 gehalten. Es wurde ein konstanter Perfusionsdruck von 9,81 kPA eingestellt. Im Anschluss an die Adaptationsphase wurden die Vitalitätsparameter zur Überprüfung der Unversehrtheit und Funktionalität der arteriellen Gefäßversorgung und des CAD getestet. Neben der myogenen Autoregulation wurde die Reaktion der Gefäße auf die Zugabe von Noradrenalin (NA), Nitroprussid (NO) und Papaverin (Papa) als Vasokonstriktor (NA) und Vasodilatatoren (NO, Papa) ermittelt. Des Weiteren wurde die Kontraktilität und Funktionalität des CAD durch Messung des intramuskulären Druckes nach elektrischer Stimulation überprüft. Zusätzlich wurden aus dem Perfusat Proben zur Messung von Laktat und der Aktivität der Laktat-Dehydrogenase (LDH) zu drei Zeitpunkten entnommen. Für die statistische Auswertung wurden eine einfache ANOVA mit wiederholten Messungen sowie der Holm-Sidak-Test als Post-hoc-Test im Falle signifikanter Unterschiede verwendet. Dabei wurde eine Wahrscheinlichkeit von p < 0,05 als statistisch signifikant angenommen. Im Hauptversuch konnten an n = 5 Kehlköpfen Perfusionen über einen Zeitraum von 352 ± 18,59 Minuten durchgeführt werden. Die myogene Autoregulation zu Beginn der Perfusion war bei drei Kehlköpfen und zum Ende der Perfusion bei vier Kehlköpfen sichtbar. Auf die Zugabe von NA reagierten am Beginn der Perfusion vier Kehlköpfe und am Ende der Perfusion alle fünf Kehlköpfe mit einer Vasokonstriktion. NO erzeugte am Versuchsbeginn bei vier Kehlköpfen eine Vasodilatation. Die Zugabe von NO und Papa führte am Versuchsende in allen Fällen zu einer Vasodilatation. Die Kontraktilität des CAD nach elektrischer Stimulation konnte in allen Fällen am Versuchsbeginn und Versuchsende gemessen werden. Bei Überprüfung der Funktionalität des CAD zeigten sich insgesamt heterogene Messergebnisse. Im Verlauf der Perfusion stiegen sowohl die Konzentration des Laktats als auch die Aktivität der LDH statistisch signifikant an, lagen aber beide im Bereich der für das Pferd in der Literatur beschriebenen Normwerte. Die vorliegende Arbeit beschreibt erstmalig die Etablierung eines druckkonstant perfundierten Ex-vivo-Modells des equinen Larynx. Über verschiedene Vitalitätstests wurde die Intaktheit und Funktionalität des perfundierten Kehlkopfes überprüft. Die myogene Autoregulation hat sich als sinnvoller aber störanfälliger Test erwiesen. Die Applikation vasoaktiver Substanzen zur Überprüfung der Funktionalität der arteriellen Gefäße hat sich als sehr zuverlässig gezeigt. Der Test auf Kontraktilität des CAD hat sich als Vitalitätsparameter mit geringer Aussagekraft gezeigt. Der Test auf Funktionalität des CAD hingegen hatte eine höhere Aussagekraft über die Vitalität des entnommenen Kehlkopfes, zeigte aber auch interindividuelle Schwankungen. Das in der vorliegenden Arbeit beschriebene Ex-vivo-Modell stellt eine solide Grundlage für weitere Untersuchungen auf dem Gebiet der Kehlkopflähmung dar.

Impact of the Maturation Status of Osteoblasts on Their Hematopoietic Regulatory Activity

Osteoblasts (OST) provide strong intrinsic growth modulatory activities on hematopoietic stem and progenitor cells via different mechanisms that include secretion of growth factors, and cellular interaction. Previously we showed that medium conditioned by mesenchymal stromal cell (MSC)-derived osteoblasts (M-OST) improve the expansion of cord blood (CB) CD34+ cells. I hypothesize that the hematopoietic supporting activity of M-OST would vary as a function of their maturation. This was tested by producing osteoblast conditioned media (OCM) from M-OST at distinct stages of maturation, and testing their growth regulatory activities in CB CD34+ cell cultures. My results showed that some of the growth promoting activity of OCM on CB cells are not dependent on the maturation status, while others are and those are largely independent of Notch signalling. In conclusion, these results provide further evidence that osteoblasts release factors that can promote the growth of immature CB progenitors in a Notch-independent way.

The Effects and Reversibility of Combination Inhaled Corticosteroids on Phonation Threshold Pressure (PTP) and Phonation Threshold Flow (PTF) in Ex Vivo Rabbit Larynges

Although combination inhaled corticosteroids (ICs) are known to cause voice disorders in otherwise vocally healthy individuals, it is unknown whether those adverse effects can be reversed by the cessation of treatment. Quantitative aerodynamic measures such as phonation threshold pressure (PTP) and phonation threshold flow (PTF) can be used to identify the development of, and recovery from, vocal pathologies. We examined the effects and reversibility of ICs on laryngeal aerodynamics. This study was a mid-project investigation as part of a larger ongoing project. The 18 larynges were from rabbits that received ICs, a control condition, or no treatment. Experimental group rabbits received ICs twice per day until inflammatory changes (e.g., erythema, edema) became visible through endoscopic observation. One experimental group (i.e., the induction group) received treatment until symptoms were observed and then larynges were harvested. The other experimental group (i.e., the reversibility group) had ICs withdrawn once visible changes were detected via endoscopy; larynges were harvested only after these changes no longer differed from baseline. Both experimental groups had corresponding control rabbits that received twice-daily nebulized saline and followed the same withdrawal and larynx harvest schedule. A final group received no treatment. During benchtop phonation trials, PTP and PTF values were determined. All data were analyzed using descriptive and parametric statistics. No significant between-group differences were observed. Descriptively, however, average PTP and PTF values for the reversibility group were lower than the induction group. Additionally, average PTP and PTF values for the reversibility group were slightly lower than the induction group. Both experimental groups had higher PTP and PTF values than the control larynges. The results of this study indicate a trend in recovery for larynges afforded a recovery period from ICs. Further testing is needed to substantiate these preliminary findings.

Yes / Background and Objective Visible light has beneficial effects on cutaneous wound healing, but the role of potential photoreceptors in human skin is unknown. In addition, inconsistency in the parameters of blue and red light‐based therapies for skin conditions makes interpretation difficult. Red light can activate cytochrome c oxidase and has been proposed as a wound healing therapy. UV‐blue light can activate Opsin 1‐SW, Opsin 2, Opsin 3, Opsin 4, and Opsin 5 receptors, triggering biological responses, but their role in human skin physiology is unclear. Materials and Methods Localization of Opsins was analyzed in situ in human skin derived from face and abdomen by immunohistochemistry. An ex vivo human skin wound healing model was established and expression of Opsins confirmed by immunohistochemistry. The rate of wound closure was quantitated after irradiation with blue and red light and mRNA was extracted from the regenerating epithelial tongue by laser micro‐dissection to detect changes in Opsin 3 (OPN3) expression. Retention of the expression of Opsins in primary cultures of human epidermal keratinocytes and dermal fibroblasts was confirmed by qRT‐PCR and immunocytochemistry. Modulation of metabolic activity by visible light was studied. Furthermore, migration in a scratch‐wound assay, DNA synthesis and differentiation of epidermal keratinocytes was established following irradiation with blue light. A role for OPN3 in keratinocytes was investigated by gene silencing. Results Opsin receptors (OPN1‐SW, 3 and 5) were similarly localized in the epidermis of human facial and abdominal skin in situ. Corresponding expression was confirmed in the regenerating epithelial tongue of ex vivo wounds after 2 days in culture, and irradiation with blue light stimulated wound closure, with a corresponding increase in OPN3 expression. Expression of Opsins was retained in primary cultures of epidermal keratinocytes and dermal fibroblasts. Both blue and red light stimulated the metabolic activity of cultured keratinocytes. Low levels of blue light reduced DNA synthesis and stimulated differentiation of keratinocytes. While low levels of blue light did not alter keratinocyte migration in a scratch wound assay, higher levels inhibited migration. Gene silencing of OPN3 in keratinocytes was effective (87% reduction). The rate of DNA synthesis in OPN3 knockdown keratinocytes did not change following irradiation with blue light, however, the level of differentiation was decreased. Conclusions Opsins are expressed in the epidermis and dermis of human skin and in the newly regenerating epidermis following wounding. An increase in OPN3 expression in the epithelial tongue may be a potential mechanism for the stimulation of wound closure by blue light. Since keratinocytes and fibroblasts retain their expression of Opsins in culture, they provide a good model to investigate the mechanism of blue light in wound healing responses. Knockdown of OPN3 led to a reduction in early differentiation of keratinocytes following irradiation with blue light, suggesting OPN3 is required for restoration of the barrier function. Evaluation of ligand modified poly (N-Isopropyl acrylamide) hydrogel for etiological diagnosis of corneal infection

Yes / Corneal ulcers, a leading cause of blindness in the developing world are treated inappropriately without prior microbiology assessment because of issues related to availability or cost of accessing these services. In this work we aimed to develop a device for identifying the presence of Gram-positive or Gram-negative bacteria or fungi that can be used by someone without the need for a microbiology laboratory. Working with branched poly (N-isopropyl acrylamide) (PNIPAM) tagged with Vancomycin, Polymyxin B, or Amphotericin B to bind Gram-positive bacteria, Gram-negative bacteria and fungi respectively, grafted onto a single hydrogel we demonstrated specific binding of the organisms. The limit of detection of the microbes by these polymers was between 10 and 4 organisms per high power field (100X) for bacteria and fungi binding polymers respectively. Using ex vivo and animal cornea infection models infected with bacteria, fungi or both we than demonstrated that the triple functionalised hydrogel could pick up all 3 organisms after being in place for 30 min. To confirm the presence of bacteria and fungi we used conventional microbiology techniques and fluorescently labelled ligands or dyes. While we need to develop an easy-to-use either a colorimetric or an imaging system to detect the fluorescent signals, this study presents for the first time a simple to use hydrogel system, which can be applied to infected eyes and specifically binds different classes of infecting agents within a short space of time. Computer modeling and experimentation in radiofrequiency-based minimally invasive therapies

Tesis por compendio / [ES] La ablación por radiofrecuencia (RF) se ha convertido en una técnica ablativa importante, ampliamente utilizada en el área de las terapias mínimamente invasivas de la medicina moderna. El avance en el campo de las tecnologías basadas en RF a lo largo de los años ha llevado a un número creciente de aplicaciones en diferentes áreas terapéuticas tales como arritmias cardíacas, epilepsia, oncología, resección asistida, apnea, dolor o cirugía estética. Sin embargo, existe una constante necesidad de desarrollar estudios computacionales y experimentales para mejorar el rendimiento de estas técnicas. El enfoque principal de esta tesis doctoral está centrado en examinar los efectos térmicos y eléctricos de ablación por radiofrecuencia de tejidos para mejorar la eficacia y la seguridad de las terapias y dispositivos basados en energía de radiofrecuencia. Las dos áreas principales de interés han sido el tratamiento del dolor y la cirugía hepática oncológica, que se han organizado en tres estudios independientes. La metodología de los estudios se ha basado en modelos computacionales y estudios experimentales sobre phantom de agar, modelos ex vivo e in vivo y ensayos clínicos. El estudio focalizado en el tratamiento del dolor ha incluido el análisis de los efectos eléctricos y térmicos del tratamiento con radiofrecuencia pulsada (PRF) y el riesgo relacionado con el daño térmico al tejido. Se han estudiado diferentes protocolos pulsados empleados en la práctica clínica utilizando modelos computacionales. La exactitud del modelo se ha validado mediante un modelo en phantom de agar. Se han propuesto también modelos computacionales adicionales para los protocolos pulsados alternativos en los cuales se reduciría el efecto térmico sin afectar al efecto eléctrico. En el estudio se ha discutido también el concepto de electroporación leve como el resultado de PRF. En el área de la cirugía hepática oncológica se han analizado dos técnicas diferentes. El primer estudio se ha centrado en examinar la hidratación del tejido durante la ablación por RF con un nuevo electrodo ICW. El nuevo diseño ha incluido dos agujas de perfusión expandibles integradas en el catéter. El objetivo principal ha sido mejorar la precisión del modelo computacional de ablación por RF de tumor utilizando una geometría realista de la distribución de solución salina en el tejido y evaluar el rendimiento del catéter de RF. Se han modelado diferentes casos de tumor infundido con solución salina y los resultados simulados se han comparado con los datos clínicos de un ensayo en 17 pacientes con cáncer hepático. Con el fin de obtener una distribución espacial realista de la solución salina infundida, se ha empleado un estudio in vivo sobre el modelo de hígado de cerdo. El segundo estudio se ha centrado en el desarrollo de una nueva técnica de sellado endoluminal basada en catéter, como una alternativa más efectiva para el manejo del remanente pancreático. El método ha consistido en una ablación por radiofrecuencia guiada por impedancia con la técnica de pullback. El ajuste del tipo de catéter de RF y del protocolo de ablación se ha realizado mediante modelos porcinos ex vivo. S'han modelat diferents casos de tumor infundit amb sèrum salí i els resultats simulats han sigut comparats amb les dades clíniques d'un assaig dut a terme sobre 17 pacients amb càncer hepàtic. Amb l'objetiu d'obtenir una distribució espacial realista del sèrum salí injectat, s'ha du a terme un estudi in vivo basat en un model de fetge de porc. El segon estudi s'ha centrat en el desenvolupament d'una nova tècnica de tancament endoluminal bassat en catèter, com una alternativa més efectiva per a gestionar el romanent pancreàtic. El mètode ha consistit en una ablació per radiofreqüència guiada per impedància amb la tècnica de pullback. L'ajust del tipus de catèter de RF i del protocol d'ablació ha sigut realitzat per mitjà de models porcins ex vivo. Posteriorment, l'efectivitat del tancament ha sigut avaluada sobre un model de porc in vivo. / [EN] Radiofrequency (RF) ablation has become an important ablative technique widely used in the area of minimally invasive therapies of the modern medicine. The advancement in the field of RF-based technologies over the years has led to a growing number of applications in different therapeutic areas such as cardiac arrhythmias, epilepsy, oncology, assisted resection, apnea, pain or aesthetic surgery. There is, however, a constant need for the development of computer and experimental studies, which would enhance the performance and safety of these techniques. The main focus of this PhD Thesis was on examining the thermal and electrical phenomena behind tissue radiofrequency ablation in order to improve the efficacy and safety of the RF-based therapies and applicators. Two main areas of interest were pain management and oncology, which were organized into three independent studies. The research methodology was based on computer modeling and experimental studies on phantoms, ex vivo and in vivo models, and clinical trials. The research on pain management involved the analysis of electrical and thermal effects of the pulsed radiofrequency (PRF) treatment and the related risk of tissue thermal damage. Different pulse protocols used in clinical practice were studied using computer modeling and the study accuracy was validated by means of agar phantom model. Additional computer models for alternative pulse protocols were also proposed, in which thermal effect would be reduced but the electrical effect would remain unchanged. The study also discussed the concept of a mild electroporation from PRF. In the area of oncology, two different techniques were analyzed. First study focused on examining tissue hydration technique during RF ablation with a novel internally cooled wet (ICW) electrode. The new design involved two expandable perfusion needles built into the catheter. The main aim was to improve the accuracy of computer model of tumor RF ablation using a realistic geometry of saline distribution in tissue, and to assess the performance of the RF catheter. Different cases of saline-infused tumor were modeled and the simulated results were compared with the clinical data from a trial on 17 hepatic cancer patients. An in vivo study on pig liver model was used to obtain a realistic spatial distribution of the infused saline. The second study focused on the development of a new catheter-based endoluminal sealing technique as more effective alternative for management of the pancreatic stump. The method consisted of the impedance-guided radiofrequency ablation with pullback. Ex vivo Binding of the Agonist PET Radiotracer [11C]-(+)-PHNO to Dopamine D2/D3 Receptors in Rat Brain: Lack of Correspondence to the D2 Recepor Two-affinity-state Model

The dopamine D2 receptor exists in vitro in two states of agonist affinity: a high-affinity state mediating dopamine’s physiological effects, and a physiologically-inert low-affinity state. Our primary goal was to determine the in vivo relevance of this two-affinity-state model for the agonist PET radiotracer [11C]-(+)-PHNO, developed for measurement of the D2 high-affinity state. Our second goal was to characterize the regional D2 versus D3 pharmacology of [3H]-(+)-PHNO binding and assess its utility for measuring drug occupancy at both receptor subtypes. Using ex vivo dual-radiotracer experiments in conscious rats, we showed that, contrary to the two-affinity-state model, the binding of [11C]-(+)-PHNO and the antagonist [3H]-raclopride were indistinguishably inhibited by D2 partial agonist (aripiprazole), indirect agonist (amphetamine) and full agonist ((-)-NPA) pretreatment. Furthermore, ex vivo [11C]-(+)-PHNO binding was unaffected by treatments that increase in vitro high-affinity state density (chronic amphetamine, ethanol-withdrawal), whereas unilateral 6-OHDA lesion, which increases total D2 receptor expression, similarly increased the ex vivo binding of [11C]-(+)-PHNO and [3H]-raclopride. These results do not support the in vivo validity of the two-affinity-state model, suggesting instead a single receptor state for [11C]-(+)-PHNO and [3H]-raclopride in conscious rat. Importantly, we also demonstrated that the increased amphetamine-sensitivity of the agonist radiotracers [11C]-(+)-PHNO and [11C]-(-)-NPA, commonly seen in isoflurane-anaesthetized animals and cited as evidence for the two-affinity-state model, is due to the confounding effects of anaesthesia. Using in vitro and ex vivo autoradiography in rat and the D3 receptor-selective drug SB277011, we found that [3H]-(+)-PHNO binding in striatum and cerebellum lobes 9 and 10 was due exclusively to D2 and D3 receptor binding, respectively, but in other extra-striatal regions to a mix of the two receptor subtypes. Surprisingly, the D3 contribution to [3H]-(+)-PHNO binding was greater ex vivo than in vitro. Also surprising, several antipsychotic drugs, at doses producing 80% D2 occupancy, produced insignificant (olanzapine, risperidone, haloperidol) or small (clozapine, ~35%) D3 occupancy, despite similarly occupying both receptor subtypes in vitro. These data reveal a significant discrepancy between in vitro and ex vivo measures of dopamine receptor binding and suggest that the D3 occupancy is not necessary for the therapeutic effect of antispychotic drugs.
