Time domain analysis of spirograms

Spirograms are widely used simple tests of ventilatory lung function. This thesis evaluates their analysis in the time domain as a means of improving their yield of diagnostic information. The most precise method for recording spirograms was determined. Errors due to non-instantaneous cooling of gas within spirometers were found to be greater than the known non-linearity of pneumotachographs when recording simulated spirograms. The analysis of spirograms inthe time domain involves considering the spirogram as a cumulative distribution of transit times and deriving the statistical moments of this distribution. A valid procedure for comparing the moments of truncated spirograms has been proposed and these truncated moments have been shown to be highly reproducible within individuals and retain discriminatory ability. A multi-exponential model of the spirogram has been considered in detail as a means of overcoming errors inherent in the moments of some spirograms. The most satisfactory technique for applying this model to describe spirograms has been determined and the model was found to reflect correctly abnormalities in spirograms for commonly found ventilatory disorders. A cross-sectional population survey of a male workforce was carried out to determine whether time domain analysis of spirograms detects abnormalities hitherto unappreciated. Normal values for conventional and time domain spirometric indices were determined. Smokers were found to have two types of spirogram abnormality previously not recognised. The more commonly found abnormality was evident early within the spirogram, and the other was only evident in the tail of the spirogram. These findings do not agree with current hypotheses that smoking damage should be first manifest in the terminal part of a maximal forced expiratory manoeuvre. Alternative hypotheses are proposed. Time domain analysis of spirograms has been found to demonstrate hitherto unrecognised changes in spirograms due to smoking.

The Effects of Expiratory Muscle Strength Training on Communication in Amyotrophic Lateral Sclerosis when Implemented via Telepractice

<p>The purpose of the current study was to determine whether implementing EMST through telepractice is feasible and effective for providing better respiratory support to improve or maintain communication and cough production in ALS. In a nine-week multiple baseline design, twelve participants with mild ALS completed six weeks of Expiratory Muscle Strength Training (EMST) at a moderate intensity level. Descriptive data revealed that it is feasible for speech-language pathologists to implement EMST through telepractice with individuals who have ALS, as minimal time and caregiver involvement was required to implement EMST successfully and adherence and attendance rates were high. EMST implemented through telepractice resulted in a significant increase in maximum expiratory pressure (MEP) at the group level. At the individual level, nine participants had significant increases in MEP, two participants MEP remained stable, and one participant’s MEP decreased Post-Training, most likely due to poor adherence. These overall improvements to MEP did not result in significant improvements to respiratory-dependent speech outcome measures such as pause frequencies and pause durations at the group level. A lack of significant changes to the respiratory-dependent speech outcome measures, but a significant declination in the control variable of articulation rate that is not dependent on the respiratory system suggests that EMST had a maintenance effect for most participants. At the individual level, EMST appears to impact neural control of speech as it alters the pause patterns uniquely for each participant, with some participants demonstrating improvements and others demonstrating maintenance or declination. Maintenance at the group level was also observed for cough strength following EMST. In conclusion, EMST can be implemented via telepractice with people who have mild ALS and may help people with ALS maintain speech and cough production by altering respiratory support. </p>

Dosagem de óxido nítrico expirado pelas narinas de pacientes com rinossinusite crônica / Dosage of nitric oxide expired by the nostrils of patients with chronic rhinosinusitis

Introdução: Várias controvérsias e diversos questionamentos existem sobre a fisiopatogenia e o melhor tratamento para a doença Rinossinusite Crônica (RSC). Alguns estudos apontam que a dosagem de Óxido Nítrico (ON) do ar expirado pelas narinas pode contribuir no diagnóstico e seguimento desta afecção. Objetivos: Determinar se há diferença na dosagem de ON expirado pelas narinas de pacientes com RSC e indivíduos sem RSC e estabelecer correlação entre os achados clínicos, exames complementares e a quantidade de ON encontrada nos pacientes. Casuística e Método: Estudo retrospectivo com 104 pacientes com RSC e 35 indivíduos sem RSC, atendidos no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo, entre os anos de 2013 e 2016. Foram realizadas dosagens de ON do ar expirado pelas narinas dos pacientes e controles. Posteriormente, realizou-se correlação entre os achados clínicos e exames complementares dos pacientes e a quantidade de ON obtida. Resultados: As dosagens de ON nos pacientes com RSC foram significativamente menores em relação aos controles. Também se observou relação entre dosagem de ON e intensidade de sintomas, queixas clínicas, achados endoscópicos e tomográficos, e o número de seios acometidos, quando realizada a cirurgia. Conclusão: Diante dos resultados obtidos, o que mais chama atenção é a possibilidade de avaliação dos pacientes portadores de RSC com ou sem pólipo nasal por um método aqui padronizado, de fácil manuseio pelos técnicos e pacientes, de baixo custo, não invasivo e que permite também comparar resultados antes e após o tratamento. / Introduction: Chronic rhinosinusitis (CRS) is a disease of the upper respiratory tract with great questions regarding its clinical management. Nitric Oxide (NO) dosing by the air expiratory by the nostrils, can contribute to the diagnosis and follow-up of this affection. Objectives: To determine if there is a difference in the NO expiratory dose in the nostrils of people with CRS and people who do not have CRS, and to establish a clinical correlation between the findings and the amount of NO found in the patients. Casuistic and Method: Retrospective study conducted in 104 patients with CRS and 35 individuals without CRS at Hospital of Clínics of Ribeirão Preto Medical School, University of São Paulo. Between the years of 2013 and 2016. NO dosages were performed by expiratory air from the nostrils of people with and without CRS, and, later an analysis of the clinical findings of the patients with the amount of NO obtained was performed. Results: The NO dosages of patients with CRS were significantly lower in relation to those without CRS. We also found a relationship between the NO dosage, and the intensity of the symptoms, clinical complaints, endoscopic and tomographic findings, and the number of sinuses involved in the surgery. Conclusions: In view of the results obtained, what is most striking is the possibility of evaluating patients with CRS with or without PN, using a low-cost, noninvasive method that is easy to handle by technicians and patients. before and after treatment.

To determine whether differences in lung morphology assessed on the CT scan influence the response to PEEP we study by a prospective study during a 53-month period in a fourteenbed surgical Intensive Care Unit of a university hospital, seventy-one consecutive patients with early ARDS. In each patient, a fast spiral thoracic CT scan was performed in ZEEP and after implementation of PEEP 10 cmH2O. Hemodynamic and respiratory parameters were also measured in the same conditions. PEEPinduced overdistension and alveolar recruitment were quantified by a specifically designed software, Lungview®. Overdistension occurred only in the upper lobes and was significantly correlated with the volume of lung characterized by a scanographic density ranging between -900 HU and -800 HU parenchyma in ZEEP conditions. Cardiorespiratory effects of PEEP were similar in patients with primary and secondary ARDS. The thoracoabdominal mobility of COPD patients alters as a result of this obstructive process that causes permanent pulmonary hyperinflation, altering respiratory mechanics. As a result, physical efforts can increase minute ventilation by reducing the time to expiration, increasing this hyperinflation. Positive airway expiratory pressure (PEP) is a physiotherapeutic resource that helps reduce air trapping during exacerbations of the COPD patient, but little is known about the effects of PEP on pulmonary hyperinflation. As a rule, PEP with high pressures has been contraindicated. Objective: Evaluate the effects of physical effort and PEP on pulmonary hyperinflation and Thoracoabdominal mobility through optoeletronic plethysmography (OEP) in patients with COPD and in healthy individuals. Materials and Methods: Thoracoabdominal mobility was evaluated by OEP in 30 subjects, in two groups, 15 of them with GOLD (GDPOC) and 15 healthy subjects (GC), with a mean age of 58.1 ± 11 years, before and after a daily life simulation activity, up and down the step and before and after performing a series of exercise with PEP. Results: Both the age and the anthropometric data of the studied population as weight and height showed no difference. The spirometric variables showed a significant difference, in favor of the CG, as expected. There were no significant differences in the contribution of the thoracoabdominal compartments at rest between groups, except for Ti / Tot in the GDPOC, with median of 0.77 characterizing possible hyperinflation or respiratory muscle fatigue. After physical exertion, for 2 minutes, the GDPOC group showed an increase in the Abd contribution in thoracoabdominal movements, higher than CG (52.2 ± 12.6% vs. 41.1 ± 14.6%). After PEP, there was an increase of abdominal compartment in the subjects of the GDPOC (50.5 ± 19.1% x 36.9 ± 16.4%). After the PEP the patients of the GDPOC had a normalization of Ti / Ttot, with a median of 0.41. As the analysis of the compartments in the GDPOC there is a greater involvement of the abdominal compartment in three moments evaluated and after PEP this abdominal participation is also greater than in the CG. Conclusion: Physical effort and the use of PEP alter the participation of thoracic and abdominal compartments differently between groups and PEP in COPD seems to contribute to the reduction of hyperinflation. / Introdução: A doença pulmonar obstrutiva crônica (DPOC) é caracterizada por obstrução persistente que é geralmente progressiva e associada a uma resposta inflamatória nas vias aéreas e pulmões, por partículas ou gases nocivos. A mobilidade toracoabdominal de portadores de DPOC se altera em decorrência deste processo obstrutivo que causa hiperinsuflação pulmonar permanente, alterando a mecânica respiratória. Em decorrência disso, esforços físicos podem aumentar a ventilação minuto reduzindo o tempo hábil para a expiração, aumentando essa hiperinsuflação. A pressão expiratória positiva (PEP) é um recurso fisioterapêutico que auxilia na redução do aprisionamento de ar durante as exacerbações do paciente com DPOC, porém pouco se sabe sobre os efeitos do PEP em tal cirunstância de hiperinsuflação pulmonar, na qual, via de regra, a PEP com altas pressões tem sido contraindicada. Objetivo: Avaliar os efeitos da pressão expiratória positiva na mobilidade toracoabdominal e na hiperinsuflação induzida pelo exercício físico, por meio da Pletismografia optoeletrônica (POE). Material e Método: A mobilidade toracoabdominal foi avaliada pela POE em 30 indivíduos, em dois grupos, sendo 15 com DPOC nível II pelo GOLD (GDPOC) e 15 indivíduos saudáveis (GC), com idade média de 58,1±11 anos, antes e após uma atividade de simulação de vida diária, subir e descer degrau e, antes e após realizarem uma série de exercício com PEP. Resultados: Tanto a idade quanto os dados antropométricos da população estudada como peso e altura não mostraram diferença. Já as variáveis espirométricas mostraram diferença significativa, à favor do GC, conforme o esperado; não houve diferenças significativas na contribuição dos compartimentos toracoabdominal, em repouso, entre os grupos, com exceção do Ti/Tot no GDPOC, com mediana de 0,77 caracterizando possível hiperinsuflação ou fadiga muscular respiratória. Após o esforço físico, por 2 minutos, houve alteração na mobilidade toracoabdominal no grupo GDPOC, apresentou aumento da contribuição do compartimento abdominal, maior que o CG (52,2±12,6% x 41,1±14,6%). O mesmo aumento houve após a PEP, no GDPOC (50,5±19,1% x 36,9±16,4%). Após a PEP os pacientes do GDPOC tiveram uma normalização de Ti/Ttot, com mediana de 0,41. Quanto a análise dos compartimentos no GDPOC há uma participação maior do compartimento abdominal nos três momentos avaliados sendo que após PEP essa participação abdominal também é maior do que no GC. Conclusão: O esforço físico e o uso de PEP alteram a participação de compartimentos torácicos e abdominais, de forma diferente entre os grupos e, a PEP na DPOC parece contribuir para a redução da hiperinsuflação.

L'activité tonique diaphragmatique chez les enfants avec et sans support respiratoire

Introduction : Les nourrissons, vu la grande compliance de leur cage thoracique, doivent maintenir activement leur volume pulmonaire de fin d’expiration (VPFE). Ceci se fait par interruption précoce de l’expiration, et par le freinage expiratoire au niveau laryngé et par la persistance de la contraction des muscles inspiratoires. Chez les nourrissons ventilés mécaniquement, notre équipe a montré que le diaphragme est activé jusqu’à la fin de l’expiration (activité tonique). Il n’est pas clair si cette activité tonique diaphragmatique compense pour l’absence de freinage laryngé liée à l’intubation endotrachéale. Objectif : Notre objectif est de déterminer si l’activité tonique diaphragmatique persiste après l’extubation chez les nourrissons et si elle peut être observée chez les enfants plus âgés. Méthode : Ceci est une étude observationnelle longitudinale prospective de patients âgés de 1 semaine à 18 ans admis aux soins intensifs pédiatriques (SIP), ventilés mécaniquement pour >24 heures et avec consentement parental. L’activité électrique du diaphragme (AEdi) a été enregistrée à l’aide d’une sonde nasogastrique spécifique à 4 moments durant le séjour aux SIP : en phase aigüe, pré et post-extubation et au congé. L’AEdi a été analysée de façon semi-automatique. L’AEdi tonique a été définie comme l’AEdi durant le dernier quartile de l’expiration. Résultats : 55 patients avec un âge médian de 10 mois (écart interquartile: 1-48) ont été étudiés. Chez les nourrissons (<1an, n=28), l’AEdi tonique en pourcentage de l’activité inspiratoire était de 48% (30-56) en phase aigüe, 38% (25-44) pré-extubation, 28% (17-42) post-extubation et 33% (22-43) au congé des SIP (p<0.05, ANOVA, avec différence significative entre enregistrements 1 et 3-4). Aucun changement significatif n’a été observé pré et post-extubation. L’AEdi tonique chez les patients plus âgés (>1an, n=27) était négligeable en phases de respiration normale (0.6mcv). Par contre, une AEdi tonique significative (>1mcv et >10%) a été observée à au moins un moment durant le séjour de 10 (37%) patients. La bronchiolite est le seul facteur indépendant associé à l’activité tonique diaphragmatique. Conclusion : Chez les nourrissons, l’AEdi tonique persiste après l’extubation et elle peut être réactivée dans certaines situations pathologiques chez les enfants plus âgés. Elle semble être un indicateur de l’effort du patient pour maintenir son VPFE. D’autres études devraient être menées afin de déterminer si la surveillance de l’AEdi tonique pourrait faciliter la détection de situations de ventilation inappropriée. / Background: Infants have to actively maintain their end expiratory lung volume (EELV) due to their high rib cage compliance. Mechanisms such as high respiratory rate, short expiratory time and laryngeal braking are implicated. In mechanically ventilated infants, the diaphragm stays activated until the end of expiration (tonic activity), contributing to EELV maintenance. It is unclear whether tonic activity compensates for the lack of laryngeal braking due to intubation or if it is normally present. Objective: To determine if tonic diaphragm activity remains after extubation in infants, and if it can be observed in older children. Methods: Prospective observational study of pediatric patients 1 week to 18 years-old ventilated for >24 hr with parental consent. Diaphragm electrical activity (EAdi) was recorded using a specific nasogastric catheter during four periods: (i) the acute phase, (ii) pre-extubation, (iii) postextubation, and (iv) at PICU discharge. EADi was analyzed in a semi-automatic manner. Tonic EAdi was defined as the EAdi in the last quartile of expiration. Results: Fifty-five patients, median age 10 months (Interquartile range: 1–48) were studied. In infants (<1 year, n=28), tonic EAdi as a percentage of inspiratory activity was 48% (30-56) in acute phase, 38% (25-44) pre-extubation, 28% (17-42) post-extubation et 33% (22-43) at PICU discharge (p<0.05, ANOVA, with statistically significant difference between recordings 1 and 3-4). No significant change was observed between pre- and post-extubation periods. In older patients (n=27), tonic activity was negligible as a whole (0.6mcv). However, significant tonic EAdi (>1mcv and >10%) was observed in 10 patients (37%). Bronchiolitis was the only independent factor associated with tonic EAdi. Conclusions: In infants, tonic EAdi remains involved in ventilatory control after extubation and restoration of laryngeal braking. Tonic EAdi can be reactivated in older patients. The interest of tonic EAdi as a tool to titrate mechanical ventilation warrants further evaluation.
