Deprimerade patienters erfarenheter av bemötande från vårdpersonal / Depressed patients' experiences of treatment from health care professionals

Bakgrund: Depression är en folksjukdom som orsakar stort lidande för dem som drabbas och deras närstående. Sjukdomen underdiagnosiseras på grund av samsjuklighet med fysiska sjukdomar. Ökad kunskap om depression krävs för att kunna främja ett bra bemötande av patient från vårdpersonal. Syfte: Syftet med studien var att beskriva hur patienten med depression upplever bemötande från vårdpersonal. Metod: En litteraturöversikt baserad på tio vetenskapliga artiklar som analyserades i enlighet med Friberg. Tio kvalitativa studier hämtades från CINAHL, PsycINFO och PubMed. Som en teoretisk utgångpunkt användes Rosemarie Rizzo Parses omvårdnadsteori. Resultat: Resultatet presenterades i två teman och sex underteman. Det första huvudtemat är relation till vårdpersonalen med tre underteman: delaktighet, information, delaktighet och behandling och tillgänglig vård. Det andra huvudtemat är interaktion med vårdpersonal med tre undertemat: kommunikation, upplevelse att bli lyssnad på och kompetent och närvarande personal. Diskussion: Resultat diskuteras utifrån Rosemarie Rizzo Parses teori och även tio vetenskapliga artiklar, med syfte att visa att deprimerade patienter som känner brist på tillit, engagemang från vårdpersonalen samt brist av god behandling och kontinuitet haft dåligt erfarenheter av bemötande med vårdpersonal. Det krävs en ömsesidig relation mellan patienter och vårdpersonal för att patienterna kunna ha en bra upplevelse med vårdpersonalen. / Background: Depression is a common disease that causes great suffering for patients and their relatives. The disease is often under diagnosed because of comorbidity with physical disorders. Increased knowledge about depression is needed to a good response between patients and health care professionals. Aim: The aim of this study was to describe how the patient with depression experiences treatment from healthcare professionals. Method: A literature review based on ten scientific articles that were analyzed in accordance with Friberg (2012). Ten qualitative studies were retrieved from the database CINAHL, PsycINFO and PubMed. As a theoretical base used Rosemarie Rizzo Parse's theory. Results: The result demonstrates two main themes and the six sub-themes. The first main theme highlights the relationship with the nursing staff with three sub-themes: participation, information, treatment and care available. The second main theme is the interaction with health care professionals with three sub-themes: communication, experience to be listened to and competent and present staff. Discussion: The results discussed from Rosemarie Rizzo Parse's theory, and also additional ten scientific articles, with the aim to show that depressed patients who feel a lack trust, commitment from health care professionals and continuity had bad experiences of treatment with health care professionals. The needed a mutual relationship between patients and health care professionals in order to patients to have a good experience with health care professionals.

Percepções de médicos e enfermeiros acerca da violência contra a mulher: uma análise comparativa / Doctors and nurses\' perceptions of violence against women: a comparative analysis

Este trabalho foi desenvolvido como subprojeto integrante da pesquisa A Interface Entre a Ocorrência e o Atendimento de Violência de Gênero Entre Mulheres Usuárias dos Serviços de Saúde da Rede Pública de Ribeirão Preto. A violência contra a mulher é uma das expressões mais perversas da subordinação feminina e gera graves conseqüências para as pessoas que vivenciam o seu ciclo. Mulheres nessa situação buscam frequentemente os serviços de saúde, que possuem um alto poder de detecção da violência. Porém, há uma série de dificuldades por parte dos profissionais, médicos e enfermeiros, em identificar e prestar assistência adequada a essas mulheres. O objetivo deste estudo foi analisar comparativamente as percepções de médicos e enfermeiros que atuam na rede de saúde de Ribeirão Preto acerca da violência contra a mulher buscando formas de aprimorar a assistência prestada. Para isso, realizamos uma pesquisa qualitativa, utilizando um banco de dados de 14 entrevistas com médicos e 10 com enfermeiros, realizadas por meio de um roteiro semi-estruturado. Por meio de análise de conteúdo temático foram identificadas as seguintes categorias: 1) Percepções sobre gênero; 2) Percepções sobre a violência contra a mulher; 3) Sobre atuação profissional; e 4) Sobre a rede de proteção. Tais categorias foram divididas em diversas subcategorias que foram analisadas a partir do referencial de gênero. Os resultados mostraram que há muitas semelhanças nas percepções relativas às questões de gênero, que são ainda bastante tradicionais. Os profissionais entendem que a violência contra a mulher ocorre devido às desigualdades perpetuadas pelo sistema social e que acabam por justificar a violência. Eles reconhecem os tipos de violência existentes e estão aptos a identificar e acolher as mulheres nos serviços de saúde, reconhecendo tais ações como suas responsabilidades. Porém, muitas vezes não o fazem por barreiras como a própria estrutura dos serviços, a falta de capacitação e aspectos emocionais, que dificultam o acolhimento e a orientação adequados. Também identificamos que a rede de apoio existente ainda é desconhecida por muitos dos profissionais e, diversas vezes, está estruturada de forma inadequada para atender às demandas existentes. Assim, é fundamental o desenvolvimento de capacitações para os profissionais da área de saúde com o objetivo de prepara-los para uma melhor assistência às mulheres em situação de violência e para o conhecimento da rede de proteção existente. Além disso, é urgente que a estrutura dos serviços seja repensada em diversos aspectos para que os princípios do SUS e as ações de humanização possam, de fato, ser colocadas em prática. / This study was conducted as part of the larger research project \"The interface between the occurrence and treatment of gender violence among women who are users of public health care in Ribeirão Preto. Violence against women is one of the most perverse forms of female subordination and leads to serious consequences to those affected by it. Women in such conditions frequently seek health care services, which are highly capable of detecting violence. However, there are a number of obstacles faced by professionals, doctors and nurses, in order to identify and provide proper assistance to these women. The objective of this study was to comparatively analyze how doctors and nurses who work in the public health care system in Ribeirão Preto perceive violence against women with a view to improving the quality of services. To attain this goal, this qualitative study made use of a database of 14 interviews with doctors and 10 with nurses following a semistructured script. Following thematic content analysis, the following categories have been established: 1) Perceptions about gender; 2) Perceptions about violence against women; 3) About the jobs; and 4) About the protection network. These categories were further divided into subcategories that were analyzed by taking gender as a reference. The findings show that there are many similarities across perceptions concerning gender issues, which are still quite traditional. Professionals believe that violence against women occurs as a result of inequalities that have been perpetuated by the social system and that ultimately justify such violence. They recognize the existing types of violence and are ready and able to identify women and offer them assistance within the health care system, which they regard to be part of their responsibilities. However, they frequently cannot do that due to barriers such as how the services are structured, lack of training, and emotional aspects, which make both reception and counseling difficult. It has also been found that the existing support network is still unknown to many professionals and, very often, that it is inadequately organized to meet current demands. Thus, it is essential to develop the skills of health care professionals in order to prepare them both to provide better assistance to women who have been affected violence and to know about the existing protection network. In addition, there is the urgent need to rethink the organization of services at various levels so that the principles of SUS - the universal health care system - and humanization actions can actually be put into practice.

Os efeitos de um programa de Mindfulness para profissionais da Atenção Primária à Saúde: um estudo antes-e-depois / The effects of a Mindfulness program for Primary Health Care professionals: a before-and-after study

Tendo em vista o aumento nos níveis prejudiciais de estresse e seu impacto na saúde da população, nota-se a necessidade de buscar estratégias eficazes de promoção de saúde na Atenção Primária para minimizar os prejuízos causados pelo estresse e aumentar a qualidade de vida das pessoas. Atualmente, o trabalho é um importante gerador de eventos \"estressores\" e, neste ambiente, as pessoas podem apresentar índices prejudiciais de estresse quando não encontram recursos de enfrentamento suficientes para lidar com tais eventos. A prática de Mindfulness surge como uma estratégia para redução dos níveis de estresse, de ansiedade e melhora na qualidade de vida. O objetivo do presente estudo é avaliar o efeito de um programa de intervenções baseadas em Mindfulness no estado funcional, nos níveis de atenção plena e no risco de adoecimento associado ao estresse no trabalho em profissionais da Atenção Primária à Saúde (APS), na cidade de Ribeirão Preto - SP, Brasil. Trata-se de estudo quasi-experimental, realizado de setembro a dezembro de 2015, com profissionais da APS, utilizando métodos quantitativos e abordagem analítica antes-e-depois de um Programa de Mindfulness de 8 semanas (1 encontro de 2h/semana). A intervenção foi avaliada antes e depois da execução do programa por meio da aplicação dos instrumentos: Questionário sociodemográfico e de características pessoais, comportamentais e do trabalho, Escala de Estresse no Trabalho, Questionário das Cinco Facetas de Mindfulness e Dartmouth Primary Care Cooperative Information Project - COOP Function. Foram observadas diferenças significantes em seis facetas de Mindfulness após a intervenção: Observar (p=0,002); Descrever - formulação positiva (p=0,01); Agir com consciência plena - piloto automático (p=0,01) e distração (p=0,05); Não reagir à experiência interna (p=0,0005) e Não julgar a experiência interna (p=0,01). Em relação ao estado funcional, foram encontradas diferenças significantes para as dimensões: mudanças na saúde (p=0,01), saúde geral (p=0,007), qualidade de vida (p=0,04) e sentimentos (p=0,01). Quanto aos níveis de estresse no trabalho,notou-se a redução do modo de organização no trabalho de alta exigência, que passou de 35% antes da intervenção para 19% após a intervenção. Houve aumento dos modos de organização ativo (antes: 19%; depois: 27%); passivo (antes: 27%; depois: 31%); e do modo de baixa exigência (antes: 19%; depois: 23%). Conclui-se que intervenções baseadas em Mindfulness aplicadas a profissionais da APS podem melhorar níveis de atenção plena, funcionalidade e podem modificar os modos de organização no trabalho, reduzindo riscos de adoecimento destes profissionais. / Considering the increasing harmful levels of stress and its impact on the population health, it is necessary to seek effective strategies of health promotion in primary care to minimize damages caused by stress and improve people\'s quality of life. Today, work is a major \"stressor\" event generator, and in this context, people may experience harmful levels of stress when they do not find enough coping resources to handle such events. Mindfulness practice is a strategy to reduce levels of stress, anxiety and improvement in quality of life. The objective of the present study is to evaluate the effect of a Mindfulness program on the functional status, in mindfulness levels and the risk of illness associated with work-related stress in Primary Health Care (PHC) professionals in Ribeirão Preto-SP, Brazil. This is a quasi-experimental study, conducted from September to December 2015, with PHC professionals, using quantitative methods and analytical approach before and after an 8-week Mindfulness Program (sessions of 2 hours/week). The intervention was evaluated before and after the Mindfulness program through the application of three questionnaires: Sociodemographic questionnaire and personal, behavioral and work characteristics, Job Stress Scale, Five Facet Mindfulness Questionnaire and the Dartmouth Primary Care Cooperative Information Project - COOP Function. Significant differences were observed in six facets of Mindfulness after the intervention: Observe (p = 0.002); Describe - positive formulation (p = 0.01); Acting with awareness - autopilot (p = 0.01) and distraction (p = 0.05); Do not react to internal experience (p = 0.0005) and Do not judge the internal experience (p = 0.01). In functional status, significant differences were found for the dimensions: perception of change in health (p = 0.01), general health (p = 0.007), quality of life (p = 0.04) and feelings (p = 0.01). Regarding the stress levels at work, it noticed a reduction in the organizational mode of high strain, from 35% before the intervention to 19% after the intervention. There was increased in active mode (before: 19%, after: 27%); in passive mode (before: 27%, after: 31%) and the low strain mode (before: 19%, after: 23%). It is concluded thatinterventions based on Mindfulness applied to PHC professionals can improve levels of mindfulness, functionality and can modify the ways of organization at work, reducing the risks of illness of these professionals.

Physiotherapy practice in the ICU has shown itself to be effective, whereas the lack of physiotherapy management in the care of critically ill patients may prolong recovery. Therefore, the knowledge and attitudes about physiotherapy management by the other Health Care Professionals of the team is essential in order to facilitate efficient and effective medical services. This study aimed to determine other Health Care Professional' knowledge, attitudes regarding physiotherapy management in the ICU. As well as, it determined the current physiotherapy practices applied by physiotherapists in ICUs in Khartoum State - Republic of Sudan. The study was conducted due to limited studies in this field.

Mammors upplevelse av amningsstöd på BVC

Bakgrund och problemformulering: Många kvinnor har bestämt sig för att amma innan förlossningen. Tidig hemgång från BB-avdelning gör att BVC-sköterskan har ett stort ansvar att stödja nyblivna mammors amning. Studier visar att BVC ibland ger bristfällig och felaktig information om amning och mammor upplever amningsstödet från BB bättre än stödet från BVC. Med bakgrund av detta utformades studiens syfte för att beskriva mammors upplevelse av amningsstöd på BVC.Syfte: Att beskriva mammors upplevelse av amningsstöd.Metod: Öppna intervjuer genomfördes med åtta mammor. Alla hade påbörjat amning men ammade under olika lång tid. Samtliga mammor hade deltagit i basprogrammet på BVC. En kvalitativ innehållsanalys gjordes av de transkriberade intervjuerna.Resultat och diskussion: Studiens resultat visade att mammor har stort behov av individuellt stöd vid amning. Mammorna berättade om bra och mindre bra erfarenheter i mötet med BVC-sköterskan. Det som framgick av studien var att tid, individuellt stöd och uppmuntran att tro på sin egen förmåga från BVC-sköterskan var det viktigaste för mammorna. De mammor som inte fick stödet de önskade kände sig utlämnade och ensamma i sin situation, de kände att de inte dög som mammor. I de berättelser där mammorna fick stöd kände de sig stärkta. När mammorna inte kände stöd från BVC-sköterskan förstärktes deras känsla av oduglighet. Slutsatsen är att BVC-sköterskans amningsstöd är viktigt men kunskaper om amning och samtalsmetodik behöver förbättras och utvecklas för att skapa vårdande relationer. / Program: Specialistsjuksköterskeutbildning med inriktning mot distriktssköterska

Health Care Professionals' Perceptions of Media Influence on Eating Disorder-Related Factors Among African American Women

Little is known about health care professionals' perceptions of eating disorder etiology among African American (AA) women. The purpose of this quantitative research study was to examine the associations among health care professionals' race, cultural awareness, and perceptions of media influence on eating disorder-related factors in AA women. Festigner's social comparison theory; Bandura's social learning theory; and Garcia, Cartwright, Winston, and Borzuchowska's transcultural integrative model served as the theoretical frameworks for this study. Specifically, this study examined whether race and cultural awareness of health care professionals relate to their perceptions of the extent to which media influences AA women's eating disorders, and whether cultural awareness moderates the association between their race and media influences. Data were obtained through a researcher-created demographic questionnaire, the Multicultural Counseling Inventory, and a modified Sociocultural Attitudes Toward Appearance Scale-3 with a purposive sample of 49 participants. Data were analyzed using descriptive statistics, Spearman's Rho correlation, Pearson correlation, and a hierarchical multiple linear regression. Compared to their Caucasian American counterparts, AA health care professionals perceived greater media pressure on AA women's body image concerns. Additionally, participants' cultural awareness was positively correlated with their ratings of AA women's desire to have more athletic bodies. The implications for positive social change stemming from this study are directed at health care professionals as additional training may increase their awareness, early detection, diagnosis, and treatment of eating disorders among AA women.

The Impact of Work-Related Stress on Medication Errors by Health Care Professionals in Saudi Arabian Hospitals

Despite increased awareness about patient safety and quality of care, errors and adverse outcomes occur frequently in clinical practice. An estimated 10% of the 35.1 million U.S. hospital patients suffered injuries caused by medical errors; the most common were medication errors, which accounted for more than 50% of all medical errors. Work-related stress is associated with medication errors for health care professionals (HCP) in Saudi Arabia (SA) hospitals; however, the specific types of stressors and their effect on the level of medication errors have not been studied in SA. The purpose of this quantitative correlational study was to examine the relationship between the overall level and sources of work-related stress using the job stress scale on the level of medication errors for a group of 269 HCPs working at King Abdul-Aziz Hospital (KAH) in SA. The theoretical framework for this research was the Donabedian patient safety model, which relates healthcare quality to personal, environmental, and organizational factors. Binary logistic regression analyses indicated there was no relationship between overall levels of stress and medication errors. However, specific sources of work related stress such as disruption to home life, excessive workload, and night/weekend call duties were associated with a significant increase in the medication error rate, while pressure to meet deadlines and difficulties with colleagues was associated with a significant decrease in the medication error rate. Positive social change implications include how understanding the impact of work-related stress on medication errors by SA HCPs may lead to specific interventions to reduce medication errors and improve patient care.

South Africa as a developing country is not exceptional on the problem of turnover rate of health care professionals. The high vacancy rate became evident in the recent years. The health institutions depend on the health professionals in providing quality health care services. The challenges that are facing the health institutions include lack of a clear understanding by managers of the factors that are contributing to high turnover rate of health care professionals as well as the formulation of effective recruitment and retention strategies. The prevailing of the rapid changes in the internal and external environment make it difficult for the health institutions to develop and implement recruitment and retention strategies in an effort to minimise high turnover of healthcare professionals. The purpose of this study is to analyse the factors that are contributing to turnover of health care professionals at Polokwane and Mankweng Tertiary Hospital in Limpopo Province. The study also provides recruitment and retention strategies that the management may apply them to minimise the high turnover of health care professionals. The population sample was randomly selected from Polokwane and Mankweng Tertiary Hospital. Data was collected and analysed from a total number of 150 respondents. A structured questionnaire was used as an instrument for the collection of primary data. In this regard, factors such as lack of medical equipment, poor management style, poor working conditions and high workload are found to be critical factors that are affecting quality health care service and eventually the morale of employees, and resulting into turnover of health care professionals
