The effects of lay counselling on posttraumatic stress in black adolescents

M.A. (Psychology) / Youth and children constitute seventy percent of the population in South Africa. It is these children who carry the potential for the future of our nation. They may be precluded from reaching their potential jf we do not urgently address the problems brought about by violence. Highly evident is the increased incidence of PTSD within the violence stricken communities in South Africa. PTSD among black youths is so high that it can be described as an epidemic. Due to the limited therapeutic resources available, it is imperative that a programme ofaction aimed atreducing the negative impact which PTSD has and continues to have on children is designed. Thus, following adiligent study of the literature, it was decided to train lay counsellors I volunteers in terms of a previously devised Cognitive Behavioural Group intervention formulated by Cowley, Hetz and Rosin (1994) in order to reach out to these children who have been so aversively affected by violence. Utilising the services of lay counsellors rather than professionals is cost effective and time effective. A large sample of PTSD positive subjects were selected from three different schools on the basis that they satisfied the DSM IV criteria for PTSD. The subjects participated in a six week intervention programme. A large number of lay counsellors were trained and selected to facilitate the intervention programme. The administration of the intervention by the lay counsellors did not alter the effectiveness of the Cognitive Behavioural intervention. The results obtained were in accordance with those obtained by Cowley et al.,(1994) in the paired sample tests which were deemed appropriate for this kind of research.

Exploring the influence of stigma, level of trauma, and social support on the experience of posttraumatic growth in adults living with HIV

The present study investigated the influence of HIV-related stigma, social support, and impact of HIV diagnosis on posttraumatic growth (PTG) in adults living with HIV (N = 126). In addition, the study aimed to identify if social support moderated the relationship between stigma and PTG. Lastly, the study attempted to determine how impactful receiving an HIV diagnosis was to the sample. One hundred and twenty-six adults living with HIV within the state of Florida (41% response rate) participated in the research. Participants were recruited from a series of support groups and HIV focused agencies throughout the state, and responded through face to face survey administration. Each assessment packet consisted of the following assessments: (a) Posttraumatic Growth Inventory; [PTGI], Tedeschi & Calhoun, 1996, (b) Multidimensional Scale of Perceived Social Support; [MSPSS], Zimet, Dahlem, Zimet, & Farley, 1988, (c) Berger HIV Stigma Scale; Berger, Ferrans, & Lashley, 2001, (d) Impact of Event Scale [IES-R . Weiss & Marmar, 1996], (e) Reynolds Short Form Social Desirability Scale; Reynolds, 1982, and (f) a demographic questionnaire. Multiple regression analysis was used to determine whether the independent variables were predictive of the outcome of PTG. Findings from the study revealed that each of the predictor variables (i.e., stigma, social support, and impact of diagnosis) contributed significantly (p<.05) to the model, and accounted for 12% of the variance in PTGI scores. In examining the moderating presence of social support between stigma and PTG, there was no found significant interaction between stigma and social support. Significant differences in PTG scores were also identified amongst differing demographic groups, specifically participant ethnicity and religious orientation. Findings also revealed that 38.5% of the sample reported their HIV diagnosis as so impactful that they would likely qualify for a clinical diagnosis of posttraumatic stress disorder (PTSD). Furthermore, a detailed discussion of previous literature, study procedures and methodology, counselor implications, counselor educator implications, and recommendations for future research are discussed.

Introduction. The HIV prevalence among men who have sex with men (MSM) in China keeps increasing sharply. A high proportion of the MSM in China have male regular sex partner (RP) and prevalence of unprotected anal intercourse (UAI) involving such RP is higher than when non-RP is involved. Trust, intimacy and cognitive factors are the factors associated with UAI with RP. Several cross-sectional studies have been demonstrated the important factors associated with UAI with RP among MSM in China. However, no study about intervention for MSM-RP is found to be conducted. To reduce UAI with RP, an intervention tailored to RP is urgently developed and identified its efficacy. / Objectives. The study aimed to describe the prevalence of UAI, as well as of which associated factors among MSM-RP in Beijing and Chengdu, China, and to evaluate the efficacy of an enhanced HIV voluntary counseling and testing (VCT) in increasing condom use with RP among MSMRP in China by a randomized controlled trial (RCT). / Subjects and Methods. A cross-sectional survey and a randomized controlled trial have been conducted. For the cross-sectional survey, total 307 HIV negative MSM who have RP have been recruited by three ways. Face to face interview has been conducted to participants. Based on the associated factors found in the cross-sectional survey, interventions including video, education leaflets and enhanced counseling contents have been tailored to RP among MSMRP. For the randomized controlled trial, total 336 MSMRP have been recruited and randomly assigned 169 subjects to the Intervention Group in which participants have been given enhanced VCT plus an audio-visual and four leaflets components and 167 subjects to the Control Group in which participants have been given only standard-of-care VCT at the baseline. Evaluation was conducted at Month 3 and 6. Statistical methods such as descriptive analyses, Chi-square test and logistic regression were used in this study. / Results. The results have been found were the prevalence of UAI with RP among MSMRP was 52.4%, and the Theory of Planned Behavior (TPB) related cognitions, trust, intimacy, depression and anxiety were associated with UAI with RP among MSMRP. In the RCT study, participants in the Intervention Group had less UAI (36.1% vs. 49.1%) than those of the Control Group at Month 3. / Conclusions. This study showed a high prevalence of UAI among MSMRP, whilst trust, intimacy and cognitive factors were associated with UAI with RP. The efficacy of Enhanced VCT tailored to RP has been identified. The acceptability and feasibility of the tailored intervention were demonstrated. The Role of Mental Health Counselors in Public Schools

Youth living in the United States are experiencing increasing rates of mental health issues (New Freedom Commission on Mental Health, 2003; U.S. Department of Education, 2006) and are less likely to receive mental health services (U.S.DHHS, 2009). Children and adolescent mental health services are fragmented, often times ineffective, and difficult to obtain (Brown, 2006; Center for Mental Health in Schools, 2008; Jacob, 2008). Children of color and children living in poverty (ASCA, 2009; Panigua, 2005; Shavers, 2013; and Vera, Buhin, & Shin, 2006), are more likely than their peers to experience mental health disorders and less likely to receive outside mental health services (U.S. Department of Health and Human Services,1999; U.S. National Institute of Mental Health, 2001). Professional school counselors, play a significant role in identifying, meeting and connecting students in need of additional mental health services (ASCA, 2012; Jacob, 2008) and are advised to collaborate with community agencies to better meet the mental health needs of all their students (ASCA, 2009; ASCA, 2012). If left untreated, mental health issues can have a direct impact on students’ learning and academic performance (Adelman & Taylor, 2006). In response, there have been many recent initiatives to promote mental health in schools, including H.R. 628: Mental Health in Schools Act currently being considered by a United States congressional committee. School-based mental health counseling programs (SBMHCPs) have been implemented to address the fragmented mental health delivery system for children and adolescents needing mental health services (Center for Mental Health in Schools, 2003; New Freedom Commission on Mental Health, 2003; & Surgeon General Report on Mental Health, 1999). There is limited research about SBMHCPs from the perspective of those who have been addressing the mental health needs of students for decades—professional school counselors and professional school counselor educators. This qualitative study examined professional school counselors’ and counselor educators’ experiences working collaboratively with school-based mental health counselors. Seventeen school counselors (n=17) and five (n=5) counselor educators participated in a structured online questionnaire. Phenomenological data analysis methods were used to analyze the results (Hays & Woods, 2011; Moustakas, 1994). Results describe professional school counselor and counselor educator’s perceptions about school-based mental health programs, the roles of professional school counselors and school-based mental health counselors, training recommendations, and funding issues related to school-based mental health programs.
