Programa de intervenção interdisciplinar em hábitos de vida em indivíduos de risco cardiometabólico: análise de fatores dietéticos associados à melhora do metabolismo glicídico / Interdisciplinary intervention program on lifestyle in individuals at cardiometabolic risk: analysis of dietary factors associated with improvement in glucose metabolism

Introdução: Doenças cardiometabólicas são problemas de saúde pública mundial com repercussões sociais e econômicas. Dieta inadequada é apontada como um dos principais fatores de risco modificáveis, de importância central na prevenção destas doenças. Apesar da constatação de que certos nutrientes se associam a doenças, ainda existem dúvidas quanto às relações causais. Objetivos: 1) Revisar a participação dos alimentos e nutrientes na gênese e controle de doenças crônicas não-transmissíveis integrantes da síndrome metabólica (manuscrito 1); 2) Descrever de forma detalhada a estrutura do programa de intervenção interdisciplinar em hábitos de vida, desenvolvido para usuários da rede pública de saúde do município de São Paulo (manuscrito 2); 3) Investigar o efeito de mudanças em fatores dietéticos sobre o metabolismo glicídico, decorrentes de intervenção em hábitos de vida em indivíduos de risco cardiometabólico da rede pública de saúde (manuscrito 3). Métodos: Foram incluídos adultos com diagnóstico de pré-diabetes ou síndrome metabólica sem diabetes em ensaio clínico aleatorizado envolvendo dois tipos de intervenções de 18 meses: tradicional e intensiva. A intervenção tradicional consistiu de consultas trimestrais com endocrinologista; na intensiva, além dos atendimentos médicos, os indivíduos participaram de sessões psicoeducativas em grupo com equipe multiprofissional. As orientações dietéticas visaram à adequação do consumo de gorduras totais (redução de saturadas e trans e elevação de insaturadas) e aumento na ingestão de fibras. As variáveis coletadas nos momentos pré- e pós-intervenção incluíram dados sócio-demográficos, antropométricos, dietéticos e bioquímicos (glicemia de jejum e póssobrecarga, perfil lipídico, insulina, adiponectina, proteína C reativa, IL-6, TNF-, apolipoproteína A1 e B). A dieta foi avaliada por recordatórios de 24 horas, processados pelo Nutrition Data System software. Foram empregados os testes: t de Student, coeficientes de correlações, ANOVA com p de tendência e análises de regressão múltipla para identificar fatores dietéticos associados à melhora do metabolismo glicídico. Resultados: O manuscrito 1 consiste de um capítulo de livro (Nutrição em Saúde Coletiva) abordando aspectos epidemiológicos de doenças do espectro da síndrome metabólica com ênfase em fatores alimentares e estado nutricional. O manuscrito 2 detalha a estrutura, a equipe integrante, as orientações preconizadas e as principais estratégias psicoeducativas utilizadas no programa de intervenção em hábitos de vida desenvolvido (artigo metodológico submetido e website). Análises do manuscrito 3 mostraram que, ao final da intervenção, o aumento na ingestão de ácidos graxos monoinsaturados e fibras solúveis associaramse de forma independente à redução da glicemia de jejum e os primeiros também à melhora da glicemia pós-sobrecarga. Ajustes por medidas antropométricas não alteraram os resultados, o que ocorreu, porém, após inclusão da variável mudança na insulina nos modelos (artigo original). Conclusão: Justifica-se divulgar programas de intervenção como o aqui desenvolvido considerando-se o atual cenário das doenças cardiometabólicas na atualidade. No presente estudo, o aumento na ingestão de ácidos graxos monoinsaturados e de fibras solúveis promoveu benefícios no metabolismo glicídico, independente da adiposidade, ao final da intervenção no estilo de vida. Possíveis mecanismos mediadores destes processos podem incluir principalmente a melhora da sensibilidade à insulina. Dessa forma, nossos achados sugerem fortemente que um aumento factível no consumo destes nutrientes deva ser estimulado como estratégia para a proteção do metabolismo glicídico em indivíduos de risco cardiometabólico / Introduction: Cardiometabolic diseases are global public health problems with social and economic repercussions. Unhealthy diet is seen as one of the main modifiable risk factors, being of central importance for the prevention of these diseases. Despite the evidence that certain nutrients are associated with diseases, causal relationships are still under discussion. Objectives: 1) To review the involvement of foods and nutrients in the genesis and control of non-communicable chronic diseases, that comprise the metabolic syndrome (manuscript 1); 2) To describe in detail the structure of the interdisciplinary intervention program on lifestyle, developed for costumers of public health system of Sao Paulo city (manuscript 2); 3) To investigate the impact of changes in dietary factors on glucose metabolism, induced by a lifestyle intervention in individuals at cardiometabolic risk attended by the public health system (manuscript 3). Methods: Adults with prediabetes or metabolic syndrome without diabetes were included in a randomized clinical trial involving two types of 18-month interventions: a traditional or an intensive one. The traditional intervention consisted of quarterly consultations with an endocrinologist, while in the intensive participants also attended psychoeducational group sessions with a multidisciplinary team, in addition to the medical visits. The dietary recommendations aimed at the adequate intake of fat (reduction of saturated and trans and increase of unsaturated fatty acids) and increase in fiber intake. Socio-demographic, anthropometric, dietary and biochemical (fasting and post-load plasma glucose, lipid profile, insulin, adiponectin, C-reactive protein, IL-6, TNF-, apolipoprotein A1 and B) data were collected at baseline and after 18-month of follow-up. Diet was assessed by 24-h dietary recalls, processed by Nutrition Data System software. Student t test, correlation coefficients, ANOVA with p for trend and multiple linear regressions analyses were employed to identify dietary factors associated with glucose metabolism improvement. Results: Manuscript 1 consists of a book chapter (Nutrition in Public Health) about epidemiological aspects of diseases of the spectrum of metabolic syndrome with emphasis on dietary factors and nutritional status. Manuscript 2 details the structure, professional team, recommendations and the main psychoeducative strategies used in the lifestyle intervention program developed (methodological article submitted and website). Analyses of manuscript 3 showed that the intervention-induced increase in monounsaturated fatty acids and soluble fiber intake were independently associated with reduction on fasting plasma glucose and the former also with 2h-plasma glucose improvement. Adjustment for anthropometric measurements did not change these results but did after including change in insulin in the models (original article). Conclusion: Intervention programs such as the developed herein should be reported taking into consideration the current scenario of cardiometabolic diseases. In the present study, increases in monounsaturated fatty acids and soluble fiber intake promoted benefits in glucose metabolism, independent of adiposity, at the end of the lifestyle intervention. Possible mechanisms mediating these processes may mainly include improvement in insulin sensitivity. Therefore, our findings strongly suggest that a feasible increase in consumption of these nutrients should be encouraged as a strategy for the protection of glucose metabolism in individuals at cardiometabolic risk

The goal of the thesis is to develop a business plan for an instant natural food replacement operating on the Czech market, analyse the potential and feasibility of this business idea and calculate the financial estimates of the business. In the first part of the thesis, theoretical background on business planning is mapped with special focus on recent developments such as canvas business model, lean canvas and lean startup approach. This is followed by business plan structure which serves as a base for the practical part. Last topic of the theoretical part summarizes current trends in the food industry and especially market with functional food, dietary supplements, powdered food replacement, and anticipated future developments. The practical part of the thesis is constituted by the business plan for the startup selling natural powdered meal-replacement on the Czech market. The goal of this part is to verify the business model by conducting a customer survey, assess the market potential, develop the company strategy and analyse its feasibility based on the financial model for potential use by external stakeholders.

Estos tres puntos de venta son cercanos a las estaciones del metropolitano : Estación Central, Canaval y Moreyra y Angamos. Los objetivos de esta jugería son remarcar los beneficios reconocidos de los jugos naturales y poner a disposición de los clientes productos saludables y de calidad de manera rápida. Los clientes se muestran cada vez más interesados por una alimentación saludable ; sin embargo, no siempre disponen del tiempo necesario para preparar sus alimentos. El Instituto Nacional de Estadística e Informática (2014) nos ha mostrado que la población de Lima Metropolitana tiende a comer fuera de casa. These three points of sales are near to the metro stations: Estación Central, Carnaval y Moreira and Angamos. The main objectives of this juice bar are to emphasize the benefits of natural juices and to provide to its customers healthy and quality products in short time. Clients seems to be more and more interested in healthy food habits, however, for some reasons, they do not have the time to prepare their meals. The National Institute of Statistics and Informatics (2014) has shown that the population of the city of Lima tends to eat out. Datum (2017) has revealed that there is a part of the population that understands that having a healthy life, a healthy diet is a must. And, Kantar Worldpanel (2019) pointed out that Peruvians recognize eating fruits and vegetables as a key elements of a healthy diet. According to the economic and financial evaluation, the project will have a Economic NPV of S/. 188,397.03 and an economic IRR of 87 %. Title: The effect of an active lifestyle for a man and a woman in their middle ages Objectives: The aim is to diagnose the lifestyle of a man and a woman in their middle ages, and on the basis of this diagnosis, determine invetrevention, both physical and dietary. When the intervention has been finished, the differences in body composition of both individuals will be determined. Methods: Analysis of literature as well as montitoring and measurment was used in this diploma thesis. The target group consists of male and female proband measured at the age of 51 and 47 years. An online application called Sebekoučink on stobklub.cz was used to monitor both physical acitivity and energy intake. Heart rate monitors Polar RS400 were used to determine the intensity of physical activity. The complete picture of the physical acitivy of individual was measured using pedometers OMRON Walking style Pro. Body composition was determined by bioelectrical device B.I.A. 2000-M. Results: The monitoring of probands showed signs of sedentary lifestyles. Significantly excessive caloric intake were also found in the male proband. With a six week intervention, there were changes in body composition, which brought loss of weight 10.4 kg for male proband and 4.2 kg for female proband. Key words: physical activity, health,...

ABSTRACT: The topic of my dissertation is orthorexia nervosa, one of the modern eating disorders, and the impact of a Czech school on its development. The aim of this dissertation is to identify what knowledge about orthorexia issues the secondary medical school students have, what is their opinion on eating disorders, and to suggest a sample lesson focusing on this problem. To prove this fact, a quantitative research through a questionnaire has been used. This research was implemented in selected secondary medical schools and grammar schools. The results clearly indicate that students do not have sufficient knowledge about orthorexia and that the difference in the knowledge of medical students at secondary schools and grammar schools is negligible, although the composition of their diet is more concerned with secondary medical school students. In view of the fact of these and other findings, a draft of a sample lesson focusing on the issue of eating disorders and orthorexia has been developed. The diploma thesis deals with the issue of a healthy lifestyle and nutrition in connection with the prevention of civilization diseases. It describes the causes of these diseases, the risks and possibilities of prevention. It also draws attention to the principles of correct eating habits, the pitfalls of various diets, the consequences of overeating, the danger of overweight and obesity, the negative influence of stress on our health, and ways and principles of healthy weight reduction. It summarizes advices and recommendations of professionals for optimal composition of our diet, a change of lifestyle, and the significance of correct breathing, relaxation, exercise, positive thinking and sleep for health. In the theoretical part of the diploma thesis there was described food of cereal character including school nutrition programmes and their representation in the diet of basic school pupils and university students. The interview focused on the representation of cereal products and their importance in the diet is described in the research part of this diploma thesis. The interview was made in the groups of pupils in higher basic schools and university students. When evaluating the questionnaire survey, there were assessed the quantity, individual types and quality of commonly consumed cereal products and the level of eating habits of respondents in accordance with the principles of good nutrition. After evaluation of the interviews it was found that basic school pupils have worse all-day eating habits than university students. It was confirmed mostly by the answers of the questions relating to breakfast, mid-afternoon snacks and total number of meals during a day. University students prefer wholegrain products more than basic school pupils. Pupils of higher basic schools have lower knowledge level of cereals than university students.

Content of the thesis includes research of service design and innovation process, study based on application of the design service approaches, introduction of the study outputs, actual project of a new service design and recommendations. Following methods and techniques were used in research: persona, contextual interview, service safari, benchmarking, service blueprints, empathy map, product box. The thesis should contribute to the theoretical determination of service design and its following aplication in the design of a specific service. Final outcome of the study is a service proposal for a canteen in Lycée Français de Prague.

Introducción: Este proyecto se llevó a cabo en el distrito de Surco, Lima durante el periodo de mayo a junio de 2020 durante la pandemia de Covid19, en una vivienda habitada por 5 personas donde una de ellas actuó como promotor de salud. Este distrito tiene una población total de 360 400 habitantes y la esperanza de vida al nacer es de 79.45 años; según el MINSA existe una prevalencia de caries en el Perú del 90%. Descripción del proyecto: Partiendo de un análisis de involucrados se identificó que los integrantes de la familia tenían una higiene oral regular y una dieta desbalanceada, por lo que se establecieron dos objetivos específicos para promover hábitos de salud oral y de dieta saludable. Se desarrollaron sesiones educativas, talleres demostrativos, control de dieta y cepillado, entre otras actividades. Se utilizaron recursos virtuales y supervisión directa. Resultados: Se realizó un monitoreo de los avances y una evaluación final de resultados. El 100% de los integrantes incrementó sus conocimientos sobre salud oral y dieta. La frecuencia de cepillado aumentó de 2 a 3 veces diarias, la eficacia para la técnica de cepillado fue del 100% y la efectividad en cuanto a la distribución de los alimentos en la dieta fue del 100%. Conclusión: El proyecto fue efectivo para la promoción de la salud oral y dieta en un entorno familiar resultando exitoso en medio del contexto por la pandemia del Covid19. / Introduction: This project was carried out in the district of Surco, Lima during the period from May to June 2020 during the Covid19 pandemic, in a house inhabited by 5 persons where one of them acted as a health promoter. This district has a total population of 360 400 inhabitants and life expectancy at birth is 79.45 years; According to the MINSA, there is a 90% prevalence of caries in Peru. Project description: Based on an analysis of those involved, it was identified that the family members had regular oral hygiene and an unbalanced diet, therefore, two specific objectives were established to promote oral health habits and a healthy diet. Educational sessions, demonstration workshops, diet control and brushing, among other activities, were developed. Virtual resources and direct supervision were used. Results: Progress monitoring and a final evaluation of the results are carried out. 100% of the members increased their knowledge of oral health and diet. The frequency of brushing increased from 2 to 3 times daily, the effectiveness for the brushing technique was 100% and the effectiveness in terms of distribution of food in the diet was 100%. Conclusion: The Project was effective for the promotion of oral health and diet in a family environment, being successful in the context of the Covid19 pandemic. / Trabajo de investigación
