Como uma alternativa para os portadores de deficiência auditiva neurossensorial profunda bilateral, surgiu o Implante Coclear - que envolve ato cirúrgico para a implantação de eletrodos na cóclea - que permite ao deficiente auditivo, uma audição útil, que viabilize ou melhore sua comunicação. Esta pesquisa refere-se aos usuários do Implante coclear, implantados no Centro de Pesquisas Audiológicas (CPA) do Hospital de Reabilitação de Anomalias Craniofaciais (HRAC) da Universidade de São Paulo (USP). Até junho de 2000, eram 163. O objetivo deste trabalho é traçar um perfil desse universo, chegando às repercussões do Implante Coclear (IC) na vida de seus usuários, por meio de uma amostra de 34 usuários. O que se constatou foi uma concentração de pacientes procedentes da região sudeste - mais especificamente Estado de São Paulo - pertencentes às classes menos favorecidas, que têm como expectativa inicial o Implante Coclear (IC). As demais categorias apresentam diferenças que, no entanto, não transparecem nas repercussões do IC na vida de seus usuários. As repercussões pesquisadas foram em nível de satisfação, mudanças na vida diária - no caso de crianças, mudança na vida dos pais também - e preocupações com relação ao IC. A maioria dos sujeitos da amostra (97.0%) esta satisfeita com o IC e concorda que, em virtude dele, ocorreram mudanças em suas vidas - a maioria positiva - sem, no entanto, descartar as preocupações decorrentes do uso do IC, que giram em torno dos dispositivos interno e externos (fragilidade e manutenção, em virtude dos custos). A realidade constatada é objeto de intervenção da Assistente Social, que... / The hearing loss, due to its high incidence has received special attention from the scientific community in the development and improvement of devices adressed to the hearing impaired people. The Cochlear Implant -surgery envolving electrode implantation inside the cochlea -an alternative to those who have bilateral profound sensorineural hearing loss. It provides to the hearing impaired people a useful hearing which improves their communication. This research is about Cochlear Implant (CI) users, who underwent surgery in the Centro de Pesquisas Audiológicas (CPA) from the Hospital de Reabilitação de Anomalias Craniofaciais (HRAC) of University of São Paulo. Until June/2000 totalized 163 cases. The aim of this study is to delineate the profile of those patients (the total) and to get to the CI repercussions in their lives, a sample of 34 cases was analysed. It was verified that there is a concentration of patients coming from the Southeast region - more specifically, from São Paulo state - belonging to the lowers classes and have the CI as a initial expectation. The others verified categories show differences that neverthless, do not appear in the CI repercussions in the users lives. The researched repercussions were related to satisfaction, changes in the daily life - with the children, changes in the parents lives too - and concerns about the CI. The majority of the subjects from the sample (97.0%) is satisfied with the CI and report positive changes in their lives without dismissing their concerns resulting from the CI use, that is about the external and internal devices (its fragility and maintenance, due the cost). The first article, accepted in Revista da Escola de Enfermagem da USP, has as title Literature Revision about the attendance of the deaf patient by health professionals, with emphasis on the communication between deaf patient and health professional. In the second one, accepted to publish in Revista Brasileira de Otorrinolaringologia and entitled Relation between deaf patient and the doctor, the legal and sociocultural aspects of the relation between deaf patient and the doctor were presented. The third article, submitted to Revista de Saúde Pública, with the title Meeting of deaf patient who uses sign language with health professionals, has as content the object of this dissertation and has as purpose to evaluate the meeting of the deaf patient who use sign language with health. It was developed in a qualitative approach, of descriptive-analytical nature. Eleven deaf people, who use the Brazilian Sign Language to communicate, including 8 males and 3 females, with ages between 19 and 36 years old, had participated of the research. For data collection a half-structured interview was used and it had questions that approach the expectations of the death person about the meeting with health professionals, the means of communication used, the presence of a Brazilian Sign Language interpreter as a mediator and what the person judges necessary to favor a good relation. The data were organized using the content analysis, proposed by Bardin, that allowed to identify three categories: Established Communication, Interpreter Presence and Lack of Autonomy. In the first category, two sub-categories had been distinguished: facilities and difficulties. As facilitator the visual resources stood out and as difficulties inefficiency in the communication, even reaching the extreme of feeling hindered to search assistance. Other difficulties were the written language and non capable professionals to attend. The category Interpreter Presence presents as eminence the complexity of this mediator. In some moments his presence is valued, while in others it turned to a barrier. In Lack of Autonomy, it was verified that their rights haven t been respected and they stay apart from the decisions taken mainly by the family. It followed that deaf patients face a lot of barriers in the meeting with health professional, since the appointment, the moment of the meeting, until the decurrent unfoldings. And in many stories it was verified that they can t solve their problems and they can t comprehend the reason of the attitudes taken by professionals, so they feel excluded and not respected in their autonomy. So an effective participation of the deaf patient in clinical meetings requires special attention from the health professionals. / Dissertação construída na modalidade artigos científicos e consta de três artigos, sendo o primeiro e o segundo de revisão sistemática da literatura sobre o tema do estudo e o terceiro tem por conteúdo a parte experimental da dissertação. O primeiro artigo, aceito pela Revista da Escola de Enfermagem da USP tem como título: Revisão de literatura sobre o atendimento ao paciente surdo pelos profissionais da saúde com ênfase na comunicação entre paciente surdo e profissional da saúde. No segundo, aceito pela Revista Brasileira de Otorrinolaringologia, intitulado Relação do paciente surdo com o médico, foram abordados os aspectos legais e socioculturais da relação do paciente surdo com o médico. O terceiro, submetido à Revista de Saúde Pública, com o título Encontro do paciente surdo que usa língua de sinais com os profissionais da saúde, que tem por conteúdo o objeto desta dissertação, cujo objetivo foi analisar o encontro entre pacientes surdos que utilizam a língua de sinais e os profissionais da saúde. Desenvolvido numa abordagem qualitativa de natureza descritivo-exploratória. Participaram 11 pessoas surdas, que utilizam a Língua Brasileira de Sinais para se comunicar, sendo 8 do sexo masculino e 3 do sexo feminino, com idades entre 19 e 36 anos. Para a coleta de dados foi utilizada uma entrevista semi-estruturada, contendo questões que abrangiam as expectativas da pessoa surda quanto ao encontro com os profissionais da saúde, os meios de comunicação utilizados, a presença de um intérprete da Língua Brasileira de Sinais como mediador da relaçao e o que julga necessário para favorecer uma boa relação. Os dados foram organizados utilizando a técnica de análise de conteúdo, proposta por Bardin, que permitiu identificar três categorias: Comunicação Estabelecida, Presença do Intérprete e Falta de Autonomia. Na primeira categoria destacaram-se duas subcategorias: facilidades e dificuldades. Como facilitador sugiram os recursos visuais e, como dificultador, ineficiência na comunicação, podendo chegar ao extremo de sentirem-se impedidos de buscar assistência. Outras dificuldades apontadas foram a língua escrita e profissionais não capacitados para atendê-los. A categoria Presença do Intérprete apresentou como destaque a complexidade deste mediador. Em alguns momentos sua presença é valorizada, enquanto, em outros, torna-se uma barreira. Na Falta de Autonomia, verificou-se que os direitos dos pacientes surdos não estão sendo respeitados, ficando à parte nas decisões tomadas principalmente pela família. Concluiu-se que os pacientes surdos deparam-se com muitas barreiras no encontro com profissionais da saúde, desde o agendamento, o momento do encontro, até os desdobramentos decorrentes e, em muitos relatos verificou-se que eles não conseguem resolver seus problemas, não compreendem o porquê das atitudes tomadas pelos profissionais, sentindo-se excluídos, não respeitados em sua autonomia. Portanto, a participação efetiva dos pacientes surdos nos encontros clínicos necessita especial atenção por parte dos profissionais da saúde.

My diploma work applies to progress of communication with a child with partial deafness at an early age. Theoretical part in its first chapter applies with the problém of hearing disability and basic subjectś terminology. Second chapter deal with communication of hearing-impaired children and shows partial communication systems. Third chapter introduces consulting institutions, which are concerned to hearing impairment. Practical part is focused to one concrete longitudinal study of a child with difficult hearing impairment. The study consist of observation the speech development and sign language of this child in his early age. Part of this work are also flow charts and graphs where you can observe the development of the vocabulary signs and spoken words. From these resource materials we can see very well the boyś development in the whole last year, where prevail spoken words, which he refills with sign Czech language, which helps to his parents to get the feedback and check out boyś understanding, but also in the communication in time, when the boy cannot have any compensatory aid. Her eis the confirmation of suitability of combining signs with spoken words for quick and general childś development in the early age. This dissertation titled Teaching English to Deaf and Hard-of-hearing University Students aims to portray a picture of the situation of teaching English to university students, more specifically of those studying at Charles University in Prague. The work is divided into two main parts. The first part brings general insight, also from linguistic perspective, into language competences and language acquisition by the hearing impaired. It describes variables and possible variables affecting spoken language acquisition by this minority. It gives basic overview of different levels of hearing loss on the grouds of medicine assessment of hearing loss. The dissertiation stresses out the neccessity of educating these students with respect to linguistic research and findings. Moreover, it emphasizes the needs to interweave linguistics and pedagogy and benefits of linguistics research and methods such as error analysis and contrastive analysis for enhancement of language productive skills of hearing impaired students. The second part of the dissertation is devoted to the research of English language teaching to hearing impaired students at Charles University in Prague. This thesis is focused on learning texts reading comprehension of pupils in primary schools for hearing impairment pupils in comparison with primary schools pupils of mainstream education, in connection with the textbooks. The whole issue is put into the theoretical framework of reading literacy and teaching texts information. Specifically, there is a reading literacy in the concept of international studies, and also a brief overview of the findings drawn from the textbooks, their functions, research, evaluation and comparison. The comparison of the concept of primary school textbooks for the hearing impaired with textbooks for mainstream schools through standardized assessment tools is included too. In this thesis, there is a research of learning texts comprehension for pupils from 3rd to 9th (or 10th) years of primary schools for the hearing impaired and analogous grades of primary school pupils of mainstream education. The research design is based on Progress in International Reading Literacy Study (PIRLS), one part of this research is translated into Czech sign language.

Objetivo: este estudo teve por objetivo verificar a eficácia de um procedimento para a identificação da deficiência auditiva em crianças pré escolares de uma comunidade de baixa renda por pessoas não especialistas. Casuística e métodos: funcionários da creche e da unidade de saúde foram treinados para aplicarem um questionário para triagem auditiva nos pais de 224 crianças de 3 a 6 anos de idade, que foram submetidas à triagem audiométrica e imitanciométrica. Resultados: verificou-se alta concordância na coleta de dados por meio do questionário entre a pesquisadora e os não especialistas. O questionário apresentou melhor sensibilidade e pior especificidade. Os não especialistas relataram aprendizado com a pesquisa e conscientização sobre a importância da audição. Conclusão: é possível treinar agentes comunitários a identificarem a deficiência auditiva em suas comunidades com instrumentos de baixo custo. / Aim: this study aimed to access the efficacy of a low cost instrument used by non professionals to identify hearing loss in pre-school children in a low income community. Methods: employees of a nursery school and a health unit were trained to use a hearing screening questionnaire with parents of 224 children aged 3 to 6 years. The children underwent audiological screening. Results: the results showed that the non - specialists were capable of reproducing the evaluation of the professional regarding the use of the questionnaire. The questionnaire presented better sensitivity than specificity. The non - specialists reported learning and awareness of the importance of hearing in children. Conclusion: the questionnaire offers a low cost option for hearing screening, specially if it is administered by non professionals resources

Uit die semi-gestruktureerde onderhoude is verskeie temas geidentifiseer wat betrekking het op die hantering van die dowe of hardhorende kind in die gesin. Praktiese riglyne is vir ouers daargestel ten op sigte van die dowe of hardhorende kind se kommunikasie, gesinslewe,die skool, asook sy/haar emosionele en sosiale funksionering. / Although previous research has been done abouth the hard-hearing child, most of the available research concentrates on early identification of hearing lost, placement and communication as well as to cope with hearing loss within the school environment. The majority of the literature which is currently available concerning guidelines for the deaf or hard-hearing children within the family group, is outdated. A phenomenological study has been done. By means of semi-structured interviews practical guidelines have been collected. Several themes were identified in the course of these interviews, namely: communication by the child with hearing loss, the effect of the hearing loss on the family members, the impact of hearing loss in the school environment and the implications of hearing loss on the child's emotional and social functioning. Guidelines were suggested for the practical management of deaf and hard-hearing children concerning communication, family life, scool and emotional and social functioning. / Educational Studies / M. This thesis is focused on problems of teaching in school subject Czech Language in 1 - 3 grade of elementary schools for hearing impaired. The whole issue is put into the theoretical framework, which delimit specify of hearing impaired pupils education and the goals which this pupils should achieve in elementary education and primarily in language education too. Special attention is paid to problems of teaching reading and writing - ways to acquiring reading literacy in hearing impaired pupils. Presented are methods of teaching reading and writing and I am thinking about their applicability in hearing impaired pupils education. Is given an idea of teaching in school subject Czech Language in Elementary schools for hearing impaired in history (1948 - 1990), in detail I analyze teaching materials used in that time. Current situation of teaching in school subject Czech Language is describe on the grounds of research probe realized in school year 2011/2012 on four selected elementary schools for hearing impaired in the Czech Republic.
