SELEÇÃO E ADAPTAÇÃO DE PRÓTESES AUDITIVAS EM IDOSOS: CARACTERÍSTICAS INDIVIDUAIS, EXPECTATIVAS E SUCESSO / SELECTING AND FITTING OF HEARING AIDS IN ELDERLY: INDIVIDUAL CHARACTERISTICS, EXPECTATIONS AND SUCCESS

This study had the objective to analyze some of the aspects involved in the process of selecting and fitting hearing aids, order to verify the possible influence on the expectations of elderly patients as to the results with the use of amplification, as well as the relationship of these expectations with the success of auditory reh abilitation. Were considered as criteria for inclusion in the sample: elderly individuals, patie nts with bilateral sensorineural hearing loss, symmetric, mild to moderately severe degree, with i ndication of using hearing aids bilaterally, without previous experience as a user of it and abs ence of any cognitive impairment. The sample was constituted of 16 subjects, aged between 64 and 94 years old. The evaluation was performed at two different times: 15 days befor e and 15 days after the fitting of hearing aids. Subjects responded, firstly, to an anamnesis, which provided information regarding age, occupation and educational level of subjects studie d, beyond the motivation for the use of hearing aids and presence or absence of a companion . For analysis of the expectations, was applied the Questionnaire for expectations evaluat ion of the adult / elderly, new user of the hearing aids , followed by the Hearing Handicap Inv entory for the Elderly - Screening Version HHIE/S, to measure the individual's participation constraint with and without the use of hearing aids. The overall benefit with the use of h earing aids was evaluated using the International Outcome Inventory for Hearing Aids IOI-HA. The communicative performance was evaluated by determining the Percentage Index S entence Recognition in Quiet (PISRQ), using the Portuguese Sentences Lists test PSL, wi th and without the use of hearing aids. The data were analyzed using parametric tests and n onparametric, with p≤0,05. Were obtained positive expectations about the results wi th the use of hearing aids to all individuals in the sample. The restriction of participation was classified as moderate for 14 individuals (87.5%) and below this level for the remaining indi viduals (12.5%). The average of overall benefit obtained was 84%, showing a correlation sta tistically significant and negative, with the results of the assessment expectations. The sub jective evaluation of the benefit, by comparing the results of HHIE/S pre and post-adapta tion, showed satisfactory results for the entire sample. The same was not observed in the obj ective assessment, comparing the PISRQs under the same conditions. It was concluded that: the expectation about the results with the use of hearing aids has been positive for the entire sample, independent of other variables considered. Also, the more positive expec tations presented, smaller were the chances of the individual to report the overall ben efit with the use of hearing aids. Nevertheless, there was some benefit in this regard . Objectively, the benefit was not observed in all individuals evaluated considering t he speech tests, however, subjectively, it was observed throughout the sample. Thus, the expec tations were not influenced by aspects of the process of selection and fitting of hearing aids. Already the influence of expectations on the success of the process occurred only in rela tion to the overall benefit evaluated. / O presente estudo teve como objetivo analisar alguns dos aspectos envolvidos no processo de seleção e adaptação de próteses auditivas, visando verificar a sua possível influência sobre as expectativas dos pacientes idosos quanto aos resultados com o uso da amplificação, bem como a relação destas com o sucesso da reabilitação auditiva. Foram considerados como critérios de inclusão da amostra: indivíduos idosos, portadores de perda auditiva neurossensorial bilateral, simétrica, de grau leve à moderadamente-severo, com indicação de uso de próteses auditivas bilateralmente, sem experiência prévia como usuário de tal e ausência de quaisquer comprometimentos cognitivos. A amostra foi constituída de 16 indivíduos, com idades entre 64 e 94 anos. A avaliação foi realizada em dois momentos distintos: 15 dias antes e 15 após a adaptação de próteses auditivas. Os indivíduos responderam, primeiramente, a uma anamnese, a qual forneceu informações referentes à idade, ocupação e escolaridade dos sujeitos estudados, além da motivação para o uso de próteses auditivas e presença ou não de acompanhante. Para análise das expectativas foi aplicado o Questionário para avaliação das expectativas do indivíduo adulto/idoso, novo usuário de próteses auditivas , seguido do Hearing Handicap Inventory for the Elderly Screening Version HHIE- S, para mensuração da restrição de participação do indivíduo sem e com o uso de próteses auditivas. O benefício global com o uso de próteses auditivas, foi avaliado por meio do Questionário Internacional Aparelho de Amplificação Sonora Individual QI-AASI. O desempenho comunicativo foi avaliado pela determinação do Índice Percentual de Reconhecimento de Sentenças no Silêncio IPRSS, utilizando-se o Teste Listas de Sentenças em Português LSP, sem e com o uso de próteses auditivas. Os dados foram analisados, utilizando-se testes paramétricos e não paramétricos, com p≤0,05. Obtiveram-se expectativas positivas, quanto aos resultados com o uso de próteses auditivas, para todos os indivíduos da amostra. A restrição de participação foi classificada como moderada para 14 indivíduos (87,5%) e de grau inferior a esse para os demais indivíduos (12,5%). O benefício global médio obtido foi de 84%, apresentando correlação estatisticamente significante e negativa, com os resultados obtidos na avaliação das expectativas. A avaliação subjetiva do benefício, pela comparação dos resultados do HHIE/S pré e pós-adaptação, evidenciou resultados satisfatórios para toda a amostra. O mesmo não foi observado na avaliação objetiva, comparando-se os IPRSSs sob as mesmas condições. Concluiu-se que: a expectativa acerca dos resultados com o uso de próteses auditivas foi positiva para toda a amostra estudada, independente das demais variáveis consideradas. Também, quanto mais positivas as expectativas apresentadas, menores foram as chances de o indivíduo relatar benefício global com o uso das próteses auditivas. Apesar disso, verificou-se algum benefício nesse sentido. Objetivamente, não foi evidenciado benefício em todos os indivíduos avaliados considerando-se os testes de fala, no entanto, subjetivamente, ele foi observado em toda a amostra. Assim, as expectativas não sofreram influência dos aspectos envolvidos no processo de seleção e adaptação de próteses auditivas. Já a influência dessa expectativa sobre o sucesso do processo ocorreu somente em relação ao benefício global avaliado.

When it is addressed, the types of disabilities are treated individually. However, it is noteworthy that public spaces and services are used by all categories simultaneously. Due to that fact, the main purpose of this work is to analyze the accessibility for three types of disabilities: physical, hearing and visual. This was done in the facilities and activities offered by the Parque Natural Municipal Victório Siquierolli, located in Uberlândia / Minas Gerais. This is a relevant place in non-formal environmental education, offering activities for local schools, researchers and society in general; which also supports the research and extension activities of the Federal University of Uberlândia. Although the location has some access for wheelchair users, there is no specific accessibility project for other disabilities. The research methodology was divided into literature review and documentary research and field research. Field research included direct observation through photographic registration of the park structures using as a gathering instrument, a protocol based on Brazilian Standard NBR 9050/2015, profiling the mediators working in the park and the from the perspective of three associations for each type of disability of this study, through semi-structured interviews. With the results, we can notice that the park does not have all the structures for accessibility that NBR 9050/2015 standard requires, and the present structures, such as ramps, signs, handicap restrooms and free space for movement are not entirely suitable, with a view only to assist people with physical disabilities, more specifically wheelchair users. So necessary adjustments should be done to benefit other disability categories cited in this study, in order to improve the structure of the park and as a consequence increase and diversify the visitors. / A acessibilidade de pessoas com deficiência como usuários de parques ainda é pouco abordada e, quando trabalhada, as deficiências são tratadas de forma isolada, sendo que os espaços e serviços públicos são utilizados por todas as demais categorias de deficiência simultaneamente. Decorrente disso, esse trabalho tem como objetivo principal analisar a acessibilidade para três tipos de deficiência: física, auditiva e visual nas dependências e atividades oferecidas pelo Parque Natural Municipal Victório Siquierolli, localizado em Uberlândia/MG. Trata-se de um local de relevância na educação ambiental não formal, oferecendo atividades para escolas do município, pesquisadores e a sociedade em geral; que também serve de apoio para as ações de pesquisa e extensão da Universidade Federal de Uberlândia. Apesar de o local possuir alguns acessos para cadeirantes, não existe um projeto específico voltado para a acessibilidade incluindo as demais deficiências. A metodologia da pesquisa foi dividida em pesquisa bibliográfica e documental e pesquisa de campo. A pesquisa de campo incluiu a observação direta por meio de registros fotográficos das estruturas do parque utilizando como instrumento de coleta um protocolo elaborado a partir da NBR 9050/2015, a caracterização do perfil dos mediadores que trabalham no parque e de três associações referentes a cada tipo de deficiência deste estudo, todas a partir da realização de entrevistas semiestruturadas. Com os resultados obtidos, percebe-se que o parque não apresenta todas as estruturas para acessibilidade que a norma NBR 9050 exige e as estruturas existentes, como rampas, placas, sanitário adaptado e espaço livre para deslocamento não estão totalmente adequadas, tendo em vista auxiliarem apenas as pessoas com deficiência física, mais especificadamente os cadeirantes. Logo, se fazem necessárias adaptações que possam beneficiar as demais categorias de deficiência abordadas nesse estudo, de forma a melhorar a estrutura do parque e consequentemente aumentar e diversificar os visitantes.

The presented diploma thesis deals with the means of communication (sign language, signed language, written language, finger alphabet, etc.) used by teachers in schools for pupils with hearing impairment in Prague and Munich. The work is interested in the topic of related educational approaches, which schools, or teachers use, and what means of communication they prefer. Using observation methods and questionnaires, it wants to find out what or who leads the educators to the chosen communication means and what they think about their own communication competencies. In addition, it compares the current situation of this issue at the elementary schools for the hearing impaired in Prague and Bavarian Munich. The first chapter focuses on which kind of pupils are attending schools for the hearing impaired and what are the possibilities for communication. The second chapter deals with approaches to the education of pupils with hearing impairment. In addition to basic approaches (oral, total, bilingual) describes the approach by American linguist D. A. Stewart, which was developed for the needs of the classroom. Influence de la cadence de stimulation sur la perception auditive : étude chez l'implanté cochléaire et chez le normo-entendant / Influence of stimulation rate on auditory perception in cochlear implant users and normal hearing listeners.

L’implant cochléaire (IC) est une prothèse auditive implantée dans l’oreille interne qui a pour fonction de restaurer le sens de l’audition aux personnes atteintes de surdité sévère à profonde lorsque les aides auditives classiques ne leur permettent plus une audition suffisante. Pour pouvoir transmettre les indices provenant des sons extérieurs, l’IC contourne une grande partie du système auditif défaillant et stimule directement les fibres du nerf auditif du patient. En pratique, les électrodes de l’implant émettent des trains d’impulsions électriques modulées en amplitude. Ces trains d’impulsions ont une fréquence de répétition (cadence) constante. Un problème limitant est que nous ne connaissons pas entièrement les effets physiologiques et perceptifs liés à l’augmentation de la cadence de stimulation. Le projet de recherche lié à la présente thèse s’inscrit dans l’optique d’améliorer le codage temporel des futurs ICs. Cette thèse a pour but de mieux comprendre l’influence de la cadence de la stimulation électrique sur la perception des sons avec, comme objectif fondamental, de caractériser les mécanismes physiologiques sollicités lors d’une stimulation électrique. La cadence de la stimulation affecte au moins trois dimensions perceptives distinctes qui sont la hauteur, la durée et la sonie. Les expériences s’intéressent à résoudre des problèmes précis concernant chacune de ces dimensions. Lorsque nous le pouvions, les expériences réalisées chez le sujet IC ont également été réalisées chez des sujets normo-entendants en utilisant des simulations acoustiques d’ICs. / Cochlear implants (CIs) are auditory prostheses directly inserted in the inner ear to partly restore the sense of audition to people suffering from severe to profound deafness. In order to convey environmental sounds, CIs bypass the peripheral auditory system and produce amplitude-modulated electrical pulses that stimulate auditory nerve fibres. In general, pulses are delivered with a constant frequency of occurrence (i.e. rate). Even if CI users reach good performances to understand speech, it roughly diminishes when they are placed more complex audiological environment.A potential problem is that stimulation rates are not the same among subjects while our knowledge concerning the physiological mechanisms provided by rate variations, is limited. This thesis aims to improve temporal coding of CIs by studying the perceptual influence of stimulation rate, and quantify the physiological mechanisms sought by an electrical stimulation. At least, stimulation rate acts on three perceptual dimensions which are pitch, sound duration and loudness. The present experiment solves specific problems concerning each dimension cited previously. In addition to electrical stimulations, acoustical simulations of a CI were conducted to compare normal hearing with hearing-impaired performances.

Orientador: José Walter Canôas / Resumo: A deficiência auditiva, em função da sua alta incidência, tem recebido da comunidade científica uma atenção especial na criação de dispositivos que viabilizem a (re)habilitação de seus portadores. Como uma alternativa para os portadores de deficiência auditiva neurossensorial profunda bilateral, surgiu o Implante Coclear - que envolve ato cirúrgico para a implantação de eletrodos na cóclea - que permite ao deficiente auditivo, uma audição útil, que viabilize ou melhore sua comunicação. Esta pesquisa refere-se aos usuários do Implante coclear, implantados no Centro de Pesquisas Audiológicas (CPA) do Hospital de Reabilitação de Anomalias Craniofaciais (HRAC) da Universidade de São Paulo (USP). Até junho de 2000, eram 163. O objetivo deste trabalho é traçar um perfil desse universo, chegando às repercussões do Implante Coclear (IC) na vida de seus usuários, por meio de uma amostra de 34 usuários. O que se constatou foi uma concentração de pacientes procedentes da região sudeste - mais especificamente Estado de São Paulo - pertencentes às classes menos favorecidas, que têm como expectativa inicial o Implante Coclear (IC). As demais categorias apresentam diferenças que, no entanto, não transparecem nas repercussões do IC na vida de seus usuários. As repercussões pesquisadas foram em nível de satisfação, mudanças na vida diária - no caso de crianças, mudança na vida dos pais também - e preocupações com relação ao IC. A maioria dos sujeitos da amostra (97.0%) esta satisfeita com o IC e concorda que, em virtude dele, ocorreram mudanças em suas vidas - a maioria positiva - sem, no entanto, descartar as preocupações decorrentes do uso do IC, que giram em torno dos dispositivos interno e externos (fragilidade e manutenção, em virtude dos custos). The Cochlear Implant -surgery envolving electrode implantation inside the cochlea -an alternative to those who have bilateral profound sensorineural hearing loss. It provides to the hearing impaired people a useful hearing which improves their communication. This research is about Cochlear Implant (CI) users, who underwent surgery in the Centro de Pesquisas Audiológicas (CPA) from the Hospital de Reabilitação de Anomalias Craniofaciais (HRAC) of University of São Paulo. Until June/2000 totalized 163 cases. The aim of this study is to delineate the profile of those patients (the total) and to get to the CI repercussions in their lives, a sample of 34 cases was analysed. It was verified that there is a concentration of patients coming from the Southeast region - more specifically, from São Paulo state - belonging to the lowers classes and have the CI as a initial expectation. The others verified categories show differences that neverthless, do not appear in the CI repercussions in the users lives. The researched repercussions were related to satisfaction, changes in the daily life - with the children, changes in the parents lives too - and concerns about the CI. The majority of the subjects from the sample (97.0%) is satisfied with the CI and report positive changes in their lives without dismissing their concerns resulting from the CI use, that is about the external and internal devices (its fragility and maintenance, due the cost). Integrating inclusive practices at Technical and Vocational Education and Training (TVET) colleges in Limpopo Province

Thesis (Ph.D. (Community and Continuing Education)) -- University of Limpopo, 2020 / Access and support for students with disabilities remain limited despite strong legislative and policy framework for addressing inclusivity in the education sector. Presently, TVET colleges encounter severe obstacles and hiccups in that they must improve the quality of education provided together with other compulsory obligations. As a result, TVET col-leges continue to encounter challenges as and when they try to improve the quality of education. In South Africa, the integration of people with disabilities has been an ongoing process since the advent of the democratic government. The study investigated the inte-gration of inclusive practices for students with disabilities at Technical and Vocational Edu-cation and Training (TVET) colleges. The study followed the qualitative research ap-proach. The purposive sampling technique was used to select participants. Twenty-seven participants took part in the study. Participants comprised campus man-agers, HoDs, student support officers, lecturers and students with disabilities. Two differ-ent methods to analyse data were used, namely, thematic categorisation (for interviews and observations) and content analysis (for documents). The study is premised on the social model of disability and Bronfenbrenner’s bioecological system theory to understand participants’ views and how they carry out inclusive practices in their lecture rooms. Data was collected using three techniques, namely, interviews, observations and document analysis. The findings of the study revealed the following: lecturers at TVET colleges are struggling to support students with disabilities due to lack of training on different types of disabilities; students with disabilities are given access to certain types of training based on the nature of their disability; students who experience mobility disabilities are faced with challenges of inaccessible of physical environments; the curriculum at TVET col-leges does not fully cater for a range of diverse needs of and students with impairments. The study recommends that: lecturers require training on different types of disabilities in order to support students with diverse learning needs; the involvement of disabled stu-dents in the planning of their support programmes; all stakeholders be encouraged to appreciate change and to contribute towards meaningful inclusive practices.

Hearing impaired toddlers present with a hearing loss, which distinguish them from toddlers with normal hearing. However, all toddlers have primary needs. Quality of life is improved by the fulfilment of needs, which influences the developmental process. These needs are fulfilled by the toddler, as well as by factors from the environment. A literature study was conducted as theoretical framework. This included theories regarding the environment and needs, protective and risk factors, and toddler development. In this qualitative study, data collection was conducted by semi-structured interviews and the compilation of ecomaps by parents of hearing impaired toddlers. Triangulation was used to verify and integrate data. Protective and risk factors were explored within the toddler, the family and the wider community. This study is exploratory and descriptive and applied research was used. Hearing impaired toddlers present with a hearing loss, which distinguish them from toddlers with normal hearing. However, all toddlers have primary needs. Quality of life is improved by the fulfilment of needs, which influences the developmental process. These needs are fulfilled by the toddler, as well as by factors from the environment. A literature study was conducted as theoretical framework. This included theories regarding the environment and needs, protective and risk factors, and toddler development. In this qualitative study, data collection was conducted by semi-structured interviews and the compilation of ecomaps by parents of hearing impaired toddlers. Triangulation was used to verify and integrate data. Protective and risk factors were explored within the toddler, the family and the wider community. This study is exploratory and descriptive and applied research was used. In this way conclusions and recommendations were made to provide appropriate support to parents of hearing impaired toddlers. / Social Work / M. Diac. (Spelterapie)
