Vårdpersonalens upplevelse av att vårda patienter som lever i hemlöshet : En litteraturöversikt / Caregivers experiences of caring for patients who live in homelessness : A literature review

Bakgrund: Begreppet hemlöshet innefattar flera situationer. Det är svårt att veta hur många hemlösa personer som befinner sig i Sverige då hemlöshet även innefattar de som lever utan uppehållstillstånd, papperslösa och asylsökande. Hemlösa personer löper en högre risk av att drabbas av både fysisk och psykisk ohälsa och dåliga erfarenheter av vården kombinerat med en begränsad ekonomi och oklarheter vart i vården de ska vända sig kan göra att hemlösa inte uppsöker vården. Sedan 2013 har både asylsökande och papperslösa rätt till vård som inte kan anstå och enligt hälso-sjukvårds lagen ska vården ges med respekt på lika villkor. Trots detta sker det att papperlösa och asylsökande nekas vård. Syfte: Att beskriva sjukvårdspersonals upplevelse av att vårda patienter som lever i hemlöshet. Metod: Litteraturöversikt med nio vetenskapliga artiklar som valdes ut från databasen Cinahl Complete, Swepub, Pubmed, PsycINFO och Academic Search Complete. Åtta var av kvalitativ karaktär och en använde kvantitativ metod. Dessa analyserades och sammanställdes till tre huvudkategorier och fem underkategorier. Resultat: Litteraturöversikten innefattar följande kategorier: Vårdpersonalens känslor inför hemlösa patienter, Utmaningar av att vårda hemlösa patienter, Hinder i vårdandet, Hinder vid kommunikation och dokumentation, Hinder och utmaningar inför hantering av komplexa vårdbehov, Förhållningsätt i mötet med hemlösa patienter, Ett öppet sinne och en vilja att vårda och Att skapa en vårdrelation med hemlösa patienter. Resultatet visar att vårdpersonalen kan ha både positiva och negativa känslor inför den hemlösa patienten och att det uppkommer hinder i vården av hemlösa. Vårdpersonalen förhållningsätt i vården av hemlösa och skapandet av en vårdrelation kan vara svårt men anses viktigt.  Diskussion: Här diskuteras delar av alla kategorier ifrån resultatet i relation till forskning och Katie Erikssons teori om att ansa, leka och lära och konsensusbegreppet vårdande.

This thesis approaches the question of homelessness in the Czech Republic, focusing specifically on homeless people with dogs who together form an alternative family unit. The purpose of this thesis is to map and present comprehensively viewed information and specifics relating to the group. The thesis aims to inspire the reader to ponder on the discussed issue, that is to be aware that this family unit requires to be approached as a whole, and not as two separate components - a homeless person and a dog. The first part of the thesis defines the theoretical aspect of the question in discussion, based primarily on academic literature sources, but also on sources from the internet which reflect on the focus of the research. Then follows a practical part where a questionnaire is conducted with homeless people with dogs, where the main research question was what the obstacles are preventing this group from living a fulfilling life. Interviews were also conducted with selected people who have been working with homeless people with dogs for a long time. Using both of these research tools, an overall result was achieved. Degree work "The Possibilities of Work Rehabilitation For Homeless People Within the Area of the Capital City of Prague" brings forward a practical survey of possibilities of the work rehabilitation for this target group. The literary - survey part consists of a concise introduction into problems of the homelessness phenomenon, deals with the welfare state relationship towards the homeless people plus the relevant legislature and, last but not least, it mentions a list of services and organizations rendering help to homeless people. The most important part of the work focuses on personal investigation of work rehabilitation. It provides a detailed description of each organization and project rendering work rehabilitation with their final comparison based on information given to the author by experts working in these organizations.

Abstract {--} Public opinion on homelessness The issue of homelessness became very relevant in the last year. At present, the number of people without home has been growing and a nation-wide problem comes into being which cannot be overlooked by the society. As well as other problems of the current world, also the issue of homelessness is one of the popular mediaʼs topics. The homelessness as extreme expression of the poverty and social exclusion is, all the time, an issue neglected not only by the specialists and the state, but also by the lay public. For this reason it is necessary to address the public and first of all to inform the public sufficiently of the given issue. Only based on the knowledge and understanding the people will be able to treat this issue thoroughly and in a thought-out way and they will not avert the eyes from this problem. The target of my thesis is to evaluate the attitude of public to the issue of homelessness. In connection with it two hypotheses were postulated: 1) The citizens of younger age (15-30 years) have a more tolerant attitude to the issue of homelessness than citizen of older age (31-70 let). 2) The women have a higher understanding for the aid to homeless persons then men. A quantitative research was made among persons in the age group of 15-70 years residing permanently in České Budějovice and surroundings, in the form of a controlled questionnaire applying the method of the accidental choice of the respondents. The whole research was performed in the hospital in České Budějovice under the supervision of the medical staff and partly also through internet. The research file consisted of 60% respondents. The questions in the questionnaire were directed on establishment of the attitude of the public to the issue of the persons without homes. On the questions formulated in this determined way the total attitude of the respondents to homeless people was estabilshed as well as the differences in the attitude to the homeless people depending on age and sex of the respondent. The results of the research part of the thesis really proved the highly negative attitude of citizens to homeless people, however, the persons of younger age (15-30 years) showed a higher tolerance rate then citizens of older age (31-70 years). The women also showed a higher extent of altruism then men. Based on these results the hypotheses postulated in advance were confirmed unambiguously and with them also the target of the thesis.

It aims at understanding their point through the city. The studyâs starting point is the homeless personâs body, which is conceived as a prime unit of social experiences. It is their body that goes throughout the city, carrying the marks of a street sociability. The practices studied do not hawed; the are shaped by their own rules of behavior, which, according to De Certeau (1996), in a tactical watt affect the instutionalized forms of power. Thus silently and invisibly, these practices builds new forms of everyday life. The city is the stage where these streets are performed. In this study , downtown is understood as throbbing organism, a space that aggregates many social actors. There the homeless are invisible exceptat night, where they come together in fron of the shops doors. A description of the main place occupied by the homeless is presented in a way that such places are understood to be defined territories by usage (Ferrara, 1999). Lastly, the ways followed by the homeless people are narrated on the bassist of their singularity, but they also are aggregated according to their nature, be is survival delirious, of female conditions.

This thesis deals with ethical dimension of homeless people´s life and dying. It is devoted to ethical questions of homelessness phenomenon and is primary aimed at dignity and reasonability of life on the street, value system of homeless persons, influenced by the way of their life, and feeling about their own mortality. The purpose of this thesis is to analyse how the homeless people feel about the reasonability of life and their own mortality, in the framework of their value system, and about their responsibility for their situation or if they devolve it to others or state. Using qualitative research techniques it analyses realized controlled interviews with homeless persons in thier own environment, that helps to clarify their perception of life and dying on the street. Research aggregate was made up of 15 homeless persons living in the territory of the city České Budějovice.

Alternativní životní styl jako varianta spokojeného bytí / Alternative lifestyle option as being satisfied

The topic of this diploma thesis is Alternative lifestyle as a form of content human existence. This thesis is focused on specification of subculture and I defined the subculture of homeless people, which as I assumed and also as my own research data showed, can demonstrate the content form of human existence. The theoretical part of this thesis is divided into four chapters. The first chapter constitutes a theoretical scope of this thesis where I put emphasis on social norms and theory of social deviation, which are crucial for homeless people who are demonstrating alternative lifestyle which is significantly different from the lifestyle of mainstream conformist society. Second chapter describes characteristics of homelessness. The third chapter includes procedure of help to homeless people in their life situation. There are also stated social services for homeless people. The last chapter is focused on the alternative lifestyle. I also deal with the topic of voluntary homelessness. The aim of this diploma thesis is description of aspects of lifestyle of the observed subculture of homeless people. The target group consists of 20 homeless people in age from 18 to 70 years. A method of qualitative approach was applied and a method of analysis of case studies was used for data collection. The main technique of data collection was an interview with directions which I used with 10 homeless people who were in asylum house and with 10 homeless people who were present directly at the public areas. In the context of this thesis five research questions were determined. The primary data was coded/indexed and then processed with the framework analysis according to Ritchie and Spencer. The outcomes of the research showed that the most frequent reason for the origin of homelessness is alcohol addiction. Most frequently it is related to single or divorced man and women. The daily regime of homeless people is from a greater half regular. Among the most frequent daily activities in the asylum house can be mentioned watching television, cleaning common areas of the asylum house, fixed activities and drinking coffee. The outdoor activities mostly constitute of collection of metal, cooking, shopping for food and searching the garbage bins. They usually spend winter nights in facilities of social services (doss house, asylum house), in a tent, in makeshift shanties, or in a squat. Most people spend their days with their friends or partners. People who live in public areas are more willing to make some extra money. There was three times higher experience with begging of the people who live in public areas than people who live in asylum houses. People in asylum houses usually pay for day board meals. People who live outside in public areas procure their food in supermarkets, day centres and doss houses and in garbage bins. For homeless people nicotine addiction is more frequent than alcohol addiction. They perform their personal hygiene in the facilities of social services (day centre, doss house, asylum house). They usually get their clothes from social or asylum clothes supplies. They do not live in relationships with partners however people who live in the public areas have more sexual contacts. More than half of respondents do not think about their future. People who live in asylum houses are more satisfied with their life situation, nevertheless more than half of respondents would like to change their life situation. On basis of the results of this research theoretical concepts were induced.

Presented thesis is called Pathways Out of Homelessness from the Respondents Point of View. It basis lays in the analysis of interviewes with nine respondents by so called grounded theory method. As an answer to the question - How formaly homeless people get back to the society from their point of view? - the model of Changeable identity emerged. Its description is the basis of this thesis. I clarify two positions "homeless person" and "person with full society membership" within it, I am observing the transformation from the first position to the second through the reintegration process. I follow influences that affect it. In conclusion, I discuss a question of the end of homelessness.
