A structured questionnaire with questions on demographic characteristics, morbidity, use of psychotropics, and occurrence of falls was used. In December 2006 all the 21 (LCIE) registered in the city. Of these, 19 agreed to take part in the study. A total of 377 elderly people were identified, but 96 were not able to answer the questionnaire and 38 refused to do it. Most of the 243 elderly included in the study were women (72.8%) and were, one average, 77.7 (dp=8.9) years old. About one third of the elderly (32.5%) had at least one fall in the previous year. The occurrence of falls was two times higher by women, by the elderly with rheumatism or spinal cord disease, and in those making use of psychotropics. It is intended that the information obtained make it possible further discussions on the health of the elderly people living in long-term care institutions / O objetivo deste estudo foi descrever a ocorrência de quedas e fatores associados em idosos residentes em Instituições de Longa Permanência para Idosos (ILPI) em Pelotas, RS. Foi utilizado um questionário estruturado com perguntas sobre características demográficas, morbidade, uso de psicotrópicos e ocorrência de quedas. Em dezembro de 2006 foram visitadas todas as 21 (ILPI) registradas na cidade. Destas, 19 consentiram participar do estudo. Foram identificados 377 idosos, sendo que, 96 não tinham condições de responder o questionário e 38 foram recusas. Dos 243 idosos incluídos no estudo, a maioria eram mulheres (72,8%) e tinham, em média, 77,7 (dp=8,9) anos. Cerca de um terço dos idosos (32,5%) sofreram pelo menos uma queda no último ano. A ocorrência de quedas foi duas vezes maior nas mulheres, nos idosos com reumatismo ou doença da coluna e naqueles que utilizavam medicação psicotrópica. A crosssectional study was conducted in order to determine the prevalence of functional dependence and its associated factors in institutionalized elderly. Data were collected in the only long-term establishment of a city of the Southwest of the State of Goiás, Midwest region of Brazil. In order to collect the data a structured form was used consisting of three sections: identification, socio-demographic and economic profile, general health condition and the functional independence measure (FIM). Its statistical analysis was performed using multivariate logistic regression. 75 elderly participated in the study, whom most were men 72.0%, among 60 and 79 years 69.3%, did not live in marital union 85.3%, had no schooling degree 69.3% and 76% lived in the facility for less than 10 years. The mean age was 76.3 ± 8.0 years and the mean residence time in long-term care institution was 6.68 ± 6.80 years. The prevalence of dependence was in 68% of the elderly. The average of the total scores of the FIM (77.0 ± 36.8), its motor (53.0 ± 29.6) and cognitive domains (24.0 ± 9.6) and categories, ranked the institutionalized elderly as moderate dependent people level. There was observed association between the variables female sex (p = 0.043; OR = 6.09) and presence of cognitive impairment (p = 0.001; OR = 20.26) and the functional dependency. The functional dependence in institutionalized elderly is multifactorial and the knowledge of these factors contribute to the development of targeted policy interventions to minimize this process. / A funcionalidade de idosos institucionalizados tem sido pesquisada e reconhecida como um importante indicador das demandas de cuidados em instituições de longa permanência. Estudo transversal analítico desenvolvido com objetivo de determinar a prevalência da dependência funcional e os fatores associados em idosos institucionalizados. Os dados foram coletados na única instituição de longa permanência de um município do Sudoeste do Estado de Goiás, região Centro- Oeste do Brasil. Para a coleta de dados foram utilizados um formulário estruturado composto de três seções: dados de identificação, perfil sociodemográfico e econômico, condições de saúde geral e avaliação da medida da independência funcional (MIF). Para a análise de associação foi utilizada a regressão logística multivariada. Participaram do estudo 75 idosos, a maioria eram homens 72,0%, tinham entre 60 e 79 anos 69,3%, não viviam em união conjugal 85,3%, não possuíam instrução escolar 69,3% e 76% residiam na ILPI em estudo há menos de 10 anos. A média de idade foi 76,3±8,0 anos e o tempo médio de residência na instituição de longa permanência foi de 6,68±6,80 anos. Foi verificada uma prevalência de dependência em 68% dos idosos. A média dos escores total da MIF (77,0±36,8), dos domínios motor (53,0±29,6) e cognitivo (24,0±9,6) e de suas categorias, classifica os idosos institucionalizados em estudo como pessoas com nível de dependência moderada. Verificou-se associação das variáveis sexo feminino (p=0,043; OR=6,09) e presença de déficit cognitivo (p=0,001; OR=20,26) com a dependência funcional. A dependência funcional em idosos institucionalizados é multifatorial e o conhecimento desses fatores contribuem para o desenvolvimento de políticas de intervenção direcionados à minimização desse processo.

Promoting psychosocial health of elderly residential care home residents: implementation of a Tai Chi program. / CUHK electronic theses & dissertations collection

Before the main study commenced, empirical work was conducted to establish and validate the Chinese version of the Satisfaction with the Nursing Home Instrument (SNHI-C), which was an instrument used to measure resident satisfaction in the main study. Testing was conducted on a cluster sample of 330 residents from 16 elderly residential care homes in Hong Kong. Findings demonstrated that the SNHI-C had good content validity with an index of .93. It also demonstrated high construct validity by having significant correlation with depression (r = -.42, p < .001), the physical component of health-related quality of life (r = .16, p < .05), the mental component of health-related quality of life (r = .41, p < .001), and global quality of care (r = .49, p < .001). The SNHI-C also demonstrated satisfactory internal consistency and good stability by having a Cronbach's alpha of .79 and intra-class correlation coefficient of .94, respectively. / Current study adds new knowledge to the psychosocial health benefits of Tai Chi for the residents living in elderly residential care homes. Findings were discussed in relation to previous research findings and in the context of elderly residential care setting. The present study has the strength of evaluating the health benefit of Tai Chi using a more stringent research design, in comparing with the previous studies. Limitations of this study included the use of convenience sampling, without random assigning participants into groups, and inadequacy in quantifying Tai Chi learning. The implications of the findings support incorporating Tai Chi training in routine elderly residential care. Based on the study experience, recommendations for a successful implementation of a Tai Chi program were provided. Future research is advised to further explore the most beneficial mode of a Tai Chi program particularly for the elderly residential care home residents. Evaluation of its effect over a longer period of time and using various approaches are also suggested for obtaining a more comprehensive picture of the health benefit of Tai Chi. Finally, it is also worthwhile to investigate the experience of practicing Tai Chi through a qualitative approach to complement present findings. / Like many areas in the world, Hong Kong is facing an aging population. To accommodate the increasing elderly population, particularly those who are frail and have limited family support, elderly residential care homes are growing in size and number. Despite of the escalating demand, it has been documented repeatedly that the nature of residential living can predispose the residents to a number of psychosocial challenges. Study findings highlight consistently that residents are prone to experience lowered self-esteem, poor social support, and poor health-related quality of life. Providing psychosocial support to the residents is an important dimension in elderly residential care. It is because older residents, despite of their deteriorating physical functions that cannot be cured, can still have the potential to live with optimum psychosocial well-being. Even though a number of interventions have been considered in the past to address the residents' psychosocial needs, evaluation on the effectiveness of these interventions is inconclusive. There is still a paucity of evidence suggesting a promising intervention that can promote psychosocial health for the residents. Given the general consensus that exercise is beneficial for the psychosocial health, and considering that the characteristics of Tai Chi are particularly suitable for the elderly population, Tai Chi is considered as a possible means to promote psychosocial health for the residents. This study aimed to examine the psychosocial effects of a Tai Chi program on the residents of the elderly residential care homes. / The main study adopted a non-equivalent pretest-posttest control group design to examine the psychosocial effect of Tai Chi on residents of elderly residential care homes. Residents were recruited from six elderly residential care homes in Hong Kong by convenience sampling. The experimental group (n = 66) joined a 26-week Tai Chi program three times per week with each session lasted for one hour. The control group (n = 73) continued their daily activities as usual. Outcome variables encompassing state self-esteem, the physical component of health-related quality of life, the mental component of health-related quality of life, social support network, and social support satisfaction were assessed at baseline, the 13th and 26th week of the intervention period. Doubly multivariate analysis of covariance was performed to examine the effect of the Tai Chi program. / With the confounding effect of resident satisfaction controlled, findings demonstrated that participants in the experimental group experienced greater improvement in the psychological composite outcome of state self-esteem, the physical component of health-related quality of life, and the mental component of health-related quality of life (p < .05). No significant changes were detected in any of the three individual psychological outcome variables. Tecendo a teia de uma instituição de longa permanência para idosos: estudo de rede social / Weaving a social support network of a long-term care institution for the elderly: a social networking study

Introdução: Na velhice podem ocorrer mais situações de vulnerabilidades (biológica, funcional, familiar, social e emocional), prejudicando o autocuidado e a provisão suficiente de recursos. Emerge então a necessidade da prestação de cuidados de longa duração. Dentre as modalidades deste tipo de cuidado, encontra-se a Instituição de Longa Permanência para Idosos (ILPI). Considerando a escassez de recursos das ILPI privadas sem fins lucrativos, são adotados diversos mecanismos de ajuda, às vezes estabelecidos informalmente pela rede de apoio destas instituições. Objetivos: mapear a rede de apoio de uma ILPI privada sem fins lucrativos, identificando seus membros, atributos, a configuração dos laços relacionais e do comportamento da rede, e os motivos da prestação de apoio. Métodos: Trata-se de estudo transversal, de abordagem quanti-qualitativa, de natureza exploratória e descritiva, utilizando-se a Análise de Rede Social (ARS). A rede de apoio da ILPI foi mapeada a partir de seus dirigentes pelo método Snowball, e as entrevistas proporcionaram a coleta de dados sobre os tipos de apoio, os atributos dos atores e as relações sociais entre eles, analisados com os softwares UCINET® 6 e NETDRAW®. Resultados: Observou-se que os dirigentes da ILPI não tinham conhecimento da amplitude da rede e dos tipos de apoio prestado. O estudo mostrou uma rede composta por 86 parceiros, sendo 50 pessoas físicas (58,13 por cento ), 17 empresas privadas (19,77 por cento ), 11 organizações não governamentais (12,80 por cento ) e 8 órgãos públicos (9,30 por cento ), que proveem apoio material, instrumental, trabalho voluntário, divulgação da ILPI, organização de eventos para captação de recursos, além de entretenimento, apoio emocional e espiritual às moradoras. A natureza filantrópica da instituição e o público assistido revelaram-se fatores motivadores para a prestação de ajuda. Constatou-se que a rede de apoio da ILPI é também um dos componentes da rede pessoal de apoio das moradoras, além de contribuir com ações que impactam positivamente o bem estar das idosas. A ARS mostrou uma rede mais centralizada, com potencial capacidade para aumentar a densidade e a reciprocidade dos laços. Os membros dirigentes ocupam papéis centrais na rede, detendo os maiores índices de centralidade de grau (Degree), de proximidade (Closeness) e de intermediação 7 (Betweenness). No entanto, membros não dirigentes com medidas de centralidade elevadas podem exercer influência sobre os outros atores, como pontes entre atores desconectados. Houve correlação linear positiva muito forte entre as medidas de centralidade de grau e proximidade da rede. Analisando separadamente o grupo de pessoas físicas, houve forte correlação linear entre as medidas de centralidade de grau, de proximidade e de intermediação. Este é o grupo que apresenta maior potencial para ampliar a rede de apoio e intensificar a frequência e a quantidade de apoios, detendo a maior parte da reciprocidade das relações, o que pode aumentar a confiança entre os membros. Conclusão: O mapeamento da rede de apoio da ILPI mostrou-se potencial ferramenta gerencial. Os motivos que levam os membros a colaborarem podem influenciar a intensidade da ajuda, e os membros dirigentes podem adotar estratégias para contemplar os elementos motivacionais dos diferentes tipos de parceiros. A rede de apoio da ILPI é importante para a sobrevivência econômica da entidade, devendo ser continuamente fortalecida e ampliada, dada a necessidade de fidelizar os membros e de potencializar conexões e captação de recursos. / Introduction: Old age is a typical life stage in which many people are likely to be vulnerable whether for biological reasons or functional, familiar, social and emotional ones which may cause not only functional impairment but also lack of both self-care conditions and enough resources for their own survival, which gives rise to exist a long-term care. Long-term care institutions for the elderly (LTCI) are sought to provide this care. Taking into account for the usual scarcity of LTCI resources, notably those non-profit organizations, several aiding mechanisms are adopted, sometimes informally established by the institutions social network. Objectives: the aims of the present work were to map a support social network of a non-profit LTCI by identifying their members, attributes, quantify the presence of social network clicks and examine their influence on the provision of support. Methods: the present investigation is descriptive and exploratory with both quantitative and qualitative approaches. The Social Network Analysis (SNA) is used to measure and visualize its indicators and sociograms, followed interpreting qualitative analysis of the results taken from the interviews with managers and collaborators of the LTCI as well as partners appointed by them, data categorization, content analysis and discussion with national and international papers. The Snowball method was used to find out its partners and collaborators from all types of support (voluntary service activities, food, clothes and money donations, leisure activities, hand-made products for funding, and publicity of events). Both UCINET® and NETDRAW® were used to characterize network indices and their attributes. Results: it was found that the LTCI directors were neither aware of its supportive network amplitude nor their types. The study showed that its network has 86 partners: 50 ordinary people (58,13 per cent ), 17 private companies (19,77 per cent ), 11 nongovernmental organizations (12,80 per cent ) and 8 public organizations (9,30 per cent ). They all provide material and instrumental means, do voluntary work, promote the LTCI, 9 organize events for gathering resources besides entertainment, and both emotional and spiritual support for the elderly people. The non-profit nature of the LTCI and the public attended were motivating factors for the delivery of aid. It was verified that the support network of the LTCI is also one of the components of the personal support network of the dwellers, besides contributing with actions that positively impact the well-being of the elderly. The SNA showed a more centralized network, with potential capacity to increase the density and the reciprocity of the ties. The directors play a major role in the network so they have high degree, closeness and betweenness centralities. However, non-directing actors with high centrality indices were found to have influence on other actors acting as bridges or link among nonconnected members. There was a very strong positive linear correlation between the measures of degree and closeness centralities network. Analyzing separately the group of ordinary people, there was a strong linear correlation between the measures of degree centrality, closeness and betweenness. Ordinary people represent a high potential to both amplify social network and the frequency of support by having most part of relationship reciprocity, which may improve trust among their members. Conclusions: mapping of LTCI support network has proved to be a potential management tool. The reasons that make members to collaborate may influence the intensity of help so that director may adopt strategies to increase motivation for different types of partners. LTCI support network is important for economical sustainability of the enterprise and must therefore be continuously strengthened and ample besides the necessity for member fidelity, more connections and resource gathering.

One of the consequences of it is the appearance of dizziness, vestibular or not. This study aims to identify the risk factors related to dizziness in institutionalized elderly, with and without vestibular disorders, in the city of Natal-RN. Method: a case-control study realized in 12 regulated by Health Surveillance Long Term Care Institutions for the elderly in Natal-RN. Elected seniors with good cognitive level and able to walk, totaling 115 individuals, and of these, 102 were selected according to the presence of dizziness in the last year (n = 51) and their controls (n = 51), paired by sex and age. The 51 elderly patients with dizziness were divided into 3 groups case: case one, for elderly with dizziness and without vestibulopathy (n=38); case two, for elderly with dizziness and vestibulopathy (n=13) and case three, for all the seniors with dizziness, or added to the case 1 case 2 (n=51). The 51 seniors who served as controls were also divided into three groups, according to the number of individuals of each case: control 1, n = 38, control 2, n = 13 control and 3 (sum of 1 control with control 2), n = 51. As possible risk factors were analyzed variables related to characteristics of the institution, to the habits of life of older people and those concerned with the health of the elderly. For statistical analysis, we used the chi-square or Fisher exact test for a significance level of 5% and calculating the association magnitude between variables by measuring the Odds Ratio. Results: as risk factors for dizziness without vestibular disorders were found the presence of hypertension and cardiovascular disease, as well as the presence of three or more disorders for elderly and use of gastric protector drugs. For the elderly group from case two were found no associated risk factor. For elderly patients with dizziness in the case group three, we observed the same risk factors found for the elderly in the case group one, plus the presence of osteoarthritis pathology, which was also significant for this group. Conclusion: dizziness in institutionalized elderly is associated with systemic common diseases in this age group and the vestibulopathy presents itself as pathology on an isolated way, not being possible, with our data, associate it with non-risk factors / O idoso institucionalizado apresenta, por ser mais fr?gil, v?rias altera??es do equil?brio corporal, podendo levar a quedas e debilidade de sua sa?de. M?todo: trata-se de um estudo caso-controle realizado nas 12 Institui??es de longa perman?ncia para idosos de Natal-RN, regulamentadas pela Vigil?ncia Sanit?ria. Foram eleitos os idosos com bom n?vel cognitivo e capazes de deambular, totalizando 115 indiv?duos, e destes, foram selecionados 102 de acordo com a presen?a da queixa de tontura no ?ltimo ano (n=51) e os respectivos controles (n=51), emparelhados por sexo e idade. Os 51 idosos com queixa de tontura foram divididos em 3 grupos caso: caso 1, para idosos com tontura e sem vestibulopatia (n=38); caso 2, para idosos com tontura e com vestibulopatia (n=13) e caso 3, para todos os idosos com tontura, ou seja, caso 1 somado ao caso 2 (n=51). Os 51 idosos que serviram de controle foram tamb?m divididos em tr?s grupos, seguindo o n?mero de indiv?duos de cada caso: controle 1, n=38; controle 2, n=13 e controle 3 (soma do controle 1 com o controle 2), n=51. Como poss?veis fatores de risco, foram analisadas vari?veis referentes a caracter?sticas da institui??o, aos h?bitos de vida dos idosos e as relacionadas com a sa?de dos idosos. Para a an?lise estat?stica, utilizou-se o teste do Qui-quadrado ou exato de Fisher para um n?vel de signific?ncia de 5% e c?lculo da magnitude da associa??o entre as vari?veis, atrav?s da medida da OddsRatio. Resultados: como fatores de risco para tontura sem vestibulopatia, foram achados a presen?a de hipertens?o arterial sist?mica e doen?a cardiovascular, assim como a presen?a de 3 ou mais patologias por idoso e o uso de medicamentos protetores g?stricos. Para os idosos do grupo caso 2, n?o foi encontrado nenhum fator de risco associado. Para os idosos com tontura pertencentes ao grupo caso 3, foram observados os mesmos fatores de risco encontrados para os idosos do grupo caso 1, acrescido da presen?a da patologia osteoartrose, que tamb?m se mostrou significativa para este grupo. Conclus?o: a tontura no idoso institucionalizado est? associada a doen?as sist?micas comuns nesta faixa et?ria, e a vestibulopatia apresenta-se como patologia de forma isolada, n?o sendo poss?vel com os dados deste estudo associ?-la a nenhum fator de risco

The recording of drug sensitivities for older people living in care homes

AIMS: The aims of this study were to determine the recording of drug sensitivities of elderly care home residents, to describe the nature of sensitivities and to identify and describe discrepancies in the documentation of drug sensitivity status in general practices, pharmacies and care homes. METHODS: A random sample of residents within a purposive sample of care homes (nursing and residential) was selected. A clinical pharmacist inspected the GP medical record, the medicines administration record, and the care home record for each resident to identify drug sensitivities and discrepancies between records and to describe the nature of the recorded sensitivities. RESULTS: The records of 121 residents in 31 care homes were studied. Thirty-one (26%) residents had at least one documented drug sensitivity in one of the sources inspected, with 48 sensitivities in total recorded. There was no description of the nature of the sensitivities recorded in 39/48 (81%) cases. The number of sensitivities recorded on the medicines administration record, care home record and the GP record were 3 (6%), 29 (60%) and 35 (73%), respectively. Only two sensitivities were simultaneously recorded on all three records. CONCLUSIONS: It was of concern that over 90% of drug sensitivities were not recorded on the medicines administration record which is the final checking document when administering medication. The reason for this was that the dispensing pharmacy was responsible for generating the medicines administration record; however, drug sensitivity status is seldom shared between the GP and the dispensing pharmacy. Printing sensitivities on prescriptions would help to resolve this.

However, there is a paucity of literature on specific instruments that assess the quality of life of older people in these contexts. The aim of this study was to culturally adapt the instrument Quality of Life Scales for Nursing Home Residents for the brazilian context. The scale has 54 items in total and includes eleven domains answered in the form of Likert scale and is used to assess the social and psychological aspects of QOL of patients. It is a methodological research, which followed the steps described in the literature on translation and adaptation of instruments: translation; synthesis of translations; back translation; review by a committee of judges and pretesting. The initial translation was developed by two independent qualified translators who investigated and corrected errors of interpretations and of ambiguous terms in the original language. Both translators and researchers conducted a synthesis of the results of the translations. In the back translation, the consensual translated version of the instrument was again passed into the original language (American English). In the next step, a review by a committee of judges aimed to produce a final version, modified and adapted after review and comparison of all translations performed. Likert scale was used for each item of the scale and calculated the Content Validity Index (CVI) in order to produce the final version and adapted. Items that had Validity Index (CVI) of 0.80 were reviewed again by experts. In the last step, the pre-test, the instrument was administered to 15 elderly patients in Homes for the Aged in order to confirm understanding, clarity of all questions in this sample. This step was developed in long-stay institutions for the elderly in a city in the state of São Paulo, in 2013. The Quality of Life Scales for Nursing Home Residents was translated and adapted to the brazilian context. The application of the instrument through the sample of institutionalized elderly showed clarity and understanding of all the items of the instrument. / As Instituições de Longa Permanência para Idosos (ILPI) podem influenciar a qualidade de vida (QV) de seus residentes. No entanto, há na literatura uma escassez de instrumentos específicos que avaliem a QV de idosos nesses contextos. O objetivo deste estudo foi realizar a adaptação cultural do instrumento Quality of Life Scales for Nursing Home Residents, para o contexto brasileiro. A escala possui 54 itens ao todo e inclui onze domínios respondidos no formato da escala Likert e é utilizada para avaliar os aspectos sociais e psicológicos da QV. Trata-se de uma pesquisa metodológica, que seguiu os seguintes passos descritos na literatura sobre tradução e adaptação de instrumentos: tradução; síntese das traduções; retrotradução; revisão por um comitê de juízes e o pré-teste. A tradução inicial foi desenvolvida por dois tradutores independentes e qualificados, que investigaram e corrigiram erros de interpretações e de termos ambíguos no idioma original. Os dois tradutores e os pesquisadores realizaram uma síntese do resultado das traduções. Já na retrotradução, a versão consensual traduzida do instrumento foi novamente passada para a língua de origem (inglês americano). Na etapa seguinte, a revisão por um comitê de juízes teve como objetivo produzir uma versão final, modificada e adaptada após revisão e comparação de todas as traduções realizadas. Foi utilizada uma escala Likert para cada item da escala e calculado o Índice de Validade de Conteúdo (IVC), a fim de produzir a versão final e adaptada. Os itens que obtiveram Índice de Validade (IVC) de 0,80 foram revisados novamente pelos especialistas. Na última etapa, o pré-teste, o instrumento foi aplicado para 15 idosos residentes em ILPI a fim de confirmar a compreensão e a clareza de todas as perguntas nessa amostra. Esta etapa foi desenvolvida em Instituições de Longa Permanência para idosos em uma cidade no interior do estado de São Paulo, no ano de 2013. A escala Quality of life Scales for Nursing Home Residents foi traduzida e adaptada para o contexto brasileiro. A aplicação do instrumento por meio da amostra de idosos institucionalizados apresentou clareza e compreensão de todos os itens do instrumento.

It is one of the national survey specific objectives, called "Long-stay institutions for low-income elderly: proposed multidimensional model of basic assistance," whose goal is to build a proposal for a multidimensional care model concerning ILPIs and to promote interinstitutional in different regions of the country (Florianópolis/SC, Ribeirão Preto/SP, Porto Alegre/RS, Jequié/BA, Passo Fundo/RS, Rio Grande/RS). The study was characterized as a descriptive transversal. The population was composed of 377 elderly from 6 ILPIs, one in each city studied, the sample was obtained in a non-probabilistic, intentional and voluntary, from a sample of 92 elderly subjects (39 men and 53 women), living in institutions, the exclusion criteria were: wheelchair user, bedridden and / or have dependency degree level III. Data were collected by using instruments that included: institution identification sheet, elderly identification sheet, the American Alliance for Health, Physical Education, Recreation and Dance battery test (AAHPERD) and the Katz scale. It had been realized some adaptations in the AAHPERD battery from the pilot study, the main one, is exchange the half-mile test by 6 minutes Walking Test to evaluate aerobic endurance. Data were analyzed using descriptive statistics, normality test, percentile scores calculation, variance analysis - ANOVA, Kruskal Wallis test, Mann-Whitney U test and Spearman's correlation tests. The elderly average age was 78,78 years and had been stayed in the nursing home for an average of 7 years, the majority were single (56,5%), with low scholarity level (80,5%) and there were no elderly that practice exercises in the nursing home. Diseases of the circulatory system were the most prevalent (66,3%). The scholarity level (p<0,001) was the only variable that showed statistically significant difference between the northeast, southeast and south regional areas. In relationship to the functional fitness, the elderly majority were classified as general function fitness index (GFFI) between regular and good (66,9%). Among the regional areas there were differences only for the agility and dynamic balance (p=0015) and aerobic endurance (p=0,003). The GFFI showed no statistically significant difference between the regional areas (p&#61504;0,5). Concerning functional capacity, most of the elderly were classified as independent (71,7%) and there was no difference in dependency between the regional areas (p&#61504;0,7). There was a moderate negative association between functional capacity and functional fitness at the national level (&#961;=-0,4, p=0,001) and in all regional areas, except the southeast. It was observed an association between functional fitness and age (&#961;=- 0,2, p=0,036). In the ILPIS routine there were no restrictions to independently elderly to carry out the ADLs. The specialized framework is scarce. None institutions provide exercise program for the residents. It was concluded that it is possible to evaluate functional fitness components in order to verify the decline function in elderly and provide improvements, whereas strategies are required to supply the professional framework deficiency in nursing homes and, we must implement physical exercise on ILPIs to provide a healthy elderly to the residents. / Esta dissertação teve como objetivo verificar a relação entre a capacidade funcional e a aptidão funcional de idosos institucionalizados. Trata-se de um dos objetivos específicos da pesquisa nacional, intitulado Instituições de Longa Permanência para Idosos de baixa renda: proposta de modelo básico de assistência multidimensional , cujo objetivo é construir uma proposta de modelo de atendimento multidimensional para as ILPIs e promover intercâmbio interinstitucional em diferentes regiões do país (Florianópolis/SC, Ribeirão Preto/SP, Porto Alegre/RS, Jequié/BA, Passo Fundo/RS, Rio Grande/RS). O estudo caracterizou-se como sendo transversal do tipo descritivo. A população foi composta por 377 idosos de 6 ILPIs, uma em cada cidade de realização da pesquisa, a amostra foi obtida de forma nãoprobabilística, intencional e por voluntariado, composta por 92 idosos (39 homens e 53 mulheres), residentes nas instituições, tendo como critérios de exclusão: ser cadeirante, acamado e/ou possuir grau de dependência III. Foram usados como instrumentos de coleta de dados: ficha de identificação da instituição, ficha de identificação do idoso, bateria de testes da American Alliance for Health, Physical Education, Recreation and Dance (AAHPERD) e, a Escala de Katz. A bateria da AAHPERD sofreu algumas adaptações a partir do estudo piloto, sendo a principal delas, a troca do teste de meia-milha para avaliar a resistência aeróbia geral pelo Teste de Caminhada de 6 minutos. Os dados foram analisados através da estatística descritiva, teste de normalidade, cálculo de escores percentis, análise de variância ANOVA, teste de Kruskal Wallis, teste U de Mann-Whitney e teste de correlação de Spearman. Os idosos apresentaram média de idade de 78,78 anos e tempo médio de institucionalização de 7 anos, na sua maioria eram solteiros (56,5%), com baixa escolaridade (80,5%) e, nenhum praticava exercícios físicos. As doenças mais prevalentes foram as do aparelho circulatório (66,3%). A única variável que apresentou diferença estatisticamente significativa entre os âmbitos regionais nordeste, sudeste e sul, foi a escolaridade (p<0,001). Com relação a aptidão funcional, a maioria dos idosos foi classificada com Índice de Aptidão Funcional Geral (IAFG) entre regular e bom (66,9%). Entre os âmbitos regionais houveram diferenças somente para as aptidões agilidade e equilíbrio dinâmico (p=0,015) e resistência aeróbia (p=0,003). O IAFG não apresentou diferença estatisticamente significativa entre os âmbitos regionais (p&#61504;0,5). Quanto à capacidade funcional, a maioria dos avaliados foi classificada como independente (71,7%) e, não houve diferença com relação à dependência entre os âmbitos regionais (p&#61504;0,7). Foi encontrada uma associação negativa moderada entre a capacidade funcional e a aptidão funcional no âmbito nacional (&#961;=-0,4; p=0,001) e em todos os âmbitos regionais, com exceção do sudeste. Também se encontrou associação entre a aptidão funcional e a idade (&#961;=-0,2; p=0,036). Na rotina das ILPIs não há nenhuma restrição com relação ao idoso independente realizar suas AVDs. O quadro de funcionários especializados é escasso. E, nenhuma das instituições proporciona algum tipo de programa de exercícios físicos para seus residentes. Concluiu-se que, é possível avaliar componentes da aptidão funcional a fim de verificar os deficitários para trabalhá-los e proporcionar melhora na capacidade funcional; que são necessárias estratégias para suprir a deficiência no quadro de profissionais das ILPIs e; é preciso implementar programas de exercícios físicos nas ILPIs para proporcionar um envelhecimento mais saudável possível para os idosos institucionalizados.
