Termo de consentimento informado em cuidados de reabilitação : considerações para atingir os objetivos bioéticos

Por isso considera-se importante avaliar a compreensão do TCI (Termo de Consentimento Informado). Embora estudos semelhantes tenham sido realizados, procurou-se identificar na percepção do paciente e pesquisador quais seriam as considerações necessárias para atingir os objetivos bioéticos. OBJETIVO: Analisar o termo de consentimento informado obtido de pacientes internados para realização de procedimentos no Hospital Sarah Brasília – Unidade Centro. MÉTODO: Foram aplicados questionários com questões fechadas e abertas para avaliação do termo de consentimento obtido no momento da internação no Hospital Sarah Brasília no período de 30 dias (outubro de 2016). Foram excluídos pacientes menores de 18 anos, com dificuldades cognitivas e da fala, considerados psicologicamente incapazes de participar da pesquisa. Os dados foram analisados no programa Excel 2007 e foi realizada análise de conteúdo das questões abertas. Realizou-se análise e discussão a luz da Declaração Universal sobre Bioética e Direitos Humanos (DUBDH). RESULTADOS: 70 pacientes foram convidados a participar do estudo. Destes, cinco se recusaram a participar da pesquisa, totalizando a amostra em 65 participantes. A média etária foi de 45,11 anos, com ligeiro predomínio para sexo feminino (50,7%), 47,7% estado civil de casado, 85% provenientes do Distrito Federal e Goiás. O grau de instrução predominante foi de ensino médio completo (36,9%) seguido de superior completo (20%). A média da renda informada foi de cinco mil reais. A religião mais frequente foi católica em 57%, seguido por evangélica em 27,7%. Referiram média de 100,17 dias com a doença (4 a 432 dias). Foram internados para procedimentos de cirurgia ortopédica em 61,3%, seguido por procedimentos de cirurgia plástica em 30,7%. Quanto ao habito de leitura encontrou-se respostas como nunca leio (1,6%) a leio diariamente em 41,5%. O meio de comunicação mais referido foi a televisão, 98,4%, seguido por rádio e jornal com a mesma proporção (60%). As respostas abertas foram categorizadas em quatro grupos definidos como utilidade, compreensão, dúvidas e sugestões. A palavra “risco” foi a mais frequente, apareceu em 17 respostas, e a palavra “consentimento” em duas respostas. DISCUSSÃO: O termo de consentimento informado foi o documento assinado pelo paciente no momento da internação em Hospital de reabilitação. Nele, obtiveramse as informações e forneceu-se o consentimento para realização de procedimentos. Neste estudo procurou-se explorar a compreensão dos pacientes acerca do termo de consentimento assinado no momento da internação para realização de procedimentos cirúrgicos eletivos e/ou reabilitação em regime de internação hospitalar. No estudo foi possível verificar que os pilares constantes na DUBDH, da autonomia, responsabilidade individual e do consentimento foram cumpridos. Os participantes tinham competência para participação da pesquisa e de voluntariamente serem internados para realização do procedimento. Entretanto, é importante as considerações bioéticas relacionadas a vulnerabilidade hospitalar e à doença crônica dos pacientes. CONSIDERAÇÕES FINAIS: Baseado nos resultados elenca-se considerações para melhorar a autonomia do paciente em vulnerabilidade hospitalar e doença crônica como a: 1) a previsão de tempo suficiente entre o procedimento e a apresentação do termo e a assinatura dele; 2) fornecer o termo no momento da consulta pré-anestésica para que o paciente leve para casa, tenha tempo de ler e discutir com seus familiares, permitindo que ele apresente suas dúvidas no momento da internação; 3) a equipe que fornece o termo deve estar disponível para responder a quaisquer perguntas que possam estar relacionadas ao termo escrito e seu tratamento, promovendo o incentivo à leitura, ao diálogo e a participação no processo decisório; 4) apresentação do formulário na forma de áudio no momento da internação; 5) aplicação do texto padrão com linguagem clara com a possibilidade de configurações específicas adequadas a necessidade do paciente e o procedimento a ser realizado. / Patients admitted to a rehabilitation hospital have particularities due to chronic diseases, with history of many hospitalizations, so we consider it important to evaluate the understanding of the Informed Consent Term (ICT). Although similar studies have been carried out, it was sought to identify in the patient's and researcher's perceptions what would be the necessary considerations to achieve bioethical goals. OBJECTIVE: To analyze the Informed Consent Term (ICT), obtained from hospitalized patients to perform procedures at Sarah Brasília Hospital - central unit. METHODS: Closed and open questions questionnaires were used to evaluate the Consent Term obtained at the time of hospitalization at Sarah Brasília Hospital during a 30 day period (October 2016). Patients younger than 18 years, with cognitive and speech difficulties were excluded, considered psychologically incapable to participate on the research. The data were analyzed on the Excel 2007 program and carried out content analysis of the open questions. Analysis and discussion were carried out in the light of the Universal Declaration on Bioethics and Human Rights (UDBHR) . RESULTS: 70 patients were invited to take participation in the study, from which five refused to participate , completing 65 participants on the sample. The mean age was 45.11 years, with a slight predominance for women (50.7%), 47.7% married, 85% from Distrito Federal and Goiás. The predominant level of education was High School Degree (36.9%) followed by Graduation Degree (20%). The average income reported was five thousand Reais. The most frequent religion was Catholic in 57%, followed by Evangelical in 27.7%. They reported a mean of 100.17 days with the disease (4 to 432 days). They were hospitalized for orthopedic surgery procedures in 61.3%, followed by plastic surgery procedures in 30.7%. As for the habit of reading, we found answers such as I never read (1.6%), I read daily 41.5%. The most mentioned means of communication was television 98.4%, followed by radio and newspaper with the same proportion (60%). The open responses were categorized into four groups defined as utility, comprehension, doubts and suggestions. The word "risk" was the most frequent, it appeared in 17 responses and the word "consent" in two responses. DISCUSSION: The informed consent form is the document signed by the patient at the time of admission to the rehabilitation hospital. The consent to perform procedures was provided in this form. It Was also de source of information for this study. The aim of this study was to explore the patients' understanding of the informed consent form at the time of admission for elective surgical procedures and / or rehabilitation in a hospital stay. In the study it was possible to verify that in the DUBDH pillars of autonomy, individual responsibility and consent were honored. Participants were able to participate in the research and voluntarily were hospitalized for their procedures. However, we have to keep in mind bioethical considerations related to hospital vulnerability and chronic illnesses are important. FINAL CONSIDERATIONS: Based on the results, we list suggestions to improve the autonomy of the patient in hospital vulnerability and with chronic diseases. Among these suggestions we can highlight: 1) Accurate estimation of the time needed between the procedure, the presenting of the form, and its signature; 2) Provide the form at the time of the pre-anesthetic medical appointment for the patient to take it home and have time to read and discuss it with family members. This will allow patients to present their question at the time of hospitalization; 3)The team that provides the form should be available to answer any questions that may be related to the written form and the patients’ treatment, encouraging the the habit of reading, the dialogue and the participation in the decision-making process; 4) Make the form available in audio format at the time of hospitalization; 5) Presenting of the standard text with clear language with the possibility of specific settings appropriate to the patient's need and the procedure to be performed.

Isso ocorre de forma contínua e o avanço da tecnologia no setor da saúde tem contribuído com o aumento da expectativa de vida. Objetivos: Conhecer o perfil sócio - epidemiológico dos idosos internados em Unidade de Terapia Intensiva (UTI) e descrever os desfechos clínicos, e processos de judicialização no acesso a vaga de UTI, em um hospital terciário. Método: Trata-se de um estudo de base epidemiológica do tipo transversal descritivo. A técnica de seleção dos documentos foi por meio de conveniência; utilizaram-se como fonte de coleta dos dados os livros de admissão/alta e o prontuário eletrônico; amostra foi composta pelo exame de 142 prontuários e os dados foram coletados no período de janeiro a dezembro de 2014. Resultados: Verificou-se que houve um predomínio de idosos masculinos internados na UTI; comorbidades prevalentes foram a HAS e DM e os principais diagnósticos neurológicos foram: tumor cerebral, 47 (33%), traumatismo crânio encefálico, 43 (30%) e acidente vascular encefálico hemorrágico, 35(25%). Em relação ao diagnóstico de TCE observou-se que a maioria foi por queda da própria altura, 18(42%). Conclusão: Concluise que a maioria dos internados foram homens e a maioria dos diagnósticos foram de tumores cerebrais, seguidos por TCEs; as internações garantidas por mandados judiciais corresponderam, a 26(8,3%). / Introduction: Aging is a natural and physiological process of the human being. This is happening continuously and the advancement of technology in the health sector has contributed to the increase in life expectancy. Objectives: To know the socio - epidemiological profile of the elderly hospitalized in an Intensive Care Unit (ICU) and to describe the clinical outcomes, and processes of judicialization in the access to the ICU vacancy, in a tertiary hospital. Method: This is a descriptive cross-sectional epidemiological study. The technique of document selection was by means of convenience; The admission / discharge books and the electronic medical record were used as data collection sources; Sample was composed by the examination of 142 medical records and the data were collected from January to December 2014. Results: It was verified that there was a predominance of male elderly hospitalized in the ICU; Prevalent comorbidities were hypertension and DM and the main neurological diagnoses were: brain tumor, 47 (33%), traumatic brain injury, 43 (30%) and hemorrhagic encephalic vascular accident, 35 (25%). Regarding the diagnosis of TBI, it was observed that the majority was due to the fall of the height itself, 18 (42%). Conclusion: It was concluded that the majority of hospitalizations were men and the majority of the diagnoses were of brain tumors, followed by TBIs; the hospitalizations guaranteed by court orders corresponded to 26 (8.3%). / Introducción: El envejecimiento es un proceso natural y fisiológico del ser humano. Esto ocurre de forma continua y el avance de la tecnología en el sector de la salud ha contribuido con el aumento de la expectativa de vida. Objetivos: Conocer el perfil socio - epidemiológico de los ancianos internados en Unidad de Terapia Intensiva (UTI) y describir los resultados clínicos, y procesos de judicialización en el acceso a vacante de UTI, en un hospital terciario. Método: Se trata de un estudio de base epidemiológica del tipo transversal descriptivo. La técnica de selección de los documentos fue por medio de conveniencia; Se utilizaron como fuente de recolección de los datos los libros de admisión / alta y el prontuario electrónico; La muestra fue compuesta por el examen de 142 prontuarios y los datos fueron recolectados en el período de enero a diciembre de 2014. Resultados: Se verificó que hubo un predominio de ancianos masculinos internados en la UTI; Las comorbilidades prevalentes fueron la HAS y el DM y los principales diagnósticos neurológicos fueron: tumor cerebral, 47 (33%), traumatismo cráneo encefálico, 43 (30%) y accidente vascular encefálico hemorrágico, 35 (25%). En relación al diagnóstico de TCE se observó que la mayoría fue por caída de la propia altura, 18 (42%). Conclusión: Se concluye que la mayoría de los internados fueron hombres y la mayoría de los diagnósticos fueron de tumores cerebrales, seguidos por TCE; Las internaciones garantizadas por mandatos judiciales correspondieron, a 26 (8,3%).

O acompanhante e o espaço de visita : um estudo no Hospital Universitário de Brasília

Tomando-se como referência a PNH, o principal objetivo deste trabalho foi o de conhecer a atenção destinada aos acompanhantes e verificar como se estabelece o espaço de visitas no Hospital Universitário de Brasília (HUB), como forma de desenvolver estratégias integrais de acolhimento nesse contexto. A abordagem metodológica utilizada foi a de Estudo de Caso, realizada por meio de dupla abordagem: pesquisa descritiva a partir de dados quantitativos obtidos pela aplicação de questionários contendo questões fechadas e abertas, e pesquisa qualitativa que incluiu análise documental, observações livres e realização de entrevistas semiestruturadas. Esses procedimentos permitiram a triangulação dos dados, o que contribuiu para a construção de uma visão ampliada do objeto de estudo. O projeto de pesquisa foi submetido e aprovado pelo Comitê de Ética em Pesquisa da Faculdade de Medicina da Universidade de Brasília. Participaram do estudo 95 acompanhantes em quatro unidades do HUB: Pediatria Clínica, Cirurgia Pediátrica, Clínica Médica e Clínica Cirúrgica. Os resultados demonstraram que o HUB recebe pacientes provenientes de Brasília, das cidades do Entorno e de outros estados brasileiros. Como essas pessoas estão a uma longa distância de sua moradia, a presença do acompanhante é cada vez mais constante nas unidades. A maioria dos acompanhantes pertence ao sexo feminino, sendo composta por mulheres adultas e jovens. No entanto, emergiu também a presença da figura do cuidador formal remunerado como acompanhante. Os entrevistados apontaram como inadequadas as acomodações a eles destinadas, especialmente aquelas destinadas ao repouso. Relataram, ainda, que o horário de visitas é pequeno e que o processo burocrático ao qual devem se submeter antes da visita restringe ainda mais esse período. Os acompanhantes referiram a presença de sentimentos conflituosos: por um lado emergem a tranquilidade pela possibilidade de poder estar junto à pessoa hospitalizada e, ainda, a gratidão relativa ao fato de contribuir para o enfrentamento da doença em um ambiente desconhecido. Por outro, despontam o esgotamento e a sobrecarga emocional por terem que se afastar de sua rotina de vida para estar no hospital. Ficou evidente que, embora a presença do acompanhante seja cada vez mais constante no espaço hospitalar, faltam, ainda, políticas específicas para sua orientação e definição do seu papel nesse contexto. Isso ocasiona insegurança sobre o que é permitido e/ou proibido fazer. Além disso, um horário de visitas restrito interfere na manutenção de vínculos da pessoa hospitalizada com seu entorno social. Cabe, portanto, aos gestores e profissionais de saúde trabalhar no sentido de construir políticas específicas que contemplem o papel do acompanhante e a inclusão de visitas abertas conforme o preconizado pela PNH. _________________________________________________________________________________ ABSTRACT / Among the main guidelines proposed by the National Policy of Humanization (PNH, from Portuguese language), the presence of the companion and the open visit as devices to promote integral CARE to the hospitalized patient are indicated. Taking as reference the PNH, the main objective of this study was to know the attention destined to the caretakers and to verify how to establish the space of visitants at the Hospital Universitário de Brasília (HUB), as a way to develop integral strategies of shelter in this context. The methodological approach used was the case study, carried through by means of double approach: descriptive research from quantitative data obtained by the application of questionnaires containing closed and open questions; and qualitative research, that included documentary analysis, free observation and accomplishment of semi-structuralized interviews. These procedures had allowed the triangulation of the data, which contributed to the construction of an extended vision of the study object. The research project was submitted and approved by the Committee of Ethics in Research of the College of Medicine of the University of Brasilia. 95 companions participated of the study in four units of the HUB: Clinical Pediatrics, Pediatric Surgery, Medical Clinic and Surgical Clinic. The results had demonstrated that the HUB receives patients proceeding from Brasilia, of cities of the Entorno and from other Brazilian states. As these people are very far from their home, the presence of the companion is each time more constant in the units. The majority of the companions are of the feminine sex, adult and young women. However, the presence of the figure of the formal remunerated caretaker also emerged as accompanying. The interviewed people had pointed as inadequate the rooms destined to them, especially those destined to the rest. They also reported that the schedule of visits is short and that the bureaucratic process to which they are submitted before the visit reduces this period still more. The companions refer the presence of conflicting feelings: by one hand, there is the tranquility by the possibility of being able to be next to the hospitalized person, and, still, the relative gratitude to the fact of contributing for the confrontation of the illness in an unknown environment; by another hand, the exhaustion and the emotional overload by moving away from the routine of life to be in the hospital are present. It was evident that, even so the presence of the companion is each more constant time in the hospital space, still they lack to specific politics for its orientation and definition of its paper in this context. It was clear that, although the presence of a companion is ever more constant in the hospital space, there are still few specific policies for their guidance and definition of its role in this context. This causes uncertainty about what is allowed and/or forbidden to make. Moreover, a restricted visiting schedule interferes with the maintenance of bonds of the hospitalized person with his social surroundings. It is therefore up to the managers and health professionals working to build specific policies that include the role of the companion and the inclusion of open visits, as recommended by PNH.
