A experiência de amamentação de um grupo de mulheres com mamoplastia redutora e de aumento / The breastfeeding experience of a group of women with reducing and enlarging mammoplasty

As transformações sociais com a excessiva valorizção da imagem corporal idealizada esteticamente, em especial das mamas como símbolo da feminilidade, submete as mulheres a procurar recursos cirúrgicos para adequar sua aparência aos padrões vigentes. A maior facilidade de acesso às cirurgias estéticas repercute na prática assistencial e observamos um número crescente de mulheres com mamoplastia. O objetivo deste estudo foi de compreender a experiência de amamentação de um grupo de mulheres com mamoplastia redutora e de aumento. A pesquisa qualitativa foi desenvolvida utilizando o modelo teórico representativo da experiência de amamentar, \"Pesando Riscos e Benefícios\", elaborado por Silva (1997), e o Discurso do Sujeito Coletivo (DSC) para organização dos dados. Participaram do estudo 14 mulheres, sendo oito com mamoplastia redutora e seis com implante mamário, as quais expressaram o desejo de amamentar e apresentavam boas condições clínicas e, mãe e recém-nascido tinham qualquer distúrbio impeditivo à amamentação. Na ocasião das entrevistas, cerca de um mês após o nascimento, no grupo de mulheres com mamoplastia redutora, a amamentação exclusiva foi observada em caráter de exceção, duas mulheres desmamaram e seis estavam em aleitamento materno. As mulheres com prótese mamária, metade estavam amamentando exclusivmente e as restantes, em aleitamento materno. Na análise dos dados foram extraídos 16 DSC, distribuídos em quatro temas: \"Opção pela cirurgia plástica e o projeto de amamentação\", \"Vivenciando a prática da amamentação\", Reflexão sobre a interface da cirurgia com o processo de amamentação\" e \"Conciliar o papel de mãe, nutriz e mulher - não é fácil\". Os DSC revelaram o movimento de mulheres em buscar a mamoplastia para obtenção de maior satisfação com a imagem corporal e as ansiedades decorrentes da opção cirúrgica que afloram na gestação ou no pós-parto ao se depararem com a prática da amamentação. Ao vivenciarem a amamentação, as mulheres avaliaram a capacidade de produzir leite em quantidade adequada às necessidades do filho. O complemento lácteo apareceu como benefício à mulher, sendo uma estratégia para prolongar o período de amamentação e postergar o desmame e, por vezes, como auxílio para recuperação dos traumas mamilares. As mulheres em sua maioria acreditam que a mamoplastia interferiu na amamentação, observando dificuldade de ejeção e produção láctea reduzida. As mulheres que não foram capazes de manter a amamentação exclusiva expressam sentimento de culpa pela opção cirúrgica e procuraram garantir o vínculo com o filho e transmitir o amor materno construindo outras estratégias. As dificuldades em amamentar foram pautadas em questões biológicas e estruturais da glândula mamária e não em questões culturais de valorização do corpo feminino. A crença social e dos profissionais da área da saúde que a mulher com mamoplastia não deseja amamentar, para não comprometer o resultado estético de suas mamas obtido pela cirurgia não se evidenciou. Percebe-se que permanece a intenção da mulher em manter sua imagem em corcordância com os padrões idealizados, projetando a possibilidade de repetir a cirurgia no futuro / The social transformations with excessive valuation of the aesthetically idealized body image and, specially the breasts as femininity, submit the women to search surgical resources in order to adequate their appearance to the present patterns. The bigger easiness of access to the aesthetical surgeries reverberates on the first-aid practice and we observe an increasing number of women with mammoplasty. The object of present study was to understand the breastfeeding experience of a group of women with reducing and enlarging mammoplasty. The qualitative research was developed utilizing the representative theoretical model of the breastfeeding experience \"Pesando Riscos e Benefícios\" [Weighing Risks and Benefits] elaborated by Silva (1997) and for the data organization the Discurso do Sujeito Coletivo (DSC) [Collective Subject Speech]. It has participated in the study 14 women being 8 with reducing mammoplasty and 6 with mammary implant, that expressed the desire for breastfeeding and that presented good clinical conditions and both, mother and newborn, did not have any impeditive disturbance to the breastfeeding. By the occasion of the interviews, about one month after the birth, in the group of women with reducing mammoplasty, the exclusive breastfeeding was observed in exception character, two women have weaned and six were in maternal feeding. On the women with mammary prosthesis, half of them was exclusively breastfeeding and the remaining, on maternal feeding. In the data analysis there were drawn 16 DSCs, distributed in four themes: \"Options for the plastic surgery and the breastfeeding project\", \"Experiencing the breastfeeding practice\", \"Reflection about the surgery interface with the breastfeeding process\" and \"Conciliating the mother, nourisher and woman role - it is not easy\". The DSCs have disclosed the women motion searching the mammoplasty for achieving greater satisfaction with the body image and the anxieties elapsing from the surgical option that arise on the gravidity or on the post-partum when facing with the breastfeeding practice. When experiencing the breastfeeding, the women have assessed the capacity for producing milk in a quantity adequte to the child necessities. The lactic complement has shown up as a benefit for the woman, being a strategy for extending the beastfeeding period and postponing the weaning and, many times, as a help for recovering from the mammilary traumas. Most of the women believe that the mammoplasty has interfered in the breastfeeding, observing ejection difficulty and reduced lactic production. The women that were not capable of sustaining the exclusive breastfeeding expressed guilty sense for the surgical option and tried to ensure the entailment with the child and to transmit the maternal love building new strategies. The difficulties for breastfeeding were guided by biological and structural questions of the mammary gland and not by cultural questions of female body valuation. The social belief and among the professionals of the area that the woman with mammoplasty do not desire to breastfeed in order to not compromise the static results of her breasts achieved through surgery has not been shown clearly, although being perceived that it remains the intention for keeping her image according to the idealized patterns, projecting the possibility for repeating the surgery in the future

Um corpo: a queixa muda da anoréxica / A Body: the silent complaint of anorexia

Fundamentando-se nas perspectivas teóricas da análise do discurso de linha francesa e da psicanálise lacaniana, busca-se investigar o significante corpo em uma paciente anoréxica. Considerando-se que o caráter primordial do significante se exerce em relação ao sujeito, buscamos compreender o sujeito anoréxico através da investigação do significante corpo. O material analisado foi constituído por uma entrevista semi-estruturada, realizada com uma paciente diagnosticada como anoréxica, em tratamento no Hospital das Clínicas de Ribeirão Preto da Faculdade de Medicina da Universidade de São Paulo. De acordo com os referenciais teóricos, a análise dos dados foi qualitativa e obedeceu ao paradigma indiciário. A análise da entrevista permitiu-nos observar que o significante \"corpo\" atrelou-se aos significantes \"peso\" e \"alimentação\" em uma mesma cadeia: como \"a única coisa que eu consigo controlar na minha vida\". O significante corpo constituiu, para o sujeito, a via encontrada na busca por sua alteridade. Desse modo, essa cadeia significante veio compor a ordem da identidade e da alteridade, sob a égide do (sem)controle. Concluímos que a queixa muda da anoréxica, enunciada sob o comportamento da recusa alimentar, era dirigida ao Outro como uma tentativa de alcançar sua alteridade. / Basing theoretical perspectives of analysis on the discourse of the Lacanian and the French line of psychoanalysis, the body signifier of an anorexic patient was investigated. Remembering that the primordial character of the signifier exerts an influence on the subject, we attempted to understand the anorexic individual through an investigation of body signifiers. The analyzed material was formed from a semi-structured interview of a patient diagnosed as anorexic, who was being treated in the Teaching Hospital das Clínicas, University of São Paulo, Ribeirão Preto. According to theoretical references, data analysis was qualitative and followed the indicated paradigm. Analysis of the interview allowed us to observe that the body signifier was linked to the ?weight? and ?feeding? signifiers in a single chain, as ?the only thing that I can control in my life?. The body signifier constituted, for the individual, the way the patient found to identify her alterity. Thus, this signifier chain established the order of the identity and alterity, by the support of (lack of)control. In conclusion, the silent complaint of anorexia, confirmed by the behavior of refusing to eat, is directed at the ?Big Other? in an attempt to attain alterity.

Fisioterapia em transtorno depressivo maior: avaliação da postura e imagem corporal / Physical therapy in major depressive disorder : assessment of posture and body image

Introdução: O Transtorno Depressivo Maior (TDM) caracteriza-se por humor polarizado para depressão, diminuição de energia e atividade geral, assim como uma visão distorcida de si, do mundo e do futuro. Os pacientes podem apresentar postura corporal recurvada sem movimentos espontâneos e olhar abatido. Fatores intrínsecos e extrínsecos influenciam a postura corporal do indivíduo, tais como condições físicas do ambiente, estado sócio-cultural e emocional, atividade física e/ou obesidade. A imagem corporal, a qual é a imagem internalizada do próprio corpo, está intimamente ligada à postura. Objetivos: Avaliar a postura e a imagem corporal em pacientes com TDM no episódio depressivo (semana 1) e na remissão total ou parcial (semana 8 a 10), e comparar ao grupo controle. Métodos: Trata-se de um estudo casocontrole observacional com 34 pacientes, com idades entre 37,62 (±8,20), sendo 26(76,5%) do sexo feminino e 8 (23,5%) do sexo masculino, portadores de TDM de acordo com os critérios do DSM IV-TR. O grupo controle é composto de 37 sujeitos sadios do ponto de vista físico e mental, com idade entre 34,78(± 6,21), sendo 29 (78,4%) do sexo feminino e8 (21,6%) do sexo masculino.A avaliação postural foi realizada através da técnica de fotogrametria utilizando um simetrógrafo, com um tripé posicionado a uma altura de 1m e a uma distância de 2,70 m do simetrógrafo, e uma base de apoio posicionada em frente a este, todos nivelados com bolha. Os pontos anatômicos foram identificados e assinalados a partir de palpação dos limites externos dos ossos e foram marcados com adesivos e bolas de isopor de 15 mm, para serem visualizados em vista lateral. O paciente estava vestido em trajes de banho, conforme a técnica de Penha et al. (2005). Foram realizadas fotografias em quatro poses na posição ortostática: frontal(anterior e posterior), sagital (esquerda e direita). Os ângulos e as distâncias entre as referências ósseas foram quantificados em graus e centímetros, respectivamente, e foram calculados com auxílio das linhas traçadas através do software Coreldraw v.12.0, com base nas linhas de referência do simetrógrafo e nos pontos ósseos marcados. Para a avaliação da imagem corporal foi utilizado o questionário Body Shape Questionnaire. Resultados: Houve diferença da postura no grupo depressivo entre a semana 1 (episódio depressivo) e a semanas 8 a 10 (remissão), onde no episódio ocorre, flexão de cabeça sagital direita e esquerda (p<0,001; p<0,001), aumento da cifose torácica direita e esquerda (p<0,001), tendência a abdução da escápula (p=0,046) e tendência a retroversão pélvica( p=0,012). Na comparação entre o grupo controle e o grupo depressivo semana 8-10 houve diferença entre apenas para a variável postura do ombro(p=0,009). Com relação à imagem corporal, houve diferença estatisticamente significativa no grupo de depressivo entre semana 1 e 8-10 (p= 0,007), a pontuação foi de 90,03 (±38,46) na semana 1 e 75,82 (± 35,30) nas semanas 8-10. O grupo controle não apresenta insatisfação com a imagem corporal assim como os pacientes depressivos na remissão. Conclusão: Os resultados mostraram que o paciente depressivo no episódio apresentou alteração da postura com: aumento da cifose, aumento da inclinação anterior da cabeça, tendência a retroversão pélvica e abdução da escápula. Na remissão há melhora desta postura sendo similar ao grupo controle. Com relação à imagem corporal os pacientes depressivos apresentaram insatisfação leve no episódio. Na remissão não há insatisfação com a imagem corporal assim como o grupo controle. Portanto o paciente depressivo apresenta alteração da postura e da imagem corporal devido aos sinais e sintomas do TDM / Introduction: The Major Depressive Disorder (MDD) is characterized by mood polarized for depression, decreased energy and general activity, as well as a distorted vision of itself, the world and the future. Patients may present curved body posture and without spontaneous movements and looking shot. Intrinsic and extrinsic factors influencing the body posture of the individual, such as physical conditions of the environment, state sociocultural and emotional, physical activity and/or obesity. The body image, which is the image of internalised own body, is closely linked to posture. Objectives: To assess the posture and body image in patients with MDD in depressive episode (week 1), and remission (week 8 a 10) in, and compared to control group. Methods: This was an observational case-control study with 34 patients, aged 37.62 (± 8.20), and 26 (76.5%) female and 8 (23.5%) of sex men, diagnostic of TDM according to the criteria of the DSM-IV TR. The control group is composed of 37 healthy subjects from a physical and mental, aged 34.78 (± 6.21), 29 (78.4%) females and 8 (21.6%) of mens. A posture assessment was performed by using a technique of photogrammetry simetrógrafo, with a tripod positioned at a height of 1m and a distance of 2.70 meters from the simetrógrafo, and a base of support positioned in front of him. The anatomical points were identified and marked from palpation of the external limits of the bones and were marked with adhesive dots and small balls to be viewed in side view. The patient was dressed in swim wear, as the technique of Penha et al. (2005). Photographs were taken in four poses in the standing position: front (anterior and posterior), sagittal (left and right). The angles and distances between references bone were measured in degrees and centimeters, respectively, and were calculated with the help of lines drawn through software CorelDraw v.12.0, based on the lines of reference of simetrógrafo and points marked bone. For the evaluation of body image was used questionnaire Body Shape Questionnaire. Results: There was a difference of attitude between the depressive group in week 1 (depressive episode) and 8 to 10 weeks (remission), with : head inclination right and left (p <0001, p <0001), increased of thoracic kyphosis (p <0001), the abduction of the scapula (p=0,046) and tendency to retroversion pelvic (p= 0,012). In the comparison between the control group and the depressed group there was a difference between 8-10 weeks only to the variable position of the shoulder (p = 0009). With regard to body image, there was a statistically significant difference in the group of depression between week 1 and 8-10 (p = 0007), the score was 90.03 (± 38.46) in week 1 and 75.82 (± 35.30) in 8-10 weeks. The control group does not show dissatisfaction with body image and depressive patients in remission. Conclusion: The results showed that patients with depressive episode showed change in the posture with: increased kyphosis, increased head anterior inclination, tendency to retroversion of pelvis abduction and abduction of scapula. In remission there is improvement this attitude is similar to the control group. With regard to body image, patients with mild depressive episode showed dissatisfaction. In remission there is dissatisfaction with body image and the control group. Therefore the patient presents depressing change of posture and body image because of the signs and symptoms of TMD

Estudo da imagem corporal, do estresse e das preferências alimentares em adolescentes e seus cuidadores / Study of body image, stress and food preferences in adolescents and their caregivers.

A obesidade infantil é influenciada por aspectos ligados à família, orientações alimentares e nutricionais, prática de exercícios físicos, mídia entre outros. Diante deste contexto, o presente estudo teve por objetivo avaliar o estresse, as preferências alimentares e a imagem corporal de adolescentes na faixa etária de 10 a 13 anos e de seus cuidadores. Foram selecionados, por nutricionista, trinta adolescentes em acompanhamento médico e nutricional no Ambulatório de Obesidade Infantil e Adolescente (AOIA) do Departamento de Pediatria da Faculdade de Medicina de Ribeirão Preto (FMRP-USP). O critério para inclusão dos adolescentes obesos foi baseado no índice de massa corporal (IMC) com percentil do IMC maior que 95 calculado após pesagem e medida destes. Foram pareados com outros adolescentes eutróficos, de acordo com sexo e faixa etária e seus cuidadores, recrutados na Unidade de Atenção Primária à Saúde (PAM) da Vila Lobato, Ribeirão Preto (SP). Para avaliar os sintomas de estresse dos cuidadores foi utilizado o Inventário de Sintomas de Estresse para Adultos e para os adolescentes, o Inventário de Estresse Infantil. A percepção da imagem corporal foi investigada por meio da Escala de Silhuetas e a preferência alimentar pelo Teste de Preferências Alimentares. Para a análise dos dados foram utilizados os seguintes testes: Teste Exato de Fisher, Coeficiente de Correlação de Pearson , Modelo de Regressão Linear Múltiplo e Teste de Wilcoxon. Os resultados mostraram diferenças entre os grupos na percepção de imagem corporal e nas preferências alimentares, mas não em relação ao diagnóstico de estresse. As diferenças na percepção da imagem corporal entre eutróficos e obesos foram observadas tanto nos adolescentes quanto em seus cuidadores. Tanto os adolescentes quanto seus cuidadores apresentaram inacurácia na percepção da imagem corporal, que foi ainda maior no grupo de obesos, assim como em seus cuidadores. Ambos os grupos, de adolescentes e seus cuidadores, apresentaram insatisfação com a imagem corporal. Não houve diferenças entre os grupos na avaliação do estresse. Comparando-se os grupos, foram observadas diferenças significativas em relação à preferência por alimentar, pelos adolescentes e seus cuidadores. Os resultados obtidos sugerem que os cuidadores têm hábitos alimentares semelhantes aos de seus filhos. O julgamento da imagem corporal dos cuidadores também foi diferente comparando os grupos e semelhante ao dos adolescentes de cada grupo. / Rezende, C.A.A. Ribeirão Preto, 2012. The environmental influence on childhood obesity includes aspects related to family, food and nutrition guidelines, physical exercises, and media among others. Given this context, the present study aimed to assess the stress level, food preferences and body image in obese adolescents aged 10-13 years and their caregivers. Sixty adolescents were recruited by a dietitian under medical and nutrition supervision at the Clinic of Child and Adolescent Obesity (AOIA) Department of Pediatrics, Faculty of Medicine of Ribeirão Preto (FMRP-USP). Inclusion criteria was based on body mass index (IMC) calculated after weighing and measuring of adolescents. They were paired with eutrophic adolescents according to gender and age and also their caregivers, which were recruited from the Unit of Primary Health Care (PAM) of Vila Lobato, Ribeirão Preto (SP). The Stress Symptoms Inventory for Adults was used to evaluate the caregiver stress symptoms and, to assess the teenager´s, the Child Stress Inventory. The body image perception was investigated by Silhouettes Scale and the food preference through Preferences Food Test. For data analysis, the following tests were used: Fisher\'s exact test, Pearson correlation coefficient, Multiple linear regression model and Wilcoxon´s test. The results showed differences between groups related to the body image perception and food preferences, but not to stress diagnosis. The differences in body image perception between obese and eutrophic were observed, in adolescents and even in their caregivers. Both, adolescents and their caregivers, showed inaccuracy in body image perception, which was even higher in the obese group, as well as in their caregivers. The two groups, adolescents and their caregivers, showed dissatisfaction with body image, which was higher in females. There were no differences between groups in the stress evaluation. Comparing the groups, significant differences were observed regarding preference for low-calorie foods by eutrophic adolescents and their caregivers. These results suggest that caregivers can exert influence in the eating habits of their children regarding food preferences and in the judgment about their body image. Satisfaction With Appearance Scale - SWAP: adaptação e validação para brasileiros que sofreram queimaduras / Satisfaction With Appearance Scale - SWAP: adaptation and validation for brazilian burn victims

Estudo metodológico, com objetivos de adaptar a Satisfaction With Appearance Scale - SWAP para o português - Brasil e avaliar a validade e a confiabilidade da versão adaptada em uma amostra de pacientes brasileiros que sofreram queimaduras. A SWAP avalia a satisfação com a imagem corporal de pessoas que sofreram queimaduras. Quanto maior o escore pior a satisfação com a imagem corporal. O processo de adaptação foi realizado como segue: tradução do instrumento original, síntese das traduções, revisão da tradução realizada por um comitê de juízes, retrotradução, síntese das retrotraduções, comparação da versão original em inglês com a versão consensual em inglês, avaliação pelo autor do instrumento original, reavaliação pelo comitê de juízes, validação semântica dos itens e pré-teste da versão final. A validade de constructo convergente foi avaliada pelo teste de correlação de Pearson entre o escore obtido pela aplicação da SWAP e os resultantes da medida de constructos correlatos - depressão (Índice de Depressão de Beck); autoestima (Escala de Auto-Estima de Rosenberg); qualidade de vida relacionada à saúde (Short Form Health Suvey-36) e estado de saúde de vítimas de queimaduras (Burn Specific Health Scale-Revised). A validade discriminante foi testada por meio de grupos conhecidos pelo teste t de Student para amostras independentes, associando a média da SWAP adaptada com as médias obtidas com sexo, superfície corporal queimada e percepção da visibilidade da queimadura por outras pessoas. A validade de constructo relacionada à dimensionalidade foi verificada por meio da Análise Fatorial Confirmatória (AFC) e Análise Fatorial Exploratória (AFE). A confiabilidade foi avaliada pela consistência interna dos itens (alfa de Cronbach) e pela correlação item-total. O nível de significância adotado foi 0,05. Participaram da etapa de validação da SWAP 106 pacientes queimados com média de idade de 37,5 anos (Desvio-Padrão - DP=13,2) em atendimento ambulatorial na Unidade de Queimados do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto. Na análise da validade de constructo convergente, de acordo com as hipóteses, as correlações entre as medidas da SWAP e a medida dos constructos correlatos variaram de moderadas a fortes e foram estatisticamente significantes (r=0,30 a 0,77). A SWAP adaptada apresentou capacidade de discriminar os grupos com percepção de visibilidade da queimadura por outras pessoas e percepção de não visibilidade (p<0,001). A consistência interna foi de 0,88, e a correlação item-total variou de moderada a forte (r=0,35 a 0,73). A primeira AFC com quatro fatores não apresentou índices de ajuste ao modelo adequados (? 2 =165,2/ p<0,0001; ? 2 /g.l=2,335; AGFI=0,69; RMSEA=0,11; CFI=0,85) A AFE resultou em três fatores com percentual de variância total explicada de 63,2%. Foi feita a segunda AFC com três fatores que apresentou índices de ajuste ao modelo mais próximos dos indicados pela literatura (? 2 =159,46/ p<0,0001; ? 2 /g.l=2,155; AGFI=0,73; RMSEA=0,10; CFI=0,86). Conclui-se que a SWAP adaptada para o português apresentou-se válida e confiável para ser utilizada com indivíduos brasileiros que sofreram queimaduras / Methodological study to adapt the Satisfaction With Appearance Scale - SWAP to Brazilian Portuguese and assess the validity and reliability of the adapted version in a sample of Brazilian burns patients. The SWAP assesses burns victims\' satisfaction with their body image. The higher the score, the worse the satisfaction with the body image. The adaptation process was developed as follows: translation of the original instrument, synthesis of the translations, review of the translation by an expert committee, back-translation, synthesis of the back-translations, comparison between the original version in English and the consensus version in English, assessment by the author of the original instrument, re-assessment by the expert committee, semantic validation of the items and pretest of the final version. The convergent construct validity was assessed using Pearson\'s correlation test between the SWAP score and the scores resulting from the measuring of correlated constructs - depression (Beck Depression Inventory); self-esteem (Rosenberg Self-Esteem Scale); health-related quality of life (Short Form Health Survey-36) and health status of burns victims (Burn Specific Health Scale-Revised). The discriminant validity was tested by means of known groups using Student\'s t-test for independent samples, associating the adapted SWAP score with the means obtained for gender, total body surface area and perceived visibility of the burn by other people. The construct validity related to the dimensionality was verified using Confirmatory Factor Analysis (CFA) and Exploratory Factor Analysis (EFA). The reliability was assessed by means of the internal consistency of the items (Cronbach\'s alpha) and the item-total correlation. The adopted significance level was 0.05. The participants in the validation phase of the SWAP were 106 burned patients with a mean age of 37.5 years (Standard Deviation - SD=13.2), receiving outpatient care at the Burns Unit of the Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto. In the convergent construct validity analysis, the correlations according to the hypotheses were moderate to strong and statistically significant between the SWAP scores and the correlated construct measures (r=0.30 to 0.77). The adapted SWAP was able to distinguish between the groups with perceived visibility of the burn to other people and perceived non-visibility (p<0.001). The internal consistency corresponded to 0.88 and the item-total correlation varied between moderate and strong (r=0.35 to 0.73). The first CFA with four factors did not show appropriate model adjustment indices (? 2 =165.2/ p<0.0001; ? 2 /g.l=2.335; AGFI=0.69; RMSEA=0.11; CFI=0.85). The EFA resulted in three factors with a total explained variance percentage of 63.2%. A second CFA was applied with three factors, which showed model adjustment indices that were closer to those indicated in the literature (? 2 =159.46/ p<0.0001; ? 2 /g.l=2.155; AGFI=0.73; RMSEA=0.10; CFI=0.86). A obesidade infantil é influenciada por aspectos ligados à família, orientações alimentares e nutricionais, prática de exercícios físicos, mídia entre outros. Diante deste contexto, o presente estudo teve por objetivo avaliar o estresse, as preferências alimentares e a imagem corporal de adolescentes na faixa etária de 10 a 13 anos e de seus cuidadores. Foram selecionados, por nutricionista, trinta adolescentes em acompanhamento médico e nutricional no Ambulatório de Obesidade Infantil e Adolescente (AOIA) do Departamento de Pediatria da Faculdade de Medicina de Ribeirão Preto (FMRP-USP). O critério para inclusão dos adolescentes obesos foi baseado no índice de massa corporal (IMC) com percentil do IMC maior que 95 calculado após pesagem e medida destes. Foram pareados com outros adolescentes eutróficos, de acordo com sexo e faixa etária e seus cuidadores, recrutados na Unidade de Atenção Primária à Saúde (PAM) da Vila Lobato, Ribeirão Preto (SP). Para avaliar os sintomas de estresse dos cuidadores foi utilizado o Inventário de Sintomas de Estresse para Adultos e para os adolescentes, o Inventário de Estresse Infantil. A percepção da imagem corporal foi investigada por meio da Escala de Silhuetas e a preferência alimentar pelo Teste de Preferências Alimentares. Para a análise dos dados foram utilizados os seguintes testes: Teste Exato de Fisher, Coeficiente de Correlação de Pearson , Modelo de Regressão Linear Múltiplo e Teste de Wilcoxon. Os resultados mostraram diferenças entre os grupos na percepção de imagem corporal e nas preferências alimentares, mas não em relação ao diagnóstico de estresse. As diferenças na percepção da imagem corporal entre eutróficos e obesos foram observadas tanto nos adolescentes quanto em seus cuidadores. Tanto os adolescentes quanto seus cuidadores apresentaram inacurácia na percepção da imagem corporal, que foi ainda maior no grupo de obesos, assim como em seus cuidadores. Ambos os grupos, de adolescentes e seus cuidadores, apresentaram insatisfação com a imagem corporal. Não houve diferenças entre os grupos na avaliação do estresse. Comparando-se os grupos, foram observadas diferenças significativas em relação à preferência por alimentar, pelos adolescentes e seus cuidadores. Os resultados obtidos sugerem que os cuidadores têm hábitos alimentares semelhantes aos de seus filhos. O julgamento da imagem corporal dos cuidadores também foi diferente comparando os grupos e semelhante ao dos adolescentes de cada grupo. / Rezende, C.A.A. Ribeirão Preto, 2012. The environmental influence on childhood obesity includes aspects related to family, food and nutrition guidelines, physical exercises, and media among others. Given this context, the present study aimed to assess the stress level, food preferences and body image in obese adolescents aged 10-13 years and their caregivers. Sixty adolescents were recruited by a dietitian under medical and nutrition supervision at the Clinic of Child and Adolescent Obesity (AOIA) Department of Pediatrics, Faculty of Medicine of Ribeirão Preto (FMRP-USP). Inclusion criteria was based on body mass index (IMC) calculated after weighing and measuring of adolescents. They were paired with eutrophic adolescents according to gender and age and also their caregivers, which were recruited from the Unit of Primary Health Care (PAM) of Vila Lobato, Ribeirão Preto (SP). The Stress Symptoms Inventory for Adults was used to evaluate the caregiver stress symptoms and, to assess the teenager´s, the Child Stress Inventory. The body image perception was investigated by Silhouettes Scale and the food preference through Preferences Food Test. For data analysis, the following tests were used: Fisher\'s exact test, Pearson correlation coefficient, Multiple linear regression model and Wilcoxon´s test. The results showed differences between groups related to the body image perception and food preferences, but not to stress diagnosis. The differences in body image perception between obese and eutrophic were observed, in adolescents and even in their caregivers. Both, adolescents and their caregivers, showed inaccuracy in body image perception, which was even higher in the obese group, as well as in their caregivers. The two groups, adolescents and their caregivers, showed dissatisfaction with body image, which was higher in females. There were no differences between groups in the stress evaluation. Comparing the groups, significant differences were observed regarding preference for low-calorie foods by eutrophic adolescents and their caregivers. These results suggest that caregivers can exert influence in the eating habits of their children regarding food preferences and in the judgment about their body image. Para tanto, reuniu-se, primeiramente, o material empírico necessário: as imagens fotográficas referidas e doze entrevistas abertas com pessoas entre as que posaram nuas para tais fotografias em situações diversas. Em seguida, construiu-se uma pequena narrativa sobre os momentos de realização de alguns desses ensaios fotográficos, visando a compreensão da natureza de cada um (para isso, procurou-se caracterizar as personagens) e da natureza dos códigos usados pelas pessoas para demonstrarem seu desejo de se verem nuas. A necessidade de se compreender as imagens partindo de sua gênese, ocorreu porque estas imagens não integravam nenhum projeto artístico ou discurso pré-elaborado ou consciente, e tanto as composições como a técnica e os modelos eram muito diversos entre si. Desta maneira, os modelos, seus corpos e expressões ganharam importância neste trabalho e questões relativas ao nu artísitico (desde o modo como o nu era concebido pelos gregos à revolução da body art) e ao uso do corpo na fotografia foram abordadas. O presente trabalho tem por objetivo traduzir, adaptar e validar o Eating Bahaviours and Body Image Test (EBBIT), para uso com crianças de uma cidade do interior do Estado de São Paulo. Foram sujeitos do estudo 261 escolares do sexo feminino, na faixa etária de 9 a 12 anos. Por meio da análise fatorial, com rotação varimax avaliou-se a consistência interna do instrumento. Esta análise, realizada pelo programa SPSS, versão 10.0, revelou dois fatores. Para o instrumento total a consistência interna foi adequada (&#8733; de Cronbach: 0,89) e para os dois fatores (1 e 2) os valores de &#8733; também foram considerados satisfatórios (&#8733;=0,90 e &#8733;=0,80 respectivamente), mostrando assim que o EBBIT é útil para uma avaliação precoce, rastreando comportamentos indicadores de possíveis distúrbios no comportamento alimentar. Foram mantidas as características psicométricas do instrumento original. / This study aimed to translate, adapt and validate the Eating Bahaviours and Body Image Test (EBBIT), to be used with children in a city in the interior of São Paulo. Study subjects were 261 female students between 9 and 12 years old. The internal consistency of the instrument was evaluated by means of factorial analysis with varimax rotation. This analysis was accomplished through SPSS, version 10.0, revealed two factors. The internal consistency was adequate for the total instrument (Cronbach’s &#8733;: 0.89) and &#8733; values were also considered satisfactory for the two factors (1 and 2) (&#8733;=0.90 and &#8733;=0.80 respectively), which demonstrated that the EBBIT is useful for an initial evaluation, tracing behavior that indicates possible eating behavior disorders. The psychometric characteristics of the original instrument were maintained.

O Body Image Quality of Life Inventory (BIQLI) é um instrumento composto por 19 itens e avalia o impacto positivo ou negativo da imagem corporal sobre alguns aspectos da vida: emoções, convívio social, alimentação, sexualidade, bem-estar emocional, exercícios, relacionamentos entre amigos, família e trabalho/escola. Originalmente proposto no idioma inglês, esse instrumento apresenta uma escala bipolar de resposta com sete opções. Os objetivos deste estudo metodológico foram adaptar o BIQLI para o português-Brasil e avaliar confiabilidade e validade da versão adaptada do instrumento, em uma amostra de pacientes brasileiros, vítimas de queimaduras. O processo de adaptação seguiu as seguintes etapas: tradução do BIQLI para o português por dois tradutores, obtenção da versão consensual em português, avaliação pelo Comitê de Juízes para análise das equivalências semântica, idiomática, conceitual e cultural, back-translation por dois tradutores americanos, obtenção da versão consensual em inglês, comparação da versão consensual em inglês com a versão original do BIQLI, envio da versão consensual em inglês a um dos autores do instrumento original, análise semântica dos itens e pré-teste da versão adaptada do BIQLI em pacientes brasileiros que sofreram queimaduras. A validade de construto foi avaliada pelo teste de correlação de Pearson entre os resultados obtidos pela aplicação do BIQLI e os resultados da avaliação de construtos correlatos - autoestima (com aplicação da Escala de Autoestima de Rosenberg) e qualidade de vida (com a aplicação da Burns Specific Health Scale- Revised). Para verificar o número de componentes do BIQLI e visando à comparação com outros estudos, foram realizados a análise dos componentes principais e o teste para comparação de grupos distintos. A confiabilidade foi avaliada pela consistência interna de seus itens (Alfa de Cronbach). O nível de significância adotado foi de 0,05. Participaram da etapa de validação 77 pacientes, com idades entre 18 e 62 anos, em atendimento no ambulatório da Unidade de Queimados do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto. A média dos valores obtidos para cada um dos 19 itens do BIQLI foi de 0,62 (Desviopadrão - D.P. = 1,1) e para o escore total do BIQLI foi de 11,8 (D.P. = 20,9). Com relação à validade de construto, obtiveram-se correlações moderadas entre as medidas de imagem corporal e autoestima (r= 0,30 e p= 0,008) e correlações moderadas entre as medidas de imagem corporal e as obtidas nos domínios afeto e imagem corporal (r= 0,45 e p= 0,000) e relações interpessoais ( r= 0,33 e p= 0,003) da BSHS-R. Com relação à confiabilidade, obtivemos um alto valor para a consistência interna da versão adaptada do BIQLI (Alfa = 0,90). A análise fatorial realizada com um único fator previamente determinado explicou 37,30% da variância de resposta, de forma que a menor carga encontrada foi 0,395. Concluímos que a versão adaptada do BIQLI atendeu aos critérios de validade e confiabilidade, mantendo as propriedades da versão original. / The Body Image Quality of Life Inventory (BIQLI) is an instrument composed of 19 items and assesses the negative or positive body-image impact over a few aspects of life: emotions, social life, diet, sexuality, emotional well-being, exercise, relationship with friends, family and work/school. Originally proposed in the English language, this instrument presents a bipolar response scale with seven options. The objective of this methodological study was to adapt the BIQLI to Brazilian Portuguese and assess the reliability and validity of the instrument\'s adapted version, in a sample of Brazilian patients victims of burns. The adaptation process followed the following steps: translation of the BIQLI into Portuguese by two translators, obtainment of the consensual Portuguese version, assessment by the Judging Committee to analyze the semantic, idiomatic, conceptual and cultural equivalence, back-translation by two American translators, obtainment of the consensual English version, comparison of the consensual English version with the original BIQLI version, forwarding of the consensual English version to one of the authors of the original instrument, semantic analysis of the adapted BIQLI version items and pre-testing on Brazilian patients that suffered burns. The validity of the construct was assessed through the Pearson correlation test between the results obtained through applying the BIQLI and the results of the correlate construct assessments - self-esteem (applying the Rosenberg Self-Esteem Scale) and quality of life (applying the Burns Specific Health Scale- Revised). To check the number of components in the BIQLI and aiming at the comparison to other studies, the main components were analyzed and the test to compare different groups was performed. The reliability was assessed through the internal consistency of its items (Cronbach Alpha). The significance level adopted was of 0.05. Seventy-seven patients, aged between 18 and 62 being treated at the outpatient clinic of the Ribeirão Preto Medical School Clinical Hospital Burn Unit participated in the validation phase. The mean of the values obtained for each one of the 19 items in the BIQLI was of 0.62 (Standard Deviation - S.D. = 1.,1) and for the total BIQLI score 11.8 (S.D. = 20.9). With regards to the construct validity, moderate correlations were obtained between the body-image and self-esteem measurements (r= 0.30 and p= 0.008) and moderate correlations between the body-image measurements and those obtained from the affection and body-image (r= 0.45 and p= 0.000) and interpersonal-relations ( r= 0.33 and p= 0.003) domains of the BSHSR. As to the reliability, we obtained a high value for the internal consistency of the adapted BIQLI version (Alpha = 0.90). The factorial analysis performed with a single predetermined factor explained 37.30% of the response variance, in such a way that the lowest load found was of 0.395. We concluded that the adapted BIQLI version met the validity and reliability criteria, maintaining the properties of the original version.

A imagem corporal, entendida como imagem do corpo formada pela mente, desenvolve-se desde o nascimento pela maturação do mundo psíquico. O objetivo do presente trabalho foi verificar a percepção da imagem corporal de adolescentes, utilizando avaliação do estado nutricional (IMC) e aplicação de uma escala adaptada de satisfação corporal. A amostra constituiu-se por 147 adolescentes, sendo 52 meninos e 95 meninas, com idade média 12 anos, que após mensuração antropométrica (peso e altura) responderam questionário sobre satisfação por áreas corporais. Dos meninos estudados, 1,92% apresentaram estado nutricional de “magreza”, 53,84% de “normalidade” e 44,23% de “sobrepeso/obesidade”, e para as meninas, 3,15%, 77,89% e 18,92% respectivamente. Os meninos identificaram insatisfação com peso corporal (34,6%) e estômago (25,0%) e as meninas, insatisfação com peso corporal (30,5%), tórax/seio (20,0%) e estômago (17,9%). Detectou-se associação significantemente estatística entre estado nutricional e grau de satisfação corporal, com meninos classificados com “sobrepeso e obesidade” inferindo maior insatisfação para as áreas do estômago, cintura e peso corporal e para meninas, todas as áreas, exceto rosto, cintura, tórax/seio, braço e altura. Associando-se fase de adolescência (maturação sexual) e grau de satisfação por áreas corporais, os meninos pré-púberes revelaram maior insatisfação para a área ombro/costas, e as meninas pós-púberes maior insatisfação para a área do rosto, cabelo, quadril, estômago, cintura, tórax/seio, tônus muscular, altura e aspectos gerais. Associando-se gênero e satisfação corporal, os meninos evidenciaram maior insatisfação para as áreas do estômago e nádegas e meninas para tórax/seio. Concluí-se, para a população estudada, que o estado nutricional, maturação sexual e sexo interferem na satisfação e imagem corporal. Os possíveis motivos geradores da insatisfação corporal na adolescência em nível de saúde pública, definem-se pela pressão da mídia, influências sociais e influências negativas parentais e de amigos. / Conti MA. The present study aimed to verify the body image perception of adolescents, through nutritional status evaluation and an adapted body satisfaction scale application. The sample was 147 teenagers (52 boys and 95 girls) with 12 years old on average who had their weight and stature measured and answered a body satisfaction questionnaire. In relation to the nutritional status, it was registered for the boys, 1.92% of leanness, 53.84% of normality and 44.23% of overweight/obesity, and for the girls, 3.15%, 77.89% and 18.92% respectively. The boys had identified complete body dissatisfaction with weight (34.6%) and stomach area (25.0%) and girls with their weight (30.5%), thorax/breast (20.0%) and stomach area (17.9%). It was detected significant association between nutritional status and body satisfaction. The boys classified as ‘overweight/obesity’ had greater dissatisfaction with the stomach area, waist and weight comparing to the boys classified as ‘normal’. The girls had greater dissatisfaction with their hair, buttocks, hip, thighs, legs, stomach, shoulder/back, muscle strenght, weight and general aspects. It was registered significant differences between the phase of adolescence (sexual maturation) and degree of body satisfaction. The pre-pubescent boys had greater dissatisfaction with the shoulder/back area than the pubescent ones. For the pos-pubescent girls, greater dissatisfaction was related to the face area, hair, hip, stomach area, waist, thorax/breast, muscle strength, height and general aspects, comparing to the pubescent ones. When gender and body satisfaction was associated, the boys had shown greater dissatisfaction with the stomach area and buttocks and girls with the thorax/breast area. In conclusion the nutritional status, sexual maturation and gender were associated with body satisfaction and body image. In the public health area, the possible body dissatisfaction causes among teenagers are midia, social, familiar and peer’s influences.
