Sjuksköterskors möte med kvinnor utsatta för våld i nära relation. : En litteraturöversikt / Nurses´ encounter with women exposed to intimate partner violence : A literature review

Bakgrund: Våld i nära relationer är ett globalt folkhälsoproblem och en diskriminering mot kvinnors mänskliga rättigheter. Av kvinnor mellan 15–49 år som varit i en relation har 27 procent blivit utsatta för våld i nära relation. Vanligtvis är det sjuksköterskor som träffar dessa kvinnor först när de söker sig till vården. I möte med dessa kvinnor behöver sjuksköterskor ha ett personcentrerat arbetssätt. Syfte: Syftet med studien var att undersöka sjuksköterskors erfarenhet av att möta kvinnor som blivit utsatta för våld i nära relation. Metod: En litteraturöversikt med kvalitativ metod och induktiv ansats. Artikelsökning gjordes i Cinahl, PubMed och Scopus. Resultatet framställdes av 12 vetenskapliga artiklar som analyserades med Fribergs 5-stegs modell. Resultat: I resultatet framkom 4 huvudteman med tillhörande 9 underteman. De fyra huvudteman var: avsaknad av träning, förutsättningar för ett bra möte, vikten av medvetenhet och samverkan och rollfördelning. Slutsats: Sjuksköterskor upplevde bristande kunskap och osäkerhet i hur de skulle agera kring hantering av våld i nära relation. De uppgav att en trygg miljö behövdes för att kvinnor skulle våga öppna upp sig. God samverkan mellan olika professioner var en viktig del av arbetet. / Background: Intimate partner violence is a global public health issue and a discrimination against women's human rights. Of women between the ages of 15 and 49 who have been in a relationship, 27 percent have been exposed to intimate partner violence. The nurse is usually the one who first sees these women when they seek care. When meeting these women nurses need to have a person-centered approach. Aim: The aim of the study was to explore nurses experience of meeting women who have been exposed to intimate partner violence. Method: A literature review with a qualitative method with an inductive approach. Articles included in this study were collected from Cinahl, PubMed and Scopus. The results are presented by 12 scientific articles that have been analyzed with Friberg's 5-step model. Results: The results showed 4 main themes with 9 sub-themes. The four main themes were: lack of training, prerequisites for a good encounter, the importance of awareness and collaboration and division of roles. Conclusion: Nurses experienced lack of knowledge and insecurity about dealing with intimate partner violence. They stated that a safe environment was needed for women to feel comfortable opening up. Good collaboration between different professions was an important part.

Intimate partner violence was one of the major social issues during the COVID-19 pandemic in Greece. During the pandemic, the cases of intimate partner violence seemed to increase in terms of women coming forward to report the abuse but also in media exposure. This study aims to explore the key factors for the rise in intimate partner violence numbers, as well as the effect it had on victims of intimate partner violence. PRISMA protocol was utilized to analyze the data collected. From the theoretical framework of feminist theory, three key factors for the increase of intimate partner violence in Greece were identified: lack of governmental incentives and action plans to prevent IPV and aid victims of IPV, the macho and impunity culture of Greek society, and police and legislation as patriarchal institutions.

Våld i nära relation är ett globalt hälsoproblem och mörkertalet är högt. Majoriteten av de som utsätts för våldet är kvinnor och våldsutövaren är oftast en man som hon har eller har haft en relation med. Våld i nära relation förekommer även under graviditeten och kan ta sig i uttryck på olika sätt, fysiskt, psykiskt och sexuellt våld. De mänskliga rättigheterna är universella och gäller alla människor men trots detta är antalet kvinnor som utsätts för våld högt. Våld i nära relation medför negativa konsekvenser för modern och det ofödda barnet. Barnmorskan har ett ansvar i vårdandet av gravida kvinnor, det innebär att ha kunskap om våld för att kunna identifiera och åtgärda vid kännedom om att våld förekommer. Syftet med föreliggande litteraturöversikt var att belysa kvinnors upplevelser av att bli utsatta för våld i nära relation under graviditeten. En allmän litteraturöversikt med kvalitativ ansats har genomförts. Datainsamlingen utfördes i databaserna PubMed, Cinahl och PsycInfo som resulterade i att 15 vetenskapliga artiklar inkluderades. Inkluderade artiklar i föreliggande litteraturöversikt har kvalitetsgranskats och den insamlade datan har analyserats utifrån Graneheim och Lundman. Resultatet visade att kvinnor utsätts för fysiskt, psykiskt och sexuellt våld under graviditeten. I många fall var graviditeten en startpunkt för eller återupptagande av eskalerat våld. Gemensamt för de våldsutsatta gravida kvinnorna var att de upplevde kontrollerande och isolerande beteenden från våldsutövaren. Kvinnorna redogjorde för att våldsutsattheten påverkade deras mentala hälsa negativt och oron för det ofödda barnet var ständigt närvarande. De våldsutsatta kvinnorna försökte skydda det ofödda barnet från våldet genom olika undvikande beteenden. Våld i nära relation mot en gravid kvinna kan ge allvarliga konsekvenser för den reproduktiva- och sexuella hälsan samt kvinnohälsan. Resultatet bekräftar att våld i nära relation är ett allvarligt förekommande globalt hälsoproblem som ställer höga krav på barnmorskan som i mötet med de våldsutsatta kvinnorna kan identifiera och agera vid kännedom om förekomst av våld. / Intimate partner violence is a global health issue and the hidden statistics are high. The majority of the victims of intimate partner violence are women and the perpetrator is most often a man that she has or has had a relationship with. Intimate partner violence does also occur during the pregnancy and comes in different forms. Physical, psychological and sexual violence. Human rights are universal and applies to all human beings but despite that, the hidden statistics of women who are exposed to intimate partner violence are high. Intimate partner violence entails negative consequences for the mother and her unborn child. The midwife has a responsibility when caring for pregnant women, it means having knowledge about violence in order to identify and to take measures if there is knowledge of occurrence of violence. The aim of the study was to illustrate women´s experience of being exposed to intimate partner violence during pregnancy. A general literature review with a qualitative approach was conducted. The data collection was performed through the databases PubMed, Cinahl and PsycInfo which resulted in 15 scientific articles. The quality of the included articles has been reviewed and the articles were analyzed from Graneheim och Lundsmans method. The results showed that the women are exposed to physical, pschycological and sexual abuse during pregnancy. In many cases pregnancy was the starting point or a restart of escalated violence. Common to all pregnant women exposed to violence was the experience of controlling and isolating behavior from the perpetrator. The pregnant women reported that intimate partner violence had a negative effect on their mental health and that the concern for the unborn child was always present. The abused women tried to protect their unborn child from the violence through avoidant behaviors. Intimate partner violence against pregnant women can have severe consequenses on the reproductive and sexual health and women’s health. The results of the present literature review confirm that intimate partner violence is a severe global health problem that requires high standards of the midwife when meeting abused pregnant women to identify and act if there is an occurrence of violence.

Våld i nära relationer är ett vanligt förekommande problem i samhället som drabbar familjer, individer och andra närstående på ett omfattande sätt. Syftet med denna studie är att undersöka hur kvinnojourens samverkan kring våldsutsatta kvinnor fungerar i praktiken. Sex kvalitativa intervjuer har genomförts med olika kvinnojourer runt om i Sverige och intervjuerna analyserades genom en tematisk analys. Denna studie visar att kvinnojourer upplever samverkan som komplex och att det finns stora hinder men också möjligheter som medföljer samverkan. Studien visar även att anställda och ideellt arbetande på kvinnojourer upplever både svårigheter och möjligheter men även ett stort behov av samverkan. Ytterligare visar den här studien att det behövs mer forskning kring detta område. Resultatet i studien presenteras och diskuteras utifrån samverkansteori, tidigare forskning och respondenternas svar. / Intimate partner violence is a common problem in society that affects families, individuals and other relatives in a profound way. The purpose of this study is to investigate how women’s shelters collaborate collaborative work on women who are victims of violence works in practice. Six qualitative interviews were conducted with people that work in different women’s shelters in Sweden and the interviews were analyzed through a thematic analysis. This study shows that people that work in women’s shelters experience collaboration as complex and that there are major obstacles but also opportunities that come with collaboration. The study also shows that the people that work in women’s shelters perceive both difficulties and opportunities, but also a great need for collaboration. Furthermore, this study shows that more research is needed in this area. The results of the study are presented and discussed in conjunction with collaboration theory, previous research and the respondents’ answers.

Intimate Partner Violence in Orthopaedic Surgery: Lessons Learned and Future Directions

The overarching theme of this thesis is to discuss the research to date on intimate partner violence (IPV) in orthopaedic surgery and to begin to study selected issues that have been understudied in orthopaedic surgery and IPV. This thesis outlines the current state of knowledge in the field of IPV and orthopaedic surgery and provides some insight into three selected “emerging issues” in the field which warrant future research including: education of orthopaedic surgeons and residents to reduce barriers and improve perceptions; IPV perpetrators; and outcomes for victims of IPV. The findings demonstrate that a short course on IPV for orthopaedic trainees led to an improvement and retention of knowledge three months after the course. IPV education should be integrated into training programs for orthopaedic surgeons. Our systematic review on IPV perpetrator factors indicates that using alcohol or drugs, experiencing child abuse, witnessing interparental aggression, low socioeconomic status, and psychological conditions like depression and anxiety were commonly associated with IPV perpetration. Perpetrator treatment programs should take into consideration modifiable and preventable factors that are associated with IPV perpetration. This thesis proposes a pilot prospective cohort study as the first step toward determining how experiences of IPV affect orthopaedic outcomes such as injury-related complications. The proposed study will determine feasibility and assist in the development a larger-scale multinational prospective cohort study that will engage health care professionals from around the world to increase awareness of how IPV affects patients’ musculoskeletal outcomes. In the past decade, the field of orthopaedic surgery has become more aware of the issue of IPV, but there are many questions that remain. Future research into the above issues will be an excellent first step to fully understanding the issue of IPV in orthopaedic patients, and may lead to improved support of victims of IPV in the future. / Thesis / Master of Science (MSc)
