Introdução: No momento, pouco se conhece sobre o desempenho diagnóstico da angiotomografia de coronárias com 64 colunas de detectores (Angio-TC 64) frente aos achados do ultrassom intracoronário com técnica de histologia virtual (USIC-HV). Este estudo compara a capacidade diagnóstica de ambos os métodos para a avaliação das dimensões vasculares e para avaliação da composição da placa aterosclerótica através da análise de toda extensão dos três vasos epicárdicos principais. Métodos e Resultados: Um total de 21 pacientes com diagnóstico de doença arterial coronária obstrutiva foi incluído neste estudo. Angio-TC 64 foi realizada em todos os pacientes antes da realização do USIC-HV, com intervalo<72horas entre os dois exames. No total, 70 vasos foram avaliados (3,3 vasos por paciente) e divididos em 641 subsegmentos de 4 mm de extensão cada. Um total de 5.972 cortes seccionais de USIC-HV e 5.233 cortes seccionais da Angio-TC 64 foram analisados. As medidas de área luminar, área do vaso, área da parede arterial e carga de placa à Angio-TC 64 e ao USIC-HV foram significativamente correlacionadas (r-Spearman: 0,81; 0,78; 0,55 e 0,49; respectivamente - p<0,001 para todas as correlações). A Angio-TC 64 subestimou a área luminar ao USIC-HV (em mediana: 0,4mm2 ,variando entre -5,6 mm2 e 10,2 mm2). A Angio-TC 64 superestimou a área do vaso, a área da parede arterial (placa+média) e a carga de placa (em mediana: 3,0 mm2; 3,2 mm2 e 13,9%, respectivamente). O aumento da densidade média da placa à Angio-TC 64 foi significativamente associado com o aumento da contribuição percentual dos componentes cálcio denso e núcleo necrótico ao USIC-HV. O aumento da densidade média da placa à Angio-TC 64 foi significativamente associado com a diminuição da contribuição percentual do componente fibro-lipídico ao USIC-HV. Parâmetros de qualidade da imagem (atenuação luminar, ruído da atenuação luminar e relação sinal ruído) influenciaram significativamente os resultados da Angio-TC 64. Conclusão: Nosso estudo demonstra que as imagens da Angio-TC 64 se correlacionam significativamente com as imagens do USIC-HV. Estes achados indicam que a Angio-TC 64 pode ser uma ferramenta útil para a avaliação quantitativa da luz arterial e da placa aterosclerótica; bem como para a avaliação da composição da placa aterosclerótica in vivo / Background: Currently, little is known about the comparative diagnostic performance for coronary assessment of 64-slice multidetector computed tomography (64-MDCT) versus virtual histology intravascular ultrasound technique (VH-IVUS). The present study compares the diagnostic ability of both methods for the evaluation of coronary lumen and vessel wall dimensions as well as plaque composition in a three-vessel whole-artery analysis protocol. Methods and Results: A total of 21 patients with diagnosed obstructive coronary artery disease was included. 64-MDCT was performed within 72 hours before the VH-IVUS examination. Overall, 70 vessels were imaged (3.3 vessels per patient), and divided into 641 subsegments of 4 mm each. Image quality parameters (i.e. lumen attenuation, image noise, signal-to-noise ratio) significantly influenced the results of 64-MDCT. Conclusion: Our study shows that 64-MDCT imaging significantly correlates with VH-IVUS. These findings indicate that 64-MDCT may be a useful non-invasive tool for quantitative evaluation of lumen and plaque parameters, as well as for the in vivo assessment of atherosclerotic plaque composition

Segmentation d’images intravasculaires ultrasonores

L'imagerie intravasculaire ultrasonore (IVUS) est une technologie médicale par cathéter qui produit des images de coupe des vaisseaux sanguins. Elle permet de quantifier et d'étudier la morphologie de plaques d'athérosclérose en plus de visualiser la structure des vaisseaux sanguins (lumière, intima, plaque, média et adventice) en trois dimensions. Depuis quelques années, cette méthode d'imagerie est devenue un outil de choix en recherche aussi bien qu'en clinique pour l'étude de la maladie athérosclérotique. L'imagerie IVUS est par contre affectée par des artéfacts associés aux caractéristiques des capteurs ultrasonores, par la présence de cônes d'ombre causés par les calcifications ou des artères collatérales, par des plaques dont le rendu est hétérogène ou par le chatoiement ultrasonore (speckle) sanguin. L'analyse automatisée de séquences IVUS de grande taille représente donc un défi important. Une méthode de segmentation en trois dimensions (3D) basée sur l'algorithme du fast-marching à interfaces multiples est présentée. La segmentation utilise des attributs des régions et contours des images IVUS. En effet, une nouvelle fonction de vitesse de propagation des interfaces combinant les fonctions de densité de probabilité des tons de gris des composants de la paroi vasculaire et le gradient des intensités est proposée. La segmentation est grandement automatisée puisque la lumière du vaisseau est détectée de façon entièrement automatique. Dans une procédure d'initialisation originale, un minimum d'interactions est nécessaire lorsque les contours initiaux de la paroi externe du vaisseau calculés automatiquement sont proposés à l'utilisateur pour acceptation ou correction sur un nombre limité d'images de coupe longitudinale. La segmentation a été validée à l'aide de séquences IVUS in vivo provenant d'artères fémorales provenant de différents sous-groupes d'acquisitions, c'est-à-dire pré-angioplastie par ballon, post-intervention et à un examen de contrôle 1 an suivant l'intervention. Les résultats ont été comparés avec des contours étalons tracés manuellement par différents experts en analyse d'images IVUS. Les contours de la lumière et de la paroi externe du vaisseau détectés selon la méthode du fast-marching sont en accord avec les tracés manuels des experts puisque les mesures d'aire sont similaires et les différences point-à-point entre les contours sont faibles. De plus, la segmentation par fast-marching 3D s'est effectuée en un temps grandement réduit comparativement à l'analyse manuelle. Il s'agit de la première étude rapportée dans la littérature qui évalue la performance de la segmentation sur différents types d'acquisition IVUS. En conclusion, la segmentation par fast-marching combinant les informations des distributions de tons de gris et du gradient des intensités des images est précise et efficace pour l'analyse de séquences IVUS de grandes tailles. Un outil de segmentation robuste pourrait devenir largement répandu pour la tâche ardue et fastidieuse qu'est l'analyse de ce type d'images. / Intravascular ultrasound (IVUS) is a catheter based medical imaging technique that produces cross-sectional images of blood vessels. These images provide quantitative assessment of the vascular wall, information about the nature of atherosclerotic lesions as well as the plaque shape and size. Over the past few years, this medical imaging modality has become a useful tool in research and clinical applications, particularly in atherosclerotic disease studies. However, IVUS imaging is subject to catheter ring-down artifacts, missing vessel parts due to calcification shadowing or side-branches, heterogeneously looking plaques and ultrasonic speckle from blood. The automated analysis of large IVUS data sets thus represents an important challenge. A three-dimensional segmentation algorithm based on the multiple interface fast-marching method is presented. The segmentation is based on region and contour features of the IVUS images: a new speed fonction for the interface propagation that combines the probability density functions (PDFs) of the vessel wall components and the intensity gradients is proposed. The segmentation is highly automated with the detection of the lumen boundary that is fully automatic. Minimal interactions are necessary with a novel initialization procedure since initial contours of the external vessel wall border are also computed automatically on a limited number of longitudinal images and then proposed to the user for acceptance or correction. The segmentation method was validated with in-vivo IVUS data sets acquired from femoral arteries. This database contained 3 subgroups: pullbacks acquired before balloon angioplasty, after the intervention and at a 1 year follow-up examination. Results were compared with validation contours that were manually traced by different experts in IVUS image analysis. The lumen and external wall boundaries detected with the fast-marching method are in agreement with the experts' manually traced contours with similarly found area measurements and small point-to-point contour differences. In addition, the 3D fast-marching segmentation method dramatically reduced the analysis time compared to manual tracing. Such a valdiation study, with comparison between pre- and post-intervention data, has never been reported in the IVUS segmentation literature. In conclusion, the fast-marching method combining the information on the gray level distributions and intensity gradients of the images is precise and efficient to analyze large IVUS sequences. It is hoped that the fast-marching method will become a widely used tool for the fastidious and difficult task of IVUS image processing.

Efeitos da farmacoterapia utilizando doses máximas de clopidogrel e atorvastatina no controle da hiperplasia neointimal pós-implante de stent coronário / Impact of optimized clopidogrel 150 mg and atorvastatin 80 mg treatment to control neointimal hyperplasia after PCI with bare metal stent: an intravascular ultrasound study

Fundamentos: O implante de stents coronários constitui-se na técnica mais prevalente de revascularização percutânea, em especial pela prevenção da reestenose, quando comparado às intervenções com o balão. No entanto, a reestenose intra-stent, que ocorre em cerca de 25% dos casos, restringe os seus benefícios clínicos e econômicos tardios. Demonstrou-se que a hiperplasia neo-intimal decorrente da reação da parede vascular causada pelo implante do stent, é responsável pelas recidivas. Interroga-se se um protocolo de medicamentos contemplando doses máximas de manutenção de clopidogrel e atorvastatina poderia reduzir a hiperplasia neo-intimal e a reestenose. Objetivos: O objetivo primário foi aferir se esta associação de medicamentos reduziria o volume de hiperplasia neo-intimal (35% ou mais), expressa pela obstrução volumétrica da luz, mensurada pelo ultrassom intracoronário, 12 meses após a intervenção. Os objetivos secundários foram: os resultados da angiografia quantitativa e os eventos cardíacos adversos maiores (óbito, infarto e revascularização do vaso-alvo). Casuística e métodos: Foram incluídos casos eletivos e com lesões primárias nas artérias naturais. Os pacientes foram tratados com stents não farmacológicos e randomizados em dois grupos: A, com 50 pacientes medicados com doses máximas de clopidogrel e atorvastatina; e grupo B com 50 casos, medicados com 75mg de clopidogrel e doses de sinvastatina rotineiramente prescritas para obtenção das metas recomendadas para controle lipídico. Resultados: Ambos os grupos não apresentaram diferenças significantes em relação às características clínicas, angiográficas e técnicas, com exceção do diabetes, mais comum em A (36% vs 16%; p=0,02). Aos 12 meses de evolução, observaram-se eventos cardíacos maiores (12% versus 18%; p = 0,56) e revascularização do vaso-alvo (4% versus 2%; p>0,99) sem diferença significante. Nova angiografia coronária foi obtida em 98% dos casos dos dois grupos, observando-se taxa de reestenose de 8,1% e 8,2% nos grupos A e B (p>0,99). A perda tardia da luz arterial foi semelhante [0,9 mm (DP 0,5 mm) versus 1,1 mm (DP 0,7 mm); p = 0,22], o mesmo acontecendo com o diâmetro mínimo da luz [2,2 mm (DP 0,6 mm) versus 1,9 mm (DP 0,6 mm); p = 0,12]. Realizou-se ultrassom intracoronário em 98% dos pacientes de ambos os grupos, observando-se obstrução do volume da luz de 36,3% (DP 10,3%) no grupo A e de 40,1% (DP 10,9%) no grupo B (p = 0,14). Conclusões: Os resultados deste estudo demonstram que: 1) a terapêutica adjunta utilizando doses máximas de clopidogrel 150 mg/dia e atorvastatina 80 mg/dia não reduz o volume de hiperplasia neo-intimal, expressa pela obstrução volumétrica da luz; 2) as variáveis da angiografia quantitativa e os eventos cardíacos adversos maiores não mostraram diferenças significativas entre os dois grupos. / Coronary stent implantation is the current technique of choice in patients undergoing percutaneous intervention. They effectively reduce acute occlusion and restenosis even in complex lesions. However, instent restenosis occurs in up to 25% of the treated cases and there are several theories elaborating the possible relation between coronary artery thrombosis and inflammation to neointimal proliferation and the mechanism of obstruction recurrence. There are questions whether an optimized medical treatment based on maximum doses of Clopidogrel and Atorvastatin could limit neointimal hyperplasia and restenosis. Objectives: Primary endpoint was to assess the efficacy of this scheme in reducing neointimal hyperplasia volume (XX% or more), according to intravascular ultrasound measurements, 12 months after the index intervention. Secondary endpoints were quantitative angiography measurements and major adverse cardiac events (death, myocardial infarction and target vessel revascularization). Methods: We included patients with de novo lesions undergoing elective implantation of uncoated stents. Patients were divided according to the drug regimen into Group A - 50 patients receiving maximal atorvastatin and clopidogrel doses; and Group B - 50 cases treated with standard clopidogrel and simvastatin doses. Results: Groups were similar concerning clinical and angiographic characteristics, except for diabetes, more frequent in Group A (36% vs 16%, p = 0.02). At the end of 12 months major cardiac events (12% versus 18%, p = 0.56) and target vessel revascularization (4% versus 2%, p> 0.99) did not show differences between groups. Coronary angiography was obtained in 98% of the cases and the restenosis rate was 8.1% (A group) and 8,2% (B group) (p> 0.99). Late luminal loss was similar [0.9 mm (SD 0.5 mm) versus 1.1 mm (SD 0.7 mm), p = 0.22], as well as the minimum lumen diameter [2.2 mm (SD 0.6 mm) versus 1.9 mm (SD 0.6 mm), p = 0.12]. IVUS was done in 98% of patients in both groups, and the volume of neointimal hyperplasia was not significantly different in both groups [61.8 mm 3 (SD 35.4 mm3) mm3 versus 66.3 (SD 31.6 mm3), p = 0.58]. Luminal volume obstruction was 36.3% (SD 10.3%) in group A and 40.1% (SD 10.9%) in group B (p = 0.14). Conclusions: According to our results we may conclude that: 1) the therapeutic regimen using maximum doses of atorvastatin and clopidogrel did not reduce the volume of neointimal hyperplasia, 2) restenosis rate, quantitative angiography results and the rate of major adverse cardiac events were not affected by the treatment regimen.

This database contained 3 subgroups: pullbacks acquired before balloon angioplasty, after the intervention and at a 1 year follow-up examination. Results were compared with validation contours that were manually traced by different experts in IVUS image analysis. The lumen and external wall boundaries detected with the fast-marching method are in agreement with the experts' manually traced contours with similarly found area measurements and small point-to-point contour differences. In addition, the 3D fast-marching segmentation method dramatically reduced the analysis time compared to manual tracing. Such a valdiation study, with comparison between pre- and post-intervention data, has never been reported in the IVUS segmentation literature. In conclusion, the fast-marching method combining the information on the gray level distributions and intensity gradients of the images is precise and efficient to analyze large IVUS sequences. It is hoped that the fast-marching method will become a widely used tool for the fastidious and difficult task of IVUS image processing.

Efeitos da farmacoterapia utilizando doses máximas de clopidogrel e atorvastatina no controle da hiperplasia neointimal pós-implante de stent coronário / Impact of optimized clopidogrel 150 mg and atorvastatin 80 mg treatment to control neointimal hyperplasia after PCI with bare metal stent: an intravascular ultrasound study

Fundamentos: O implante de stents coronários constitui-se na técnica mais prevalente de revascularização percutânea, em especial pela prevenção da reestenose, quando comparado às intervenções com o balão. No entanto, a reestenose intra-stent, que ocorre em cerca de 25% dos casos, restringe os seus benefícios clínicos e econômicos tardios. Demonstrou-se que a hiperplasia neo-intimal decorrente da reação da parede vascular causada pelo implante do stent, é responsável pelas recidivas. Interroga-se se um protocolo de medicamentos contemplando doses máximas de manutenção de clopidogrel e atorvastatina poderia reduzir a hiperplasia neo-intimal e a reestenose. Objetivos: O objetivo primário foi aferir se esta associação de medicamentos reduziria o volume de hiperplasia neo-intimal (35% ou mais), expressa pela obstrução volumétrica da luz, mensurada pelo ultrassom intracoronário, 12 meses após a intervenção. Os objetivos secundários foram: os resultados da angiografia quantitativa e os eventos cardíacos adversos maiores (óbito, infarto e revascularização do vaso-alvo). Casuística e métodos: Foram incluídos casos eletivos e com lesões primárias nas artérias naturais. Os pacientes foram tratados com stents não farmacológicos e randomizados em dois grupos: A, com 50 pacientes medicados com doses máximas de clopidogrel e atorvastatina; e grupo B com 50 casos, medicados com 75mg de clopidogrel e doses de sinvastatina rotineiramente prescritas para obtenção das metas recomendadas para controle lipídico. Resultados: Ambos os grupos não apresentaram diferenças significantes em relação às características clínicas, angiográficas e técnicas, com exceção do diabetes, mais comum em A (36% vs 16%; p=0,02). Aos 12 meses de evolução, observaram-se eventos cardíacos maiores (12% versus 18%; p = 0,56) e revascularização do vaso-alvo (4% versus 2%; p>0,99) sem diferença significante. Nova angiografia coronária foi obtida em 98% dos casos dos dois grupos, observando-se taxa de reestenose de 8,1% e 8,2% nos grupos A e B (p>0,99). A perda tardia da luz arterial foi semelhante [0,9 mm (DP 0,5 mm) versus 1,1 mm (DP 0,7 mm); p = 0,22], o mesmo acontecendo com o diâmetro mínimo da luz [2,2 mm (DP 0,6 mm) versus 1,9 mm (DP 0,6 mm); p = 0,12]. Realizou-se ultrassom intracoronário em 98% dos pacientes de ambos os grupos, observando-se obstrução do volume da luz de 36,3% (DP 10,3%) no grupo A e de 40,1% (DP 10,9%) no grupo B (p = 0,14). Conclusões: Os resultados deste estudo demonstram que: 1) a terapêutica adjunta utilizando doses máximas de clopidogrel 150 mg/dia e atorvastatina 80 mg/dia não reduz o volume de hiperplasia neo-intimal, expressa pela obstrução volumétrica da luz; 2) as variáveis da angiografia quantitativa e os eventos cardíacos adversos maiores não mostraram diferenças significativas entre os dois grupos. / Coronary stent implantation is the current technique of choice in patients undergoing percutaneous intervention. They effectively reduce acute occlusion and restenosis even in complex lesions. However, instent restenosis occurs in up to 25% of the treated cases and there are several theories elaborating the possible relation between coronary artery thrombosis and inflammation to neointimal proliferation and the mechanism of obstruction recurrence. There are questions whether an optimized medical treatment based on maximum doses of Clopidogrel and Atorvastatin could limit neointimal hyperplasia and restenosis. Objectives: Primary endpoint was to assess the efficacy of this scheme in reducing neointimal hyperplasia volume (XX% or more), according to intravascular ultrasound measurements, 12 months after the index intervention. Secondary endpoints were quantitative angiography measurements and major adverse cardiac events (death, myocardial infarction and target vessel revascularization). Methods: We included patients with de novo lesions undergoing elective implantation of uncoated stents. Patients were divided according to the drug regimen into Group A - 50 patients receiving maximal atorvastatin and clopidogrel doses; and Group B - 50 cases treated with standard clopidogrel and simvastatin doses. Results: Groups were similar concerning clinical and angiographic characteristics, except for diabetes, more frequent in Group A (36% vs 16%, p = 0.02). At the end of 12 months major cardiac events (12% versus 18%, p = 0.56) and target vessel revascularization (4% versus 2%, p> 0.99) did not show differences between groups. Coronary angiography was obtained in 98% of the cases and the restenosis rate was 8.1% (A group) and 8,2% (B group) (p> 0.99). Late luminal loss was similar [0.9 mm (SD 0.5 mm) versus 1.1 mm (SD 0.7 mm), p = 0.22], as well as the minimum lumen diameter [2.2 mm (SD 0.6 mm) versus 1.9 mm (SD 0.6 mm), p = 0.12]. IVUS was done in 98% of patients in both groups, and the volume of neointimal hyperplasia was not significantly different in both groups [61.8 mm 3 (SD 35.4 mm3) mm3 versus 66.3 (SD 31.6 mm3), p = 0.58]. Luminal volume obstruction was 36.3% (SD 10.3%) in group A and 40.1% (SD 10.9%) in group B (p = 0.14). Conclusions: According to our results we may conclude that: 1) the therapeutic regimen using maximum doses of atorvastatin and clopidogrel did not reduce the volume of neointimal hyperplasia, 2) restenosis rate, quantitative angiography results and the rate of major adverse cardiac events were not affected by the treatment regimen.

Multi-modality imaging of atherosclerotic plaque using optical coherence tomography / Imagerie multi-modalité en tomographie par cohérence optique de la plaque d’athérosclérose

Les technologies d'imagerie intravasculaire c’est à dire l’échographie endo-coronaire (IVUS)et la tomographie par cohérence optique (IV-OCT) sont des outils précieux pour aider audiagnostic de la plaque d’athérosclérose et guider les gestes thérapeutiques.Le chapitre 1 présente les inconvénients de la coronarographie dans la pratique clinique.Dans plusieurs circonstances, l’IVUS et l’OCT ont clairement une valeur supplémentaire quandils sont utilisés comme un outil diagnostique en cas d'ambiguïtés angiographiques. Nousavons récemment rapporté la première observation utilisant l’OCT pour décrire une dissectioncoronaire d'une branche septale perforante responsable d’un infarctus du myocarde.Le chapitre 2 porte sur la reproductibilité de I’OCT entre différents instituts concernant denombreux paramètres qualitatifs et quantitatifs de la plaque d'athérome. Le but de notre étudeétait d'étudier en plus de la reproductibilité inter- et intra-observateur, la variabilité inter-institutsde I’OCT concernant les mesures quantitatives et qualitatives et de la comparer à celle del’IVUS. Dans ce travail, nous avons observé que, dans la mesure de la surface de la lumièreendocoronaire, des diamètres minimum et maximum de cette même lumière endocoronaire,de la surface endo-luminale du stent, des diamètres minimum et maximum endo-luminaux dustent, par des analystes provenant de 2 laboratoires différents, la reproductibilité inter-institutsde I’OCT était nettement supérieure à celle de l’IVUS. Le Chapitre 3 présente 2 nouvelles technologies robustes d'imagerie intravasculaire hybridesdéveloppées dans le laboratoire du Professeur Guillermo J. Tearney: l’OCT couplée à laspectroscopie dans le proche infra-rouge (OFDI-NIRS) et l’OCT couplée à l’autofluorescencedans le proche infra-rouge (OFDI-NIRAF). Les premières procédures d’OFDI-NIRS chezl’homme sont prévues dans un avenir proche. Les premières procédures chez l’homme ontété réalisées chez 12 patients porteurs d’un angor stable entre Juillet 2014 et Janvier 2015.Les résultats de cette étude pilote ont montré que le signal d’autofluorescence recueilli(NIRAF) a été focalement trés élevé dans des endroits de la plaque d’athérome où la plupartdes phénotypes morphologiques en OCT d’une plaque à haut risque de rupture étaientévidents. Les substrats biochimiques de ce signal d’autofluorescence (NIRAF) sont encore àélucider. / Intravascular imaging technologies i.e. IVUS and IV-OCT are valuable tools for interventionguidance and diagnostic plaque imaging.Chapter 1 introduces the drawbacks of coronary angiography in the clinical practice. The purpose of our study was to investigatefurther inter- and intra-observer reproducibility, the inter-institute variability for IV-OCT (OFDI)quantitative and qualitative measurements vs. IVUS measurements using publishedconsensus document definitions. In our work, we observed that in the measurement of lumenCSA, maximum and minimum lumen diameters, stent CSA, maximum and minimum stentdiameters by analysts from 2 different laboratories, inter-institute reproducibility of OFDI wasfound to be more consistent than IVUS.Chapter 3 present 2 new robust hybrid intravascular imaging technologies developed in Dr.Tearney’s laboratory i.e. OFDI-NIRS and OFDI-NIRAF, which may offer supplementary criteriafor plaque vulnerability. First-in-human OFDI-NIRS imaging procedures are anticipated in thenear future. First-in-human OFDI-NIRAF imaging procedures have been performed in a firstpioneering series of 12 stable patients between July 2014 and January 2015. Findings of thispilot study showed that NIRAF was focally elevated in plaque locations where most high-riskmorphologic phenotypes were evident. The biochemical substrates of the NIRAF signal stillhave to be elucidated.
