Fricção das dânulas na desinfecção e o risco de dispersão: é possível controlar? / Three-way tap friction in disinfection and risk of dispersion: is it possible to control?

O uso de dispositivos para infusões intravasculares representa um desafio, principalmente, pela possibilidade da dispersão microbiana do local de inserção até a ponta do cateter. O procedimento de desinfecção poderá reduzir a colonização no sítio de inserção desses dispositivos, entretanto instiga uma série de questionamentos acerca da possibilidade de dispersão para o interior do lúmen, espectro de ação do antimicrobiano, e a técnica do procedimento de desinfecção. O objetivo deste estudo foi avaliar in vitro o procedimento de desinfecção das dânulas/torneiras de três vias contaminadas propositalmente com Staphylococcus aureus e Pseudomonas aeruginosa, bem como a dispersão de soluções para o interior dos lúmens. Trata-se de um experimento laboratorial in vitro, controlado e desenvolvido em duas etapas: avaliação da dispersão bacteriana por meio da contaminação intencional com cepas padrão: S. aureus (ATCC 25923) e P. aeruginosa (ATCC 27853), e a dispersão de líquidos para o interior do conector por meio do corante cristal violeta a 1%, após a fricção com solução fisiológica ou álcool etílico a 70%, visando simular o processo de desinfecção. Todos os experimentos foram realizados em triplicata por três pesquisadores distintos. A fricção dos conectores com solução fisiológica demonstrou crescimento bacteriano (P. aeruginosa e S. aureus) no interior de 41,7% dos lúmens, no entanto não houve crescimento bacteriano nas amostras após a desinfecção com solução alcoólica a 70% (p<0,001). Com relação aos percentuais das ausências de dispersão de soluções para o interior dos lúmens das dânulas, observou-se que a fricção com as soluções fisiológica e alcoólica foram de 81,5% e 66,7%, respectivamente (p=0,079). Assim, a ausência do crescimento bacteriano no lúmen das dânulas após a fricção com solução alcoólica a 70% está associada a uma série de variáveis controladas as quais remetem a preocupação, principalmente, na possibilidade de dispersão de soluções desinfetantes para o seu interior. Nesse sentido, infere-se sobre os riscos que ameaçam a segurança das pessoas submetidas a infusões intravenosas, especialmente, no que concerne a execução do procedimento de desinfecção das dânulas / The use of intravascular infusion devices presents a challenge, mainly, due to the possibility of microbial dispersion of insertion site up to catheter tip. Disinfection procedure can reduce colonization at insertion site of these devices, but it instigates a series of questions about the possibility of dispersion into the lumen, antimicrobial action spectrum, and the technique of the disinfection procedure. The objective of this study was to evaluate in vitro disinfection procedure of three-way taps purposely contaminated with Staphylococcus aureus and Pseudomonas aeruginosa as well as the dispersion of solutions into the lumens. This is an in vitro laboratory experiment, it was controlled and developed in two steps: evaluation of bacterial dispersion by intentional contamination with standard strains: S. aureus (ATCC 25923) and P. aeruginosa (ATCC 27853), and liquid dispersion into the connector through 1% violet crystal dye, after friction with physiological solution or 70% ethyl alcohol, in order to simulate disinfection process. All experiments were performed in triplicate by three distinct researchers. The friction of connectors with physiological solution showed bacterial growth (P. aeruginosa and S. aureus) within 41.7% of the lumens, but there was no bacterial growth in the samples after disinfection with 70% alcoholic solution (p<0.001). Regarding the absence of dispersion percentages of solutions into the lumens from three-way taps, it was observed that the friction with physiological and alcoholic solutions were 81.5% and 66.7%, respectively (p=0.079). Thus, the absence of bacterial growth in the lumen from three-way taps after the friction with 70% alcoholic solution is associated to a series of controlled variables which refer, mainly, to the possibility of dispersion of disinfectant solutions to its interior. In that sense, it is inferred about risks that threaten the safety of people undergoing intravenous infusion, especially, concerning the disinfecting procedure execution for three-way taps

ANALYTICAL METHODS TO QUANTIFY RISK OF HARM FOR ALERT-OVERRIDDEN HIGH-RISK INTRAVENOUS MEDICATION INFUSIONS

<p>The medication errors associated with intravenous (IV) administration may cause severe patient harm. To address this issue, smart infusion pumps now include a built-in dose error reduction system (DERS) to help ensure the safety of IV administration in clinical settings. However, a drug limit alert triggered by DERS may be overridden by the practitioners which can potentially cause patient harm, especially for high-risk medications. Most analytical measures used to estimate the associated risk of harm are frequency-based and only consider the overall drug performance rather than the severity impact from individual alerts. Unlike these other measures, the IV medication harm index attempts to quantify risk of harm for individual alerts. However, it is not known how well these measures describe the risk associated with alert-overridden scenarios. The goal of this research was (1) to quantitatively measure the risk for simulated individual alert-overridden infusions, (2) to compare these assessments against the risk scores obtained among four different analytical methods, and (3) to propose better risk quantification methods with a higher correlation to risk benchmarks than traditional measures, such as the IV Harm index. </p> <p>In this study, 25 domain experts (20 pharmacists and 5 nurses) were recruited to assess the risk (adjusted for risk benchmarks) for representative scenarios created based on hospital alert data. Four analytical methods were applied to quantify risk for the scenarios: the linear mixed models (Method A), the IV harm index (Method B), Huang and Moh’s matrix-based ranking method matrix-based method (Method C), and the analytical hierarchy process method, adjusted by linear mixed models (Method D). Method A used seven alert factors (identified as key risk factors) to build models for risk prediction, and Methods B and C used two out of seven factors to obtain risk scores. Method D used pairwise comparison surveys to calculate the risk priorities. The quantified scores from the four methods were evaluated in comparison to the risk benchmarks.</p> <p>Risk assessment results from the domain experts indicated that overdosing scenarios with continuous and bolus dose field limit types had significantly higher risks than those of bolus dose rate type. About the soft limit type, the expected risk in the group with a large soft maximum limit was significantly higher than the group with a small soft maximum limit. This significant difference could be found in the adult intensive care unit (AICU), but not in adult medical/surgical care unit (AMSU). The comparisons between four analytical methods and risk benchmarks showed that the risk scores from Method A (<i>ρ</i> = 0.94) and Method D (<i>ρ </i>= 0.87) were highly correlated to the risk benchmarks. The risk scores derived from Method B and Method C did not have a positive correlation with the benchmarks.</p> <p>This study demonstrated that the traditional IV harm index should include more risk factors, along with their interaction effects, for increased correlation with risk benchmarks. Furthermore, the linear mixed models and the adjusted AHP method allow for better risk quantification methods where the quantified scores most correlated with the benchmarks. These methods can provide risk-based analytical support to evaluate alert overrides of four high-risk medications, propofol, morphine, insulin, and heparin in the settings of adult intensive care unit (AICU) and adult medical/surgical care unit (AMSU). We believe that healthcare systems can use these analytical methods to efficiently identify the riskiest medication-care unit combinations (e.g. propofol in AICU), and reduce medication error/harm associated with infusions to enhance patient safety.</p> <p> </p>

Secondary infusions is a common method to deliver short infusions of intravenous (IV) drugs and fluids. Errors associated with this infusion method have led to patient safety concerns. This study's objective was to empirically evaluate interventions to mitigate secondary infusion risks. Venopunção periférica em recém-nascidos prematuros: desafios para o cuidado de enfermagem e a segurança do paciente / Peripheral venipuncture in newborns: challenges for nursing care and patient safety

The peripheral venipuncture in premature newborns (RNPT) and the challenges for nursing care and patient safety are the subject of this study, which has as its guiding questions: How the practice of nursing care with peripheral venipuncture in preterm infants? What are the challenges experienced in nursing care considering patient safety and promoting comfort and minimizing pain? Aimed to comprehend the practice of nursing care with peripheral venipuncture form on RNPT and its challenges to promote comfort and minimize pain with a view to patient safety. Specifically: 1) To describe the profile of respondents;" 2) characterize the peripheral venipuncture procedure in preterm infants, while care practice performed by the nursing team; 3) identify the knowledge of professionals about this care practice; 4) relate aimed at professional attention to manifestations of preterm infants before, during and after the procedure with a view to patient safety; 5) verify the measures to promote comfort and minimize pain in relation to peripheral venipuncture in preterm infants. Descriptive and qualitative study, developed in the neonatal unit of the Universitary Hospital Professor Alberto Antunes (HUPAA / UFAL), Maceió/AL, between April and September 2014, after approval by the Ethics Committee of the Federal University of Alagoas, in the opinion under No. 608,622. The population was made up of 42 professionals of the nursing staff of this unit being applied semi-structured interview, plus a daily non-participant observation. The recorded data were transcribed and validated by each professional. There was use of the Content Analysis, proposed by Laurence Bardin, resulting in the following categorization: 1) Professional Practice in Neonatology and care peripheral venipuncture; 2) humanizing aspects and promotion of comfort by the team in RNPT. The results showed: the sensitivity of professionals, nursing care in trans peripheral venipuncture period; appreciation of humanization of such care; scientific knowledge associated with professional practice; leveling the knowledge reported in the same professional category; applicability of non-pharmacological techniques for pain relief; attention to manifestations of assisted RN; interest and appreciation of the professional development; consistency between reported and care the product of observation made. The study revealed the sensitivity of the participants in order to promote comfort and ease the pain of RNPT subjected to a peripheral venipuncture. Regardless of the busy and institutional link position, characterized individualized and humanized care by interest whether transpose the difficulties encountered, whether related to RN or provided by the dynamics of service. It identified the expansion of nursing practice dimension, bringing the academic and healthcare faces, resulting in identification of interest for the provision of nursing care quality coated, not limited to the activities of compliance. / A venopunção periférica em recém-nascidos prematuros (RNPT) e os desafios para o cuidado de enfermagem e a segurança do paciente constituem o objeto deste estudo, que traz como questões norteadoras: Como ocorre a prática do cuidado de enfermagem com a venopunção periférica em RNPT? Quais os desafios vivenciados no cuidado de enfermagem considerando-se a segurança do paciente e a promoção de conforto e minimização da dor? Objetivou-se compreender a prática do cuidado de enfermagem com a venopunção periférica em recém-nascidos prematuros, seus desafios para a promoção do conforto e minimização da dor com vistas à segurança do paciente. Especificamente: 1) descrever o perfil dos participantes da pesquisa; 2) caracterizar o procedimento de venopunção periférica em RNPT, enquanto prática de cuidado executada pela equipe de enfermagem; 3) identificar o conhecimento dos profissionais acerca deste cuidado; 4) relacionar a atenção voltada pelo profissional às manifestações do RNPT, antes, durante e após o procedimento, na perspectiva da segurança do paciente; 5) verificar as medidas de promoção do conforto e minimização da dor em relação à venopunção periférica em RNPT. Estudo descritivo, qualitativo, desenvolvido na unidade neonatal do Hospital Universitário Professor Alberto Antunes (HUPAA/UFAL), Maceió/AL, entre abril e setembro de 2014, após aprovação do Comitê de Ética em Pesquisa da Universidade Federal de Alagoas, segundo o parecer sob nº 608.622. A população compôs-se por 42 profissionais da equipe de enfermagem desta unidade, sendo aplicada entrevista semi-estruturada, acrescida por diários de observação não-participante. Os dados gravados foram transcritos e validados, por cada profissional. Fez-se uso da Análise de Conteúdo, proposta por Laurence Bardin, resultando na seguinte categorização: 1) Prática profissional em Neonatologia e cuidados com a venopunção periférica; 2) Aspectos de humanização e promoção do conforto pela equipe diante da venopunção periférica em RNPT. Os resultados demonstraram: sensibilidade dos profissionais, aos cuidados de enfermagem, no período de transvenopunção periférica; valorização da humanização destes cuidados; conhecimento científico associado à prática profissional; nivelamento do conhecimento relatado, em uma mesma categoria profissional; aplicabilidade de técnicas não farmacológicas para alívio da dor; atenção às manifestações do RN assistido; interesse e valorização do aprimoramento profissional; coerência entre os cuidados relatados e o produto da observação realizada, ou seja, os detalhes do cuidado que puderam ser, pela pesquisadora, visualizados. O estudo revelou a sensibilidade dos participantes, no intuito de se promover conforto e amenizar a dor do RNPT submetido a uma venopunção periférica. Independente do cargo ocupado e vínculo institucional, caracterizou-se uma assistência individualizada e humanizada, mediante interesse por se transpor as dificuldades encontradas, seja relacionadas ao RN ou proporcionadas pela dinâmica do serviço. Identificou-se a ampliação da dimensão da prática de enfermagem, aproximando as faces acadêmica e assistencial, com consequente reconhecimento do interesse pela prestação de cuidados de enfermagem revestidos de qualidade, não limitados ao cumprimento de atividades.

Adotou-se múltiplos métodos e técnicas. Na abordagem quantitativa foi realizado um survey e no delineamento qualitativo foram utilizadas entrevistas individuais com aplicação das técnicas de “recorte e colagem de gibi” e “evocação livre”. A aplicação destas técnicas visou captar as variáveis intervenientes sobre o processo de punção de vasos periféricos e subsidiar a identificação dos elementos e caracterizar a relevância da representação social sobre tal processo. Dados obtidos no período de outubro/2012 a fevereiro/2013, por meio de entrevistas individuais gravadas e registro cursivo em diário de campo, realizados numa instituição hospitalar (setor de internação convencional) e num serviço de terapia renal substitutiva. Amostra de seleção completa. Após a aplicação dos critérios de elegibilidade e exclusão, foram realizadas 365 abordagens de coleta de dados, tendo participado 235 pessoas na fase do survey, das quais 120 estavam internadas, 75 estavam em hemodiálise e 40 coletaram sangue. Houve abordagem individualizada do questionário com auxílio para preenchimento. Técnica de evocação aplicada a partir de entrevista com participação de 90 pessoas, sendo 30 delas internadas, em coleta de sangue ou em tratamento hemodialítico. Foram obtidas 444 palavras/expressões evocadas para cinco das solicitadas. Na aplicação da técnica de “recorte e colagem de gibi” e gravação do discurso explicativo participaram 60 pessoas, sendo 20 em cada situação abordada. Foi oferecido o mesmo material de figuras para os participantes desta etapa. Um mesmo participante integrou uma ou mais técnicas. Os dados do survey foram tratados em software Statistical Package for the Social Sciences (SPSS) versão 20 com análise descritiva. Os dados provenientes da evocação formaram o corpus, sendo consolidados no software EVOC versão 2000 com análise pela técnica do quadro de quatro casas. E os conteúdos obtidos pela técnica de recorte e colagem de gibi foram consolidados no software NVivo versão 10 e analisados segundo similitude de expressões. Todas as informações foram abordadas com padrões de anonimato, utilizando nomes de flores, pedras ou cores. Foram atendidos todos os requisitos éticos e legais de pesquisa envolvendo seres humanos. No perfil dos participantes da fase survey predominou a participação de mulheres na coleta de sangue e de homens internados ou em tratamento hemodialítico; com idade mais nova entre mulheres que coletavam sangue; predomínio de pele declarada como branca (40%), menos de nove anos de escolaridade (65%), união estável (55%) e filhos. Sendo o processo de punção de vasos um procedimento técnico/científico, os usuários, porém, se apropriam do mesmo de forma particular, havendo destaque de algumas etapas em detrimento de outras. Foi possível identificar as variáveis intervenientes (exposição às diferentes situações e ocasiões de ocorrência do processo de punção de vasos) comparando-as com as variáveis de desfecho, ou seja, com os sentimentos, percepções e comportamentos dos participantes diante do processo de punção de vasos sanguíneos. Houve modificação de percepções, de sentimentos e de comportamentos ao longo das etapas do processo de punção dos vasos. No período que antecedeu a introdução da agulha no interior do vaso houve predomínio de sentimentos de medo, desconforto, ansiedade, indiferença e normalidade e foram adotados comportamentos de silêncio, colaboração, normalidade e de "acostumado", embora xx acompanhados pelo nervosismo das pessoas que fizeram hemodiálise. O período de introdução da agulha foi vivenciado com sentimentos de dor e desconforto e comportamentos de colaboração, silêncio e nervosismo, com ocasional desvio do olhar para não acompanhar o procedimento técnico realizado. Manter a agulha no interior do vaso gerou predomínio de sentimentos de limitação de movimento e desconforto quando o período foi prolongado e houve comportamento de colaboração. O sentimento de alívio e a sensação de ardência foram aqueles que predominantemente emergiram por ocasião da remoção do cateter do interior do vaso e os participantes mencionaram adotar comportamentos de colaboração e de indiferença frente à remoção. No período pós-remoção predominaram os sentimentos de indiferença, alívio, normalidade e nada e comportamentos de silêncio e colaboração, caracterizando a não percepção deste momento como uma etapa do processo, exceção feita às pessoas que realizaram hemodiálise. Os resultados evidenciaram que a estrutura das representações sociais do processo de punção de vasos foi composta pelos elementos centrais de dor e medo. Como elementos periféricos apareceram a insegurança, quem punciona, habilidade profissional, resultado, repuncionar e sangrar, configurando uma abordagem negativa ou neutra expressa em diferentes situações de punções que se apoiaram em experiências, informações, julgamento de valores, concepção de sua finalidade e percepção sensorial, o que foi reafirmado pela técnica de recorte e colagem. Captar as representações sociais através dos sentimentos e comportamentos, bem como, conhecimentos, valores e experiências das pessoas que possuem seus vasos puncionados para fins de coleta de sangue, terapêutica farmacológica ou hemodialítica durante o processo de punção possibilitou fazer uma releitura da punção de vasos. Enquanto atividade processual, evidenciaram-se demandas de cuidado para as quais o enfermeiro dispõe de oportunidades para imprimir ao cuidado orientações educacionais, ações terapêuticas e gerenciais e captar/utilizar evidências a partir da investigação. Foram caracterizadas respostas dos indivíduos para as quais o enfermeiro é capaz de minimizar sentimentos e comportamentos negativos e favorecer formas positivas de enfrentamento. Ao propor-se uma abordagem quantiqualitativa, utilizando múltiplos métodos e técnicas a partir de um recorte conceitual e metodológico, a presente investigação mostrou-se enriquecedora e criativa e trouxe como limite a não comparação entre os participantes enquanto grupos que possuem vivências peculiares no processo de punção de vasos sanguíneos. / Abstract: Quanti-qualitative research whose methodological route intended to analyze the social representations of venipuncture among adults who have experienced the peripheral vessels puncture. Multiple methods and techniques were adopted. During the quantitative approach a survey research was conducted, while in the qualitative approach individual interviews were taken, applying the techniques of "cutting and pasting of comics" and "free recall". The application of these techniques aimed to capture intervening variables on the process of puncturing peripheral vessels and support the identification of elements and characterize the relevance of the social representation of this process. Data obtained from October 2012 to February 2013 through individually recorded interviews and field diary, both conducted in a hospital (regular hospitalization area) and in a program of renal replacement therapy. Sample selection complete. After applying the eligibility and exclusion criteria, there were 365 approaches for data collection, 235 people participated in the phase of the survey, of whom 120 were hospitalized, 75 were on hemodialysis and 40 collected blood samples. There was individualized approach and help to fill the questionnaire. Evocation technique applied in the interviews, with participation of 90 people, 30 of them hospitalized for blood collection or hemodialysis. With the inductive term appointed (process of vessel puncture). We obtained 444 words/expressions evoked to five of the requested. In applying the technique of "cutting and pasting of comics" and recording the explanatory discourse 60 people participated, 20 in each situation addressed. The same comics were shown to the participants of this step. A participant joined one or more techniques. For the treatment of the survey data, with descriptive analysis, was used the Statistical Package for Social Sciences (SPSS) version 20. Evocation data formed the corpus, consolidated in EVOC 2000 version, and were analyzed through the fourhouse board technique. The contents obtained by the technique of cutting and pasting the comics were consolidated into NVivo software version 10 and analyzed according to similarity of expressions. All information has been addressed with patterns of anonymity, using names of flowers, stones or colors. We met all ethical and legal requirements for research involving humans. In the survey phase participants profile predominated the participation of women in blood sample collection and men hospitalized or undergoing hemodialysis, with younger age among women who collected blood; predominance of skin declared as white (40%), less than nine years of education (65%), marital stability (55%) and children. As the process of vessel puncture is a technical/scientific procedure, users, however, interpret it in a particular way, featuring some steps over others. It was possible to identify the intervening variables (exposure to different situations and times of occurrence of vessels puncture processes) compared with the outcome variables, i.e., the feelings, perceptions and behaviors of the participants before the process of puncturing blood vessels. There was a modification of perceptions, feelings and behaviors throughout the stages of the process of vessels puncturing. In the period preceding the introduction of the needle inside the vessel there was a predominance of feelings of fear, discomfort, anxiety, indifference and normality and behaviors of xxii silence, collaboration, normality and "being used to" were adopted, although accompanied by the nervousness of those people in hemodialysis. The length of the needle insertion was experienced with feelings of pain and discomfort and behaviors of collaboration, silence and nervousness, with occasional gaze deviation, not to follow the technical procedure performed. Maintaining the needle in the vessel predominantly generated feelings of limitation of movement and discomfort when the period was extended. There was also the behavior of collaboration. The feeling of relief and burning sensation were predominantly those who emerged during the removal of the catheter inside the vessel and the participants mentioned to adopt behaviors of collaboration and indifference to the removal. In the post-removal time predominated feelings of indifference, relief, normality and nothing and behaviors of silence and collaboration, showing the non-perception of this moment as a step of the process, except to people who underwent hemodialysis. The results showed that the structure of social representations of vessel puncture was composed by the central elements of pain and fear. The peripheral elements appeared insecurity, who punctures, professional skill, result, repuncturing and bleed, setting a neutral or negative approach expressed in different puncture situations that relied on experience, information, value judgment, conception of its purpose and sensory perception, what was reaffirmed by the technique of cutting and pasting. To capture the social representations through the feelings and behaviors, as well as knowledge, values, experiences of people own their vessels punctured for the purpose of blood collection, drug therapy or hemodialysis during puncture, allowed to reread the puncture of vessels. While a procedural activity, care demands showed up for which the nurse has opportunities to print educational guidelines, therapeutic and management actions and to capture/use evidences from the investigation. Answers of individuals were characterized for which the nurse is able to minimize negative feelings and behaviors and to encourage positive ways of coping. In proposing a quanti-qualitative approach, using multiple methods and techniques from a conceptual and methodological framework, this research proved to be enriching and creative and brought as its limitation the not comparison of the participants while groups that have peculiar experiences in the process of blood vessels puncturing.

Nesse estudo trataremos das TIVs centrais e contínuas. Hoje temos a tecnologia e inovação ao nosso favor disponibilizando no mercado uma gama de produtos que irá contribuir para um cuidado de Enfermagem eficiente e seguro durante as TIVs. Por isso, a sensibilização dos hospitais para a qualidade é uma importante ferramenta na ampliação da segurança dos pacientes e na vigilância pós - comercialização. Têm-se como objeto desse estudo, as tecnologias em saúde utilizadas na TIV central contínua durante a assistência de Enfermagem. Foram traçados os seguintes objetivos: identificar as tecnologias em saúde disponíveis para uso durante a TIV central contínua no Centro de Terapia Intensiva (CTI), verificar a utilização dessas tecnologias pela equipe de enfermagem no cuidado às TIVs centrais contínuas instaladas nos pacientes internados no CTI e discutir os desafios no uso das tecnologias durante a TIV central contínua. Estudo descritivo de abordagem qualitativa realizado em um hospital universitário por meio de observação não participante norteada por roteiro com base nos padrões internacionais da Joint Commission International e entrevista semi estruturada. O corpus de dados obtidos a partir das entrevistas através do Softwear Alceste. Os resultados da observação foram agrupados em Estrutura, Processo e Resultado na utilização da TIV, dos quais pôde-se constatar: inexistência de monitoramento dos registros dos trabalhos pela engenharia clínica; de treinamentos com simulações práticas e testes documentados; não conformidade no manejo das tecnologias no preparo das TIVs; medicamentos de emergência indisponíveis, não monitorados e sem segurança. Dos dados resultantes das entrevistas emergiram duas categorias: As Tecnologias em Saúde nas Terapias Intravenosas Centrais Contínuas e Desafios no Manejo das Tecnologias em Saúde nas Terapias Intravenosas. Concluiu-se que o gerenciamento das tecnologias em saúde visa a organização do trabalho permitindo ao enfermeiro estabelecer prioridades durante sua prática, minimiza desperdícios com redução de custos, evita a ocorrência do (re)trabalho e principalmente fortalece a qualidade assistencial / The VIT is considered as an important therapeutic resource worldwide, and is indicated for the majority of hospitalized patients, representing sometimes a basic condition in its treatment, it is a common practice in the daily lives of nursing professionals. In this study we will deal with the continuous and central VITs. Today we have the technology and innovation to our favor making available on the market a range of products that will contribute to an efficient and safe Nursing care during the VITs. Therefore, the awareness of hospitals for the quality is an important tool in the expansion of patient safety and post-marketing surveillance. It has as an object of this study, the health technology used in continuous and central VIT during nursing care. The following objectives were tracings: identify the available health technology for use during the continuous and central VIT in Intensive Care Unit (ICU), check the use of these technologies by the nursing team in the care of continuous and central VITs installed in hospitalized patients in the ICC and discuss the challenges in the use of technologies during the continuous and central VIT. Qualitative descriptive study conducted in a university hospital by not participant means of observation guided by roadmap based on international standards of the Joint Commission International and semi structured interview. It was drawn up a corpus of data obtained from the interviews which was analyzed by Alceste Softwear. The results were grouped into structure, process and result in the use of VIT, noting: lack of monitoring of the records of the work by clinical engineering; trainings with practical simulations and documented tests; bad practices of technologies management in the preparation of VITs; unavailable emergency drugs, not monitored and no security. The interviews emerged two categories: The health technologies in continuous and central intravenous therapies and challenges in the health technologies management in intravenous therapies. It was concluded that the management of technologies in health aims to the organization of work allowing the nurse to establish priorities during his practice, minimizes waste with cost reduction, prevents the occurrence of (re)working and mainly strengthens health care quality
