Kroniska bensår : Personernas upplevelser / Chronic leg ulcers : People's experiences

Bensår är ett ökande problem i samhället, detta som följd av att vi blir allt äldre. Bensår är svåra att läka och det är vanligt att de återkommer. Kroniska bensår definieras som sår som inte läker inom sex veckor. De vanligaste orsakerna till bensår är venös insufficiens, arteriell insufficiens eller en bladning av dessa. Syftet med litteraturstudien var att beskriva personers upplevelser av att leva med kroniska bensår. Det här arbetet har utförts som en litteraturstudie, där sökning av artiklar har gjorts i två databaser CINAHL och PubMed. Efter sökning i databaserna har 16 artiklar valts ut och granskats. Analysen av artiklar har gjorts genom att plocka ut förekommande fenomen i texten och kategorisera dessa i huvudteman med underteman. Analysen resulterade i tre huvudteman: smärta, aktivitetsbegränsningar och lukt och vätskande sår. Det finns många olika faktorer som påverkar personernas upplevelser av att leva med bensår, där smärta, rörelseförmågan och lukt och vätska från såret har en central roll. Studien visar på att många personer upplever att deras problem inte tas på allvar. Det holistiska synsättet inom vården måste förbättras och mer hänsyn bör tas till personernas existentiella, psykiska, sociala och kroppsliga upplevelser.

Überprüfung der Validität eines funktionell kurzen Beines auf der Seite der Ilium-Subluxation

Die Anwendung von Beinlängentests spielt in der Chiropraktik als auch in der manuellen Medizin eine große Rolle. In den meisten Fällen ist beschrieben, dass Beinlängendifferenzen durch Subluxationen im Beckengürtel entstehen (Ausnahmen werden in Punkt 5.4 dargelegt). Diese These zu überprüfen und mit eigenen Erfahrungen abzugleichen, ist dem Autor schon seit geraumer Zeit ein Anliegen. Es ist auch zu klären, welche Art der Beckensubluxation zur Beinlängendifferenz (BLD) führt. Die Voraussetzungen für diese Untersuchung sind in der eigenen Praxis des Autors, im Haus der Chiropraktik gegeben, da alle Patienten ausschließlich nach Vorlage von Röntgenbildern behandelt werden. Diese werden in dem dafür entwickelten Programm X-Ray Solution vermessen und gespeichert. In der Sacro Occipital Technik (SOT) von Dr. M. B. DeJarnette DC, der Aktivator Technik, als auch der Thompson Terminal Point Technik (TTPT) wird auf der Seite des kurzen Beines von einem PI oder Ex Ilium ausgegangen. Als PI Ilium wird eine Beckenverschiebung nach posterior und inferior beschrieben. Referenzpunkt dafür ist die Spina iliaca posterior superior (SIPS). Verschiebt sich diese nach extern, so wird diese Subluxation als Ex Ilium bezeichnet. Je nach Technik werden aber vorher die Halswirbelsäule als Verursacher sowie eine bestimmte Art von Knie- und Fuß-Subluxationspattern ausgeschlossen.

Effects of Unilateral and Bilateral Plyometric Training on Power and Jumping Ability in Women

The purpose of this study was to examine the effects of unilateral and bilateral plyometric exercise on peak power and jumping performance during different stages of a 12-week training and detraining in women. Forty-nine untrained but physically active female college students were randomly assigned to 1 of × groups: unilateral plyometric group (n = 16), bilateral plyometric group (BLE; n = 18), and a control group (n = 15). Peak power and jumping ability were assessed by means of the alternate leg tests (10-second Wingate test and 5 alternate leg bounds), bilateral leg test (countermovement jump [CMJ]) and unilateral leg test (unilateral CMJ). Performance indicators were measured pretraining, midtraining, posttraining, and detraining. Differences between dependent variables were assessed with a × × 4 (group × time) repeated analysis of variance with Tukey's post hoc test applied where appropriate. Effect size was calculated to determine the magnitude of significant differences between the researched parameters. Only the unilateral plyometric training produced significant (p < 0.05) improvement in all tests from pretraining to midtraining, but there was no significant (p < 0.05) increase in performance indicators from midtraining to posttraining. The BLE group significantly (p < 0.05) improved in all tests from pretraining to posttraining and did not significantly (p > 0.05) decrease power and jumping ability in all tests during detraining. These results suggest that unilateral plyometric exercises produce power and jumping performance during a shorter period when compared to bilateral plyometric exercises but achieved performance gains last longer after bilateral plyometric training. Practitioners should consider the inclusion of both unilateral and bilateral modes of plyometric exercise to elicit rapid improvements and guard against detraining.

The aetiology and pathogenesis of tropical ulcer

Tropical ulcer is a very specific form of cutaneous ulceration. It occurs worldwide in most tropical and a number of subtropical areas. The disease occurs mainly in older children and young adults with children under the age of 5 and adults over 45 years rarely being affected. Ulcers occur most commonly on the lower leg but may occur on the upper limb. Although most ulcers normally heal slowly over many weeks or months, some ulcers may recur. Recognised complications include squamous cell carcinoma, gangrene and osteitis, although these are rare. A number of authors have reported on the disease and suggested diet, trauma and infection as aetiological factors for this condition. This survey was thus conducted to assess as many of these factors as possible. The study took place in 5 tropical areas, namely Zambia, Gambia, southern India, Fiji and Papua New Guinea. Consultations took place at hospitals, rural clinics, health centres and villages. Although many authors have suggested that the disease is related to malnutrition, few have objectively assessed the nutritional status of the patients and compared it with controls. Those studies which included objective assessments were limited to small areas and only investigated specific parameters. In order to investigate the immune response of the host to an anaerobic infection, the antibody levels to the organisms isolated from the ulcers were measured by an ELISA test. The local host response to an infection with a Fusobacterium species was assessed by the number of antibody secreting B-lymphocytes at the site of the ulcers. These parameters may play a role in the localisation of the ulcers and account for recurrent infections.

MEASUREMENT OF TEMPERATURE ON THE LEG OF A LUNAR LANDER

<p> The last decade has seen a proliferation of spaceflight ventures, sparking a new commercial Space Race. Companies ranging in size from SpaceX, Blue Origin, and Virgin Galactic to startups with just a few employees have submitted designs for a Lunar Lander. As the world shifts its attention back to the Moon, reducing mass and increasing safety in these systems has become vital. One avenue to weight reduction may be the legs of the lander. This experiment measures the heating of lunar lander legs from interaction with the lander’s rocket exhaust plume. The temperature of the legs was measured using thermocouples and thermochromic paint. Thirty-two thermocouples were attached in a grid pattern to generate a temperature map of the leg. Removable thermochromic paint shields provide an immediate temperature indicator so that leg distance and angle can be quickly adjusted without having to view the thermocouple temperature data. Heat transfer is also measured on the shield, finding radiation and convection. Ground tests show that the test methods, instruments, and hardware are reliable. Ground tests also show that the most significant heating and convection in ground-leg interactions occur on the bottom foot of the lander, with little heating on the top half of the leg. Further study of this heating will be vital for the future of lunar research </p>

Leg Length Discrepancy: A Study on In-Shoe Plantar Pressure Distribution

no / Leg length discrepancy (LLD) refers to the medical a condition where legs are of different lengths. This condition might affect gait and posture, and may lead to various orthopedic disorders that can have serious repercussions on the individual, be it physiological, psychological, social, economic, or ergonomic. In order to ameliorate the rehabilitation of individuals with LLD, it is imperative to understand the biomechanics of LLD in these individuals. Hence, the current study’s objective is to characterize the plantar pressures of individuals with Leg Length Discrepancy in comparison to those of asymptomatic individuals. This pilot study employs a pedar-x in-shoe pedobarograph system to gather the history of dynamic plantar pressures in one adult male individual with LLD and a representative adult normal volunteer with no diagnosed neurological or musculoskeletal disorders. The obtained results reveal quantifiable differences in the study metrics between the two individuals during walking. These results provide a proof-of-concept for this study, and may serve as diagnostic tools to better rehabilitate individuals with LLD and, thus, provide them with a better quality of life. Future work is to incorporate an extended study of 10 normal individuals versus 10 individuals with LLD, and includes both males and females, as well as both adults and adolescents.

Syftet med studien var att undersöka kommunsjuksköterskan omvårdnadsdokumentation om bensår i hemsjukvård. Som metod valdes journalgranskning. Resultatet visade att omvårdnadsdokumentationen brister på många sätt. / Background: In Sweden, approximately 50 000 people suffers from slow-healing leg ulcers. The number of municipal registered nurses ́ has decreased in recent years. The municipal registered nurses ́ often hold function as an advisory consultant for the nursing staff and without participating in nursing care. This documentation is based on second-hand information from the nursing staff. Purpose: The aim was to investigate how municipal registered nurse care documents about leg ulcers in patients in home care practice. Method: An audit of patient journals was chosen to investigate how municipal registered nurse ́ care documents about leg ulcers in patients in home care practice. Results: The results are presented in five themes: contact cause, health and medical history, skin functions alternatively skin / tissue, skin action alternatively action / prescription skin / tissue and Ulcer case record. Beneath these themes the assessment of documentation was into incomplete, partially complete and complete. The result shows that the municipal registered nurse often documents in chronological order and not applying to the nursing process. The results also showed that the old documentation was not completed in the journal, which meant that it was difficult to follow a course of treatment. Discussion: The result shows that documentation in home care failures in many ways. Municipal registered nurse works as a consultant and is not involved in daily care. The documentation is based on second-hand information from the nursing staff and resulting in the documentation of leg ulcers is poor. Patient journals documentation was divided into different keyword, which means that it was difficult to follow the nursing process.

A corelation study between step length, step frequency and the length of the leg : A study of running

Background: Running is one of the most popular branches of athletics. Running has several physiological benefits, such as a higher maximal oxygen consumption, increased muscle mass and reduced blood pressure. There have been plenty of discussions about what the most efficient running technique might be, should you use a short step length and a high step frequency or vice versa. It is generally known that you can increase your running speed by increasing your step frequency or your step length, and also both. However, maximal speed is achieved by increasing your step frequency rather than your step length. For endurance runners it is important to have a good running economy, because runners with a good running economy use less energy while running, and therefore require less oxygen than a runner with poor running economy. Previous research have not found any correlations between the length of the leg with step frequency or step length.  Aim: The aim of this study was to examine whether there are any significant correlations between the length of the leg in relation to the step frequency or the step length while running in different speeds.  Method: 15 trained males participated in this correlation study. All participants’ leg length were measured with a measuring tape. Three running trials, with the speeds of 12, 16 and 20 km/h were performed on a treadmill during 1 minute each. Between each trial the participants rested for 3 minutes. The trials were recorded with a video camera, and the participants individual step frequency, step length and heel or fore foot strike were collected. Persons coefficient of correlation test was used to analyze if there were any significant correlations between the length of the leg with step frequency and step length. Also a paired samples T test was performed to see if there was a significant increase between step frequency and step length with the speeds.  Result: There were no significant correlations (p&gt;0,05) found between the length of the leg and the step frequency or the length of the leg and step length in any of the different speeds. The paired samples t-test showed a significant (p&lt;0,001) increase in step frequency and step length with the different speeds.  Conclusion: In conclusion, according to the present study, there were no significant correlations found between the length of the leg with step frequency or step length, which is in consensus with previous research.  Therefore, it is not advisable to predict an individual’s step length based on their leg length. There are various factors involved in determining a person’s step length and step frequency, and this study showed that running speed is one important factor. This information may be useful for runners and their coaches to apply, so they avoid making interventions involving alternations in the runners step length based on their body dimensions, such as leg length.
