Estimativa das forças e torques articulares em seres humanos durante o andar em piscina com água rasa / Estimates of joint forces and torques during human walking in pool with shallow water

O ambiente aquático é um recurso bastante utilizado na prática de atividade física, reabiltação e lazer. No entanto, apesar de seu amplo uso, pouco se sabe sobre a demanda mecânica a que nosso corpo está sujeito durante a atividade física neste ambiente. O conhecimento das cargas sobre o corpo durante exercícios em água poderá permitir uma melhor compreensão das adaptações realizadas pelo ser humano para se movimentar neste ambiente e, ainda, auxiliar na prescrição correta de atividades físicas. O objetivo deste trabalho foi, portanto, estimar as forças e torques articulares sobre o tornozelo, joelho e quadril de adultos jovens, saudáveis, durante o andar em ambiente terrestre e aquático (água rasa). Para tanto, conduzimos um experimento no qual 10 indivíduos caminharam com velocidade auto-selecionada nos dois ambientes. Durante o andar medimos as componentes da força de reação do solo, usando uma plataforma de força, e filmamos os movimentos, com auxílio de um de sistema de aquisição de imagens. As forças e torques internos foram determinadas através da técnica da dinâmica inversa e de estimativas das forças de arrasto e respectivos torques, realizadas a partir da teoria das faixas (\"strip theory\"). Os resultados referentes à cinemática articular e descritiva e às forças de reação do solo foram, no geral, semelhantes aos resultados apresentados em estudos anteriores, indicando que não há mudanças na coordenação dos movimentos articulares nem re- organização temporal da passada durante o andar em água. Entretanto, foi possível perceber mudanças nos sentidos dos torques articulares durante o ciclo para o joelho e o quadril, sugerindo que, para manter a mesma coordenação articular sob a ação do empuxo e do arrasto, é necessário promover mudanças nos padrões dos torques internos nestas articulações. Com relação à articulação do tornozelo, os resultados mostram que há diminuição nos torques articulares necessários para gerar a flexão plantar durante o apoio. A estimativa das forças de arrasto se mostrou adequada para descrever a situação experimental, o que indica que os valores apresentados neste estudo são adequados para descrever a carga mecânica no aparelho locomotor durante o andar em piscina com água rasa. / The aquatic environment is a resource often used for the practice of physical activity, rehabilitation, and leisure. However, despite its extensive use, little is known about the mechanical demand that our body is subjected during physical activity in this environment. The knowledge of the load on the human body during water exercise will give us a greater understanding of the adaptations of the human being in water and assist the health professional to the correct prescription for physical activities. The goal of this work was, therefore, to estimate the internal joint forces and moments on the ankle, knee, and hip joints of young healthy adults during walking in the terrestrial and aquatic (shallow water) environments. For that, we conducted an experiment in which 10 subjects walked with self-selected speed in both environments. During the experiment, we measured the components of ground reaction forces, with a force plate, and recorded the subject\'s movements with a video analysis system. To find the forces and moments we used the inverse dynamics procedure and the calculation of drag forces and moments were based on the strip theory. The results concerning the joints and descriptive kinematics and the ground reaction forces were, in general, similar to the results presented in previous studies, indicating that there aren\'t changes in the coordination of joint movements or a re-organization of the stride during walking in water. Meanwhile, we were able to see changes in the directions of joint moments during the gait cycle for the knee and hip joints, suggesting that to keep the same joint coordination under the action of drag and buoyancy, it is necessary to promote changes in the patterns of internal moments in these joints. As for the ankle, the results showed that there is a decrease in joint moments necessary to generate the plantar flexion during the support phase. The estimation of the drag forces was adequate to describe the experimental situation, which suggests that the values presented here can describe correctly the mechanical load on the locomotor system during walking in shallow water.

Lombalgies chroniques : évaluation des facteurs mécaniques des membres inférieurs au moyen des relations moment-vitesse / Low back pain : evaluation of lower limbs mechanical parameters from torque-velocity relationships under isokinetic conditions

La lombalgie est un problème de santé public induisant de nombreuses répercussions sur le plan économique et social. Le passage à la chronicité concerne moins de 10% des lombalgies, mais représente 85% des coûts liés à la prise en charge de cette pathologie. Dans ce contexte, il parait indispensable de pouvoir optimiser les programmes de réentrainement à l’effort du lombalgique chronique afin de les rendre le plus efficace et le plus pertinent possible. Ce travail de thèse a pour objectif d’évaluer les facteurs mécaniques des membres inférieurs au moyen des relations moment-vitesse dans des conditions isocinétiques. Dans une première étude, nous avons évalué les muscles extenseurs du genou et les muscles fléchisseurs et extenseurs du tronc chez des sujets lombalgiques et des sujets sains. Nous avons, ainsi mis en évidence une faiblesse significative au niveau de la force et de la puissance des muscles extenseurs du genou chez les sujets lombalgiques associée à la faiblesse musculaire du tronc classiquement évoquée pour cette population. Après avoir mis en place, dans une seconde étude, un protocole d’évaluation permettant d’établir des relations moment- et puissance-vitesse pour les muscles extenseurs et fléchisseurs de la hanche, nous avons évalué la force et la puissance de ces groupes musculaires chez des sujets lombalgiques. Les résultats ont, comme pour la première étude, mis en avant une faiblesse significative au niveau de la force et de la puissance de ces groupes musculaires, par rapport à des sujets sains, avec un déficit plus marqué au niveau des extenseurs, comme pour le tronc. L’ensemble des résultats obtenus dans le cadre de ce travail de thèse démontre qu’il est important de proposer un travail de force et de puissance au niveau des membres inférieurs pour les patients lombalgiques chroniques afin de mieux lutter contre le syndrome de déconditionnement qui touche l’ensemble des chaines musculaires des patients. / Low back pain is a public health problem inducing economical and social consequences. Chronicity involves less than 10% of low back pain, but represents 85% of the total costs related to this pathology. In this context, it seems essential to optimize chronic low back pain rehabilitation programs to make them more effective and relevant. The purpose of this phD was then to evaluate lower limbs mechanical factors using torque- and power-velocity relationships. In a first study, knee extensors muscles and trunk flexor and extensor muscles were evaluated in chronic low back pain and healthy subjects. A significant knee extensor strength and power weakness weas observed for chronic low back pain subjects, associated with the typical trunk weakness referred to this population. The second study focused on establishing a protocol allowing assessing hip flexor and extensor torque- and power-velocity relationships. This protocol was then applied to evaluate flexor and extensor hip muscles in chronic low back pain subjects. Results, in accordance with the first study, highlighted a significant strength and power weakness for hip muscle groups in chronic low back pain relative to healthy subjects, with a greater hip extensor deficit, as it is generally observed for the trunk.In conclusion, these different studies showed the importance of proposing torque and power lower limbs rehabilitation for chronic low back pain patients to better fight against the deconditioning syndrome that affects all muscular chains in this population.

Title: Selected parameters of postural stability in elite sport dance group. Objectives: The aim of this study is to evaluate selected parameters of postural stability in elite sport dancers in Czech Republic during their preparatory period and to define the relation of postural stability to body composition, equality and ability of explosive strength production of lower limbs. Methods: The group of 10 elite sport dancers, composed of men (n=5, age=25,1±2,4 years, height=181,9±3,8 cm, weight=73,7±7,5 kg) and women (n=5, age=24±3,2 years, height=169,1±5,7 cm, weight=53,8±3,6 kg), was longitudinally observed during their preparatory period. At the beginning and on the top (national championship) postural stability, body composition and dynamic performance were evaluated. These following devices were used for testing: force plate FootScan (RScan International, Belgium), bioimpedance analyzer BIA 2000M (Datainput, Germany) and Tanita (Tanita Corporation, Japan), dynamometric force plates Kistler 8611 (Kistler, Switzerland). Results were evaluated by descriptive and inductive (paired t-test) statistics. Results: Results showed high similarity of followed selected parameters in elite sport dancers at the beginning and also at the top of preparatory period. TMS stimulus-response asymmetry in lower limbs : Difference in stimulated muscles between dominant and non-dominant leg

Transcranial magnetic stimulation (TMS) is a 37-year-old non-invasive tool and can be used for diagnostic, therapeutics, and research purposes. In research, TMS is mostly used to stimulate the motor cortex, resulting in a neuroelectric excitatory response called a motor evoked potential (MEP). The resulting nerve signal leads to muscle movement, which can be measured by electromyography (EMG). Majority of previous research has targeted muscles of the upper limbs, due to the relative inaccessibility of the cortical leg area. Thus, the aim of this study is to investigate whether asymmetry occurs during lower limb stimulation and if there is a difference in stimulated muscles between dominant and non-dominant leg. Nine healthy adults conducted cortical stimulation over the motor cortex using double cone coil. EMG was recorded from the rectus femoris, tibialis anterior and abductor hallucis on both left and right leg. Depending on the subject's tolerance, data was collected by delivering 30 or 35 pulses. For each intensity, five MEPs were recorded, starting at 30% of the intensity and increasing in steps of 10%. Results showed no significant difference (p>0,05) between the dominant and non-dominant leg in all three muscles. In conclusion, the study no stimulus response asymmetry between the dominant and non-dominant leg in the respective muscle.

A doença arterial coronariana (DAC) pode alterar o balanço simpato-vagal do Sistema Nervoso Autônomo (SNA) e aumentar os riscos de arritmias fatais e morte súbita. O exercício físico pode reverter essa condição, porém poucos estudos analisam o ajuste hemodinâmico ao exercício dinâmico resistido, especialmente em portadores de DAC. Além disso, exercícios de membros superiores podem induzir diferentes repostas comparado aos de membros inferiores, porém os estudos comparando os dois tipos de exercícios são escassos. O objetivo do estudo foi analisar as respostas cardiovasculares no exercício resistido de membros superiores e compará-las com o exercício resistido de membros inferiores, em idosos saudáveis e em portadores de DAC. Para a realização do estudo, foram selecionados 20 indivíduos do sexo masculino, sendo 10 idosos saudáveis e 10 idosos portadores de DAC, não etilistas, não tabagistas, e sem distúrbios respiratórios, neurológicos, metabólicos e articulares. Foram realizados: teste de 1RM em exercício resistido no supino inclinado e no leg-press 45°; teste de esforço físico dinâmico resistido com diferentes percentuais de 1RM, com carga inicial de 10% da 1RM e increntos de 10% da 1RM, e a partir de 30% os incrementos passaram a ser de 5% da 1RM. No supino inclinado, houve diminuição significativa do índice rMSSD a partir de 30% da 1RM (GC: de 20±2 ms para 11 ±3 ms; GDAC: de 19±3 ms para 9±1 ms) em ambos os grupos, semelhantemente ao índice SD1 (GC: de 14±2 ms para 8±1; GDAC: 14±2 ms para 7±1 ms). O índice RMSM se manteve sem diferenças entre as cargas no grupo controle (GC) (de 28±3 ms para 45±9 ms), porém aumentou significativamente no grupo DAC (GDAC)(22±2 ms para 79±33 ms). A FC aumentou significativamente a partir de 30% da 1RM em ambos os grupos (GC: de 69±3 bpm para 93±6 bpm; GDAC: 59±3 bpm para 75±4 bpm). No leg-press 45º, houve diminuição do índice rMSSD a partir de 30% da 1RM em ambos os grupos (GC: 29±5 ms para 12±2 ms; GDAC: 28±4 ms para 18±3 ms). O índice SD1 diminuiu no GC a partir de 30% da 1RM (de 23±4 mas para 7±1) e no GDAC a partir de 20% da 1RM (de 16±3 para 11±1). O índice RMSM diminuiu a partir de 30% da 1RM no GC (34±5 ms para 14±3 ms) e aumentou a partir de 35% da 1RM no GDAC (28±4 ms para 43±5 ms). A FC aumentou a partir de 30% da 1RM no GC (65±3 bpm para 92±4 bpm) e a partir de 35% da 1RM no GDAC (61±1 para 76±3). Comparando as variáveis entre os equipamentos, observaram-se menores valores do índice SD1 no GDAC a partir de 35% da 1RM no supino. Também se observaram maiores valores do índice RMSM no GDAC a partir de 30% da 1RM no supino inclinado. Como conclusão, pode-se inferir que houve diminuição parassimpática e aumento da modulação simpática a partir de 30% da 1RM em ambos os equipamentos, sendo que o supino inclinado produziu marcada atenuação parassimpática associada a maior modulação simpática. / Coronary artery disease (CAD) can alter the balance of the sympatho-vagal autonomic nervous system (ANS) and increase the risk of fatal arrhythmias and sudden death. Exercise can reverse this condition, but few studies analyze the hemodynamic adjustment to dynamic resistance exercise, especially in patients with CAD. In addition, upper exercise may induce different responses compared with the lower limbs, but studies comparing the two conditions of resistance exercise are scarce. The objective of the study was to assess the cardiovascular responses in resistance of the upper and compare them with the lower limb resistance exercise in healthy elderly and in patients with CAD. We selected 20 males, 10 healthy elderly and 10 elderly patients with CHD, non-alcohol drinkers, non-smokers, and without respiratory, neurological and metabolic diseases. Were performed: 1RM test in the bench press inclined and leg-press 45 °, and an incremental dynamic resistance test different percentages of 1RM, with initial load of 10% of 1RM and increments of 10% of 1RM, and from 30 % increments was 5% of 1RM. On the bench press inclined, there was significant decrease in RMSSD index from 30% of 1RM (CG: 20 ± 2 ms to 11 ± 3 ms; GDAC: 19 ± 9 ms to 3 ± 1 ms) in both groups, similarly SD1 index (CG: 14 ± 2 to 8 ± 1 ms; GDAC: 14 ± 2 ms to 7 ± 1 ms). The RMSM index remained no differences between the loads in the control group (CG) (28 ± 3 ms to 45 ± 9 ms), but increased significantly in the CAD group (GDAC) (22 ± 2 ms to 79 ± 33 ms). The HR increased significantly from 30% of 1RM in both groups (CG: 69 ± 3 bpm to 93 ± 6 bpm; GDAC: 59 ± 3 bpm to 75 ± 4 bpm). In the leg-press 45º, the RMSSD index decreased from 30% of 1RM in both groups (CG: 29 ± 5 ms to 12 ± 2 ms; GDAC: 28 ± 4 ms to 18 ± 3 ms). The SD1 index decreased in GC from 30% of 1RM (23 ± 4 ms to 7 ± 1 ms) and GDAC from 20% of 1RM (16 ± 3 ms to 11 ± 1 ms). The RMSM index decreased from 30% of 1RM in the GC (34 ± 5 ms to 14 ± 3 ms) and increased from 35% of 1RM in the GDAC (28 ± 4 ms to 43 ± 5 ms). The HR increased from 30% of 1RM in the GC (65 ± 3 bpm to 92 ± 4 bpm) and from 35% of 1RM in the GDAC (61 ± 1 to 76 ± 3). Comparing the variables between the equipment, there were lower values for SD1 from GDAC in 35% of 1RM on the bench press. Also observed higher values in the index RMSM in GDAC from 30% of the 1RM bench press inclined. It can be inferred that decreased parasympathetic and increased in sympathetic modulation from 30% of 1RM in both equipments, and the bench press inclined produced marked attenuation in parasympathetic modulation with an increased in sympathetic modulation.

Darbe yra atliekama čiurnos sąnario biomechanikos analizė, įtvarų konstrukcijų ir medžiagų analizė, nagrinėjama ilgaamžiškumo reikšmė ir tyrimo būdai. Sudaroma tyrimo metodika. Kompiuterinio modeliavimo būdu, panaudojant SolidWorks programinę įrangą, yra sudaromas įtvaro modelis, nustatomos veikiančios apkrovos, atliekami įtempių ir poslinkių pasiskirstymo tyrimai. Atliekami modelio nuovargio bandymai. Atsižvelgiant į tyrimų rezultatus, yra sudaromi įtvarų iš aukštos temperatūros plastikų tinkamo pritaikymo nurodymai. Pateikiamos išvados ir literatūros sąrašas. / Final thesis presents the research of durability of lower limbs splints. The problem of fatigue behaviour of ankle joint splint is raised and its solution is suggested. Final thesis contains ankle joint biomechanics analysis, lower limbs splints design and material analysis. Review of durability research methods is also done. The methodology of research is arranged. Using the computer aided modeling with SolidWorks software the model of ankle joint splint is created. Load parameters are determined, research of stresses and deformations are performed. Fatigue test is also performed. Due to the results of research, the instructions for ankle joint splints made of high density polyethylene and polypropylene use and adaptation are listed. Final thesis also contains conclusions and list of literature. Size of work – 77 pages of text without attachments, 55 pictures, 9 tables and 25 bibliographical sources.

Déterminants du handicap moteur en République de Guinée : causes et conséquences des déficiences des membres inférieurs chez les habitants de Conakry / Determinants of motor disability in the Republic of Guinea : causes and consequences of lower limbs deficiencies among the inhabitants of Conakry

Lorsqu’elle s’inscrit dans un contexte tel que celui des pays en développement, la notion de handicap se caractérise par des tendances bien singulières. C’est notamment en République de Guinée que nous avons décidé de centrer notre travail, traitant des problématiques liées aux handicaps physiques et plus particulièrement aux déficiences des membres inférieurs. Les objectifs fixés dans cette recherche consistent à expliquer les causes et les conséquences de la prédominance de ce type de déficiences à Conakry. De plus, parmi d’autres problématiques que soulève notre travail, nous nous intéressons aux représentations que reflètent les handicaps et aux influences qu’elles subissent de la part des croyances traditionnelles et religieuses. Notre travail s’inscrit donc à la fois dans la volonté d’établir un état des lieux de la situation actuelle et dans la réalisation d’un important travail de terrain qui a été mené à Conakry entre 2008 à 2011 auprès d’un millier de personnes. D’après nos résultats, la principale cause des déficiences des membres inférieurs est liée aux séquelles de la poliomyélite et à la présence d’autres facteurs de risques tels que l’insécurité routière. De plus, selon nos analyses, près de 80% des personnes atteintes de ce type de déficience résidant à Conakry sont issues de l’exode rural. Une fois installées à Conakry, elles optent pour les trois modes de subsistance à savoir : la pratique de la mendicité (48%), l’occupation d’un emploi (16%) et la prise en charge familiale (18%). Par ailleurs, chaque mode semble déterminer leur participation et leur intégration au sein de la société. Enfin, nos résultats montrent que près de la moitié des Guinéens croit en la sorcellerie et en la nature maléfique des handicaps. Comme nous l’expliquons, ces croyances influencent négativement les représentations des handicaps. Pour conclure, selon nos analyses la majorité des Guinéens s’accorde à définir le handicap comme un phénomène biomédical et le considère comme un problème individuel et non social. / While dealing with the context of the developing countries, the notion of handicap is characterized in a specific way. We carried out our study in the Republic of Guinea, particularly related to the problems of the handicap, more specifically with the disability of inferior members. The main objectives of this research consist in explaining the causes and the consequences of the ascendancy of this type of deficiencies in Conakry. Furthermore, among the other problems analyzed in this work, we are interested in the representations which reflect the handicaps and the influence of traditional and religious beliefs on handicap. Our work is aimed to present the current conditions of the situation and to carry out a significant work on the site. In this framework, we organized two surveys during 2008 and 2009, dealing with approximately 1000 persons. According to our results, the main cause of the deficiencies of lower limbs is associated to the aftereffects of the poliomyelitis and to the presence of the other risk factors, such as the lack of safety on the roads. Furthermore, according to our analyses, about 80 % of the people questioned, belonged to the rural depopulation. Once installed in Conakry, they opt for three modes of subsistence: the practice of begging (48 %), working (16 %) and the family care (18 %). Moreover, each mode seems to determine the participation and the integration of these individuals in the society. Finally, our results show that nearly half of the Guineans believe in the witchcraft and in the malefic nature of the handicaps. Consequently, the faith influences negatively on the interpretation of the handicaps. To conclude, according to our analyses, the majority of the Guineans agrees to define the handicap as a biomedical phenomenon and considers it as an individual and not a social problem.

Title: The relevance of Bosco test for determination of healthy oriented efficiency adolescents Objective: The main objective is to find and describe the relationship of Bosco test (60 s) to a specific level of health-related fitness among adolescents based on the comparison of the output parameter of Bosco test and test items EUROFIT at physically active and inactive boys and girls. The partial objectives are to describe the influence of anthropometric parameters, gender and competence to physically active people on the current level of power parameters of the lower limbs. Methods: The dissertation has the character of empirical research, it is a descriptive study in which the research has a nature of the association, ie., that the required data are obtained by observation. The study was carried out on a group of purposefully selected probands n=100 age 18.40 ± 0.83 years, a period of adolescence. In the study group was equal representation of boys n=50 (age 18.50 ± 0.84 years) and girls n=50 (age 18.38 ± 0.82 years).
