Mödrars upplevelse av stöd vid amningsstarten

Introduction Statistics show a declining trend despite the many benefits of breastfeeding. Aim The aim was to investigate how new mothers experienced the care associated with childbirth, with focus on breastfeeding. Method The study included mothers who gave birth in a hospital in the middle region of Sweden during 2011, and who responded to an online survey (n≈1000). The present study was based upon answers from the open questions concerning dissatisfaction and suggestions for improvement and have been categorized using content analysis. Results Mothers described failure in several areas, with a negative impact on breastfeeding. Many experienced that they had not been seen and treated like an individual, and that the healthcare staff had been unpleasant, impersonal, ignorant and stressed. Mothers wanted more credit from the staff concerning their own ability and some reported that they had been controlled and ”run over”. They experienced care work as unstructured with failure in both procedures and communication. Lack of staff continuity made it difficult for mothers to feel trust and create a relationship. Furthermore, major failure in information and support was reported. Conclusion Although the majority of mothers were satisfied with the early care after giving birth, the open answers showed several areas in need of improvements, among other things individualized care was requested.

Bakgrund: Amning är ur flera aspekter det bästa för både moder och barn, då bröstmjölken bidrar till barnets hälsoutveckling och amningen främjar anknytningen dem emellan. Därav finns det tydliga amningsrekommendationer, men trots detta ses en nedåtgående amningsfrekvens i Sverige. En bidragande faktor till den låga amningsfrekvensen skulle kunna vara de amningssvårigheter många mödrar upplever initialt, vilket kan föranleda ett behov av amningsstöd. Då vårdtiderna postnatalt är korta hinner inte alltid amningssvårigheter upptäckas innan hemgång. Detta ställer krav på barnhälsovårdssjuksköterskan som ofta blir den första kontakten efter hemgång med en viktig uppgift att stödja mödrars fortsatta amningsprocess. Syfte: Syftet med studien var att belysa mödrars upplevelse av barnhälsovårdssjuksköterskans amningsstöd. Metod: Studien har en kvalitativ design och datainsamlingen genomfördes via semistrukturerade intervjuer med elva informanter från babycaféer och familjecenter i södra Skåne. Insamlat material analyserades utifrån en kvalitativ innehållsanalys med induktiv ansats. Resultat: Analysen av intervjumaterialet mynnade ut i två kategorier: Att bli sedd och Att känna sig ensam. Konklusion: Studiens resultat har synliggjort att barnhälsovårdssjuksköterskan har en central roll i att stödja mödrar i sin amning. Relationen dem emellan och på vilket sätt barnhälsovårdssjuksköterskan bemöter mödrarna är av betydelse för upplevelsen av amningsstödet. Mödrarna beskrev intresse, kunskap samt att få ett stöd anpassat efter sin unika situation som viktiga komponenter i att känna ett tillfredsställande amningsstöd. / Background: Breastfeeding is in many aspects the best for both mother and child, as breast milk contributes to the child's health development and breastfeeding promotes the connection between them. Breastfeeding is clearly recommended but nevertheless there are a downward trend in breastfeeding in Sweden. A contributing factor to the low breastfeeding rate may be the lactation difficulties many mothers initially experience, which can cause them to require breastfeeding support. Since post-natal care times are short, breastfeeding difficulties are not always detected before returning home. This place demands on the child health nurse who often becomes the first contact after returning home with an important task to support the mothers’ continued breastfeeding process. Aim: The purpose of this study was to shed light on mothers´ experience of the child health nurse lactation support. Method: The study is approached with a qualitative design and the data was collected via semistructured interviews with eleven participants from baby cafes and family centers in Skåne. The collected material was analyzed based on a qualitative content analysis with inductive approach. Results: The analysis of the interview material resulted in two categories: Being acknowledged and Feeling alone. Conclusion: The result of this study has highlighted that the child health nurse has a central role in supporting mothers´ in their breastfeeding. The relationship between them and in what way the child health nurse met with the mothers is of importance to the experience of lactation support. The mothers described interest, knowledge, and support according to their unique situation as important components of feeling satisfactory lactation support.
