Among the various specialties, oncology is among those dealing with the greatest difficulties in the management of drug therapy. It is known that patients treated with various drugs have variability of response and susceptibility to drug toxicity. In present work, we study the role of Multiple Drug Resistance gene (MDR1) polymorphisms C1236T and C3435T frequencies and response to treatment with imatinib mesylate in 96 patients with chronic myeloid leukemia (CML). A total of 96 patients with CML were treated according to the Brazilian National Cancer Institute (INCA) guidelines and the blood samples were collected for genotyping. Genomic DNA was extracted and C1236T and C3435T polymorphisms genotyping was performed by the polymerase chain reaction with restriction fragment length polymorphism (PCR-RFLP), which detects a variation in length of a DNA fragment generated (370pb and 340pb) by a specific endonuclease in a specific site of the genome (HaeIII and MboI). Of the 96 CML samples, 31 samples were homozygous (CC), 13 homozygous (TT) and 52 heterozygous (CT) for exon 12 (1236). For the exon 26 (3435), 35 were homozygous (CC), 12 homozygous (TT) and 49 heterozygotes (CT). All frequencies for both polymorphisms were in Hardy-Weinberg equilibrium (p = 0.229 and q = 0.414). We found percentage association between polymorphisms and their distribution in different populations, and the response to treatment both cytogenetic and molecular difference was not statistically significant (p <0.05) when compared to age and sex presented response by patients and also no statistical difference (p <0,050). We conclude that the observed allele frequency for exons 1236 were 59.4% for C and 40.6% for T and the frequencies for the exon 3435 were 62.0% for C and 38.0% for T. That the relationship between the frequencies of polymorphisms of MDR1 in populations of different geographic locations, can provide tools that help in choosing a more appropriate and effective treatment of CML. / Neste estudo, o papel dos polimorfismos C1236T e C3435T do gene de Resistência a Múltiplas Drogas (MDR1) foram investigados em relação à frequência e a resposta ao tratamento com imatinibe em pacientes com leucemia mielóide crônica (LMC). Um total de 96 pacientes com LMC foram tratados de acordo com as diretrizes do Instituto Nacional do Câncer (INCA) e amostras de sangue foram coletadas para genotipagem do gene MDR (Resistência à Multiplas Drogas). O DNA genômico foi extraído e a genotipagem dos polimorfismos C1236T e C3435T foi realizada por meio da reação em cadeia da polimerase com fragmentos de restrição (PCR-RFLP), que detectou uma variação no comprimento de um fragmento de DNA gerado (370pb e 340pb) por uma endonuclease específica em um sítio específico do genoma (HaeIII e Mbol). Analisando as 96 amostras de pacientes para o polimorfismo no éxon 12 (1236) com LMC, 31 amostras apresentaram homozigose (CC), 13 homozigose (TT) e 52 heterozigose (CT). Para o estudo do polimorfismo no éxon 26 (3435), 35 foram homozigotas (CC), 12 homozigotas (TT) e 49 heterozigotas (CT). Todas as frequências para ambos os polimorfismos apresentaram-se em equilíbrio de Hardy-Weinberg (p = 0,229 e q = 0,414). Foi encontrada associação do percentual dos polimorfismos estudados em relação à distribuição dos mesmos em grupos de diferentes localizações geográficas, e sobre a resposta ao tratamento tanto citogenética e molecular, não houve diferença estatísticamente significante (p<0,05), quando foi comparado à idade e ao gênero apresentados pelos pacientes e a resposta também não houve diferença estatística (p<0,05). Conclui-se que as frequências alélicas observadas para o éxon 1236 foram de 59,4% para C e 40,6% para T e as frequências para o éxon 3435 foram de 62,0% para C e 38,0% para T e que a relação entre as frequências de polimorfismos de MDR1 nas populações de diferentes localizações geográficas, pode fornecer ferramentas que auxiliem na escolha de um tratamento mais adequado e eficaz da LMC.

Estudo dos polimorfismos C1236T, G2677T e C3435T do gene MDR1 em pacientes portadores de doenças inflamatórias intestinais / Study of C1236T, G2677T, C3435T MDR1 gene polymorphisms in patients with inflammatory bowel disease

Estudos recentes têm avaliado a presença de polimorfismos do gene multidroga resistente 1 (MDR1), que codifica o transportador de membrana de efluxo chamado de P-glicoproteína, seu potencial papel na suscetibilidade das doenças inflamatórias intestinais (DII) e suas possíveis correlações com aspectos clínicos das DII. Dados conflitantes podem resultar da análise genética de populações distintas. Investigamos se os polimorfismos do gene MDR1 estão associados com as DII em população do sudeste do Brasil e suas possíveis correlações com fenótipos, atividade de doença, resposta ao tratamento e efeitos colaterais. Como métodos, a presente pesquisa trabalhou com 146 pacientes com Doença de Crohn (DC) e 90 com Retocolite Ulcerativa Idiopática (RCUI), que foram recrutados através de critérios diagnósticos estabelecidos. Os polimorfismos do MDR1 mais comumente descritos na literatura, C1236T, G2677T e C3435T, foram avaliados por PCR. As frequências genotípicas de pacientes com RCUI e DC foram analisadas na população de estudo. Associações de genótipo-fenótipo com características clínicas foram estabelecidas e riscos estimados para as mutações foram calculados. Nenhuma diferença significativa foi observada nas freqüências genotípicas para os polimorfismos G2677T/A e C3435T do MDR1 na DC ou na RCUI. O polimorfismo C1236T foi significativamente mais comum na DC do que na RCUI (p = 0,036). Na RCUI foram encontrados mais homens nos polimorfismos C1236T e G2677T no grupo de heterozigotos. Foram encontradas associações significativas entre o polimorfismo C3435T do gene MDR1 em pacientes com fenótipo estenosante na DC (OR: 3,16, p = 0,036), em oposição ao comportamento penetrante (OR: 0,31, p = 0,076). Na DC, associações positivas também foram encontradas entre o polimorfismo C3435T, à atividade moderada/severa da doença (OR: 3,54, p = 0,046), e à resistência / refratariedade ao corticosteróide (OR: 3,29, p = 0,043) nos homozigotos polimórficos. Nenhuma associação significativa foi encontrada entre os polimorfismos do MDR1 e categorias fenotípicas, atividade de doença ou resposta ao tratamento da RCUI. Em conclusão, os resultados do presente estudo sugerem que os polimorfismos do gene MDR1 poderiam estar implicados na susceptibilidade a DC e no seu fenótipo estenosante, como também estarem associados com uma resposta inadequada ao tratamento em um grupo de pacientes com DC. A forte relação com a DC suporta a existência de papéis adicionais para o MDR1 em mecanismos específicos subjacentes na patogênese da DC, como o controle da microbiota intestinal, mediação e regulação da fibrose. Além disso, compreender os efeitos de vários fármacos associados a estas variantes do MDR1 pode contribuir para a prescrição personalizada de regimes terapêuticos.

This thesis is focused on polymorphism of MDR1 gene c.-6-180T>G, which causes phenobarbital resistance. Phenobarbital is one of the most commonly used drug against epilepsy, which is one of the most common neurological disease in dogs with frequency between 0,5-5 % according to breed. This mutation has so far been detected only in the border collie breed, which manifests up to 30 percent resistance to the treatment of epilepsy. In the text is subscribed border collie breed, dog epilepsy and gene MDR1. Methodical part is focused on detection of the polymorphism in model population of 82 dogs. The results agree with previous studies and confirm frequency of mutant alleles 32 % within the population.

Analysis of the mechanism and the regulation of histatin 5 resistance in \(Candida\) \(albicans\) / Analyse des Mechanismus und der Regulierung von Histatin 5 Resistenz in \(Candida\) \(albicans\)

Antimycotics such as fluconazole are frequently used to treat C. albicans infections of the oral mucosa. Prolonged treatment of the fungal infection with fluconazole pose a risk to resistance development. C. albicans can adapt to these stressful environmental changes by regulation of gene expression or by producing genetically altered variants that arise in the population. Adapted variants frequently carry activating mutations in zinc cluster transcription factors, which cause the upregulation of their target genes, including genes encoding efflux pumps that confer drug resistance. MDR1, regulated by the zinc cluster transcription factor Mrr1, as well as CDR1 and CDR2, regulated by the zinc cluster transcription factor Tac1, are well-known examples of genes encoding efflux pumps that extrude the antimycotic fluconazole from the fungal cell and thus contribute to the survival of the fungus. In this study, it was investigated if C. albicans can develop resistance to the antimicrobial peptide histatin 5, which serves as the first line of defence in the oral cavity of the human host. Recently, it was shown that C. albicans transports histatin 5 outside of the Candia cell via the efflux pump Flu1. As efflux pumps are often regulated by zinc cluster transcription factors, the Flu1 efflux pump could also be regulated by a zinc cluster transcription factor which could in a hyperactive form upregulate the expression of the efflux pump, resulting in increased export of histatin 5 and consequently in histatin 5 resistance. In order to find a zinc cluster transcription factor that upregulates FLU1 expression, a comprehensive library of C. albicans strains containing artificially activated forms of zinc cluster transcription factors was screened for suitable candidates. The screening was conducted on medium containing mycophenolic acid because mycophenolic acid is also a substrate of Flu1 and a strain expressing a hyperactive zinc cluster transcription factor that upregulates FLU1 expression should exhibit an easily recognisable mycophenolic acid-resistant phenotype. Further, FACS analysis, quantitative real-time RT-PCR analysis, broth microdilution assays as well as histatin 5 assays were conducted to analyse the mechanism and the regulation of histatin 5 resistance. Several zinc cluster transcription factors caused mycophenolic acid resistance and upregulated FLU1 expression. Of those, only hyperactive Mrr1 was able to confer increased histatin 5 resistance. Finding Mrr1 to confer histatin 5 resistance was highly interesting as fluconazole-resistant strains with naturally occurring Mrr1 gain of function mutations exist, which were isolated from HIV-infected patients with oral candidiasis. These Mrr1 gain of function mutations as well as artificially activated Mrr1 cause fluconazole resistance by upregulation of the efflux pump MDR1 and other target genes. In the course of the study, it was found that expression of different naturally occurring MRR1 gain-of-function mutations in the SC5314 wild type background caused increased FLU1 expression and increased histatin 5 resistance. The same was true for fluconazole-resistant clinical isolates with Mrr1 gain of function mutations, which also caused the overexpression of FLU1. Those cells were less efficiently killed by histatin 5 dependent on Mrr1. Surprisingly, FLU1 contributed only little to histatin 5 resistance, rather, overexpression of MDR1 mainly contributed to the Mrr1-mediated histatin 5 resistance, but also additional Mrr1-target genes were involved. These target genes are yet to be uncovered. Moreover, if a link between the yet unknown Mrr1-target genes contributing to fluconazole resistance and increased histatin 5 resistance can be drawn remains to be discovered upon finding of the responsible target genes. Collectively, this study contributes to the understanding of the impact of prolonged antifungal exposure on the interaction between host and fungus. Drug therapy can give rise to resistance evolution resulting in strains that have not only developed resistance to fluconazole but also to an innate host mechanism, which allows adaption to the host niche even in the absence of the drug. / Antimykotika wie Fluconazol werden häufig zur Behandlung von C. albicans Infektionen der Mundschleimhaut verwendet. Dabei stellt eine langzeitige Behandlung der Pilzinfektion mit Fluconazol ein Risiko zur Resistenzentwicklung dar. C. albicans kann sich an solche Umweltveränderungen anpassen, indem es die Genexpression reguliert oder genetisch veränderte Varianten produziert, welche in der Population entstehen. Adaptierte Varianten tragen häufig aktivierende Mutationen in Zink-Cluster-Transkriptionsfaktoren, welche die Hochregulierung der Expression von Genen verursachen, darunter solche, die für Multidrug-Effluxpumpen kodieren und dadurch Antimykotikaresistenz verleihen können. MDR1, reguliert durch den Zink-Cluster-Transkriptionsfaktor Mrr1, sowie CDR1 und CDR2, reguliert durch den Zink-Cluster-Transkriptionsfaktor Tac1, sind bekannte Beispiele für Effluxpumpen, die das Antimykotikum Fluconazol aus der Pilzzelle extrudieren und somit zum Überleben der Pilzzelle beitragen. In dieser Arbeit wurde untersucht, ob C. albicans eine Resistenz gegen das antimikrobielle Peptid Histatin 5 entwickeln kann, das in der Mundhöhle des menschlichen Wirtes als erste Verteidigungsbarriere gegen den Pilz dient. Kürzlich wurde gezeigt, dass C. albicans Histatin 5 über die Effluxpumpe Flu1 aus der Candia-Zelle heraustransportiert (Li et al., 2013). Da Effluxpumpen häufig durch Zink-Cluster-Transkriptionsfaktoren reguliert werden, könnte auch die Flu1-Effluxpumpe durch solch einen Transkriptionsfaktor reguliert werden, der in einer hyperaktiven Form die Expression der Effluxpumpe hochregulieren könnte, was wiederrum zu einem erhöhten Export von Histatin 5 und folglich zur Histatin 5 Resistenz führen könnte. Um einen Zink-Cluster-Transkriptionsfaktor zu finden, der die FLU1-Expression hochreguliert, wurde mit Hilfe einer Bibliothek von C. albicans-Stämmen, die künstlich aktivierte Formen von Zink-Cluster-Transkriptionsfaktoren enthält, nach geeigneten Kandidaten gesucht. Das Screening wurde auf Mycophenolsäure-haltigem Medium durchgeführt, da Mycophenolsäure ebenfalls ein Substrat von Flu1 ist. Folglich sollte ein Stamm mit hyperaktivem Zink-Cluster-Transkriptionsfaktor, welcher die FLU1-Expression hochreguliert, einen leicht erkennbaren Mycophenolsäure-resistenten Phänotyp aufweisen. Weiterhin wurden FACS-Analysen, quantitative real-time RT-PCR-Analysen, Broth microdilution-Assays sowie Histatin 5-Assays durchgeführt, um den Mechanismus und die Regulierung der Histatin-5-Resistenz zu analysieren. Mehrere Zink-Cluster-Transkriptionsfaktoren verursachten Mycophenolsäure-Resistenz und erhöhten die FLU1-Expression. Von diesen war nur hyperaktives Mrr1 in der Lage, eine erhöhte Histatin-5-Resistenz zu verleihen. Das Auffinden von Mrr1 als Regulator der Histatin 5-Resistenz war hochinteressant, da fluconazolresistente Stämme mit natürlich vorkommenden MRR1 gain-of-function Mutationen existieren, die aus HIV-infizierten Patienten mit oropharyngealer Candidiasis isoliert wurden. Diese gain-of-function Mutationen sowie künstlich aktivierendes Mrr1 verursachen Fluconazol-Resistenz durch Hochregulation der Effluxpumpe MDR1 und anderer Zielgene. Im Verlauf der Studie wurde herausgefunden, dass verschiedene natürlich vorkommende MRR1 gain-of-function Mutationen im SC5314 Wildtyp Hintergrund eine erhöhte FLU1-Expression und eine erhöhte Histatin-5-Resistenz verursachten. Das Gleiche galt für Fluconazol-resistente klinische Isolate mit Mrr1 gain-of-function Mutationen, welche die Überexpression von FLU1 verursachten. Zellen dieser Isolate wurden, abhängig von Mrr1, weniger wirksam durch Histatin 5 abgetötet. Überraschenderweise trug FLU1 nur wenig zur Histatin-5-Resistenz bei, vielmehr trug die Überexpression von MDR1 hauptsächlich zur Mrr1-vermittelten Histatin-5-Resistenz bei, aber auch weitere Mrr1-Zielgene waren daran beteiligt. Diese Mrr1-Zielgene gilt es nun noch zu entdecken. Ob ein Zusammenhang zwischen diesen noch unbekannten Mrr1-Zielgenen hergestellt werden kann, die zur Fluconazolresistenz sowie zu einer erhöhten Histatin-5-Resistenz beitragen, wird erst nach dem Auffinden der verantwortlichen Zielgene geprüft werden können. Zusammenfassend trägt diese Studie zum Verständnis der Auswirkungen einer anhaltenden antimykotischen Exposition auf die Interaktion zwischen Wirt und Pilz bei. Eine medikamentöse Therapie kann zu einer Resistenzentwicklung führen, aus der Stämme hervorgehen, welche nicht nur eine Resistenz gegen Fluconazol entwickelt haben, sondern gleichzeitig eine Resistenz gegen einen angeborenen Wirtsabwehrmechanismus, der eine Adaption an die Wirtsnische auch in Abwesenheit des Antimykotikums ermöglicht.

HINTERGRUND UND ZIELE: Das zentrale Ziel dieser Studie war es der personalisierten Medizin einen Schritt näher zu kommen, bei der für jeden Patienten für die entsprechende Erkrankung das optimale Arzneimittel in der optimalen Dosierung gewählt wird. Dazu ist es notwendig herauszufinden, wie hoch der genetische Anteil auf die Wirkungsweise von Medikamenten ist. Wenn Ergebnisse von klinischen Studien ausreichend belegen, dass der Einfluss von genetischen Faktoren bedeutsam für die Wirkungsweise von Medikamenten ist, kann sich eine genetische Analyse vor Therapiebeginn als sinnvoll erweisen. In dieser Studie erfolgten Untersuchungen zur erblichen Variation der Aktivitäten der Efflux-Transportproteine MDR1 und MRP2. Um den Einfluss von Genen und Umweltfaktoren auf interindividuelle Unterschiede zu erforschen, eignen sich am besten Zwillingsstudien. METHODEN: So haben wir eine Zwillingsstudie mit 20 monozygoten und 9 dizygoten gleichgeschlechtlichen Zwillingen durchgeführt. Dabei wurde Talinolol als in-vivo-Testsubstanz für die Aktivität der Membran-Transportproteine MDR1 und MRP2 analysiert. Es wurden an drei verschiedenen Studientagen, die mindestens eine Woche auseinander liegen mussten, jeweils die gleiche Menge an Talinolol oral verabreicht und anschließend in festgelegten regelmäßigen Abständen die Blutkonzentrationen und weitere pharmakokinetische Parameter bestimmt. Da Talinolol nahezu nicht metabolisiert und unverändert wieder ausgeschieden wird, hängt dessen Bioverfügbarkeit stark von der Funktion und Expression seiner Transporter (MDR1 und MRP 2) ab. Die Erblichkeit wurde mittels drei unterschiedlicher Formeln berechnet. ERGEBNIS: In dieser Studie zeigte sich eine insgesamt große Variabilität der Blutkonzentrationsverläufe sowohl zwischen den Personen (interindividuelle Variabilität) als auch innerhalb einer Person zwischen den unterschiedlichen Studientagen (intraindividuelle Variabilität). Desweiteren war die Variabilität unter monozygoten Zwillingen größer als die unter dizygoten Zwillingen. Diese Konstellationen und die daraus errechnete Erblichkeit sprechen für einen geringen genetischen Einfluss und einen großen Einfluss von Umweltfaktoren auf die Variation in der Pharmakokinetik von Talinolol. Bezüglich der verschiedenen Genvarianten von MDR1 und MRP2 konnten keine signifikanten Unterschiede in den Blutkonzentrationsverläufen gezeigt werden. FAZIT: Es zeigte sich insgesamt ein geringer genetischer Einfluss auf die Variation in der Pharmakokinetik der In-vivo Testsubstanz Talinolol. Desweiteren zeigten die Probanden mit verschiedenen Genvarianten auch keine signifikanten Unterschiede in den Konzentrationsverläufen, sodass in dieser Studie kein relevanter Zusammenhang zwischen dem Vorhandensein bestimmter Genvarianten und der Transporteraktivität von MDR1 und MRP2 gezeigt werden konnte.

Caractérisation et régulation des lymphocytes T CD4+CD73+ en contextes physiologique et pathologique / Characterization and regulation of the CD4 CD73 T lymphocytes in physiological and pathological contexts

L'étude des populations de lymphocytes T CD4+ effecteurs (Teff) chez l'Homme présente un intérêt croissant dans les enjeux actuels que constitue l'élaboration de nouvelles immunothérapies. Ces travaux détaillent la caractérisation et la régulation d'une population de Teff exprimant l'ecto-nucléotidase CD73 ayant pour fonction de dégrader l'AMP extracellulaire en adénosine (Ado) immunosuppresseur. Cette population, enrichie en lymphocytes T helper de type Th1.17, est très polyfonctionelle et pro-inflammatoire. Les Teff CD73+ expriment peu de points de contrôles immunitaires inhibiteurs mais sont régulés par leur production autocrine d'Ado qui limite leurs fonctions effectrices et leurs capacités prolifératives. Les Teff CD73+ expriment par ailleurs fortement le transporteur ABC multidrug-resistance 1 (MDR1), responsable de l'exclusion de nombreuses drogues du cytoplasme des cellules. L'étude de cette population dans différents contextes pathologiques a permis de détailler le fonctionnement de cette population. J'ai pu mettre en évidence que l'expression de CD73 est en effet dynamique. Elle est notamment diminuée dans des pathologies arthritiques auto-immunitaires (la polyarthrite rhumatoïde et le rhumatisme psoriasique) dans lesquelles les Th17 et les Th1.17 sont fortement activés. La diminution d'expression de CD73 sur ces cellules constitue la levée d'un frein qui leur permet de contribuer pleinement à l'inflammation chronique caractérisant ces pathologies. Les Teff CD73+, présents dans les tumeurs solides humaines de sein et d'ovaire, pourraient en revanche présenter un avantage sélectif en contexte tumoral de par leur expression de MDR1 leur permettant de résister aux traitements de chimiothérapie. Ces traitements substrats de MDR1, combinés à des thérapies inhibant la fonction enzymatique de CD73, pourrait permettre la restauration d'une réponse immunitaire anti-tumorale efficace médiée par cette population pro-inflammatoire / Requirement for CD4+ effector T lymphocytes (Teff) comprehensive study in human is increasing since it can contribute to the emergence of new immunotherapy strategies. This work brings up important information concerning the characterization and regulation of a Teff population expressing the CD73 ecto-nucleotidase, which is able to degrade extracellular AMP into immunosuppressive adenosine (Ado). This population, highly polyfunctional and pro-inflammatory, is enriched in Th1.17 cells. CD73+ Teff express low levels of inhibitory immune checkpoints but are negatively regulated by the autocrine Ado production that limits their pro-inflammatory function and proliferative capacities. In addition, CD73+ Teff express high levels of the ABC transporter multi-drug resistance 1 (MDR1), responsible for the exclusion of cells’ cytoplasm of many drugs. The study of this population in different pathological contexts enabled to decipher its functions. I could evidence that CD73 expression is dynamic. CD73 is notably decreased in autoimmune arthritic pathologies (rheumatoid arthritis (RA) and psoriatic arthritis (PsA)) in which Th1.17 and Th17 are highly activated. CD73 decreased expression by these cells is a mechanism that alleviates self-inhibition by autocrine Ado production and enables them to fully contribute to chronic inflammation characterizing these pathologies. In tumor context, CD73+ Teff present in breast and ovarian tumors could on the contrary bear a selective advantage due to their high MDR1 expression enabling them to resist MDR1 substrates-based chemotherapy treatments. These chemotherapy treatments combined to therapies blocking CD73 enzymatic function could allow the restauration of an efficient anti-tumor immune response

Die Bedeutung der ABC-Transportsysteme ABCB1 und Abcb11 in der Arzneimitteltherapie und bei cholestatischen Lebererkrankungen

ABC-Transmembrantransporter sind an der Aufnahme, Verteilung und Ausscheidung vieler Arznei- und Fremdstoffe beteiligt. Sie spielen eine Schlüsselrolle in der Pharmakokinetik und in der Ausscheidung toxischer endogener oder exogener Substanzen. Das Ziel der hier präsentierten Untersuchungen war deshalb, den Einfluss genetischer Polymorphismen des bekanntesten Vertreters dieser Proteinfamilie, MDR1 (ABCB1) zu untersuchen. Darüberhinaus sollte der ebenfalls zur ABC-Transporterfamilie gehörende hepatozelluläre Exporter für monoanionische Gallensäuren identifiziert und charakterisiert werden. MDR1 erwies sich als ein hochpolymorphes Gen mit zahlreichen Einzelbasenaustauschen (SNPs). Die meisten SNPs waren intronisch oder stumm. Für den nichtkodierenden SNP im Exon 26 3435C>T ergab sich bei homozygoten Trägern des T-Allels eine im Vergleich zum Wildtyp geringere intestinale P-Glykoprotein Expression mit einer entsprechend höheren und schnelleren Absorption von Digoxin. Die Auswertung pharmakokinetischer Profile von Digoxin in Individuen mit MDR1-Haplotypen der miteinander verbundenen SNPs in Exon 21 2677 und Exon 26 3435 untermauerte die beobachteten pharmakogenetischen Effekte. Nach oraler Einzelgabe von 1 mg Digoxin konnten wahrscheinlich aufgrund der Überschreitung der P-Glykoprotein Transportkapazität keine genotypischen Unterschiede beobachtet werden. Der biliäre Exporter für monoanionische Gallensäuren (Bsep) konnte als ein 160 kDa Glykoprotein aus einer Rattenleber cDNA-Bibliothek identifiziert werden und gehört ebenfalls zur ABC Transporter-Familie. Die transkriptionelle Regulation und Möglichkeiten der Modulation der Expression des Bsep-Gens wurden in vitro und in Tiermodellen der Cholestase untersucht. Dabei zeigte sich, dass Gallensäuren über ein proximales FXRE-Motiv die Bsep Promotoraktivität stimulierten. Arzneistoffe hatten ebenfalls einen Einfluss auf die Transkription des Bsep-Gens. Die adaptive Regulation hepatozellulärer Transporter während der Cholestase ergab eine verminderte Expression der meisten basolateralen Aufnahmetransporter und eine unveränderte oder heraufregulierte Proteinmasse kanalikulärer (apikaler) Efflux-Transporter. Dieses Regulationsmuster dient dem Schutz der Leberzelle, indem eine intrazelluläre Anreicherung toxischer Gallensäuren vermindert und der Gallefluss für eine intakte biliäre Clearance aufrechterhalten wird. / ABC transmembrane transporters are involved in absorption, distribution and excretion of diverse drugs and xenobiotics. They are key factors in pharmacokinetics and in the elimination of toxic endogenous or exogenous compounds. Therefore, the aim of the present study was to investigate the influence of genetic polymorphisms of the best known member of this protein family, MDR1 (ABCB1). In addition, the identity of another ABC transporter assumed to be the major hepatocellular export pump for monoanionic bile acids should be revealed and characterized. MDR1 turned out as a highly polymorphic gene with many single nucleotide polymorphisms (SNPs). Most of the SNPs were intronic or silent. Homozygous carriers of the non-coding SNP in exon 26 3435C>T had lower intestinal P-glycoprotein expression rates and thus enhanced absorption of the model compound digoxin as compared to wildtype controls. The analysis of pharmacokinetic profiles in different MDR1-haplotypes of the linked SNPs in exon 21 2677 and exon 26 3435 supported the above data. An oral single dose of 1 mg digoxin did not result in genotypic differences of tested genotypes, probably because this dose was above the maximal transport capacity of P-glycoprotein. The biliary export pump for monoanionic bile acids (Bsep) was identified as an 160 kDa glycoprotein of the ABC transporter family by screening a rat liver cDNA library. The transcriptional regulation and modulatory factors of Bsep (Abcb11) gene expression were analyzed in vitro and in animal models of cholestasis. The promoter activity of Bsep was stimulated by bile acids via a proximal FXRE motif. Drugs were also able to modify Bsep gene transcription. Adaptive regulation of hepatocellular transporters during cholestasis followed a pattern of diminished expression of most basolateral uptake carrier systems and maintained or even upregulated protein mass of canalicular (apical) exporters. This pattern serves as a protective mechanism of the liver cells preventing intracellular accumulation of toxic bile acids and providing unimpaired biliary flow and clearance.

Untersuchungen zur subzellulären Lokalisation und zu den Funktionen von YB-1, einem Y-Box-Protein in Säugerzellen

YB-1, ein Y-Box-Protein in Säugerzellen, konnte sowohl im Zytoplasma als auch in den Zellkernen von HeLa-Zellen nachgewiesen werden. Es wurde eine Abhängigkeit der intrazellulären Lokalisation von YB-1 vom Verlauf des Zellzyklus beobachtet. In jeder Phase des Zellzyklus war YB-1 im Zytoplasma zu finden. Eine Kernlokalisation von YB- 1 konnte nur in den HeLa-Zellen festgestellt werden, die sich im Übergang von der G1- in die S-Phase oder in der frühen S-Phase des Zellzyklus befanden. Die Abhängigkeit der Lokalisation von YB-1 vom Verlauf des Zellzyklus unterstützt die These, daß YB-1 und andere Y-Box-Proteine an der Regulation der Zellproliferation beteiligt sind. Es wurden verschiedene Proteine identifiziert, die im Zytoplasma von HeLa-Zellen mit YB-1 assoziiert vorkommen. Alle identifizierten Proteine erfüllen Aufgaben im RNA-Metabolismus, was auf eine Beteiligung dieser Proteinkomplexe an der Regulation der mRNA hinweist. Die Interaktion von P32/SF2 (P35) mit YB-1 erwies sich als abhängig vom Zellzyklus, wobei eine maximale Assoziation dieser beiden Proteine beim Übergang der HeLa-Zellen von der G1- in die S-Phase zu beobachten war. In Multidrug-resistenten MCF7/ADR-Zellen konnte eine deutlich verstärkte Interaktion von P32/SF2 mit YB-1 im Vergleich zu den sensitiven MCF7-Zellen festgestellt werden. Im Zytoplasma von HeLa-Zellen konnte YB-1 in Verbindung mit membrangebundenen Polysomen nachgewiesen werden. Eine Assoziation von YB-1 mit freien oder zytoskelettgebundenen Polysomen konnte nicht festgestellt werden. Damit wurde erstmalig gezeigt, daß YB-1 eine Spezifität für eine bestimmte Gruppe von Polysomen besitzt. Die Assoziation mit membrangebundenen Polysomen legte die Vermutung nahe, daß YB-1 an der Translationskontrolle von Polypeptiden beteiligt ist, die am rauhen endoplasmatischen Retikulum synthetisiert werden. Es konnte gezeigt werden, daß YB-1 die Translation von P-Glykoprotein, einem integralen Membranprotein, positiv reguliert. Ein Einfluß auf die Translation der untersuchten sekretorischen Proteine (a-Faktor und Präprolactin) konnte nicht beobachtet werden. Diese Ergebnisse belegen, daß YB-1 ein spezifischer Regulator der Translation bestimmter Membranproteine ist. Am Hand von P-Glykoprotein konnte des weiteren demonstriert werden, daß YB-1 sowohl die Transkription als auch die Translation dieses Proteins positiv reguliert. Die in den Zellkulturen beobachtete Korrelation von YB-1 mit der Expression von P-Glykoprotein konnte auch in primären Mammakarzinomen nachgewiesen werden. Somit ist YB-1 ein entscheidender Faktor bei der Ausbildung einer intrinsischen multiplen Resistenz von Mammakarzinomen gegen die Behandlung mit Chemotherapeutika. Aus diesem Grunde könnte YB-1 einen Ansatzpunkt für die künftige Diagnose und Therapie von Mammakarzinomen und eventuell auch von anderen Tumoren bieten. / YB-1, a mammalian Y-box protein was detected in the cytoplasm as well as in the nuclei of HeLa cells. The intracellular localisation of YB-1 depends on the cell cycle. In every part of the cell cycle, YB-1 was found in the cytoplasm. A nuclear localisation of YB-1 was only detectable in the G1- to S-phase transition and in the early S-phase. These observations underline the hypothesis, that Y-box proteins are envolved in the regulation of cell proliferation. Different proteins interacting with YB-1 were identified in the cytoplasm of HeLa cells. All identified poteins are envolved in the RNA metabolism, indicating a role of these protein complexes in the regulation of mRNA. The interaction of P32/SF2 (P35) with YB-1 alternates during the cell cycle with a maximum at the G1- to S-phase transition. A remarkable increase of the association of YB-1 and P32 was observed in the multidrug-resistant MCF7 cells compared with the parental cell line. Furthermore, YB-1 was detected in association with membrane-bound polysomes, suggesting a role of YB-1 at the translational regulation of the synthesis of polypeptides at the rough endoplasmic reticulum. It was shown, that YB-1 stimulate the translation of P-glycoprotein. This influence is specific, beause the translation of a set of control proteins (alpha factor, preprolactin, luciferase) was not effected by YB-1. It was shown, that YB-1 stimulate the expression of P-glycoprotein at the level of transcription as well as at the level of translation. This indicates a central role of YB-1 in the regulation of the biosynthesis of this protein. The correlation of the nuclear expression of YB-1 and the expression of P-glycoprotein was demonstrated in primary breast cancers. Taken together, YB-1 is a important factor for the development of a resistant phenotyp and therefore a possible new target for anti-cancer therapy.
