INTRODUÇÃO: A doença de Legg-Calvé-Perthes (DLCP) é caracterizada pela necrose idiopática da epífise femoral proximal na criança. A interrupção do suprimento arterial para a cabeça femoral é o fator etiopatogênico mais aceito. Embora as angiografias sejam a base para as investigações das causas vasculares, elas não avançaram na DLCP devido a limitações éticas, por serem procedimentos invasivos com exposição à radiação ionizante e pela potencial toxicidade do contraste iodado. O objetivo primário foi avaliar a rede arterial da epífise proximal do fêmur em crianças com DLCP unilateral, particularmente a artéria circunflexa femoral medial (ACFM) que nutre a referida epífise, comparando o quadril afetado com o quadril contralateral não afetado. A técnica usada foi a angiografia por ressonância magnética (angioRM) que permite o estudo vascular sem as limitações e os riscos inerentes à angiografia convencional. A sequência angiográfica usada foi TRICKS (Time Resolved Imaging of Contrast KinecticS) que utiliza técnica de aquisição de imagens com foco na resolução temporal do contraste (identificação da passagem do contraste pelos vasos ao longo do tempo). MÉTODOS: Os dados foram obtidos a partir de uma amostra consecutiva composta por 24 pacientes com DLCP unilateral admitidos no ambulatório de ortopedia pediátrica de um hospital terciário de maio de 2013 a dezembro de 2016. Os exames de angioRM-TRICKS foram avaliados por dois radiologistas e o consenso foi usado para a análise. RESULTADOS: Foram realizados 24 exames. A idade média foi de 7,9 +/- 2,1 anos. O trajeto da ACFM foi analisado no plano coronal dos exames de angioRM-TRICKS. No segmento 1 (da origem até a borda medial do colo femoral), havia 100% de visibilidade no lado da DLCP, bem como no lado contralateral. No segmento 2 (da borda medial do colo femoral até a borda lateral do colo femoral), houve 63% de visibilidade no lado DLCP e 71% de visibilidade no lado contralateral. No segmento 3 (ponto de entrada no sulco intertrocantérico), houve 25% de visibilidade no lado com DLCP e 17% de visibilidade no lado contralateral. O segmento 4 (ramo terminal e ascendente na face lateral do colo do fêmur) foi 100% não visibilizada no quadril com DLCP nem no lado sem a doença. CONCLUSÕES: A angioRM-TRICKS fornece imagens bem definidas do suprimento arterial para a epífise femoral proximal em crianças com a DLCP. Não foram encontradas diferenças significativas nos trajetos analisados da ACFM entre o lado acometido e o lado sem a DLCP. Com os exames de angioRM-TRICKS não foi possível visibilizar o trecho ascendente epifisário lateral, ramo terminal da ACFM, tanto nos quadris afetados quanto nos quadris sem a DLCP / INTRODUCTION: Legg-Calvé-Perthes disease (LCPD) is characterized by idiopathic necrosis of the proximal femoral epiphysis in the child. Interruption of the arterial supply to the femoral head is the most accepted etiopathogenic factor. Although angiograms are the basis for investigations of vascular causes, they have not progressed in LCPD due to ethical limitations, because they are invasive procedures with exposure to ionizing radiation and the potential toxicity of iodinated contrast. The primary objective was to evaluate the arterial network of the proximal femoral epiphysis in children with unilateral LCPD, particularly the medial femoral circumflex artery (MCFA) that supplies the epiphysis, comparing the affected hip with the unaffected contralateral hip. The technique used was magnetic resonance angiography (MRA) that allows vascular study without the limitations and risks inherent in conventional angiography. The angiographic sequence used was TRICKS (Time Resolved Imaging of Contrast KinecticS), which uses an image acquisition technique focused on temporal resolution of contrast (identification of the passage of contrast by vessels over time). METHODS: Data were obtained from a consecutive sample composed of 24 patients with unilateral LCPD who were admitted to the pediatric orthopedic outpatient clinic of a tertiary hospital from May 2013 to December 2016. Two radiologists evaluated the MRA-TRICKS exams and the consensus was used for the analysis. RESULTS: Twenty-four examinations were performed. The mean age was 7.9 +/- 2.1 years. The MCFA trajectory was analyzed in the coronal plane of the MRA-TRICKS exams. In segment 1 (from the origin to the medial border of the femoral neck), there was 100% visibility on the LCPD side, as well as on the contralateral side. In segment 2 (from the medial border of the femoral neck to the lateral border of the femoral neck), there was 63% visibility on the LCPD side and 71% visibility on the contralateral side. In segment 3 (entry point in the intertrochanteric groove), there was 25% visibility on the LCPD side and 17% visibility on the contralateral side. Segment 4 (terminal and ascending limb on the lateral aspect of the femoral neck) was 100% not seen in the hip with LCPD or on the side without the disease. CONCLUSIONS: MRA-TRICKS provides well-defined images of the arterial supply to the proximal femoral epiphysis in children with LCPD. No significant differences were found in the analyzed ACFM pathways between the affected side and the non- LCPD side. With the MRA-TRICKS exams, it was not possible to visualize the ascending lateral epiphyseal segment, terminal branch of the MCFA, both in the affected hips and in the hips without the LCPD

INTRODUÇÃO: Os métodos diagnósticos não invasivos, como a ultra-sonografia com Doppler colorido (USDC) e a angiografia por ressonância magnética contrastada (ARMC) têm sido cada vez mais utilizados para o estudo da estenose das artérias carótidas internas (ACI). Nos testes comparativos a arteriografia por subtração digital bidimensional (ASD 2D) tem sido o padrão de referência, porém é um método caro, invasivo e com diversas complicações descritas, além disso, com a realização da arteriografia por subtração digital rotacional (ASD 3D), começou a se interrogar se a ASD 2D não estaria subestimando as estenoses. OBJETIVOS: Comparar as medidas de estenose obtidas pelos métodos não invasivos com a ASD 2D e 3D, utilizando o critério do North American Symptomatic Carotid Endarterectomy Trial (NASCET), além de comparar os dois métodos invasivos entre si, tentando identificar se os métodos não invasivos podem substituir a ASD 2D na rotina e se a ASD 2D tende subestimar a estenose em relação a ASD 3D. MÉTODOS: 92 pacientes que haviam sido indicados de maneira prospectiva e consecutiva para realização da ASD 2D, foram selecionados para o estudo, sendo encaminhados para realizar também a ASD 3D, a ARMC e o USDC. Os resultados das medidas da maior estenose, realizadas através das imagens fonte em estação de trabalho, por dois observadores em consenso, obtidas em cada um dos testes (USDC, ARMC) e dos resultados concordantes dos dois, foi comparada com os exames de referência (ASD 2D e ASD 3D). Foi feita também uma avaliação dos resultados dos exames invasivos entre si. RESULTADOS: 98 ACI foram incluídas no trabalho. Os resultados obtidos pelo coeficiente de correlação de Pearson e pelo coeficiente de correlação intraclasse, que variaram de 0,91 a 0,96, demonstraram respectivamente uma excelente correlação entre as modalidades diagnósticas e que há uma forte concordância entre os seus resultados (ambos com p <0,001). Os valores de sensibilidade, especificidade, valor preditivo positivo, valor preditivo negativo e acurácia das diversas comparações variaram respectivamente de 76 a 100%, de 92 a 100%, 79 a 100%, 90 a 100% e 90 a 100%. DISCUSSÃO: As comparações com maior sensibilidade, especificidade e acurácia foram respectivamente a (USDC+ARMC) x ASD 3D com 100%, 100% e 100%; ARMC x ASD 3D com 100%, 96% e 97% e ASD 2D x ASD 3D, com 89%, 94% e 93%. A ASD 2D categorizou as estenoses em 11 casos (11,3%) uma classe abaixo das obtidas na ASD 3D, além de classificar 4 estenoses a menos que a ASD 3D na classe de 70-94%. CONCLUSÕES: Os métodos não invasivos principalmente se avaliados em conjunto podem substituir a ASD 2D na avaliação da estenose da ACI. A ASD 2D tendeu a subestimar levemente as estenoses quando comparada com a ASD 3D / INTRODUCTION: Noninvasive techniques such as Doppler ultrasound (DUS) and contrast enhanced magnetic resonance angiography (CEMRA) are each day more used in the evaluation of the internal carotid arteries (ICA). On comparison studies 2D digital subtraction angiography (2D DSA) has been considerate as the reference standard, however its an expensive and invasive method, with well known risks, besides the use of 3D digital subtraction angiography (3D DSA) showed that 2D DSA can potentially result in an underestimation of the stenosis. OBJECTIVES: Compare stenosis measurements of noninvasive methods with 2D and 3D DSA, using the North American Symptomatic Carotid Endarterectomy Trial (NASCET) criteria, compare the two noninvasive methods with each other, trying to identify if they can substitute 2D DSA on the daily basis and if that 2D DSA tends to underestimate the stenosis compared with 3D DSA. METHODS: 92 patients that were prospective and consecutive scheduled for 2D DSA were selected to participate in the study which included perform also a 3D DSA, CEMRA and DUS. The measurements of the greatest stenosis, made with the raw images in a workstation, by two observers in consensus, obtained with each modality (DUS, CEMRA) and the concordant results of both, were compared with the reference standard (2D and 3D DSA). Also a comparison between the invasive methods was made. RESULTS: 98 ICA were included. The results obtained by Pearson correlation coefficient and intraclass correlation coefficient, that range from 0.91 and 0.96, showed respectively a excellent correlation between the diagnostic modalities and that there are a great agreement between them (both with p<0.001). Sensitivity, specificity, positive and negative predictive values, and accuracy values of the different comparisons range respectively from 76% to 100%, 92 to 100%, 79 to 100%, 90 to 100% and 90 to 100%. DISCUSSION: Comparisons with higher sensitivity, specificity and accuracy were respectively (DUS+CEMRA) x 3D DSA, with 100%, 100% and 100%; CEMRA x 3D DSA with 100%, 96% and 97% and 2D DSA x 3D DSA with 89%, 94% and 93%. 2D DSA categorized 11 cases (11,3%) of ICA stenosis as one category lower than 3D DSA, including 4 less cases of the 70-94% class. CONCLUSIONS: The noninvasive methods, especially if evaluated together, can replace 2D DSA in the study of cervical carotid steno-occlusive disease. 2D DSA tends to lightly underestimate carotid stenosis when compared to 3D DSA

Classification angiographique d’anévrismes cérébraux traités par voie chirurgicale ou endovasculaire : revue systématique et étude d’accord inter & intra-observateur

INTRODUCTION Les anévrismes cérébraux représentent une condition potentiellement mortelle. Ils sont traités par voie chirurgicale (« clipping ») ou endovasculaire (dont le « coiling »), et plusieurs essais cliniques randomisés comparant ces traitements utilisent les résultats angiographiques comme mesure du succès du traitement. Le développement d'une classification standardisée et applicable aux deux traitements et aux trois modalités angiographiques (conventionnelle, angioscan et angio-IRM) est nécessaire. Nos objectifs furent de revoir systématiquement les classifications existantes, d’évaluer la fiabilité inter/intra-observateur d’une nouvelle classification angiographique commune, et d’en vérifier la pertinence clinique. MÉTHODES Deux revues systématiques furent effectuées dans 5 bases de données pour recenser les classifications (et leur fiabilité, lorsque rapportée) pour chaque traitement. Les résultats d’angiographie conventionnelle post-clipping (n=30) ou coiling (n=30) de 60 patients furent évalués de manière indépendante, à l’aide d’une nouvelle classification à 3 catégories, par 19 cliniciens ayant diverses pratiques et expériences. Les observateurs devaient également indiquer la prise en charge clinique appropriée pour chaque cas: un suivi espacé, étroit, ou un retraitement. Quinze lecteurs ont procédé à une 2e évaluation des cas (présentée selon un ordre permuté) à 1 mois d’écart. Le même travail fut reconduit, dans un deuxième temps, par 24 observateurs avec 60 cas évalués par angioscan (n=30) ou angio-IRM (n=30). Les accords inter/intra-observateurs furent estimés avec la statistique Alpha de Krippendorff (aK); la corrélation entre le choix de classe et la prise en charge fut calculée avec un test V de Cramer. RÉSULTATS Soixante-dix classifications (44 post-clipping, 26 post-coiling) furent recensées dans la littérature. La fiabilité variait de 0.12 à 1.00, avec une hétérogénéité significative en termes d’observateurs et de patients. L’accord inter-observateur pour la classification retenue était substantiel pour les résultats d’angiographie conventionnelle (aK=0.76; 0.67–0.83) et des autres modalités (aK=0.63; 0.55–0.70); peu importe le traitement, la spécialité ou l’expérience des évaluateurs. La classe est fortement corrélée à la prise en charge (V de Cramer de 0,77 à 0,80 selon la modalité angiographique; p<0,001). CONCLUSION La classification proposée est fiable et pertinente cliniquement. Elle servira d’outil valide pour évaluer les résultats angiographiques d’anévrismes dans les futurs essais cliniques comparant les deux traitements, peu importe la modalité d’imagerie utilisée. / INTRODUCTION Intracranial aneurysms are a medical condition associated with serious morbidity and mortality when ruptured. They are mainly treated with surgical clipping or endovascular coiling, and randomized clinical trials comparing both treatments often use angiographic results as a surrogate outcome of treatment success. The development of a standardized, reliable method to report angiographic results, applicable to both treatments using any angiographic modality (Conventional [CA], Computed tomography [CTA], or magnetic resonance [MRA] angiography) is needed. We sought to systematically review existing methods of classifying treated aneurysms and to assess the inter-/intra-rater reliability of a new angiographic grading scale, as well as its clinical relevance. METHODS We conducted two separate systematic reviews in 5 databases to collect post-clipping and post-coiling grading scales, along with their reliability when reported. The CA of 60 cases of clipped (n=30) and coiled (n=30) aneurysms were independently evaluated by 19 raters of various backgrounds and experiences using a new 3-grade scale. Raters were also asked how they would manage each case (delayed, close follow-up, or retreatment). Fifteen raters performed a 2nd evaluation of the same cases (in a permuted order) at least one month apart. The same methodology was used to study the reliability of the scale when 60 noninvasive angiography cases of clipped (30 CTA) or coiled (30 MRA) aneurysms were evaluated by 24 raters. Inter and intra-rater agreements were assessed using Krippendorff’s Alpha statistics (aK), and the relationship between occlusion grade and clinical management using Cramer’s V test. RESULTS The systematic reviews found 70 grading scales (44 post-clipping, 26 post-coiling) with significant heterogeneity in terms of reliability (kappa values from 0.12 to 1.00), raters, and patients. Overall inter-rater agreement for the grading scale was substantial for CA results (aK=0.76; 0.67–0.83) as well as for noninvasive results (aK=0.63; 0.55–0.70); regardless of background, experience, or treatment used. A strong relationship was found between angiographic grades and management decisions (V=0.80±0.12 for CA; V=0.77±0.14 for CTA/MRA [p<0.001]). CONCLUSION A standardized 3-grade scale was found to be a reliable and clinically meaningful tool to compare the results of clipping and coiling of aneurysms using any angiographic modality. It could be used as a valid tool to adjudicate results of comparative randomized trials, and to standardize the reporting of aneurysm treatments.

Changes of cine cerebrospinal fluid dynamics in patients with multiple sclerosis treated with percutaneous transluminal angioplasty: a case-control study

No / The purpose of this article is to investigate characteristics of cine phase contrast-calculated cerebrospinal fluid (CSF) flow and velocity measures in patients with relapsing-remitting (RR) multiple sclerosis (MS) receiving standard medical treatment who had been diagnosed with chronic cerebrospinal venous insufficiency (CCSVI) and underwent percutaneous transluminal angioplasty (PTA). This case-controlled, magnetic resonance (MR) imaging-blinded study included 15 patients with RR MS who presented with significant stenoses (>/=50% lumen reduction on catheter venography) in the azygous or internal jugular veins. Eight patients underwent PTA in addition to medical therapy immediately following baseline assessments (case group) and seven had delayed PTA after 6 months of medical therapy alone (control group). CSF flow and velocity measures were quantified over 32 phases of the cardiac cycle by a semiautomated method. Outcomes were compared between groups at baseline and at 6 and 12 months of the study by mixed-effect model analysis. At baseline, no significant differences in CSF flow or velocity measures were detected between groups. At month 6, significant improvement in flow (P<.001) and velocity (P = .013) outcomes were detected in the immediate versus the delayed group, and persisted to month 12 (P = .001 and P = .021, respectively). Within-group flow comparisons from baseline to follow-up showed a significant increase in the immediate group (P = .033) but a decrease in the delayed group (P = .024). Altered CSF flow and velocity measures were associated with worsening of clinical and MR outcomes in the delayed group. PTA in patients with MS with CCSVI increased CSF flow and decreased CSF velocity, which are indicative of improved venous parenchyma drainage.

Este estudo teve, por finalidade, estabelecer o valor da ressonância magnética em 30 doadores potenciais do lobo direito do fígado, na determinação dos seguintes fatores: esteatose hepática; anatomia biliar; anatomias arterial hepática, venosas portal e hepática, e volume hepático lobar, comparando-os, respectivamente, com os achados anatomopatológicos da biópsia hepática, da colangiografia intraoperatória, da angiografia digital e/ou com os achados cirúrgicos, e com o peso real do enxerto. The main goal was to determine the following factors: steatosis; biliar anatomy; hepatic arterial anatomy; portal and hepatic venous anatomy, and lobar liver volume, comparing them to liver biopsy results, to intraoperative colangiography, to digital angiography and/or surgical findings, and to the real graft weight, respectively. The MR has underestimated liver steatosis; it has identified biliar anatomy with 83% of agreement; it has had 100% of agreement in the evaluation of arterial and portal and hepatic venous anatomy, and it has overestimated with small degree the lobar liver volume

Este estudo teve, por finalidade, estabelecer o valor da ressonância magnética em 30 doadores potenciais do lobo direito do fígado, na determinação dos seguintes fatores: esteatose hepática; anatomia biliar; anatomias arterial hepática, venosas portal e hepática, e volume hepático lobar, comparando-os, respectivamente, com os achados anatomopatológicos da biópsia hepática, da colangiografia intraoperatória, da angiografia digital e/ou com os achados cirúrgicos, e com o peso real do enxerto.
