Shared Trauma and Resiliency Among Military Mental Health Veterans: A Heuristic Inquiry

With the wars in Iraq and Afghanistan lasting over a decade, hundreds of military mental health providers have deployed to hostile environments and some on multiple occasions. Military mental healthcare providers can experience symptoms of acute and chronic stress resulting from exposures to horrific events while caring for soldiers in a deployed environment. Indeed, in treating these soldiers, clinicians may continue to experience the same traumatic events as their clients. The purpose of this phenomenological study was to understand and describe shared trauma and resilience for military mental health professionals who have deployed. The concept of shared trauma was defined as the experiences providers faced as they attempted to deliver clinical services while simultaneously addressing the same issue within their own lives. The concept of resilience was explored as how stressful situations were interpreted in relation to individuals' overall life experiences. The principle research question for this study examined how military mental health providers described dilemmas faced as they attempted to navigate the personal and professional aspects of shared trauma. This study utilized a purposive sample of 7 military mental health veterans who deployed during Operation Iraqi Freedom and Operation Enduring Freedom. Data were collected through semi-structured interviews and analyzed using Moustakas's 5 steps of heuristic analysis. Key findings indicated participants' interpretations of experiences involved posttraumatic growth, lessons learned, and changes in belief systems. Findings of this study can assist military mental health leaders in developing support and protection programs to assist this overwhelmed population, ensuring service members receive the proper care they are entitled.

Training Program Evaluation for a Prototype Command, Control & Communication System: Soldier Requirements, Performance & Feedback

An Army training program was evaluated in order to present recommendations for program refinement. The training program, developed as part of two larger Army combat development (CD) studies (DuBois & Smith, 1989a, 1989b), represents an attempt to define program design requirements for training tank commanders to use the Intervehicular Information System (IVIS). IVIS is a computer-based command, control, and communications system proposed for future M1A1 tank upgrades. Twenty-four Army tank commanders (TCs) participated in the research effort. TCs were of various rank and age. Training and testing were conducted over a 4 day period. Training lasted 1-1/2 days followed by 2-1/2 days of testing. The Biographical Questionnaire, the IVIS Knowledge Test, the IVIS Performance Test, the SIMNET Knowledge Test, the IVIS Survey, and the IVIS Training Reactions Questionnaire were administered to each TC. TC biographical data, reactions to training, post-training performance, and post-operational performance were evaluated. Scale reliability was evaluated for each IVIS instrument. Descriptive statistics were calculated for the soldier reaction and performance measures. Regression analyses were performed to assess the impact of TC background characteristics on commander performance. Results of the above analyses indicate: (a) the IVIS Performance and Knowledge tests are reliable, (b) the soldiers learn during training and the learning transfers to the operational setting (performance measured after two days of testing shows soldiers performance significantly increased from that of post-training performance), and (c) soldier biographical characteristics do not predict IVIS proficiency. The results call for more training time allotted to teaching IVIS skills with some IVIS functions deserving more attention. Additionally, it appears personnel requirements may not be an issue for selecting IVIS system users as the study found no correlation between biographical characteristics and IVIS proficiency. And finally, it is recommended that observational raters receive greater rater training as interrater reliability estimates were low.

The Development of a Knowledge Test for Armor Trainees

A study was conducted to develop a written tank knowledge test for United States Army armor trainees. Such a test may be employed as a tool for training program evaluation and as a criterion measure for assessing trainees. The Tank Knowledge Test (TKT) was developed from items written by subject matter experts familiar with the armor training program. After pilot testing, a revised version of the TKT had an internal consistency reliability of .79. Item difficulty levels ranged from .16 to .92 with a mean of .57. A factor analysis performed to examine the possibility of deriving subscales from four clusters of topically related items revealed no support for a four factor solution.

Combat veterans partake in exit interviews that may be inadequate in addressing their health and mental health needs prior to returning to a civilian lifestyle. Exit interviews have not been thoroughly evaluated from the perspective of veterans to determine their helpfulness in meeting the needs of those that have returned home. It is vital to the reintegration process and quality of life of our veterans to assess their needs as a priority over the feedback and inquiry of potential changes the military can make in the future for active military members, considering many veterans suffer from mental and physical illnesses and combat injuries. These war acquired injuries leave many veterans without adequate resources, facing homelessness and utilizing unhealthy coping mechanisms. Which should concern and require action on the part of social workers in carrying out our professional values, fulfilling the needs that are not met and a potential role in a multidisciplinary team. This study seeks to explore the veteran’s perspective on the helpfulness of exit interviews and how these interviews may better serve the health and mental health needs of veterans returning to a civilian lifestyle. The research findings suggest that military exit interviewers are not as helpful as they could be in regard to veteran reintegration into society. Three main themes surfaced from the qualitative analysis including: Inadequate Exit Interviews, Diminished Desire to Reenlist, subtheme Pressure to Reenlist, and Face to Face Communication.

Veterans in Transition: A Correlational Investigation of Career Adaptability, Confidence, and Readiness

Thousands of service persons and veterans may be leaving military service annually without required skills and not receiving timely career counseling and interventions needed to aid in their career transitions. Knowledge about service persons' career adaptability, confidence, and readiness could enhance the actions of all stakeholders to address the challenges that accompany career transitions and may aid in identifying needed counseling and interventions. Using a survey containing the Career Transitions Inventory and the Career Futures Inventory-Revised, perspectives were obtained from service persons (N = 264) while attending Transition Assistance Program workshops. Two research questions for the study examined associations between individuals' career adaptability and 2 transition variables: confidence and readiness. Statistical testing was accomplished using Pearson correlation coefficient, t test, and 1-way analysis of variance. Correlations of transition confidence and overall career adaptability scores indicated a low negative correlation (r (262) = -0.4299, p < .01), and correlations of transition readiness and overall career adaptability scores indicated a low positive correlation (r (262) = 0.3988, p < .01). In addition, significant differences were noted when examining survey results based on demographic variables such as race, education, marital status, highest pay-grade achieved, and years of service. This study contributes to social change by demonstrating techniques for assessing personal traits. Implications are discussed for using self-reported data for counseling and interventions for individuals, which could enhance their career transition experiences.

The Influence of the Ready Intelligence Program on Crewmembers' Perception of Proficiency in an Air Force Weapon System

A lack of evaluation and evidence of effectiveness prompted this study of the Distributed Common Ground System's (DCGS) proficiency maintenance tool, Ready Intelligence Program (RIP). The goal was to close the gap between research and practice and inform stakeholders at the local Distributed Ground Station (DGS) of evaluation results. Guided by a logic model as the theoretical foundation, this study examined how proficiency is perceived by DCGS crewmembers because of RIP at a military installation with intelligence, surveillance, and reconnaissance missions. This qualitative study used an outcomes-based program evaluation report based on interviews with 5 crewmembers, observations of program participant activities, and reviews of training documents and program reports. Data were transcribed into NVivo 10 for organization, and inductive code words and categories were applied. Data interpretations were confirmed via triangulation and then sent to the participants for member-checking. An external evaluator reviewed the study's methodology, data, and findings for veracity. The project that resulted from the study was a program evaluation report that identified 4 overarching themes. It was concluded that (a) there was a lack of awareness of RIP, (b) RIP had minimal impact on perception of proficiency, (c) the program was occasionally applied ineffectively, and (d) management of the program was insufficient. It is recommended that existing RIP training be emphasized to crewmembers to increase awareness. Additionally, an ongoing program evaluation is recommended with a quantitative measure of proficiency achievement. This study promotes social change by improving attitudes toward positional proficiency and RIP as a maintenance tool, improving program maintenance, and facilitating regular program evaluations.

Military Enlistment: The Motivations of Former Military Personnel Coming From Single-Parent Homes

The purpose of this study was to examine how the single-parent home environment, together with other demographic attributes like socioeconomic status, influenced young adults' decisions to enlist in the military. Adolescents transitioning into adulthood are often faced with the choices of what to do after high school, such as whether or not to join the military. Previous studies on youth enlistment in the military do not address in-depth the question of why they enlist or their motivating factors. Interviews via face-to-face, over the phone or via Skype, were conducted with 9 participants between the ages of 25-35 years who were high school graduates or GED holders who enlisted within 2 years from graduation, from single-parent homes, and separated from the military on their own accord prior to starting the study. Participants were voluntarily recruited through social media using purposeful sampling. Guided by Carl Rogers' theory of self-concept, data were analyzed via coding techniques to draw out common themes, and 4 composite themes emerged: (a) those individuals raised by absentee mothers were more likely to enlist in the military service, (b) troubled homes predicted the desire of the young adults to leave their homes, (c) evasion from the family conflict and financial stability were motivations for enlistment in the military, and (d) deprivation from human basic needs influenced the decision to enlist in the military. The findings can help with determining factors that cause disconnect in the family structure along with gaining a greater understanding a child's sense of belonging such as what is found in the military culture. Results can also be used to influence military professionals/recruiters about the specific types of individuals who are likely to be recruited into the military and how to develop and/or use tools to further assess the ability to sustain the military culture.

U.S. Army Enlisted Soldiers' Adherence to Prescribed Malaria Chemoprophylaxis in Afghanistan

Over the past 13 years, the United States Army has been engaged in armed conflict within Afghanistan. Unfortunately, the United States Army has been forced to evacuate soldiers from the battlefield because of malaria, a parasitic disease that is endemic in Afghanistan. Even though the U.S. Army has adopted an effective chemoprophylaxis protocol, soldiers' adherence to their prescribed medication has been historically low. This research addressed a gap in literature regarding the adherence rates of U.S. Army enlisted soldiers to their prescribed oral malaria chemoprophylaxis. In addition, this research investigated self-reported reasons for soldiers' nonadherence to this medication. The study employed an experimental, correlational research design to aid in understanding the relationship between adherence to malaria chemoprophylaxis and age, gender, military rank, education level, and previous deployment experience. Ninety-four active-duty U.S. Army personnel deployed to Afghanistan participated in the study. The frequency distribution of responses to the 8-questions Morisky Medication Adherence Scale were presented and indicated that for almost all of the questions, the percentage of participants who answered yes was larger than the percentage who answered no, indicating low levels of adherence among the study participants. The findings indicated that age, gender, and perception of risk all significantly contributed to the models predicting medication adherence. With the scientific and medical advances of the 20th and 21st centuries, few if any military personnel should contract malaria. These findings contribute to a greater awareness of medication adherence, which directly supports positive social change within the Armed Forces of the United States.

Exploring Protégé Perceptions of Success and Failure in Formal

Since the 1993 inception of the mentoring program in the U.S. Navy, little evidence has been collected on its effectiveness, primarily because of difficulties with instrumentation and conceptualization in conducting such assessments. The purpose of this correlational study was to identify external factors affecting military protégés' satisfaction with their mentoring experience. The conceptual framework of this study was based on Kram's mentor model theory, which includes career and psychosocial support functions. A 5-item Likert survey instrument was designed to measure the dependent variables of satisfaction with career mentoring and satisfaction with personal mentoring against 10 independent variables: dyad compatibility, mentor training, dyad geography, mentoring functions, mentor/protégé gender, challenging job assignments, protégé visibility, mentor leadership, time management, and protégé career expectations. The survey was completed by a total of 538 participants, selected among the service personnel of 17 U.S. Navy aviation squadrons in the enlisted ranks of E1 through E6. Ten simple linear regressions were performed with a level of significance of .001. All 10 independent variables were significantly related to satisfaction with both career and personal mentoring. The study results suggested, however, that career mentoring was favored to a greater extent than was personal mentoring by protégés, with the effect sizes ranging from 5% to 48% for career mentoring and from 3% to 22% for personal mentoring. Furthermore, Kram's theory was a useful lens to evaluate mentoring in this population. The implications for positive social change include informing program administrators in the U.S. Navy of the benefits to their units to evaluate and improve the design and the implementation of career and personal mentoring.

Navy Personnel and Effects of Select Factors on Posttraumatic Stress Disorder Symptomology

Since the establishment of the individual augmentee role within the U.S. Navy, little research has examined this nontraditional role associated with combat units. The majority of combat-related posttraumatic stress disorder (PTSD) research has been dedicated to Army and Marine Corps personnel with little research conducted on the Navy population. The purpose of this nonexperimental study was to identify the prevalence of combat-related PTSD symptomology for Navy personnel returning from an augmentee tour. The link between component and tour length and the presence of individual resilience factors on PTSD were examined. The theoretical foundation of this research included the cognitive link between the single and multiple exposures to traumatic events and the automatic conditioned responses related to the combat-related trauma using a retrospective view of archival datasets. Data analysis included a chi square test of independence and factoral analysis of variance to identify the combat-related PTSD symptoms and its associated variables. The sample size was a stratified random sampling of 570 cases. The results of this analysis support an association between location of tours and PTSD symptomology as well as a small effect between number of deployments and PTSD symptomology irrespective of status. These results will benefit the U.S. Navy enlisted personnel by increasing the awareness of a trend in combat-related PTSD, identify protective factors in resilience, and showcase the need for greater focus of these issues within Navy policy and leadership.
