Process of Motivational Enhancement Therapy: Relationships between Therapist and Client Behaviours, and Alcohol Use Outcome

Motivational Interviewing (MI) is an evidence-based, directive, client-centered therapy designed to develop discrepancy and resolve ambivalence by eliciting and reinforcing client Change Talk. However, the exact link between the process engaged in during MI and outcome is only starting to be uncovered. The present thesis has replicated and expanded on the current knowledge of the relationship between Therapist and Client Behaviours during a MI-based intervention (Motivational Enhancement Therapy; MET) and outcome, and has provided support for the emergent theory of the inner workings of MI. This was achieved by coding 106 audiotaped MET sessions primarily by the methods outlined in the Motivational Interviewing Skill Code Version 2.0. Data was drawn from 28 participants who received 3-4 sessions of MET within the context of a randomised controlled trial for mild-moderate alcohol dependence at the Community Alcohol and Drug Service of Christchurch. Therapist and Client Behaviours were analysed within sessions (categorised into Early, Mid, or End Intervals) and across sessions, and compared with whether the client had drank within national drinking guidelines during the 6-months after MET (Controlled Drinkers). In terms of Client Behaviours during MET it was found that Uncontrolled Drinkers (compared with Controlled Drinkers) uttered a significantly higher frequency of Sustain Talk, lower Ability Language strength (over all MET and during End Intervals), and lower Commitment Language strength (during Session 2 and 4, and change over MET). Giving Information was the only Therapist Behaviour where significant differences were observed over all MET, with a higher frequency given to the Uncontrolled Drinkers. However, during End Intervals within MET Sessions, Controlled Drinkers received a significantly higher frequency of Advise without Permission and a lower frequency of Emphasise Control statements. In most instances MI-Consistent Therapist Behaviours were associated with higher strength of Ability and Commitment Language, and a lower frequency of Sustain Talk. MI-Inconsistent Therapist Behaviour, Direct, was associated with lower Client Language strength. Limitations to these results include small sample, limited ability to make inferences about causality, coder biases, and uneven reliability. However, this exploratory study was unique in investigating the relationship between Therapist Behaviours and the strength of Client Language, and in examining these factors within and across multiple sessions, and has produced a number of potentially valuable findings that warrant further investigation.

THE EFFECT OF LEGAL STATUS ON RESPONSES TO BRIEF MOTIVATIONAL INTERVIEWING WITH SUBSTANCE USERS

The effectiveness of drug abuse treatment for clients coerced into care remains controversial. Some studies find clients with legal pressure do better than those without legal pressure, while others report the exact opposite. Opposing views are often fueled by the wide-ranging models that guide delivery of addiction treatment. The present study examined how participants with and without legal pressure to attend treatment responded to a motivational (MET) vs. traditional (TAU) form of addiction treatment. Additionally, the predictive value of the Readiness to Change (RTC) score, from the URICA, was assessed across days of substance use and treatment retention. Legal status was shown to have a significant effect on days of primary substance use per week and treatment retention, regardless of intervention condition. The RTC score was shown not to be predictive of days of primary substance use or treatment retention. Research and clinical implications and future directions are discussed.

This thesis describes the applied research of motivating factors conducted among paramedical staff in the Hospital "Nemocnice Rudolfa a Stefanie Benešov" in 2010. The objective was to study several dimensions of work motivation, with special emphasis on motivational factors and motivational potential of work. Purpose of the project also represented a compilation of the consultancy recommendations arising from the results and their providing to the hospital staff. These recommendations should contribute to more effective application of targeted motivating of subordinates by managers of paramedical staff in this hospital. We found out that the strongest motivating factors for respondents include patient satisfaction, good interpersonal relationships at work, expressions of recognition and work achievement. It was also suggested that the caregivers see the highest motivation potential of work in the importance of the performed tasks; that is the possibility to participate actively in influencing people's health. Goals, affect and appraisal within the stressful transaction.

This study explored how personal strivings constructs of Goal Conflict and Complementarity and primary appraisal dimensions of Motivational Relevance and Congruence (separately and in combination), related across time to immediate and long-term effects of a stressful transaction. The study was located within the transactional model of stress and integrated aspects of motivational theory, focusing on the theoretical position that within the stressful transaction the relationship between motivational factors and the individual response to an event is mediated by cognitive processes, including appraisal. Advances in transactional theory highlight the role of motivational factors (such as personal strivings) as linked to primary appraisal in the form of Motivational Relevance and Congruence. The utility of personal strivings in exploring the role of motivational factors in the stress process were highlighted. Despite an increasing theoretical focus on motivation and appraisal, research in the area is limited. Data was collected for the study through the administration of questionnaires to university students (N=152) prior to (time 1) and into (time 2) an examination period. The questionnaires used a range of self-report measures. Correlations, partial correlations and ANOVAs were used to analyze the data. The findings indicated that Goal Complementarity and Conflict directly influenced primary appraisal processes but not affective and wellbeing outcomes. It was proposed that primary appraisal processes were the conduit through which the impact of Goal Complementarity and Conflict were expressed within the stressful transaction. The results also suggested the impact of anticipatory Motivational Relevance which seemed to imply a highly “loaded” event with negative affective and long-term consequences into the event. Subjects entering the event wit h an “optimistic” demeanour indicated by high anticipatory Congruence and positive affect had increased Congruence into the event with consequent amplified positive emotions and dampened negative effects. Subjects with high anticipatory Relevance and low Congruence across the event had relatively higher scores on negative outcomes. High anticipatory Relevance and Congruence was associated with negative immediate and long-term outcomes into the event. High Relevance Congruence was generally associated with a strong emotional response, which also elicited strong positive emotion as the event unfolded. Subjects with low Relevance did not seem to hold as strong an investment in the event and reported reduced emotions and symptomology. These findings were discussed in relation to the stress, appraisal and motivation literature and their limitations and implications were explored.

Therefore, various other motivational practices have been developed, which take into consideration such issues as the work environment and the job itself.</p><p>This thesis is a comparative study between Sweden and Finland. The study has an objective of exploring how corporate culture affects the use of motivational practices in the Information Technology industry.</p><p>According to the findings gathered from two organizations, Sasken Finland Oy and SYSteam, culture does influence the choice of motivational practices. The issue of how culture affects, depends on whether corporate culture is task or person-oriented.</p>

The impact of motivational interviewing feedback and coaching on school counseling motivational interviewing skills

School counselors have potential to make significant gains in closing the achievement gap for all students as advocated for by the American School Counseling Association. School counseling is moving away from the no-model model of services delivery that places counselors at the whim of principals, parents and teachers who traditionally define counseling duties according to their needs. Instead, it is moving toward a school counseling framework advocated for by national and state organizations. A core concept of these frameworks is the use of evidence-based practices to meet the needs of all learners. Motivational Interviewing (MI) is an evidence-based counseling practice developed to assist with resolution of ambivalence and behavior change. Researched throughout the world in a variety of settings from the health professions to prisons, the results show promise in MI for assisting with behavior change in both adult and adolescent populations. MI training is currently available to school counselors at conferences, school district trainings and through self-study. There is little research on the efficacy of MI and school counseling training and implementation. The purpose of this dissertation is to produce two manuscripts related to school counseling and MI. Using a concurrent multiple baseline design, this study focused on the impact of MI feedback and coaching supervision of school counseling graduate students (N=3) with previous beginning MI training in a two day workshop. The hypothesis stated that subsequent feedback and coaching supervision improved school counseling graduate the percentage of MI adherent behavior on the Motivational Interviewing Treatment Integrity (MITI 3.1). The global rating scores increased with feedback and coaching supervision and the participants moved from levels of beginning MI competency to proficiency in their mean scores. Participants did not, however, increase the percentage of MI adherent behavior. These score began high and remained high throughout the 10 week duration of the research. Implications for this study include highlighting the importance of feedback and coaching supervision for school counseling graduate students wanting to increase beginning MI skills, and it marks the beginning of research on training and implementing MI in school counseling programs. / Graduation date: 2013

The purpose of this study is to examine, both teachers’ attitude towards motivation in school and teachers’ description of how they work with pupils’ motivation. A particular interest is directed towards pupils who have difficulties motivating themselves for their schoolwork. The survey is based on ten qualitative e-mailinterviews with teachers in schoolyear 1-6, all qualified in each subject they teach. The result of the study shows that the teachers find motivation essential for pupils learning and how different strategies are used to motivate their pupils. The strategies used by the teachers largely agree with the theories presented in the theory section, which involves creating participation, context and a sense of competence for pupils learning. Lastly, the results are discussed in relation to the theories described, and for a didactic significance.
