Nasceu do desejo de investigar nessas gestantes, através da análise das sessões clínicas, aspectos subjetivos e intersubjetivos específicos desse encontro de condições: gravidez e doença infecto-contagiosa. Para isso foram utilizadas falas em sessões clínicas de gestantes portadoras de Toxoplasmose atendidas no Instituto de Diagnóstico e Prevenção da APAE-Goiânia. A psicanálise se constitui como referencial e embasamento teórico para a referida prática clínica no âmbito da saúde. Questões como mulher e feminilidade; desejo e não desejo materno; culpa; luto materno; construção da maternidade; criação do espaço psíquico para o bebê; relação mãe/bebê; adoecimento, aceitação da doença e adesão ao tratamento; e, principalmente, ambivalência, serão discutidas neste trabalho. O entendimento deste conflito traz a possibilidade de elaboração. Partimos de um questionamento para, após a discussão, obtermos um novo e diferente encontro com o conflito, trazendo-lhe um novo caminho, uma nova significação. ________________________________________________________________________________________ ABSTRACT / This paper aims at approach between Freud’s theory and the context of pregnant women, more specific in illness situation. Originated form the desire to investigate in those pregnant patients, through the analysis of clinic sessions, subjective and inter-subjective appearances of this meeting of conditions: pregnancy and infectious diseases. For that have been used talks in clinic sessions of pregnant patients with toxoplasmosis attended in APAEGoiânia’s Prevention and Diagnostic Institute. Psychoanalysis account as referencing and theory basement for the referred clinical practice into the health ambit. Issues as woman and femininity; maternal desire and not desire; guilt; maternal mourning; maternity building; creation of the psychic space for the baby; mother/baby relation; illness, acceptability of the disease and adherence the treatment; and above all, ambivalence, will be discuss in this paper. The understanding of this conflict brings the possibility of elaboration. We began with one big question for, after the discussion, to be able to attain a new and different meet with the conflict, bringing him a new way, a new meaning.

Análises moleculares em gestantes com diabete e hiperglicemia leve e em seus recém-nascidos /

Orientador: Marilza Vieira Cunha Rudge / Coorientador: Paula Helena Ortiz Lima / Banca: Daisy Maria Fávero Salvadori / Banca: Daniel Araki Ribeiro / Resumo: O grupo IB corresponde a gestantes portadoras de hiperglicemia leve, ou seja, gestantes que apresentam rastreamento positivo, mas diagnóstico negativo para diabete gestacional (DMG) teste de tolerância a glicose (TTG100g normal), e resposta alterada no perfil glicêmico (PG). Este grupo foi, acidentalmente, identificado em 1983, quando projeto prospectivo foi desenvolvido para padronização do PG comparando-o com o TTG100g no diagnóstico do diabete na gestação. Essas gestantes apresentam resistência à insulina, intolerância à glicose, maior susceptibilidade de desenvolver Diabete Tipo 2 alguns anos após o parto, e 53,8% de seus recém-nascidos (RN) são grandes para idade gestacional e/ou macrossômicos, semelhantes às mulheres com DMG. A sinalização da insulina é mediada por uma complexa e altamente integrada rede que controla vários processos, sendo responsável pela maioria das ações metabólicas da insulina, pela regulação da expressão de alguns genes e controle do crescimento e diferenciação celular. O gene do substrato receptor de insulina 1 (IRS-1) produz a proteína IRS-1, que é uma molécula expressa em muitos tecidos sensíveis à insulina, no qual tem um papel muito importante na regulação dos efeitos da insulina na célula. O gene IRS-1 é altamente polimórfico, e esses polimorfismos podem prejudicar gravemente a função do IRS-1. A substituição mais prevalente é o polimorfismo Gly-Arg no codon 972 (Arg972) do gene IRS-1 que mostra ligação direta a resistência à insulina, alterações lipídicas, Diabetes mellitus gestacional e Diabetes mellitus tipo 2. O presente estudo objetiva avaliar o quadro de diabetes e hiperglicemia leve na gestação, para identificar possíveis alterações genéticas relacionadas a essa alteração glicêmica, que podem aumentar o risco para o desenvolvimento de futuras doenças degenerativas na própria gestante ou em seus descendente / Abstract: Although not diagnosed with diabetes, mild gestational hyperglycemic women might show high levels of oxidative DNA damage. The aim of this study was to evaluate genotoxicity (basal and oxidative DNA damage levels) in lymphocyte samples from pregnant women with diabetes and with mild gestational hyperglycemia. This study included 113 pregnant women who were distributed into four groups based on oral glucose tolerance test (OGTT) results and glycemic profiles (GP), as follows: IA - Normal OGTT and GP (Non-diabetic group); IB - Normal OGTT and abnormal GP (mild gestational hyperglycemic group); IIA - Abnormal OGTT and normal GP (gestational diabetes group); IIB - Abnormal OGTT and GP (gestational or overt diabetes group). Maternal blood samples (5 -10 mL) were collected and processed for the determination of basal and oxidative DNA damage using Fpg and Endo III by comet assay. In groups IIA and IIB analyzed together, there was a larger number of Fpg-sensitive sites, while in IB there was a greater numbers of Endo III-sensitive sites. Apesar das evidências que associam a ocorrência de IU ao diabete, pouco se sabe dos mecanismos pelos quais o diabete leva à IU. Objetivo: verificar a associação entre a ocorrência de IU e função muscular do AP com os distúrbios hiperglicêmicos gestacionais. Materiais e Métodos: As gestantes foram avaliadas em dois momentos. O primeiro momento (M1) entre a 24a e 32a, e o segundo (M2) entre a 34ª e 38a semanas gestacionais. A análise estatística foi realizada com nível de significância de 5%. Resultados: As hiperglicêmicas apresentaram idade cronológica e gestacional mais avançadas, além de menor estatura. Houve diferença significativa na distribuição peso-altura entre os grupos no M1. Não houve diferença entre os grupos, no M2 quanto à ocorrência de IU. Na análise intergrupos da palpação vaginal, observamos redução da força muscular, enquanto que algumas mantiveram ou aumentaram. Na comparação das avaliações com perineômetro e eletromiógrafo, entre e intragrupos, nos grupos, não houve diferenças significativas. Conclusão: Devido às limitações do estudo, principalmente a dificuldade de conscientização e aprendizado da contração do AP, ressaltamos a importância de estudos nessa área, e padronização dos métodos de avaliação, para possivel comparação com diferentes estudos, tendo em vista à qualidade de vida da gestante / The pelvic floor (PF) can suffer alterations in the different stages of women life that may predispose to the Urinary Incontinence (UI). Despite the evidences which associate the UI to the diabetes, there is little information about the mechanisms which take diabetes to the UI. Objective: to analyze the association between the UI occurrence and the PF muscle function with the gestational hyperglycemic disorders. Material and Methods: the pregnant women were evaluated in two moments. The first moment (M1) was between the 24th and 32nd, and the second one (M2) was between the 34th and 38th gestational week. The statistical analysis was realized considering 5% relevance. Results: The hyperglycemic ones presented more advanced chronological and gestational ages, besides they were shorter. There was relevant difference in the weight-height distribution between the groups in the M1. There was no difference between the groups in the M2 considering the UI occurrence. In the intergroup analysis of digital palpation, muscle strength reduction was observed, although for some there was an increase and some kept the same. In the comparison of the evaluations with perineometer and electromyographer, inter and intra groups, there were no relevant differences. Conclusion: Due to the limitations of the study, mainly the awareness and learning of the PF contraction difficulty, we emphasize the importance of developing studies in this area and standardization of the evaluation methods to enable the comparisons with different studies aiming the pregnant women life quality

Objetivo: estimar a prevalência e fatores associados da sífilis em parturientes do Brasil. Método: estudo transversal de base populacional, realizado entre parturientes de 15 a 49 anos de idade, atendidas em maternidades do sistema público de saúde e conveniadas, no período de janeiro de 2010 a dezembro de 2011. Os fatores associados foram computados pela razão de prevalência, estimada pelo modelo de regressão logística. O nível de significância utilizado foi de 5%. Resultados: foram analisadas informações de 36 713 parturientes. A prevalência geral da sífilis foi estimada em 0,89%; nas regiões Norte, Nordeste, Sudeste, Sul e Centro-Oeste, foi de 1,05%, 1,14%, 0,73%, 0,48% e 1,20%, respectivamente. Das parturientes, 98,5% acessaram o pré-natal, 96,9% realizaram pelo menos uma consulta e 93,5% pelo menos um exame de VDRL no curso da gestação; contudo, 53,1% das parturientes diagnosticadas durante o pré-natal permaneciam infectadas quando da admissão para o parto. As parturientes pretas, amarelas e pardas e as de menor escolaridade apresentam mais risco do que as brancas e de maior escolaridade. Conclusões: de maneira geral, observa-se uma diminuição da prevalência da sífilis no Brasil. Porém, entre as parturientes amarelas, pretas e pardas, assim como as analfabetas, observa-se um aumento na ocorrência da doença, o que reforça a associação desse agravo com as desigualdades sociais. A baixa qualidade da assistência pré-natal parece contribuir ainda mais para agravar a situação nesse contexto de desigualdades, colocando o tratamento como uma grande barreira para o país seguir avançando na redução da prevalência da sífilis no Brasil. / Introduction: Despite the secular discovery of the causative agent of syphilis, the existence of reliable methods of diagnosis, simple, affordable and available therapy since 1943, syphilis remains a public health problem. Objective: To estimate the prevalence and the associated factors of syphilis in pregnant women in Brazil. Method: Cross-sectional population-based study conducted among pregnant women from 15-49 years of age, met in maternity hospitals in the public health system and hospitals that have partnership with the public service, from January 2010 to December 2011. The associated factors were computed by the prevalence ratio estimated by logistic regression model. The level of significance was 5%. Results: information from 36,713 pregnant women was analyzed. The overall prevalence of syphilis was estimated at 0.89%; in the North, Northeast, Southeast, South and Midwest, was 1.05%, 1.14%, 0.73%, 0.48% and 1.20%, respectively. Of the mothers, 98.5% accessed prenatal, 96.9% had at least one visit and 93.5% at least one test of VDRL in the course of pregnancy; however, 53.1% of pregnant women diagnosed during prenatal remained infected at admission for delivery. Black mothers, yellow and brown and with less education have higher risk than white and level of education. Conclusions: in general, there is a decrease in the prevalence of syphilis in Brazil. However, among the yellow, black and brown women as well as the illiterate, there is an increased occurrence of the disease, which reinforces the association of this condition with social inequalities. The low quality of prenatal care appears to contribute even more to aggravate the situation in this context of inequality, making the treatment as a major barrier to the country move forward in reducing the prevalence of syphilis in Brazil.

Dopplervelocimetria nas gestações complicadas por hiperglicemia : curva do índice de pulsatilidade da artéria umbilical /

Orientador: Iracema de Mattos Paranhos Calderon / Coorientador: Cláudia Garcia Magalhães / Banca: Marilza Vieira da Cunha Rudge / Banca: Roseli Mieko Yamamoto Nomura / Resumo: Construir uma curva de PI umbilical exclusiva de gestações complicadas por diabete melito (DM) ou hiperglicemia e compará-la com curvas de referência (Arduini e Rizzo, 1990; Sakamoto, 2007). Estudo longitudinal, onde foram incluídas 163 gestantes e excluídos os casos de gemelaridade, malformação, dificuldade técnica e menos de cinco consultas no pré-natal. Foram avaliados idade materna, índice de massa corporal (IMC) pré-gestacional, número de gestações e associação com tabagismo e hipertensão arterial (HA), além dos níveis de hemoglobina glicada (HbA1c) no final da gestação. No total, foram avaliadas 431 medidas de PI umbilical, entre 24 e 41 semanas de gestação. Para cada semana gestacional foram realizadas medidas de tendência central e de dispersão dos valores atribuídos e calculados os índices dos respectivos percentis (P5, 10, 25, 50 e 95). A correlação, a equação de predição e os intervalos de confiança (IC 95%) foram estimados por análise de regressão linear simples. Realizou-se comparação qualitativa por sobreposição das três curvas e, quantitativa, entre as curvas do estudo e de Sakamoto (2007). Adotou-se o limite de significância de 95% (p < 0,05). A equação de predição do PI umbilical foi expressa por [PI umb = 1,521 - 0,018* semana de gestação]. Os limites de P95 do PI umbilical evoluíram sempre abaixo dos limites da curva de referência de Arduini e Rizzo (1990) e apresentaram queda acentuada a partir da 37ª. semana, quando se confirmou diferença estatística em relação aos índices de Sakamoto (2007). Antes de sua proposição, outros estudos deverão validar a curva desenvolvida nesse estudo / Abstract: To construct a curve PI umbilical exclusive of pregnancies complicated by diabetes mellitus (DM) or hyperglycemia and compare it with reference curves (Arduini and Rizzo, 1990; Sakamoto, 2007). A longitudinal study where 163 women were included and excluded cases of multiple births, malformations, technical difficulty and less than five prenatal visits. We assessed maternal age, body mass index (BMI) before pregnancy, number of pregnancies and is associated with smoking and hypertension (HA), and levels of glycated hemoglobin (HbA1c) in late pregnancy. In total, 431 measurements were evaluated umbilical PI, between 24 and 41 weeks gestation. For each gestational week were performed measures of central tendency and dispersion values assigned and calculated indices of the respective percentiles (P5, 10, 25, 50 and 95). The correlation equation prediction and confidence intervals (95% CI) were estimated by linear regression analysis. Estudo longitudinal, onde foram incluídas 163 gestantes e excluídos os casos de gemelaridade, malformação, dificuldade técnica e menos de cinco consultas no pré-natal. Foram avaliados idade materna, índice de massa corporal (IMC) pré-gestacional, número de gestações e associação com tabagismo e hipertensão arterial (HA), além dos níveis de hemoglobina glicada (HbA1c) no final da gestação. No total, foram avaliadas 431 medidas de PI umbilical, entre 24 e 41 semanas de gestação. Para cada semana gestacional foram realizadas medidas de tendência central e de dispersão dos valores atribuídos e calculados os índices dos respectivos percentis (P5, 10, 25, 50 e 95). A correlação, a equação de predição e os intervalos de confiança (IC 95%) foram estimados por análise de regressão linear simples. Realizou-se comparação qualitativa por sobreposição das três curvas e, quantitativa, entre as curvas do estudo e de Sakamoto (2007). Adotou-se o limite de significância de 95% (p < 0,05). A equação de predição do PI umbilical foi expressa por [PI umb = 1,521 - 0,018* semana de gestação]. Os limites de P95 do PI umbilical evoluíram sempre abaixo dos limites da curva de referência de Arduini e Rizzo (1990) e apresentaram queda acentuada a partir da 37ª. semana, quando se confirmou diferença estatística em relação aos índices de Sakamoto (2007).
