Společný ošetřovatelský lůžkový fond z pohledu sestry a managementu / Merged Nursing Inpatient Ward from the Perspective of Nurses and Management

Merged nursing inpatient ward is in the last few years becoming a trend of smaller, but also medium-sized hospitals. Due to the restructuring of beds of acute inpatient care in the Czech Republic, separate inpatient wards are merged to one joint ward, in the context of surgical as well as internal medicine. Merged inpatient ward is a combination of various disciplines (specialisms) of inpatient care under one senior consultant, or there is a common nursing ward, where senior consultants of individual disciplines are retained, while nursing care is merged. The main reason for the establishment of a merged inpatient ward is the optimization of beds and the related effective use of bed capacity in connection with an effort to preserve access to health care. The aim of the thesis Merged Nursing Inpatient Ward from the Perspective of Nurses and Management was to evaluate the contribution of a merged nursing inpatient ward, to identify the reasons for the project and the complications that accompanied the project. This research should analyze the problems in nursing care in connection with the origination of the project, to assess the demands of nursing on the ward and map errors in the field of nursing care in the context of the realization of the project. The thesis should evaluate interdisciplinary collaboration and map the occurrence of conflicts in a multidisciplinary team. During the survey it was found that the main reasons for the project were the financial savings associated with the restructuring of acute care beds in the Czech Republic. The establishment of a merged nursing inpatient ward helped the hospital to keep some expertise and it unified nursing care of the separate wards. In connection with the establishment of a joint nursing care, some complications and disadvantages that accompanied the project initially were analyzed. The survey showed how sensitively employees responded to the major change in the organization and how the cooperation between superiors and subordinates functioned. The study analyzed in what form the selection of employees for a common bed fund was realized and whether managers themselves could choose subordinates for their departments. One part of the research was the evaluation of the quality of nursing care on the merged inpatient ward, which showed that the establishment of the project did not have a negative impact on the quality of nursing care. In contrast, nursing care was united and there was also unified organization of nursing on the ward. The research evaluated the feedback from the subordinates to the top managers which was implemented through meetings and sessions. The survey refers to the greater workload on nurses in the working process on the merged ward. The origin of the project had an impact on the creation of new multidisciplinary teams and interdisciplinary collaboration. The project of a joint nursing inpatient ward was realized that the hospital can continue to provide quality health services to all patients. Research has shown that although the project had certain disadvantages and complications, it was certainly beneficial for the organization. The conclusions of the thesis may be presented in seminars, conferences and may be offered to professional journals for publication. The results of the research can help organizations that are going to realize the project of merged nursing inpatient ward.

Koordinovaná rehabilitace z pohledu pacienta po poškození mozku a členů multidisciplinárního týmu

Abstract This diploma thesis aims to describe coordinated health rehabilitation of people after severe brain injury provided by multidisciplinary team within home environment. It comprises both theoretical and practical part. The theoretical part presents the history and development of coordinated rehabilitation services and the function of multidisciplinary team. Furthermore, it characterizes a brain injury in more details. The practical part was carried out by means of qualitative research strategy semi structured interview. The first part of the research was aimed at the interviews with clients after brain injury and cerebrovascular accident. The interviews started after three months from entering the project and were realized by social workers. Its main propose was to evaluate the care of multidisciplinary team. Clients were questioned about work, time, communication and co-operation of individual experts involved in the process of recovering. The second part of the research used a method called focus group, where 12 members of the team were involved. The goal of the research was to describe how the multidisciplinary team works from both the point of view of a patient after the brain injury and the members of the multidisciplinary team within the project GAJU. The GAJU project is entitled "Coordinated rehabilitation of patients with brain injury (reg. number GAJU 138/2016/S)". The selection of patients was based on the communicative skills criteria e.g. skills to participate in the directed conversation based on the FIM and WHODAS and the follow-up questions concerning social issues, physiotherapeutic and ergo therapeutic methods and techniques. To achieve the aim of the diploma thesis two research questions were defined: "How is the function of multidisciplinary team perceived by clients involved in GAJU project?" and "How is the function perceived by the members of multidisciplinary team involved in GAJU project who put the coordinated rehabilitation into practice?" It was found out that after the discharge from a hospital being with family members was the most important. Besides, multidisciplinary team who regularly commuted to clients helped them. The members of the team were physiotherapists, ergo therapists, social workers and doctors. In view of the fact that physiotherapist attended the client the most frequently, the function of this expert is perceived as crucial during recovery. No less important is ergo therapist who is useful concerning motor activity and presenting various tools and aids leading to improvement. Social worker is apprehended rather as a psychological or informative support. Positively valued is attending clients at home, interconnection of the team members and the progress of the client from all points of view. On the other hand the lack of working experience of some team members is pointed out. Additionally, some clients were not satisfied that the data were collected by means of questionnaires.

The diploma thesis at hand aims to find out to what extent Vojta method can stimulate child?s ability to receive and actively participate in other means of the comprehensive rehabilitation system. The diploma thesis observes application of Vojta method within the comprehensive system of rehabilitation provided in ARPIDA in České Budějovice. The diploma thesis is divided into two parts. The theoretical part deals with the current state of the issue in question, provides characteristics of individual means of the comprehensive system of rehabilitation, and acquaints the reader with the technique named reflex locomotion, which is referred to as the Vojta method. The practical part comprises of two chapters. The first chapter provides the reader with a description of the selected facility. The second chapter introduces case reports of the clients completed with analyses of interviews held with parents and other members of the multidisciplinary team. Using the mentioned materials, the author produced six case reports of selected clients of ARPIDA centre. Five out of six observed clients were diagnosed with infant cerebral palsy in different forms with combined defects. For the purposes of the diploma thesis, the author used the method of qualitative research. The practical part is based on analyses of medical records and personal documentations of selected clients of ARPIDA centre; author?s own direct observation; analyses of interviews held with parents and other members of the multidisciplinary team; and observations made by the author while performing her duties as a physiotherapist. Finally, it was concluded that application of Vojta method is necessary within the comprehensive system of rehabilitation which strives to reduce the effects of the disability and improve the quality of life in subjects with the said disability. The author?s own research was designed to verify the findings learned from the literature reviewed for the purposes of the theoretical part. The conclusion of the practical part contains a summary of results, assessment of accomplished goals; and research outcomes and recommendations. The thesis may be found useful by both experts and students who have professional interests in the issue; yet, it should also be made available to the members of the multidisciplinary team as a feedback.

This study is refered to the meanings attributed to PAD on oneâs eight workers side who joined the interdisciplinary team, active in the refered program established in Fortaleza, CearÃ. The research has theoric basis in the qualitative method and the semi-structured interview. The Dejours ideas, concerning about work psychodynamic, and its connection with the workers psychological pleasure and suffering, based the comprehension of its contents work psychodynamic, and it made real trough. The subjects were male and female, genders ages between 29 and 45 years old, and they joined PAD team for more than a year. The interviews were transcripled and analyzed, highlighting questions according to several dataâs, longing for understand them. His discoveries, that donât apply to generalizations, are abridged on final consideration, where it also highlighted that their results showed, that PAD means to these workers a strategy, of acting that works as inducer agent and life and health, producer in an interdisciplinary approach, that cause the rise of âlivesâ quality under its monitoration, providing the financial costs racionalization to CAFAZ, in a view of current health, hegemonic and centerhospital. The research participants, recognize the existence of situations, that cause pleasure and suffering in the work developed at PAD, and admit that is necessary, to be developed better politics of Human Resources in Organization, that can, intervene in suffering situation of PAD team, to aim at reduce it and potencialize the experiences of pleasure at work. / Este estudo tratou sobre os significados atribuÃdos ao Programa de Atendimento Domiciliar - PAD, da Caixa de AssistÃncia aos FazendÃrios Estaduais - CAFAZ, por parte de oito trabalhadores que integravam a equipe interdisciplinar atuante no citado programa instaurado em Fortaleza, no CearÃ. A pesquisa teve fundamentaÃÃo no mÃtodo qualitativo e na tÃcnica de entrevista semi-estruturada. As idÃias de Dejours, concernentes à psicodinÃmica do trabalho e sua relaÃÃo com o prazer e o sofrimento psÃquico dos trabalhadores, subsidiaram a anÃlise dos dados e fundamentaram a compreensÃo de seus conteÃdos. Os sujeitos foram dos gÃneros masculino e feminino, com idades entre 29 a 45 anos, e integravam a equipe do PAD hà mais de um ano. As entrevistas foram transcritas e analisadas, destacando-se questÃes recorrentes a anÃlise dos fatores de entrave e facilitaÃÃo no trabalho por eles indicados, ao conhecimento das atividades desenvolvidas por esses profissionais, segundo Ãs suas percepÃÃes, e a avaliaÃÃo dos conteÃdos emocionais instigados, por conta do atendimento domiciliar, em meio aos dados plurais, almejando-se decifrÃ-los. Suas descobertas, que nÃo se aplicam a generalizaÃÃes, foram resumidas nas consideraÃÃes finais, onde tambÃm se destacou, que o PAD como um agente idutor e produtor de vida e saÃde, dentro de um enfoque interdisciplinar e que produz a elevaÃÃo da qualidade das "vidas" sob sua monitoraÃÃo, proporcionando a racionalizaÃÃo dos custos financeiros para a CAFAZ. Os resultados do estudo mostraram que existem nexos entre os objetivos especÃficos e as descobertas da pesquisa, como tambÃm, que os membros do PAD consideram, que à preciso desenvolver melhores polÃticas de Recursos Humanos na OrganizaÃÃo, as quais possam minimizar o sofrimento psÃquico destes profissionais e potencializar suas vivÃncias de prazer no trabalho

Former les soignants à la communication en oncologie :contribution à l’étude du transfert des compétences dans la pratique clinique

Si l’importance de la communication dans la prise en charge des patients en oncologie n’est plus à démontrer, elle n’en demeure pas moins une difficulté pour les soignants. Les soignants se retrouvent fréquemment confrontés à des contextes communicationnels complexes pour lesquels ils se sentent bien souvent démunis et insuffisamment formés. Des formations adaptées et spécifiques sont nécessaires afin d’assurer un changement durable des stratégies de communication utilisées par les soignants dans leur pratique clinique. Cette thèse vise à apporter une contribution à la formation des soignants à la communication en oncologie et à l’étude du transfert des compétences dans la pratique clinique. Ce travail de thèse porte sur l’analyse de deux contextes de communication différents :l’annonce de mauvaises nouvelles et la communication autour des traitements. Au travers de ces deux contextes, trois objectifs ont été poursuivis :(a) la modélisation d’un programme de formation adapté à un contexte de communication spécifique, (b) l’évaluation de l’efficacité de ce programme de formation ou l’évaluation du transfert des compétences apprises dans la pratique clinique et (c) l’évaluation de l’impact de ce programme de formation à la communication sur le patient (et sur le proche) simulé ou réel.Dans le cadre de ce travail de thèse, nous avons donc pu évaluer l’efficacité de deux formations à la communication spécifiquement développées pour ces deux contextes de communication. La formation à l’annonce de mauvaises nouvelles a permis une amélioration de la répartition du temps consacré à chaque phase du processus d’annonce ainsi que l’acquisition de stratégies de communication. Elle a également permis une augmentation de l’activation physiologique des médecins, reflétant un meilleur niveau d’engagement cognitif et émotionnel dans la tâche. Concernant la formation à la communication donnée en radiothérapie aux équipes multidisciplinaires, elle a permis le transfert des compétences acquises durant la formation dans la pratique clinique. Ces formations ont également montré un impact sur les patients et les proches réels ou simulés en permettant que les soignants leur laissent plus de place pour s’exprimer. La formation à la communication en radiothérapie a également montré un impact sur la satisfaction des patients. Tetraplegia : the psychosocial problems encountered by black patients once discharged from the hospital

The purpose of this study was to explore the psychosocial problems encountered by Black patients with tetralpegia once discharged from the hospital. Phenomenology as a research strategy was used with the aim of understanding the worldview of patients. The medical aspects of spinal cord injury were discussed, firstly with emphasis placed on the incidence of spinal cord injury, causes of spinal cord injury, different levels of tetraplegia and the management of patients with tetraplegia. The psychosocial problems encountered by black patients with tetralpegia once discharged from the hospital were discussed. The emphasis was placed on the patients’ emotional reaction to the injury, adjustment towards the injury and the guidelines for social work intervention with regard to the patient with tetraplegia. Ten respondents participated in the study after they were selected by using availability sampling and the study revealed the following. Tetraplegia results in a variety of psychosocial problems for both the patient and his/her family, which are: Negative marital relationships and desertion by the healthier spouse. Disturbed family relationships. Negative self-esteem and lack of self-confidence due to physical limitations. Poor quality of life, stigmatization by the community and inaccessibility in as far as public transport is concerned. Tetraplegia triggers psychological, social and financial problems. The diploma thesis focuses on the work of social and health-social workers in a selected medical facility. The paper is designed as a theoretical introduction to the issue of social work in health care, especially focused on the position of the social or health-social worker at a healthcare facility. Furthermore, there are described competencies of social workers, and the specifics of cooperation in multidisciplinary teams of medical facilities. The theoretical part also consists of chapters focused on human motivation and stress, which social workers may experience during the performance of their work. The empiric part of this diploma thesis presents the research, which aim is to describe the position of social workers in the selected medical facility, characterize the limits in the work of social workers resulting from this position and define their satisfaction with their current position within the organizational structure of the selected medical facility. Traumatic brain injury (TBI) patients’ post-acute rehabilitation : the experience of family members

A traumatic brain injury (TBI) is what its name suggests: an extremely traumatic event that affects the most supremely powerful but fragile organ that controls all our bodily functions and holds the essence that makes individuals unique. Some patients with a traumatic brain injury will recover without realising what the effect could have been and others will be left with effects that will last throughout their lives. “Many sufferers will remain severely incapacitated and a lamentably large number will become part of the statistics on the mortality after TBI” (Burns, 2008a:76). The impairments that an individual faces after a TBI will be dealt with in an acute rehabilitation setting. Significant emotions are experienced by patients with a traumatic brain injury. The researcher believes that this emotional reaction is also experienced by the family members. The event may be even more devastating to the family members, because of the impaired cognitive functions of the injured person. Ross and Deverell (2004:36) state that when individuals are diagnosed with disabling conditions, they experience strong emotional reactions. “Feelings of grief, anxiety, inadequacy, anger, guilt, vulnerability and confusion are some of the more common emotions that clients and their families experience when they encounter a disability in themselves or a family member” (Ross and Deverell, 2004:41). After a traumatic brain injury the patient is usually admitted to an intensive care unit (ICU). Once medically stable and able to participate in an active rehabilitation programme, provided this is authorised by the patient’s medical aid, the patient is transferred to a rehabilitation unit. While in ICU, the outcome is very uncertain and the the family often receives very limited support. Patients are often in a coma and unable to communicate. In working with TBI patients, the researcher found that family members receive very limited support, education on the condition and feedback on the patient’s medical state from team members, and therefore become very anxious when the patient is eventually transferred to a rehabilitation unit. In this study, the researcher focused on the family’s experience from the time of the patient’s admission into ICU until discharge from rehabilitation. During the rehabilitation process, family members are encouraged to support their family member in the rehabilitation unit. This is often very difficult for them, as it entails being confronted with reality and the often devastating impact of the injury. The focus of this study is on the experiences of family members while the patient with a TBI is in hospital and in the rehabilitation unit and the challenges they face. The researcher strove to understand the experiences of family members of patients with TBI, from ICU through until discharge from acute rehabilitation. The goal was to explore the experiences of adult family members of the traumatic brain-injured person in post-acute rehabilitation. The research question was: What were the experiences of adult family members of the person with a traumatic brain injury (TBI) during post-acute rehabilitation? A qualitative approach was used in this study, with a collective case study research design. The population for this study was all the family members of patients who underwent rehabilitation as a result of traumatic brain injury in the Life Eugene Marais Hospital, Pretoria, Gauteng. In this study purposive sampling was used to choose participants who were family members of TBI patients. The criteria for sampling of family members as participants were as follows. The patient <ul> <li> had suffered a traumatic brain injury;</li> <li> had undergone acute rehabilitation at Life Eugene Marais Hospital for the last four years;</li> <li> had completed rehabilitation, in other words had to be post discharge;</li> <li> lived in the province of Gauteng; and</li> <li> was able to speak and understand English or Afrikaans, irrespective of gender, race, religion, culture or age.</li> </ul> Eight participants who were family members of eight TBI patients were chosen for this study. Semi–structured individual interviews were conducted with participants. Interviews were voice recorded with the permission of the participants and were transcribed by the researcher. The data were analysed by the researcher and the themes and sub-themes generated from the data. The research findings were presented by providing a profile of the research participants and then presenting the themes and sub-themes, including literature control and verbatim quotes from the transcriptions. The themes included the following: Theme One – Understanding of TBI; Theme Two – Period of hospitalisation; Theme Three – Family members’ emotional experience of TBI; Theme Four – Period of rehabilitation; Theme Five – Period post discharge; Theme Six – Support systems; Theme Seven – Effects of TBI and Theme Eight – Future. The conclusions of this study are that traumatic brain injury (TBI) is experienced as severe trauma by family members, who struggle to cope not only initially but especially once the person is discharged and has to be cared for at home. In the long term this experience leads to changes in the family regarding structure, roles, functioning, relationships, communication, finances and social life. The aim of the thesis is to obtain the view of the employees of the assisting professions who work in hospitals on social work and, as such, work of health-social employees. The entire thesis is divided into two parts - theoretical and practical. The theoretical part introduces readers to the profession of social work, its ethics and values. The diploma thesis on the topic Special pedagogical support in the field of work training of persons with multiple disabilities aims to clarify the issue of work training of students of the One-Year Practical School and the Two-Year Practical School and their subsequent employment. The work is divided into several parts, the theoretical part is focused on information from professional publications and electronic sources, which describes the basics of this issue, which is an integral part of the topic. Furthermore, the theoretical work reveals the definition of the term person with disabilities, the specification of multiple disabilities and related terms such as education of people with multiple disabilities, vocational training and employment. It also describes the possibilities of connecting social services with education, social services that students can use within the Arpida Center.
