Representações e sentidos sobre a revelação do diagnóstico da tuberculose: relações com adesão ou não adesão ao tratamento / Representations and meanings about diagnosis revelation of tuberculosis: relations with adherence or non adherence to treatmentRoberta Andrea de Oliveira 06 July 2012 (has links)
Pesquisa descritiva e qualiquantitativa que resgatou representações sociais de profissionais de saúde e pacientes referentes ao modo de comunicação do diagnóstico da tuberculose e adesão ao tratamento. O trabalho buscou analisar o conteúdo discursivo baseado nas discussões sobre Conscientização desenvolvidas por Paulo Freire. Realizou-se entrevista semi-estruturada com 39 profissionais envolvidos com a entrevista inicial de diagnóstico e 34 pacientes adultos em tratamento da tuberculose em 22 unidades de saúde da região Sul/São Paulo. Questionados sobre a entrevista inicial de diagnóstico, os pacientes entendem este momento de modo amplo, para além da consulta onde receberam a notícia oficial. Entretanto, para profissionais, o momento da notícia é bem delimitado e revela que o diagnóstico possui um caráter tecnológico. Questionados sobre o modo de dar a notícia de sorte a incentivar o paciente a se tratar, uma das categorias expressadas pelos pacientes traz a importância de se discutir tudo aquilo que envolve a doença, contexto e vida do paciente. Em contrapartida, profissionais se importam com o conteúdo a ser informado, mantendo o caráter tecnológico da informação. Questionados sobre os modos de dar a notícia que podem desmotivar o paciente a se tratar, pacientes e profissionais trazem a importância do entendimento do tratamento (aspecto cognitivo) e do respeito e vínculo (aspecto psíquico ou afetivo) para não haver desistência. Com relação ao tratamento parecer complicado quando explicado durante a notícia (aspecto comportamental) pacientes e profissionais discordam desta opção. Algumas ideias centrais destes dois grupos concordam que o tratamento é complicado e isso pode fazer o paciente desistir. Segundo Paulo Freire, toda ação educativa deve ser precedida de reflexão sobre o homem e o meio de vida para que não se torne pré-fabricada e inoperante. Por isso ampliar as discussões sobre o que envolve a doença, contexto e vida do paciente pode contribuir para um entendimento mais completo da situação. Apesar do tratamento da tuberculose ser complicado e, portanto, desmotivante, promover o entendimento do paciente, respeitá-lo e construir um vínculo podem contribuir para a adesão, na visão de pacientes e de profissionais de saúde / Descriptive and qualitative-quantitative research that rescued social respresentations of health professionals and patients in relation to mode of communication of tuberculosis diagnosis and treatment adherence. The study aimed to analyze the discursive content based on discussions about Conscientization developed by Paulo Freire. Semi-structured interviews were performed with 39 professionals involved with the initial diagnostic interview and 34 adult patients in 22 health units in South Region, Sao Paulo. Asked about initial diagnostic interview, patients understand this point broadly, far beyond the consultation where they received the official news. However, for professionals, the moment of the news is well defined and reveals that the diagnosis has a technological character. Asked how to break the news so as to encourage the patient to treatment, one of the categories expressed by patients brings the importance of discussing everything that involves the disease, context and patients life. In contrast, professionals care about the content to be informed, keeping the technological character of the information. Asked about ways to break the news that might discourage the patient to treatment, patients and professionals bring the importance of understanding about treatment (cognitive aspect) and respect and bond (psychic or affective aspect) so as to avoid abandonment. Regarding the fact that the treatment seems complicated when explained in the news (behavioral aspect), both patients and professionals oppose to this option. Some central ideas of these two groups agree that treatment is complicated and can make patient to give it up. According to Paulo Freire, all educational activity must be preceded by reflection on the human being and the lifestyle so that it does not turn out prefabricated and inoperative. Therefore, expanding the discussions on what involves the disease, context and patients life can contribute to a more complete understanding of the situation. Although the treatment of tuberculosis is complicated, and therefore disappointing, to promote the understanding of the patient, to respect him and to build a bond with him can contribute to the adherence, both in patients and health professionals point of view

Service engagement in psychosis : the role of psychological variablesReid, Caroline January 2018 (has links)
INTRODUCTION: In psychosis, low engagement with mental health services is both prevalent and frequently associated with negative outcome. The overarching objective of this thesis was to investigate the role of psychological variables in service engagement in people with psychosis. A systematic review was conducted to examine the evidence for clinical and psychological correlates of engagement. An empirical study sought to investigate the association between engagement and psychological variables of a relational nature (i.e. mentalizing and interpersonal functioning). METHODS: A systematic search strategy across four electronic databases yielded seventeen journal articles. For the empirical study, forty-two people with multiple-episode psychosis completed self-report measures of service engagement, symptoms, mentalizing and interpersonal functioning, within a cross-sectional design. RESULTS: The review found relatively robust evidence supporting the association between engagement and numerous clinical variables. Eleven psychological variables were revealed as significant correlates of service engagement, encompassing developmental, individual and relational factors. Assessment of quality and risk of bias highlighted a number of limitations within included studies. In the empirical study, greater cognitive/disorganization symptomology was predictive of lower service engagement. Service engagement was significantly correlated with mentalizing, but not with interpersonal functioning. The relationship between cognitive/disorganized symptomology and engagement was not mediated by mentalizing performance. CONCLUSION: Numerous psychological variables are associated with service engagement, which has the potential to inform clinical practice in view of enhancing engagement. Qualitative and longitudinal studies with both service user and provider samples are required to capture the contextual information surrounding fluctuations in levels of engagement.

A adesão ao tratamento ocorre quando o conselho médico ou de saúde coincide com o comportamento do indivíduo, ao uso de medicamentos, cumprimento da dieta e mudanças no estilo de vida, não sendo, portanto, um ato não passivo do paciente. Em pacientes com hipertensão arterial sistêmica a adesão ao tratamento pode ser definida como o grau de cumprimento das medidas terapêuticas indicadas, sejam elas medicamentosas ou não, com o objetivo de manter a pressão arterial em níveis pressóricos normais. A não adesão em pacientes com doenças crônicas em tratamento a longo prazo em países desenvolvidos é em média de 50%, revelando a importância de serem avaliados os motivos que levam a esse comportamento. O estudo teve como objetivo avaliar a não adesão em idosos hipertensos de uma unidade pública de saúde de Ribeirão Preto - SP. Trata-se de um estudo de corte transversal, desenvolvido com uma amostra de 196 pessoas. A coleta de dados ocorreu entre agosto de 2014 até junho de 2015, após aprovação do Comitê de Ética em Pesquisa. Para essa etapa foram utilizados os instrumentos Brief Medication Questionnaire, Medical Outcomes Studies 36-item Short Form Survey, Escore de Risco Global e Escore de Risco pelo Tempo de Vida. Após a coleta dos dados, as entrevistas foram codificadas, os dados foram tabulados e foi realizada a análise estatística descritiva e de correlação. Como resultado, constatou-se que houve predomínio de mulheres, com idade média de 69,4 anos, casados/união estável, não moravam sozinhos, com 1,85 pessoas na casa em média, de cor branca, com ensino fundamental incompleto, renda de até dois salários mínimos e aposentados/pensionistas, atendidos pelo SUS. Apresentaram hábitos de vida razoáveis, sem predomínio de consumo de bebidas alcoólicas, tabagismo, uso excessivo de sal e sedentarismo. A mais frequente comorbidade associada à HAS foi a dislipidemia. Foi observado elevado predomínio de fatores de risco cardiovasculares como obesidade abdominal, obesidade geral, comorbidades, razão de lipídeos e fatores agravantes como proteína c reativa ultrassensível, microalbuminúria e síndrome metabólica. A maioria da amostra foi classificada como sendo portador de risco cardiovascular alto após estratificação do risco. A percepção da qualidade de vida relacionada à saúde foi considerada baixa na maioria principalmente devido a limitações emocionais. A não adesão esteve presente em quase metade dos idosos, relacionada principalmente à complexidade da farmacoterapia e dificuldade em lembrar sobre o uso de seus medicamentos. Não foi observada correlação entre a não adesão e as variáveis estudadas. Conclui-se que o comportamento de não adesão observado não esteve relacionada às variáveis estudadas nessa amostra e que são necessárias intervenções urgentes para reduzir o risco cardiovascular e prevenir doenças cardiovasculares e mortalidade, bem como melhora da percepção da qualidade de vida relacionada à saúde. / Adherence to treatment is when medical or health advice corresponds with the individual\'s behavior, the use of medication, diet compliance and changes in lifestyle. Therefore, it is a non-passive act of the patient. In patients with systemic arterial hypertension, adherence to treatment can be defined as the degree of compliance with therapeutic measures, whether medicinal or not, in order to maintain blood pressure in normal blood pressure levels. In developed countries, non adherence rate is about a half in patients with chronic diseases in long-term treatment, showing the importance of the evaluation of the reasons for this behavior. This study aimed to assess non adherence among elderly hypertenses in a public health unit of Ribeirão Preto-SP. This is a cross-sectional study, conducted with a sample of 196 people. Data collection took place from August 2014 untill June 2015, after approval by the Research Ethics Commitee. For this step, we used the instruments Brief Medication Questionnaire, Medical Outcomes Studies 36-item Short Form Survey, Global Risk Score and Lifetime Risk Escore. After data collection, the interviewers were codified, data were tabulated for the descriptive and correlation statistical analysis. We found out that there was predominance of women, mean age of 69.4 years, married individuals, and most of them lived with other people (mean 1.85), white individuals, with incomplete Elementary School, income of two minimum wage at least, retired and using public health system. We identified individuals with fair health habits, without predominance of alcohol, tabacco and salt use. A minority claimed physical inactivity. Dyslipidemia were the most frequent comorbidity associated with SAH. We observed a high presence of risk factors for cardiovascular diseases such as abdominal and general obesity, comorbidities, lipids rates, ultra-sensitive c-reactive protein, microalbuminuria and metabolic syndrome. The sample showed a very high presence of elevated cardiovascular risk after risk escore calculation. The perception of health related quality of life was considered low in most, mainly due to emotional limitations. Non adherence was present in almost half of the elderly and was mostly related to the complexity of pharmacotherapy and the difficulty to remember about the use of their drugs. No correlation was observed between non adherence and the variables studied in this sample. We strongly advice that interventions are made to reduce cardiovascular risk and prevent cardiovascular disease and mortality, and to improve mental health status.

Microsporidia infect invertebrate and vertebrate animals. Human pathogenic microsporidia are associated with severe disease in immunocompromised individuals, and mostly asymptomatic infection in the immunocompetent. Treatment options for microsporidiosis are limited, incompletely effective, and associated with toxicity. Furthermore, microsporidia infection of healthy individuals is poorly understood, and the consequences of asymptomatic infection have not been determined. Little is known about the molecular mechanisms of microsporidia infection, but such information is essential for the development of new therapies. Spores adhere to host cell surfaces in vitro. Our laboratory has focused on determining specific host cell and microsporidia spore surface participants in spore adherence. Our previous studies have shown that host cell sulfated glycosaminoglycans and the spore surface protein EnP1 participate in spore adherence to host cells. Additionally, in vitro inhibition or augmentation of spore adherence decreased or increased host cell infection, respectively. These studies demonstrated the importance of spore adherence in host cell infection and began to characterize the host cell and spore determinants of adherence. The goal of this research was to further characterize host cell and spore participants in microsporidia adherence and infection of host cells in vitro. We characterized an intracellular microsporidia protein and related antibodies for analyses of microsporidia spore surface proteins; characterized a spore surface protein, MsADAM, involved in spore adherence to and infection of host cells in vitro; and suggested a role for host cell integrins in microsporidia adherence to and infection of host cells in vitro.

Thesis (MBA.) -- University of Limpopo, 2016 / Project systems allow project managers to carry out work in a professional and well organised manner. These systems are created and maintained to advance project performance. Eskom spends a lot of resources on the creation and maintenance of project systems. The literature on project systems shows that, in spite of advancement in project managements processes, systems and tools, project success has not significantly improved. This problem raises questions about the value and effectiveness of project systems. Therefore this paper reports about the correlation between the degree of project manager’s compliance to project systems and project performance in Eskom Distribution Limpopo Operating Unit.
The study looked at the performance of 10 projects and used empirical data on designers, planners, managers and project managers working in Eskom Distribution LOU to measure the compliance level of employees to project systems when carrying out the 10 chosen projects or any other projects not listed.
A total of 45 completed questionnaires were analysed. Correlation analysis tests found a negative correlation between project manager’s project systems compliance level and project performance in terms of schedule and cost. The conclusion found was that as the compliance level on project systems increases, project performance decreases. Meaning there is an inversely proportional relationship between project system compliance level and project performance.
Additionally, a lower level of knowledge than expected on the project managers, designers, and planners was found.

Implication de l'endotheline-1 dans l'adhérence et l'activation des polynucléaires neutrophiles dans la drépanocytose / Role of Endothelin-1 in neutrophils activation and adhesion in sickle cell diseaseKoehl, Bérengère 11 January 2017 (has links)
La drépanocytose est une maladie génétique du globule rouge, due à une mutation ponctuelle du gène β de la chaine de l’hémoglobine. Néanmoins, la physiopathologie de la maladie va bien au-delà des anomalies érythrocytaires, avec notamment un dysfonctionnement vasculaire et leucocytaire qui font toute la complexité de la maladie. Au cours de ce travail, nous avons étudié le rôle de l’endothéline-1, peptide vaso-constricteur impliqué de nombreuses pathologies notamment vasculaires, sur l’activation et l’adhérence des polynucléaires neutrophiles dans la drépanocytose. Nous avons travaillé à la fois sur un modèle murin de drépanocytose (les souris SAD) et sur des échantillons sanguins de patients. Sur le modèle animal, nous avons réalisé des expériences de microscopie intravitale, permettant de tester in vivo l’effet des antagonistes des récepteurs ETA et ETB à l’endothéline sur le recrutement des polynucléaires neutrophiles. Sur les échantillons de sang de patients drépanocytaires, nous avons testé in vitro l’adhérence en flux des neutrophiles à l’endothélium vasculaire en réponse au blocage des récepteurs ETA et ETB. Nous avons enfin étudié l’expression des récepteurs ETA et ETB à la surface des neutrophiles et les voies de signalisations découlant de leur activation.Sur le modèle de souris drépanocytaire, l’inhibition des récepteurs ETA, mais surtout ETB permet de limiter le recrutement leucocytaire important provoqué par un stimulus inflammatoire. Ces résultats confirment le rôle d’ETA et celui, plus inattendu, d’ETB dans toutes les étapes d’adhérence des polynucléaires neutrophiles et dans leur transmigration tissulaire en contexte drépanocytaire.Sur des échantillons humains, nous avons confirmé le rôle crucial d’ETB dans l’adhérence des polynucléaires neutrophiles. Nous avons également confirmé la présence de récepteurs ETA et ETB à la surface des neutrophiles. Le récepteur ETB active une voie de signalisation responsable d’une mobilisation du calcium intra cytoplasmique, mais indépendante de l’activation de la phospho-inositide 3-kinase. Enfin, nous avons montré la capacité de ces cellules à synthétiser et excréter elles-mêmes de l’endothéline-1, pérennisant ainsi probablement la réponse inflammatoire et le recrutement leucocytaire. En conclusion, notre travail a permis de mettre en évidence le rôle important du récepteur ETB dans le recrutement des polynucléaires neutrophiles dans la drépanocytose. Ces données suggèrent que les antagonistes des récepteurs à l’endothéline pourraient être bénéfiques en prévention des phénomènes vaso-occlusifs chez les patients drépanocytaires. / Sickle cell disease is a genetic disorder affecting red blood cells, due to a point mutation in the β chain of the hemoglobin gene. However, the pathophysiology of the disease goes well beyond the erythrocyte abnormalities, including vascular and white blood cell dysfunctions that contribute to the complexity of the disease. In this project, we investigated the role of endothelin-1, a powerful vasoconstrictor peptide involved in many vascular diseases, on activation and adhesion of neutrophil in sickle cell disease.We worked on both a mouse model of sickle cell disease (SAD mice) and blood samples from patients. In mice, we performed intravital microscopy experiments, to test the in vivo effect of endothelin receptor antagonists ETA and ETB on neutrophils recruitment. On blood samples from patients with sickle cell disease, we tested in vitro adhesion of neutrophils to vascular endothelium in response to the blocking of ETA and ETB receptors. Finally, we studied the expression of ETA and ETB receptors on neutrophils and the signaling pathways resulting in their activation.In our mouse model of sickle cell disease, the inhibition of both endothelin receptors ETA and ETB limits the major leukocyte recruitment caused by an inflammatory stimulus. These results confirm the role of ETA and the more unexpected important role of ETB in all stages of neutrophil adhesion and transmigration in sickle cell context.On human samples, we demonstrated the crucial role of ETB in neutrophils adhesion. We also confirmed the expression of ETA and ETB receptors on neutrophils. ETB receptor activates a signaling pathway responsible for intracytoplasmic calcium mobilization, but not involving the activation of phosphoinositide 3-kinase. Finally, we have shown the ability of neutrophils to synthesize and secrete endothelin-1, which can contribute to sustain the inflammatory stimulation and increased leukocyte recruitment.In conclusion, our work has highlighted the important role of ETB receptor in the recruitment of neutrophils in sickle cell disease. These data suggest that the antagonists of endothelin could be beneficial in prevention of vaso-occlusive events in sickle cell patients.

L’efficacité d’une thérapeutique repose, en grande partie, sur la bonne observance de la prescription. Une mauvaise observance est susceptible de provoquer un échec thérapeutique et une escalade dans les traitements. Elle est également génératrice de surcoûts pour le système de protection sociale. La base de données Erasme, base de remboursement du régime général de l’assurance maladie (Cnam-TS), constitue une source d’information importante pour la mesure de l’utilisation des médicaments et donc, possiblement, de l’observance. Hors, depuis de nombreuses années, il a été proposé des descripteurs de l’observance calculable à partir des remboursements de différents systèmes d’assurance maladie dans le monde. Le but de ce travail était de : - recenser ces indicateurs et les appliquer à la base Erasme ; - proposer de nouveaux indicateurs ; - catégoriser ces indicateurs. Dans une première partie nous faisons l’état des lieux de la notion et du concept d’observance médicamenteuse et de sa mesure à partir des bases de données de remboursement. Dans une deuxième partie, nous présentons la base Erasme et proposons des nouveaux indicateurs. Afin d’illustrer nos propos, nous présentons les études que nous avons réalisé à partir de l’étude de médicaments ayant des profils de consommation différents : traitement au long cours d’une affection symptomatique (asthme), traitement au long cours d’une affection asymptomatique (hypercholestérolémie), traitement au long cours d’une affection grave (diabète), traitement « minute » (infection urinaire) et un traitement de durée moyenne (contraception orale). / The efficacy of a therapeutic response depends largely on good adherence to the prescription. Poor adherence may lead to therapeutic failure and an escalation of treatment. Furthermore, this generates excess cost for the health insurance system. The Erasme reimbursement database of the largest health insurance system in France (régime général de l'assurance maladie, Cnam-TS), represents an important source of information on the use of medicines, and, therefore, possibly adherence. This is particularly the case as several indicators have been proposed to describe adherence from reimbursements in the different health insurance systems around the world. The objective of the current work was to: - identify the indicators and to apply these to the Erasme database; - propose new indicators; - categorise these indicators. The first part introduces the notion and concept of medication adherence and its measurement from reimbursement databases. The second part describes the Erasme database and the propositions for new indicators. This will then be illustrated by the studies that we have performed on medicines that have different profiles of use: long-term treatment of symptomatic disease (asthma), long-term treatment of asymptomatic disease (hypercholesterolaemia), long-term treatment of serious disease (diabetes), short-term treatment (urinary infection), and medium-term treatment (oral contraceptives).

Enhancing Adherence to Prescribed Opioids Using a Mobile-Base Application: A Pilot Study of feasibility in Chronic Non-Cancer PainSop, Daniel M 01 January 2018 (has links)
In this study we present feasibility of a mobile monitoring and reporting system that would provide an accurate unbiased screening tool to systematically analyze opioid adherence in Sickle cell disease patients. In addition, the software simultaneously measures pain. The Mobile Applications Rating Scale: a new and validated tool for assessing the quality of health mobile apps for engagement, functionality, aesthetics, information quality, subjective quality, relevance and overall impact was administered post usage to evaluate the application.
A total of 28 patients were recruited to review and test the software at one sitting. The majority of the population found the application to be relevant for their care. Patients were also asked to report on the completeness of information within the app, the majority (96%) reported on the application’s completeness while 4% estimated the information to be minimal or overwhelming. The quality of information as it pertains to sickle cell patients was overwhelimingly reported to be relevant (91.7%); only 8.3% found the application to be poorly relevant to sickle cell disease. The application’s performance was positively rated while the ease of its use positively rated at 91.7%. Most participants (85.7%) found the application to be interesting to use while 74% found it entertaining. All users found the application’s navigation to be logical and accurate with consistent and intuitive gestural design.
We conclude that surveyed patients believe it is feasible to use a smartphone application specifically targeted to monitor opioid use and behavior in patients with sickle cell disease (SCD)-associated pain

A PRACTICE ELEMENTS-BASED APPROACH TO ADHERENCE MEASUREMENTViolante, Stephanie 01 January 2018 (has links)
The measurement of treatment adherence (i.e., the frequency and thoroughness with which a therapist delivers a treatment as designed) is critical to the field of implementation science, as adherence is often considered an indicator of successful implementation. Most existing instruments of treatment adherence are treatment protocol specific; however, this approach is costly, inefficient, and disallows cross-treatment and cross-study comparisons. Thus, there is a need for adherence instruments that can be used across treatment protocols. It has been suggested that an instrument that captures adherence at the practice element level would meet this need, as it would have utility across protocols that share practice elements. The current study examines the extent to which an observational adherence instrument designed to assess the core common practice elements found in individual cognitive-behavioral therapy (ICBT) for youth anxiety (the Cognitive-Behavioral Therapy Adherence Scale for Youth Anxiety; CBAY-A) can produce scores that can be reliably and validly interpreted across two separate ICBT protocols: Coping Cat (a standard manualized treatment; SMT) and MATCH (a modular manualized treatment; MMT). This study provides initial psychometric data for scores on an SMT subscale (comprised of CBAY-A items found in the SMT protocol) and an MMT subscale (comprised of CBAY-A items found in the MMT protocol) of the CBAY-A. Treatment sessions (n = 359 SMT; n = 243 MMT) from 38 youth participants (n = 22 SMT; n = 16 MMT; M age = 9.84 years, SD = 1.65; 52.6% male, 60.5% Caucasian) in an effectiveness study were independently coded by two coders using the CBAY-A. Inter-rater reliability intraclass correlation coefficients (2,2) for the item scores averaged 0.83 (SD = 0.07) for the SMT group and 0.80 (SD = 0.09) for the MMT group. CBAY-A SMT and CBAY-A MMT subscale scores demonstrated evidence of convergent and discriminant validity via associations with observational instruments of therapist adherence designed specifically for the SMT and MMT protocols, and observational instruments of competence and alliance. Results provide preliminary evidence that the CBAY-A can be effectively used in place of two separate protocol-specific adherence instruments, indicating that it may be a flexible, efficient, and useful tool for capturing adherence to ICBT protocols for youth anxiety in a way that allows for comparisons across treatment protocols and research studies.

The first phase of this research developed an instrument to measure workers’ cybersecurity attitudes. An iterative process resulted in a scale with good psychometric properties - The Cybersecurity Attitudes Scale. The scale measures two factors: cyber policy adherence attitudes and perceived vulnerability to a cyberattack. The second phase of this research used the theory of planned behavior as a theoretical framework to model the relationship between personality facets, policy adherence attitudes, perceived vulnerability, locus of control, cybersecurity climate, and cybersecurity behaviors. While the hypothesized model had poor fit for the data, there was a strong relationship between cybersecurity attitudes (i.e. policy adherence attitudes and perceived vulnerability) and dutifulness, altruism, compliance, cybersecurity climate, and cybersecurity behavior. This research provides practical value to academic researchers and organizations by providing a scale to measure cybersecurity attitudes and to help organizations better understand the nature of the antecedents that lead to cybersecurity attitudes and behavior.
