A Social Work assessment of the emotional needs of HIV/AIDS orphans

HIV/AIDS has assumed epidemic proportions in the world of today. The main age group infected by this disease is between the ages of 20 – 40 years. The result of this is that the children who are not infected by the disease will be orphaned and forced to assume the role of head of the house and breadwinner, at a stage in their lives when they should not be burdened with these responsibilities. Orphans are perhaps the most tragic long-term legacy of the HIV/AIDS pandemic. The stigma attached to HIV/AIDS exacerbates the trauma, hampers the bereavement process and exposes children to discrimination and victimization in their community and their extended family. Investigation into existing literature has revealed that insufficient previous studies have been conducted on HIV/AIDS orphans and even fewer pertaining to the emotional needs of the HIV/AIDS infected population in the South African context. For the purpose of this study it was decided to assess the emotional needs of HIV/AIDS affected orphans in the developmental stage of middle childhood. A literature investigation into the historical background of HIV/AIDS was done and the demographic impact of the disease on South Africa was given. The impact that HIV/AIDS has on the patient was discussed along with the impact that it has on the affected significant others of the patient. Secondly a literature investigation on middle childhood was done. The child in middle childhood as well as the family with children in middle childhood was discussed. Attention was given to the developmental tasks, developmental characteristics and the child’s comprehension of death during middle childhood. Attention was also afforded to the family with children in middle childhood with specific emphasis on the developmental tasks and needs of the family and the developmental tasks of siblings. The aim of the research study was to assess the emotional needs of HIV/AIDS orphans. An exploratory research design by means of a qualitative approach was followed. Five respondents were purposively sampled that complied with the set criteria. They were assessed through the utilization of six different Gestalt therapy techniques over a period of one week. Themes were identified through the empirical study. The research question “What are the emotional needs of HIV/AIDS orphans?” was answered and the following emotional needs of the respondents were identified: · Longing for the deceased mother. · Loneliness. · Stigmatization. · Preoccupation with the physical features of the deceased. · Anger. · Insecurity. · Preoccupation with death. · Fear of death. Cette thèse explore la Première Guerre mondiale comme modèle historique de stress psychologique subi dans l'enfance. L'Hypothèse d'Origine Développementale des Maladies (DOHaD) prévoit une susceptibilité accrue aux maladies chroniques à l'âge adulte des individus exposés à des événements traumatiques aux premiers stades du développement. Nous avons constitué une cohorte d'orphelins nés en 1914-1916, et ce grâce au statut de "pupilles de la Nation", créé par une loi de 1917 et accordé sur requête à tous les orphelins, quel que soit le statut socioéconomique de la famille. L'attribution du statut de pupille était inscrite en marge de l'acte de naissance. Les registres de naissance ont donc permis un recensement exhaustif de tous les pupilles nés dans les villes incluses ainsi qu'un suivi de la mortalité à l'âge adulte. Les actes de naissance de 7,250 pupilles ont été transcrits à ce jour. L'appel à la Base des Morts pour la France a fourni la date de décès du père, et donc sa position dans le calendrier de développement de l'enfant. Des matched non-orphans (MNOs) ont été sélectionnés dans les mêmes registres. Le critère d'intérêt était la longévité de ceux ayant atteint l'âge de 31 ans. Un écart orphelin - MNO de ~ 2.5 années a été trouvé en cas de perte prénatale du père, mais aucune différence n'a été mise en évidence dans le cas d'une perte postnatale. La conjonction de ces deux résultats suggère qu'un traumatisme in utero a un effet de programmation de la susceptibilité biologique à l'âge adulte assez fort pour altérer la longévité. / This thesis explores the First World War as a historical model in early life psychological stress. The Developmental Origins of Health and Disease (DOHaD) hypothesis predicts increased susceptibility to chronic diseases in adulthood for those exposed to an extreme psychological trauma in very early life. We collected vital information on French orphans born 1914-1916 thanks to the “pupille de la Nation” distinction, a legal status created in 1917 and granted upon request to all orphans. Notification of “adoption by the Nation” was by law inscribed on the birth certificate of a newly adopted child. Birth registers thus provided a census of all pupilles born in the included cities during the inclusion period as well as long-term mortality follow-up. The birth certificates of 7,250 pupilles have been digitized. Call to the Died for France Database enabled us to retrieve the paternal date of death. Matched non-orphans (MNOs) were drawn from the same birth registers. For each orphan, his MNO was therefore chosen born in the same district at the same time. The outcome of interest was longevity of those who survived to 31 y. An orphan-MNO difference in adult longevity of ~2.5 years was found for orphans who had lost their father before) birth (prenatal orphans), but no difference in adult longevity could be measured between postnatal orphans and their MNOs. These two results suggest early trauma in utero has programming effects on biological susceptibility in adulthood strong enough to alter longevity. The fact that no loss of lifespan was found in the case of a postnatal loss of father further suggests efficient buffers to early postnatal stress existed in French society.

Ten-year stability and variability, drinking patterns, and impairment in community youth with diagnostic orphan status of alcohol dependence

Objective: Some adolescents and young adults who do not fulfill criteria for DSM-IV alcohol abuse (AA) report symptoms of DSM-IV alcohol dependence (AD) below the diagnostic threshold (diagnostic orphans, DOs; 1 or 2 symptoms). Contemporarily, little is known on the long-term stability, risk of progression to AD, impairment, and drinking patterns possibly associated with this status in the first decades of life. Aim: (1) To identify prevalence rates of the DO status from adolescence to early adulthood. To investigate (2) stability and variability of the DO status over time and (3) associations between DO status, drinking patterns and impairment in comparison to subjects with AA, with AD, or without any symptoms. Method: N = 2039 community subjects (aged 14–24 years at baseline) were assessed at baseline and at about four and ten years after baseline. DSM-IV AUD diagnoses were obtained with the DIA-X/M-CIDI. Results: About 11–12% of the sample was classified as DOs at all waves. Over a period of ten years, 18% of DOs were stable in their diagnosis and additional 10% progressed to AD. DOs were comparable to subjects with AA in drinking patterns, impairment and stability of diagnostic status. DOs progressed to AD significantly more often than AA. AD was associated with highest levels in all outcomes of interest. Conclusions: The DO status in adolescence and early adulthood is associated with considerable stability, risk of progression and problematic alcohol intake. In consequence, it can be meaningful for the timely identification of early stages of clinically relevant alcohol problems. For subjects with DO status early specific interventions are required.

Panua Partners in Hope, Naivasha, Kenya: Legal and Domestic Related Challenges

The complexities of issues existing in Kenya create a domino effect directly affecting not only the effective management and economic growth of Kenya, but also social and legislative constraints to the lower socioeconomic classes. Land dispossession, proof of ownership to property and nationality are ongoing troubling issues affecting increased orphans and vulnerable children (OVC) in Kenya, most noticeably within predominantly poverty stricken rural enclaves. This research focuses on the bold move of stakeholders of Trinity United Methodist Church in Naivasha, Kenya partnering with First United Methodist Church of Winter Park, Orlando, United States of America. In 2010, they conceptualized Panua Partners in Hope, a transformational ministerial assisted non-profit organization program. These churches recently established support to orphans and vulnerable children (OVC) in Naivasha, Kenya. Decisively apply all their available resources to stem the varying issues these orphaned and vulnerable children (OVC) are facing. Whereby, the program is designed to teach self-sufficiency, wealth creation through sustainable income generation and entrepreneurship, and religious relationship development. Continued education is a key component of which Panua mandates. Ongoing (learning) education ensures each child remain in school – (basic, primary and secondary school levels). In addition to ensuring orphans and vulnerable children (OVC) are legally aware of their civil, human and property rights. Conversely, these issues being perpetual worldwide challenges, however, the aim of this research is to specifically address its impactive progression on orphans and vulnerable children (OVC). Thus, outlining their local conditions, the provisions of governmental policies, and assistance, or lack thereof to remedy such current challenges. Additionally, the research will outline reformed constitutional statutes despite being ratified, have not addressed the major issues currently impinging on the freedom and rights of rural communities in Naivasha, Kenya.

The utilisation of the memory box as a medium in gestalt play therapy with AIDS orphans in middle childhood

This study aimed to test the efficacy of the memory box as a medium in Gestalt Therapy, specifically within a therapeutic group with AIDS orphans in the middle childhood phase of development. The aim was to explore and describe the utilisation of the memory box, as a medium for telling one's story, within the safety of the Gestalt play therapy group. It was felt that the increased self-awareness fostered by belonging to a supportive group could enhance self-concept. The research included qualitative and quantitative data. Theoretical and the meta-theoretical assumptions affecting children, in the middle childhood phase, orphaned through HIV/AIDS, were reviewed. The group sessions were described and the data obtained from the pre and post-test was graphically illustrated. The information was then compared to ascertain whether the objectives had been met. The use of business information and management systems to administer concepts such as “virtual adoption” is deemed an important innovative contribution. The James 1:27 Trust and its model is studied as a contributor in finding solutions to scale and multiply levels of care by community and faith-based organisations to orphans and vulnerable children. Addressing behavioural challenges of orphaned learners who head households : a psycho-educational programme to enhance learning

The changing structures of families and the increase in the number of child headed families are progressively becoming noticeable both nationally and internationally. Reviewed literature has confirmed that the increasing trend in orphan status among school going age over the period 2002-2009, is of major concern. A growing body of literature also indicates that child headed families and orphan hood are becoming common phenomena in many African countries in general and South Africa in particular. This scenario presents certain challenges especially on the orphaned children‘s learning and schooling experiences as well as their behavioural and social interaction patterns. The phenomenon calls for greater educational support for such learners at school and home. Increasingly, teachers and schools world over are expected to respond to the changing educational environment and contextual realities of schooling that include the orphaned learners who head households. The aim of this thesis was to address the behavioural challenges that orphaned learners who head household experience and to develop a psycho-educational programme to support their learning. A qualitative instrumental case study research design was used with a study sample of 30 orphaned learners living in child headed household aged 10 to 16 years, ten Life Orientation Teachers, four members of School Management Team, eight members of School Based Support Team (SBST) and four Social Workers who were purposively sampled to participate in the study. The study was conducted in 15 classes located at four schools (3 secondary, 1 primary) in Taledi Area, Mamusa Circuit located in Ipelegeng Township of Schweizer Reneke town under Dr R.S. Ruth Momphati District-North West Province, South Africa. In order to answer the research question, data was collected using a variety of data collection techniques. These were semi structured interviews, observations and documentary analysis. Data obtained was analysed using the interpretational analysis method to determine the common themes and sub themes describing the behavioural challenges that the orphaned learners experience during schooling in the playgrounds and in the classroom. Data analysed revealed that although orphaned learners are socially strong with regard to interaction with their peers, they experience learning difficulties in the classroom leading to underperformance and grade repeating. The study also noted that orphaned learners exhibit negative conduct behaviours and negative social interaction patterns with their LO teachers and members of the school management team. The study also revealed that emotional problems and lack of material support are common phenomena amongst these learners. The study arrived at the recommendation that a psycho-educational programme to address the behavioural challenges be developed to enhance the academic performance of the learners. / Psychology of Education / D. Mission as hospitality : the ministry of the Free Methodist church to orphaned and vulnerable children in Kawama township in Ndola, Zambia

This study focusses on the missiological response of the Pamodzi Free Methodist Church to the plight of orphans and vulnerable children (OVCs), specifically girl children in the Kawama Township in the Republic of Zambia. The major question to answer was “How can the cultural and biblical heritage of hospitality help the church particularly the Pamodzi Free Methodist Church to respond to the problems of OVCs, particularly girl children as victims of urban problems?” The study discovered that forces such as poverty, HIV/AIDS, child abuse, family breakdown, cultural beliefs and practices as well as a lack of care from the extended family have contributed to the vulnerability of OVCs. It recommends that the members of God’s household should have a mandate to restore the dignity and identity of OVCs in a holistic manner by linking the biblical and traditional values of hospitality as well as to develop intervention measures in order to meet the felt needs of OVCs. / Christian Spirituality, Church History & Missiology / M.
