Assessment of hip fracture risk using cross-section strain energy determined from QCT-based finite element model

Accurate assessment of hip fracture risk is very important to prevent hip fracture and to monitor the effect of a treatment. A subject-specific QCT-based finite element model was constructed to assess hip fracture risk at the critical locations of femur during the single-leg stance and the sideways fall. The aim of this study was to improve the prediction of hip fracture risk by introducing a more proper failure criterion to more accurately describe bone failure mechanism. Hip fracture risk index was defined using the strain energy criterion, which is able to integrally consider information such as stresses, strains and material properties in bone failure. It was found that the femoral neck and the intertrochanteric region have higher fracture risk than other part of the femur, probably owing to the larger content of cancellous bone in these regions. The study results also suggested that women are more prone to hip fracture than men. The effects of different parameters such as age, body height, weight, and BMI on hip fracture risk were also investigated in this study. The findings in this study have a good agreement with those clinical observations reported in the literature. The main contributions from this study include: (1) introducing an algorithm for hip fracture risk assessment at the critical locations of femur using the strain energy criterion and QCT-based finite element modeling, (2) theoretically more reasonable definition of hip fracture risk index based on the strain energy criterion, and (3) a semi-automatic finite element analysis and automatic calculation of hip fracture risk index at the critical locations of femur using in-house developed computer codes. The proposed hip fracture risk index based on the strain energy criterion will be a promising tool for more accurate assessment of hip fracture risk. However, experimental validation should be conducted before its clinical applications. / October 2015

Densidade mineral óssea de mulheres na pós-menopausa em diferentes sítios e avaliação do risco de fratura / Bone mineral density in postmenopausal women in different sites and fracture risk assessment

O rápido envelhecimento da população brasileira cria um contexto de assistência prolongada e específica a morbidades que tendem a ampliar a duração do tratamento, as incapacidades dos indivíduos, os gastos com exames complementares, internações hospitalares e medicação. Dentro desse contexto, a osteoporose, doença intimamente relacionada com o envelhecimento, pode ter um aumento considerável nos próximos anos. Conhecer quem são os indivíduos em risco de desenvolver a doença é fundamental, uma vez que a fratura, sua mais importante conseqüência clínica, representa gastos elevados com serviços de saúde e está associada à alta taxa de morbidade e mortalidade. O exame indicado pela Organização Mundial da Saúde (OMS) como padrão-ouro para o diagnóstico da osteoporose é o exame de densitometria óssea (DXA), Devidos aos custos e acesso restrito e assim, selecionar candidatos ao exame é uma questão com importantes implicações clínicas e sócioeconômicas. O objetivo deste estudo foi avaliar a validade diagnóstica da radiografia panorâmica para identificação de mulheres na pós-menopausa com baixa massa óssea. Foram utilizados: questionário baseado nos fatores clínicos de risco para osteoporose, exame de densitometria óssea (fêmur, coluna e antebraço), radiografia panorâmica digital e o São Paulo Osteoporosis Risk Index (SAPORI). O estudo é do tipo observacional transversal. Os valores de sensibilidade e especificidade, valor preditivo positivo e negativo foram calculados. A amostra foi constituída por 88 mulheres na pós-menopausa com média de idade de 61 anos. A baixa massa óssea no quadril foi observada em 62 mulheres (70,5%), na coluna em 61(69,3%), no antebraço em 78 (88,6%) e 52 (59,1%) na mandíbula. Fratura após os 50 anos de idade foi observada em 17 mulheres (19,3%) e 37 (42%) relataram ocorrência de queda nos últimos 12 meses. Introdução: A tomografia computadorizada periférica quantitativa de alta resolução (HR-pQCT) é um método diagnóstico que avalia microarquitetura óssea e estima resistência óssea por análise de elemento finito, facilitando a compreensão da fisiopatologia das fraturas. Objetivo: Avaliar associação de fraturas vertebrais moderadas e graves com parâmetros obtidos na HR-pQCT, escore trabecular ósseo (TBS) e densidade mineral óssea areal em idosos da comunidade. Métodos: Foram recrutados 276 idosos da coorte SPAH. Dados clínicos foram obtidos a partir de um questionário específico. A fratura vertebral (FV) foi avaliada por DXA-VFA utilizando-se o método semiquantitativo de Genant. HR-pQCT e DXA foram realizados no mesmo dia da coleta de sangue. O modelo de regressão logística foi realizado para verificar quais fatores foram associados de forma independente com a fratura vertebral moderada e grave. Resultados: Na tíbia, mulheres com FV moderada/grave apresentaram menor densidade mineral óssea volumétrica, menor número de trabéculas, menores valores dos parâmetros de resistência óssea e maior separação trabecular; e os homens apresentaram menor número de trabéculas, menor parâmetro de resistência óssea e maior separação trabecular. No rádio, as mulheres com FV moderada/grave tinham menor densidade mineral óssea volumétrica, espessura trabecular e cortical e menor valor do parâmetro de análise do elemento finito; e os homens tinham menor densidade mineral óssea volumétrica trabecular e menor valor de parâmetro de análise do elemento finito. Não foram observadas diferenças no TBS em ambos os sexos. A análise de regressão logística revelou que uma menor densidade mineral óssea volumétrica trabecular na tíbia em mulheres (OR: 0,980, 95% CI: 0,963-0,997, p = 0,022) e uma menor densidade mineral óssea areal do colo femoral e fêmur total em homens (OR: 0,002, IC 95%: 0-0,607, p = 0,033 e OR: 0,003, IC 95%: 0-0,623, p = 0,033) foram independentemente associados com FV moderada/grave. HR-pQCT was performed at radius and tibia. TBS was performed using DXA. Results: At tibia, women with Moderate/severe VF had lower volumetric bone density (vBMD), trabecular number (Tb.N), strength parameters and higher trabecular separation (Tb.Sp); and men had lower Tb.N, strength parameters and higher Tb.Sp. At radius, women with moderate/severe VF had lower v.BMD, trabecular and cortical thickness and strength parameters; and men had lower trabecular v.BMD and strength parameters. No differences were observed in TBS in both genders. Logistic regression analysis revealed that lower trabecular vBMD at tibia in women (OR: 0.980, 95%CI: 0.963-0.997, p = 0.022) and lower femoral neck aBMD and total hip in men (OR: 0.002, 95%CI: 0-0.607, p = 0.033 and OR: 0,003, IC 95%: 0-0,623, p = 0,033) were independently associated with VF. Bone and Aluminium

<p>Osteoporosis is a major health care problem, by reason of its devastating consequences, in particular hip fractures. Worldwide it has been estimated that the incidence of hip fracture will increase to more than 6 million per year by 2050 compared to 1.7 million per year in 1990. Osteoporosis can be caused by various factors namely, genetic, lifestyle and environmental factors, and since the rising incidence of its consequences is not fully explained by the growing age of the population, there is an urgent need to identify individual causal factors of this condition. </p><p>The present research has focused on aluminium, one potential environmental factor of importance for bone disease, and its possible relation to osteoporosis, since it is known to cause osteoporosis-like bone disease and has been associated with induction of progressive central nervous system diseases.</p><p>Aluminium is the third most common element in the earth’s crust and the most abundant metal (8%). It is widely utilized industrially and it is also naturally present in many foods. Although aluminium is ubiquitous in the human environment, evolution has not given it an essential biological function.</p><p>The aluminium content of bone was measured by inductively coupled mass spectrometry in a large group of patients suffering from hip fractures, high energy fractures and osteoarthrosis. An exponential increase in aluminium content of bone with age was found (p=0.0004). However, no significant association of aluminium in bone with occurrence of hip fracture or dementia could be found, and no indirect evidence was obtained, e.g. through bone mineral density or biomechanical properties, that aluminium is involved in the pathogenesis of osteoporosis. Although we accumulate aluminium in bone throughout our lives, and there are experimental suggestions that aluminium induces premature cell death, the body content of this metal does not seem to influence the overall mortality risk. </p>
