Intestinal Transport as a Potential Determinant of Drug Bioavailability

Orally administered drugs are generally absorbed by the small intestine and transported either to the lymphatic system or to the hepatic portal system. In general, lipid soluble drugs and vitamins are transported by the small intestine to the lymphatics, and water-soluble drugs are transported to the hepatic portal system. By avoiding the early hepatic first pass effect, the lymphatic transport system may increase drug bioavailability. In addition to its transport systems, the small intestine may affect drug bioavailability through drug uptake, intestinal first pass effect, recruitment of drugs by chylomicrons, formation and secretion of chylomicrons, and enterohepatic circulation. All of these factors should be considered when formulating orally administered lipophilic drugs. Our data also suggest that Caco-2 cells may serve as a valuable in vitro model to study the intestinal transport of orally administered drugs.

Implementation of an ADHD Electronic Portal in Pediatric Primary Care

Clinical practice guidelines (CPGs) for Attention Deficit Hyperactivity Disorder (ADHD) recommend documentation of symptom scales and comorbidity screenings at the time of diagnosis and routinely throughout care. Rates of documentation for these tools in pediatric primary care are consistently low, creating a gap in care that impairs diagnosis, management, and patient outcomes. The purpose of this quality improvement project was to implement an online ADHD portal that emails, collects and scores these tools. The aims of the project are to improve utilization of CPGs by increasing documentation rates of symptom scales and comorbidity screenings for pediatric patients with ADHD. One primary care pediatrician located in Middle Tennessee piloted the project, with the goal to expand this practice change to additional partners in the future. Following submission to the IRB, the committee determined this type of quality improvement project to not require their oversight. A six-month retrospective chart review was conducted to determine baseline rates of documentation for the two outcomes. During the ten-week project, the portal was activated and evaluation tools were sent. A second chart review will be conducted at the conclusion of the project to determine if utilization of CPGs for the two outcomes has improved. Limitations included small sample size and accessibility barriers. The expected outcomes of this project include successful implementation of the online portal with improved rates of documentation for symptom scales and comorbidity assessments. This project could potentially improve CPG utilization and ultimately impact the quality of care for pediatric patients with ADHD.

THE CONNECTED ELEPHANT IN THE ROOM: A study on adoption factors for the Internet of Things in the smart home environment

Majoriteten av svenskarna (86%) tror att vi kommer se allt fler uppkopplade produkter i våra hem, men det är bara 33% som faktiskt vill ha fler smarta produkter i sina hem. I denna studie har författarna undersökt vilka faktorer som är centrala i svenska konsumenters adoption av IoT-produkten Facebook Portal, för att bidra med ökad förståelse för konsumenters motstånd mot användandet av denna typ av produkter. Utifrån en värdebaserad acceptansmodell (VAM) genomfördes en kvantitativ enkätundersökning där potentiella konsumenters attityd till användandet av Facebook Portal undersöktes utifrån faktorerna upplevd användbarhet (PU), upplevt nöje (PE), upplevd integritetsrisk (PPR) och upplevd påflugenhet (PI), i relation till det uppfattade värdet (PV) av produkten. Studien visade att upplevd användbarhet (PU) och upplevt nöje (PE) har ett starkt positivt samband med potentiella användares upplevda värde (PV) av Facebook Portal samt att upplevd dataintegritetsrisk (PPR) och upplevd påflugenhet (PI) har medelstarka negativa samband med potentiella konsumenters upplevda värde (PV) av produkten. Studien visade också att respondenter som identifierar produkten som användbar (PU) och upplever produkten som rolig (PE) också tenderar att vara mindre medhållande i att produkten utgör ett hot mot integriteten (PPR) eller upplevs påträngande (PI). / The majority of Swedes (86%) believe that smart home devices is a rising trend, yet only 33% actually want more smart objects in their homes. In this study, the authors elucidate the factors central to the Swedish consumers’ adoption of the IoT product Facebook Portal, to contribute to the understanding of their resistance to the use of this type of technology. Through a value based acceptance model (VAM), the authors conducted a quantitative survey, investigating consumers' attitudes toward the use of Facebook Portal through the factors perceived usefulness (PU), perceived enjoyment (PE), perceived privacy risk (PPR) and perceived intrusiveness (PI), in relation to the perceived value (PV) of the product. Perceived usefulness (PU) and perceived enjoyment (PE) showed a strong positive correlation to potential users' perceived value (PV) of Facebook Portal and perceived privacy risk (PPR) and perceived intrusiveness (PI) showed a medium strong negative correlation to potential consumers' perceived value (PV) of the product. The study also showed that respondents who identify the product as useful (PU) and perceive the product as enjoyable (PE) also tend to be less supportive in the claims that the product comes with a privacy risk (PPR) and/or is perceived as intrusive.

This paper describes the development of a proof-of-concept application, which can be used to train people in the field of programmable logic controller (PLC) and robot programming. This concept leads to a better understanding, what kind of infrastructure and software packages can be combined, to not only have a visual representation of an industrial like scenario, but also the possibility to use commonly established programming environments such as TIA Portal, to set up a PLC program. In addi-tion, it provides insights on how OPC UA can be used and is suitable for controlling a typical six-axis industrial robot with the help of a PLC.

Help: defining the usability requirements of a breast cancer long-term survivorship (LTS) navigator

Indiana University-Purdue University Indianapolis (IUPUI) / Long-term survivors (LTSs) of breast cancer are defined as patients who have been in remission for a year or longer. Even after being declared breast-cancer-free, many LTSs have questions that were not answered by clinicians. Although online resources provide some content for LTSs, none, or very little, provide immediate answers to specific questions. Thus, the aim involves proposing specifications for a system, the Health Electronic Learning Platform (HELP), that can assist survivors by becoming an all-inclusive resource for LTSs of breast cancer. To achieve this, relevant information from the literature was used to assess the needs of LTSs. Also, data from a study involving the breast cancer survivor’s forum project that had been filtered to include posts with mentions of features to be added to the website and usability issues encountered. To complete the actual design of the system, a synthesis of the results obtained from these two sources was performed. HELP is simple in terms of its layout and consists of a main search-bar, where LTSs are able to ask questions using their own terms and language. This navigator should not be taken as definitive solution, but instead, should be used as a starting point toward better patient-centered care.

Comparison of load-bearing system in steel / Jämförelse av stålstommar

In single storey industrial buildings, the most common material used for the load-bearingsystem is steel. The load bearing system can be designed in different ways. The two mostcommon load-bearing systems of steel industrial buildings are portal frame and thetruss/pillar-system. The benefit of using these systems is the possibility of having largespan structures. These systems have also a high load capacity in function of its weight.Geometrical parameters like width, height, length, and roof slope of the building effectsthe design and the selection of which load-bearing system will be more suitable.The purpose of this thesis is to compare the two systems in terms of weight of steel, priceand global warming potential (GWP) and to see how the load-bearing systems differ. Tofulfil the purpose, it is necessary to find the geometrical parameters to design an optimalportal frame system and use the same parameters in order to design the truss/pillarsystem. By using the same geometrical parameters, the two systems can be comparable.The results of this thesis show that the truss/pillar system has less weight, lower price andlower global warming potential than the portal frame.As conclusions of this parametrical study, it was seen that the truss/pillar system isperforming better than the portal frame system. In general, it is not possible to say that thetruss/pillar-system is better. This is because this thesis work did not consider amongstithers the bracing system, foundation, connections, mounting or fabrication. But it ispossible to say that the differences between the two systems are not major.

Introduction: The reason for the inability of performing the liver resection for colorectal carcinoma metastasis is usually insufficient remnant liver parenchyma after liver resection (future liver remnant volume - FLRV). The current standard method of increasing FLRV is the embolization of the branch of portal vein (portal vein embolization - PVE) on the side of the tumor, and then suspended after hypertrophy of the non-embolised lobe liver resection. Unfortunately, there are some patients who do not increase liver volume despite perfectly executed PVE. Besides that, FLRV occurs during the time necessary for hypertrophy progression of metastatic disease. Therefore, we are trying to find the appropriate way to encourage the growth of remaining liver parenchyma and accelerate hypertrophy of the contralateral liver lobe. From our previous experience (IGA MZ NS 10240), it is possible to be optimistic that there hope is the way of hematopoietic progenitor cells (HPC - adult stem cells) after previous PVE to non-embolised branches of the portal vein. These cells do not only accelerate liver regeneration, but are also able to improve its function (function of the liver) which is especially important for patients after neoadjuvant chemotherapy (steatohepatitis or steatofibrosis), and for patients with...
