Young, sexually active, senior high school women in the australian Capital Territory: prevalence and risk factors for genital Human papillomavirus infection

An association between persistent Human papillomavirus (HPV) infection in women and cervical cancer has been established. Young women are particularly at risk of acquiring sexually transmitted infections such as HPV because of risky sexual activity and physiological immaturity. While at risk though, young women have been shown to be amenable to health promoting initiatives. There are a small number of international studies concerning adolescent HPV infection and the risk factors associated with infection, but there is currently no evidence on the prevalence and risk factors for HPV in an Australian, sexually active female adolescent population. This study aimed to provide evidence of the prevalence of HPV, risk factors associated with infection and the patterns of sexual activity in a female sexually active, senior high school population in the Australian Capital Territory. Participants in this study were a convenience sample of 161 sexually active 16-19 year old females who had an HPV test who were attending a senior high school in the Australian Capital Territory. Nurses and doctors using a clinical record collected information about sexual and other risk behaviours. Self-obtained vaginal swabs were tested for HPV DNA using the polymerase chain reaction method and genotyping was undertaken. The HPV prevalence in this cohort of young women was 1 1.2%. High-risk genotypes were found in 55.5% and multiple genotypes were found in 38.8%. There was a significant association found between HPV infection and having had more than one male partner with whom vaginal intercourse had occurred in the previous six months. No statistically significant association was found between HPV and the age of coitarche, length of time young women had been sexually active, condom use, and smoking or alcohol intake. A young age at coitarche was common for this group. Smoking and alcohol use was seen in large proportions in this group. This is the first Australian study that has examined the prevalence and risk factors for genital HPV in this demographic group. The HPV prevalence is lower than in international studies in comparable groups, in similar age groups and much lower than in older women both in Australia and overseas. With the comparatively low prevalence comes an opportunity for important public health interventions for this group including routine Pap smears, vaccination against the high-risk types of HPV when this becomes available and strategies for young women to reduce their number of male sexual partners. A substantial amount of young women in this study were sexually active aged under 16 years. Whilst this was not identified as being a risk factor in this study, it is both a health and personal safety issue for these young women. There is a demonstrated need for health promotion strategies for this cohort about the consumption of safe levels of alcohol and for smoking cessation. Further research is recommended that includes a repetition of this study with a larger sample, the use of a prospective study design to identify trends in infection and examination of HPV prevalence and risk factors for a variety of populations.

Analisamos uma coorte de mulheres no sul do Brasil, com objetivo de identificar associações epidemiológicas para persistência e cura da infecção pelo HPV e realizamos uma metanálise para determinar a acurácia da telomerase nas lesões precursoras do câncer cervical. Métodos: O estudo de coorte foi iniciado em fevereiro de 2003. Foram coletados espécimes cervicais para citologia oncótica e para detecção do DNA HPV na entrada do estudo e no seguimento. O desfecho foi dividido em quatro categorias: (1) persistência, (2) conversão (3) cura. A quarta categoria (referência) eram mulheres negativas no início que permaneceram negativas. Foram usados o teste χ2 de Pearson, regressão logística multinomial e Kaplan- Meier para análise estatística. Para a metanálise foram incluídos estudos que comparavam o teste de telomerase (TRAP) e anatomopatológicos, obtidos por biópsias cervicais para diagnóstico de lesões cervicais. Resultados: A Incidência de HPV foi 12,3%. O HPV16 foi o tipo mais encontrado (18,6%), entre as 501 mulheres do estudo.Trinta e quatro mulheres (6,78%) ficaram persistentemente infectadas pelo HPV, estando essa categoria associada à idade da sexarca inferior a 21 anos (OR = 3,14, IC 95%, 1,43-6,87) e a quatro ou mais parceiros durante a vida (OR = 2,48 IC 95%, 1,14-5,41). No período mediano de 19 meses, 80,7 % das mulheres tinham curado o HPV, a cura foi significativamente associado à cor preta (OR= 3,44 IC 95%, 1,55-7,65), co-infecção com C. trachomatis no arrolamento (OR= 3,26, IC 95%, 1,85-5,76) e história de já ter realizado exame de Papanicolaou (OR= 3,48, IC 95%, 1,51- 8,00). Na metanálise dez estudos foram analisados, os quais incluíram 1069 mulheres. Para lesões intraepiteliais de baixo grau (LIEBG) vs. normal ou lesões benignas, encontrou-se uma positividade do teste da telomerase, sendo que o resultado da odds ratio para diagnóstico (DOR) foi de (DOR = 3,2, IC 95%,1,9-5,6). Nas lesões intraepiteliais de alto grau (LIEAG) vs LIEBG, normal ou benigna: (DOR = 5,8, IC 95%, 3.1-10). )]. Encontrou-se uma DORelevada de 8,1 (IC 95%: 3,2-20) nas lesões de câncer cervical vs LIEAG. Da mesma forma, nas lesões de câncer cervical vs. LIEBG, a razão de chance foi elevada, com uma DOR de 40,9 (IC 95%: 18,2-91). Conclusões: A persistência da infecção pelo HPV foi associada com a sexarca precoce e ao número de parceiros sexuais na vida, sugerindo que estratégias de orientação sexual podem modificar as taxas de persistência do HPV. A associação da cura do HPV com história prévia de realização de Papanicolaou salienta a importância de aprimorar os programas de rastreamento de câncer cervical. Futuros estudos da associação de infecções ginecológicas com cura da infecção pelo HPV são necessários. Na metanálise nossos dados suportam a corrente hipótese da atividade da telomerase como um evento precoce na carcinogênese e que poderia estar associado ao início e à progressão de lesões cervicais. / We analysed a cohort of women in Southern Brazil with the aim to identify epidemiological correlates for persistence and clearance of cervical HPV infection. A quantitative systematic review was performed to estimate the accuracy of telomerase assay in cervical lesions. Methods: A cohort study was started on February 2003. Cervical smears were collected to perform Pap cytology and HPV DNA detection at baseline and during the follow up. The outcome was constructed in four categories (1) persistence of HPV DNA; (2) conversion; (3) clearance of HPV. Pearson’s χ2 test, multinomial logistic regression and univariate analysis using the log-rank test were performed. Meta-analysis studies that evaluated the telomerase test (telomerase repeated amplification protocol) for the diagnosis of cervix lesions and compared it to paraffin-embedded sections as the diagnostic standard were included. Results: Incidence of HPV DNA: 12.3%. HPV16 was the most frequent type (18.6%) among 501 women in the study. Thirty-four women were persistently infected with HPV, which was associated with age below 21 years at first intercourse (OR 3.14, 95% CI, 1.43-6.87) and ≥ 4 sexual partners during lifetime (OR 2.48, 95% CI, 1.14-5.41). In a median period of 19 months, 80.7% of women had clearance of HPV, which was associated with black race (OR 3.44, 95% CI, 1.55-7.65), co-infection with C. trachomatis at baseline (OR 3.26, 95% CI, 1.85-5.76) and history of previous Pap smear (OR 3.48, 95% CI, 1.51-8.00). In meta-analysis ten studies were analyzed, which included 1,069 women. The diagnostic odds ratio (DOR) for a positive telomerase test for Lo-SIL vs. normal or benign lesions was 3.2 (95% CI, 1.9-5.6). The DOR for a positive telomerase test for Hi-SIL vs. Lo-SIL, normal or benign lesions was 5.8 (95% CI, 3.1-10). For cervix cancer vs. Hi-SIL, the DOR for a positive telomerase test was 8.1 (95% CI, 3.2-20.3) and for cervix cancer vs. Lo-SIL, normal or benign lesions, it was 40.9 (95% CI, 18.2-91). Conclusions: Persistence of HPV infection wasassociated with early age at first intercourse and number of sexual partners during lifetime, suggesting that strategies for sexual orientation may modify the rates of HPV persistence. The association of HPV clearance with a history of previous Pap smear screening highlights the importance of improving cervical screening programs. Further studies on the association of gynaecological infections with HPV clearance are needed. In meta-analysis our data support the current hypothesis that telomerase may activate an early event in cervical carcinogenesis, that could be associated with the initiation and progression of cervical lesions.

The diploma work concerns the actual problem of papilomavirus infection and the vaccination against its originators. The gravity lies in the human-organism attack and the further proceeding changes in the human cells and mucous membranes. In many cases this leads to the pre-cancer states or even worse, to the cervix carcinoma. The work is divided into two parts, first theory, and second practice. The aim was to evaluate the vaccination against papilomviruses at the female-students of the University firstly, secondly to map the topic knowledge, and thirdly to monitor if the female-students visit their gynaecologist on regular basis. The theory concerns all the epidemiology, virology, clinical and diagnosis aspects of the papilomavirus infections, including the vaccination as a prevention possibility. In practice, there were run two anonymous questionnaire searches, first aimed on the female-students, second on the male-students only. The questions were focused on the level of the topic knowledge and the level of the both sex students vaccination, including the reasons of their non-interest in the possible vaccination, next the frequency of the girls? screenings in the gynaecologist practices. The research proved certain level of the respondents? topic knowledge and compared the existing differences among faculties. Another research conclusion is that the vaccination of the female-respondents is not very high, only 23,75% has already been vaccinated against the virus. Many young ladies underestimate the vaccination, and have more or less serious reasons why not to have them vaccinated. More than half of the respondents don?t do so because of the vaccine high price. This work should serve the women and young ladies, but also the young men as a source of the needed information. This means to improve the virus knowledge itself, its means of transmission, the diagnosis process or the treatment possibilities; overall the ways of primary prevention, with the ways of infection expanding prevention.

Frequência de Chlamydia trachomatis e sua associação com a expressão de p16/Ki-67 em mulheres com lesões intraepiteliais cervicais / Prevalence and Association of Chlamydia trachomatis with the expression of p16/Ki-67 in women with cervical intraepithelial lesions

Objetivos: Verificar a frequência de Chlamydia trachomatis e a sua associação com a expressão de p16/Ki-67 em mulheres com lesões intraepiteliais cervicais. Analisar a associação entre a positividade para Chlamydia trachomatis e variáveis demográficas selecionadas, antecedentes sexuais e obstétricos, resultados anormais citológicos e anatomopatológicos. Também verificar a associação entre a expressão da dupla coloração para p16/Ki-67 com os resultados citológicos e anatomopatológicos. Métodos: Estudo de corte transversal em 1481 mulheres de 18 a 64 anos, participantes de projeto de rastreamento para câncer cervical realizado em São Paulo. As citologias foram coletadas em meio líquido e no líquido residual foi pesquisada a presença do DNA da Chlamydia trachomatis. Nos resultados de citologia anormal foi feita a pesquisa da expressão das proteínas p16/Ki-67. A análise estatística foi realizada usando-se os testes qui-quadrado e da razão de verossimilhanças. Foram estimados os valores de odds ratios (OR) com os respectivos intervalos com 95% de confiança. Resultados: A frequência de detecção da Chlamydia trachomatis foi 15,6%. Não houve associação estatisticamente significativa entre a presença de Chlamydia trachomatis e a expressão de p16/Ki-67 [OR=1,35 (0,5-3,4)]. A idade e o número de parceiros sexuais apresentou associação significativa com a presença de Chlamydia trachomatis [OR= 2,01 (1,1-3,6) e 4,14 (1,7-10,3)]. Não foi encontrada associação significativa entre citologia alterada e a positividade para Chlamydia trachomatis [1,21 (0,46-3,2)]. Não foi observada associação significativa entre os resultados anatomopatológicos e a presença de Chlamydia trachomatis (p = 0,112). A expressão do p16/Ki-67 mostrou associação estatisticamente significativa com lesões intraepiteliais cervicais de alto grau tanto nos resultados citológicos quanto anatomopatológicos. Conclusões: A frequência de infecção por Chlamydia trachomatis na amostra estudada foi de 15,6%. A associação da Chlamydia trachomatis com a dupla coloração para p16/Ki-67 nas citologias anormais não foi significativa, não sendo possível estabelecer uma associação clara entre a presença de Chlamydia trachomatis e a persistência da infecção por HPV oncogênico detectada por este marcador. Dentre as variáveis demográficas pesquisadas, a faixa etária apresentou associação estatisticamente significativa com a presença do DNA da Chlamydia trachomatis; mulheres com idade entre 35 e 45 apresentaram a maior frequência da infecção; entretanto, mesmo as outras faixas etárias mostraram uma alta frequência da presença desse patógeno; foi observada maior frequência da infecção entre as mulheres com mais de 10 parceiros sexuais, quando comparadas com as com menor número de parceiros durante a vida e essa associação foi estatisticamente significativa; não foi demonstrada associação significativa entre os resultados anormais da citologia com a positividade para a presença do DNA da Chlamydia trachomatis. A positividade da dupla coloração para p16/Ki-67 foi significativamente maior nas lesões intraepiteliais cervicais de alto grau. Foi demonstrada associação estatisticamente significativa entre a expressão do p16/Ki-67 com os resultados anatomopatológicos das biopsias dirigidas pela colposcopia / Objective: To verify the frequency of Chlamydia trachomatis and its association with the expression of p16/Ki-67 on women with cervical intraepithelial lesions. To analyze the association between Chlamydia trachomatis presence and the selected demographic variables such as sexual and obstetric history, abnormal cytology and histopathology, as well as analyzing any association between the expression of dual staining for p16/Ki-67 with cytological and histopathological results. Methods: Cross-sectional study on 1481 women with ages between 18 and 64 years, who were enrolled in a screening project for cervical cancer held in São Paulo. The cytology was collected in liquid based medium and the residual liquid was submitted for examination to find the presence of Chlamydia trachomatis DNA. The expression of protein p16/Ki-67 was performed in the abnormal cytology results. Statistical analysis was performed using chi-square tests and likelihood ratio. The values of the odds ratios (OR) with respective intervals of 95% confidence were estimated. Results: The frequency of detection of Chlamydia trachomatis was 15.6%. There was no statistical significant association between the presence of Chlamydia trachomatis and the expression of p16/Ki-67 [OR = 1.35 (0.5 to 3.4)]. Both the age and number of sexual partners presented a significant association in presence of Chlamydia trachomatis [OR = 2.01 (1.1 to 3.6) and 4.14 (1.7 to 10.3)]. There was no significant association between abnormal cytology and positivity for Chlamydia trachomatis [1.21 (0.46 to 3.2)]. No significant association was found between histopathological results and presence of Chlamydia trachomatis (p= 0.112). The expression of p16/Ki-67 showed a significant statistical association with high grade intraepithelial lesions in both cytological and histopathological results. Conclusions: The prevalence of Chlamydia trachomatis infection on the sample studied was 15.6%. The association of Chlamydia trachomatis with dual staining for p16/Ki-67 in abnormal cytology was not significant, therefore, it is not possible to establish a clear association between the presence of Chlamydia trachomatis and the persistence of oncogenic HPV infection detected by this marker. Among the demographic variables, the age range showed statistically significant association with the presence of Chlamydia trachomatis; women aged between 35 - 45 years showed the highest rate of infection, nevertheless the other age ranges showed a high frequency of the presence of this pathogen; It has been observed a higher number of infected women who had more than 10 sexual partners compared to woman who had less partners throughout life and this association was statistically significant; No significant association was found between abnormal cytology with positivity for the presence of Chlamydia trachomatis DNA; There was no significant association between pathological results directed by colposcopy with the positivity of Chlamydia trachomatis; The positivity of the double staining for p16/Ki-67 was significantly higher in the cervical high-grade intraepithelial lesions; Statistical significant association was demonstrated between the expression of p16/Ki-67 with histopathological results of biopsy directed by colposcopy

Objetivos: Verificar a frequência de Chlamydia trachomatis e a sua associação com a expressão de p16/Ki-67 em mulheres com lesões intraepiteliais cervicais. Analisar a associação entre a positividade para Chlamydia trachomatis e variáveis demográficas selecionadas, antecedentes sexuais e obstétricos, resultados anormais citológicos e anatomopatológicos. Também verificar a associação entre a expressão da dupla coloração para p16/Ki-67 com os resultados citológicos e anatomopatológicos. Métodos: Estudo de corte transversal em 1481 mulheres de 18 a 64 anos, participantes de projeto de rastreamento para câncer cervical realizado em São Paulo. As citologias foram coletadas em meio líquido e no líquido residual foi pesquisada a presença do DNA da Chlamydia trachomatis. Nos resultados de citologia anormal foi feita a pesquisa da expressão das proteínas p16/Ki-67. A análise estatística foi realizada usando-se os testes qui-quadrado e da razão de verossimilhanças. Foram estimados os valores de odds ratios (OR) com os respectivos intervalos com 95% de confiança. Resultados: A frequência de detecção da Chlamydia trachomatis foi 15,6%. Não houve associação estatisticamente significativa entre a presença de Chlamydia trachomatis e a expressão de p16/Ki-67 [OR=1,35 (0,5-3,4)]. A idade e o número de parceiros sexuais apresentou associação significativa com a presença de Chlamydia trachomatis [OR= 2,01 (1,1-3,6) e 4,14 (1,7-10,3)]. Não foi encontrada associação significativa entre citologia alterada e a positividade para Chlamydia trachomatis [1,21 (0,46-3,2)]. Não foi observada associação significativa entre os resultados anatomopatológicos e a presença de Chlamydia trachomatis (p = 0,112). A expressão do p16/Ki-67 mostrou associação estatisticamente significativa com lesões intraepiteliais cervicais de alto grau tanto nos resultados citológicos quanto anatomopatológicos. Conclusões: A frequência de infecção por Chlamydia trachomatis na amostra estudada foi de 15,6%. A associação da Chlamydia trachomatis com a dupla coloração para p16/Ki-67 nas citologias anormais não foi significativa, não sendo possível estabelecer uma associação clara entre a presença de Chlamydia trachomatis e a persistência da infecção por HPV oncogênico detectada por este marcador.
