Author: Barbora Říhová Title: Kinematic Analysis of Gait in Patients with Hip Osteoarthrosis Objectives: The aim of this thesis was to analyse the gait patterns in probands with Stage III osteoarthritis of the hip joint, to compare these patterns with published norms and to confirm a set of pre-established hypotheses. Method: The thesis was designed as a case study, the purpose of which was to observe changes in the pelvis and trunk extension during gait, associated with significant degenerative changes in the hip joint. A total of 9 probands with Stage III osteoarthritis of the hip joint (6 females, 3 males) took part in the study. A complete physiotherapy assessment was carried out as part of the study. The temporal gait parameters were used to measure the probands' walking speed and relative stance phase duration. The following variables were measured in the frontal plane: lateral displacement of the pelvis and trunk toward the stance limb (cm) and pelvic obliquity toward the swing limb (degrees). A 3D motion analysis and evaluation of kinematic parameters were carried out using the Qualisys motion capture system device consisting of 6 infrared cameras. A postura bípede reduz a área de apoio e demanda a atuação de um sistema de controle para manter o equilíbrio e a orientação dos segmentos corpóreos. A avaliação postural é muito importante nas intervenções fisioterapêuticas e a busca de sistemas confiáveis para avaliar a postura tem sido uma constante dentro da área. Nesta linha, a avaliação do equilíbrio sagital dado pela relação entre coluna lombar e pelve e pelo ângulo de incidência pélvica (IPE) tem sido usado para as alterações da coluna vertebral. Objetivo: fazer a mensuração dos parâmetros posturais pelo programa de avaliação postural (SAPO) e do equilíbrio sagital pelo programa Keops e fazer a correlação entre eles. Metodologia: foram avaliados 80 indivíduos saudáveis de ambos os sexos com idade entre 20 e 35 anos. Foi feita a biofotogrametria (marcação dos pontos anatômicos do indivíduo e fotografias com os marcadores) do programa SAPO. Foram utilizadas as variáveis obtidas nas vistas lateral esquerda e direita. As medidas dos parâmetros do equilíbrio sagital (ângulo de incidência pélvica é o mais relevante) foram obtidas pela análise de uma radiografia panorâmica em perfil da coluna vertebral, na qual se fazia a marcação digital de pontos anatômicos de referência. O cálculo dos ângulos era feito de forma automática pelo programa Keops. Resultados: pela avaliação do programa Keops: 17,5% dos indivíduos tinham ângulos de incidência pélvica altos ( > 60o), 51,2% tinham ângulos incidência pélvica médios (46-59o) e 31,5% tinham ângulos de incidência pélvica baixos ( < 45o). O SAPO mostrou 12,5% de lordose, 40% de retificação e 47,5% com curvatura normal. Na vista lateral direita, o ângulo de incidência pélvica teve correlação moderada e positiva com o alinhamento vertical do tronco e com o alinhamento vertical do corpo e negativa e moderada com o alinhamento horizontal da pelve. Na vista lateral esquerda o ângulo de incidência pélvica teve correlação fraca e negativa com o alinhamento horizontal da pelve. Estudo quantitativo das fibras nervosas do periósteo acetabular em cães / A quantitative study of actabular periosteum nerve fibers in dogs

A displasia coxo-femoral tem sido descrita como moléstia de origem genética, caracterizada por incongruência articular, com alterações dos tecidos conectivos da articulação. Podendo afetar qualquer raça, é particularmente prevalente em raças grandes e gigantes e está apontada como a principal causa do desenvolvimento de osteoartrite em cães. Muitas técnicas são apontadas para o tratamento destes quadros, e novas pesquisas têm sido desenvolvidas no intuito de se obter técnicas mais eficazes e menos cruentas, diminuindo também o tempo de recuperação. Destas, a denervação da articulação coxo-femoral surge como técnica simples e eficaz que recupera a atividade funcional dos membros afetados em tempo significativamente menor em relação às demais. Esta técnica consiste em remoção do periósteo acetabular, eliminando as fibras nervosas sensitivas da região com conseqüente promoção da analgesia. Este estudo visou quantificar as fibras nervosas presentes na região descrita, relacionando a densidade encontrada com a técnica cirúrgica, na intenção de torná-la mais precisa e eficaz. Foram utilizados 30 acetábulos, provenientes de 15 cães, machos e fêmeas, com peso variando entre 22,0 Kg e 62,0 Kg. A dissecção macroscópica revelou simetria bilateral com variação individual da enervação regional, com ramos originados dos nervos glúteo cranial, isquiático e femoral. A enervação do periósteo acetabular foi estudada por método de impregnação pela prata e os resultados obtidos apontam uma diferença estatística significativa entre a densidade (fibras/mm2) da região crânio-lateral em relação à região caudo-lateral do acetábulo. Demonstraram ainda haver simetria entre os antímeros estudados. As fibras nervosas apresentam disposição quase vertical no periósteo acetabular, em direção à cápsual articular, o que sugere a densidade nervosa também presente na região. / Hip dysplasia has been described as a genetic disease, caracterized by articular incongruence with alterations in the articular connective tissues. It may affect all breeds, prevailing in large and very large ones and it is supposedly the main cause in the development of osteoarthrites in dogs. Many techniques are indicated for the treatment of this disease, and new researches are being developed in the meaning of obtaining more efficient and less traumatic techniques, keeping the post operative time as short as possible. Among these, the denervation of the hip joint capsule comes as a simple and effective technique, that recovers the functional activity of the affected limbs in significantly less time then the others. This surgical procedure consists in removing the acetabular periosteum, eliminating the nervous fibers with consequent analgesy This study tried to quantify the nervous fibers present in the mentioned region, relating the density found with the surgical technique, intending to make it more efficient. Thirty acetabular fragments, from 15 dogs, both male and female, weighting between 22,0 and 62,0 Kg, were used. Macroscopic dissection showed bilateral symmetry, with individual variation of regional innervation, and branches from the cranial gluteal, ischiaticus and femoral nerves. The acetabular periosteal innervation was studied by silver staining and the results showed a significatn difference between the nervous fiber density (fibers/mm2) of the cranial lateral portion and caudal lateral portions of the acetabulum. They also showed both antimeres are symmetric. Nervous fibers are positioned almost in a vertical position in the acetabular periosteum, running directed to the joint capsule, suggesting nervous density also in that region.

Secular Change of the Modern Human Bony Pelvis: Examining Morphology in the United States using Metrics and Geometric Morphometry

The human bony pelvis has evolved into its current form through competing selective forces. Bipedalism and parturition of large headed babies resulted in a form that is a complex compromise. While the morphology of the human pelvis has been extensively studied, the changes that have occurred since the adoption of the modern form, the secular changes that continue to alter the size and shape of the pelvis, have not received nearly as much attention. This research aims to examine the changes that have altered the morphology of the human bony pelvic girdle of individuals in the United States born between 1840 and1981. Secular changes in the human skeleton have been documented. Improvements in nutrition, decreased disease load, exogamy, activity, climate, and other factors have led to unprecedented growth in stature and weight. The size and shape of the pelvic canal, os coxa, and bi-iliac breadth were all examined in this study. Coordinate data from males and females, blacks and whites were digitized. Calculated inter-landmark data was analyzed using traditional metric methods and the coordinate data was analyzed using 3D geometric morphometrics. After separating the samples into cohorts by sex and ancestry, results indicate that there is secular change occurring in the modern human bony pelvis. Changes in shape are significant across the groups while only white males exhibit increases in size. The dimensions of the pelvic canal have changed over time. The birth canal is becoming more rounded with the inlet anteroposterior diameter and the outlet transverse diameter becoming longer. These diameters, once limiters, are believed to have led to an adoption of the rotational birth method practiced by modern humans. In addition, the bowl of the pelvis is becoming less flared. Childhood improvements in nutrition and decreases in strenuous activity may be the cause of the dimension changes in the bony pelvis. The similar changes across both sexes and ancestries indicate a similar environmental cause. However, it is likely a combination of factors that are difficult to tease apart. Whether the increases continue remains to be determined.

This project presented a biomechanical model of the lumbosacral spine and pelvis, including novel methodologies associated with the measurement of human mechanics. This research has, for the first time, produced accurate three-dimensional geometric models of the human skeleton from living subjects using magnetic resonance imaging technology, enabling the prediction of physiological muscle action within individuals. The model was used to examine changes in the mechanics of the lumbopelvic musculoskeletal system between the standing and seated postures due to the increasing prevalence of the seated posture in the work and home environment. The outcomes of this research included a novel bone wrapping algorithm used to describe the effect of muscle-bone interactions. a novel method for creating three-dimensional in vivo spinal reconstructions using MRI, three dimensional in vivo helical axis measurements and subject specific normalised moment data.

Title: Kinematic of cervical thoracic spine in the context of whole body posture Problematics: This work describes the kinematics of upper thoracic spine during specific movement of the shoulder girdle. The kinematic changes are evaluated with the relationship of the whole body posture. Aim: To develop the objective evaluation of the posture and the amount of the segmental spinal movement of cervical thoracic spine. To describe the relationship between the specific movement of arm - adduction and external humeral rotation - and the posture using 3D kinematic analysis. To objectify this tested movement as a diagnostic tool or a therapy for forward head posture. Methods: The theoretical part of this work summarizes the current knowledge of poor posture and there is defined the upright posture. The experimental part describes the measurement of the movement of shoulder girdle and its response to the axial system using 3D kinematic analysis. Result: The results of kinematic 3D motion analysis show that the external humeral rotation with adduction of the shoulder leads to the upright posture if there is the horizontal position of the thorax and pelvis.
