Investigação das mutações responsáveis pela doença de acúmulo de glicogênio tipo II e pela miastenia hereditária em bovinos da raça Brahman no Brasil

Nenhum estudo avaliou previamente a prevalência de mutações responsáveis pelo GSD II (E7, c.1057_1058delTA e E13, c.1783C> T) ou CMS (CHRNE, c.470del20) nos bovinos Brahman brasileiro. O objetivo deste estudo foi investigar a presença dessas mutações em 276 amostras de bulbos pilosos de bovinos PO brasileiros e em 35 amostras de sêmen de touros da raça Brahman, rotineiramente utilizadas em programas de melhoramento genético no Brasil. Dos 276 bovinos Brahman testados, 7,3% foram identificados como heterozigotos para E7. Enquanto, todos os bovinos Brahman estudados eram wild-type para E13. Para as amostras de sêmen foi identificado 8,6% (3/35) heterozigotos para a E7 e para a E13 nenhum animal foi identificado. A mutação CHRNE, 0,73% das amostras de bulbo piloso são heterozigotos, enquanto para as amostras de sêmen, nenhum animal foi considerado heterozigoto. Este resultado indica que as mutações E7 e CHRNE estão presentes no rebanho Brahman brasileiro, e medidas de controle devem ser adotadas para evitar um aumento na incidência de GSD-II e CMS no gado Brahman no Brasil. / Abstract: Glycogen storage disease type II (GSD-II) and congenital myasthenic syndrome (CMS) are important autosomal recessive disorders in Brahman cattle. No study has previously evaluated the prevalence of mutations responsible for GSD II (E7, c.1057_1058delTA; and E13, c.1783C>T) or CMS (CHRNE, c.470del20) in Brazilian Brahman cattle. The objective of this study was to investigate the presence of these mutations in 276 hair roots from purebred Brazilian Brahman cattle and in 35 semen samples from purebred Brahman bulls that were routinely used in breeding programmes in Brazil. Of the 276 Brahman cattle tested, 7.3% were identified as heterozygous for E7. All Brahman cattle studied were homozygous for the wild-type E13 allele. The E7 mutations was identified as heterozygous in 8.6% (3/35) of the commercial semen samples, whereas the E13 mutations was not identified. The CHRNE mutation was identified as heterozygous in 0.73% of the hair root samples, but this mutation was not present in any semen sample assessed. Glycovecteurs pour le ciblage thérapeutique d'une maladie rare lysosomale : la maladie de Pompe / Glycovectors for therapeutic targeting of a rare lysosomal diseases : Pompe disease

Sur la cinquantaine de maladies rares lysosomales, seules 8 peuvent être traitées par enzymothérapie substitutive avec plus ou moins d’efficacité. Il y a donc un réel besoin de développer de nouveaux traitements mais aussi de mieux caractériser les causes de ces maladies. Durant cette thèse nous nous sommes intéressés à la maladie de Pompe qui résulte de l’absence ou de la carence en enzyme lysosomale alpha-glucosidase acide (GAA) responsable de la dégradation du glycogène en glucose dans de nombreux tissus. Actuellement seule la forme infantile de cette maladie peut être traitée alors que la forme juvénile/adulte est faiblement améliorée par le traitement Myozyme®. Cette thèse a eu pour but de développer une nouvelle enzymothérapie qui, à terme, permettrait d’empêcher la progression de la maladie et de soigner, de manière satisfaisante, les formes juvéniles et adultes de la maladie de Pompe. Dans ce but, nous avons utilisé des dérivés monosaccharidiques « Analogues du Mannose-6-phosphate (M6P) Fonctionnalisés sur l’Aglycone (AMFA) », qui sont ensuite greffés sur la GAA recombinante humaine (rhGAA) afin d’améliorer son adressage aux lysosomes obtenant la rhGAA-AMFA.Une première étude in vitro, sur des fibroblastes de patients atteints de la forme adulte de la maladie, a démontré que le greffage des AMFA sur la rhGAA produite en cellules d’insectes Sf9 améliorait significativement l’affinité pour le récepteur du M6P (RM6P), l’internalisation et l’activité de l’enzyme et lui conférait une efficacité sur les souris GAA-/-, modèles de la maladie de Pompe, par rapport au traitement actuel (Article 1). Puis nous avons pu démontrer, pour la première fois, l’efficacité de la rhGAA-AMFA produite en cellules CHO sur des souris GAA-/- âgées. Ces résultats suggèrent, ainsi, la possibilité d’utiliser cette néo-enzyme dans le traitement de la forme adulte de la maladie (Article2). Enfin, le greffage des AMFA permet d’obtenir une maturation intracellulaire complète de la rhGAA sous forme active dans des myoblastes et myotubes de patients adultes et dans les quadriceps de souris âgées modèles Pompe, ce qui n’a pas été observé pour Myozyme® (Article 3). Lors de cette thèse, nous avons également démontré que de nouveaux analogues disaccharidiques, ayant une meilleure affinité que les monosaccharides pour le RM6P, peuvent efficacement cibler la rhGAA pour le traitement de la maladie de Pompe. Un brevet a été déposé sur ces résultats (Brevet PCT/FR2016/052339).En conclusion, ce travail a permis de développer une nouvelle technologie de ciblage plus efficace des enzymes lysosomales par des analogues synthétiques. La désignation de médicament orphelin pour l’alpha glucosidase acide conjuguée aux analogues du mannose-6-phosphate a été obtenue suite à ces travaux auprès de l’Agence Européenne du Médicament pour le traitement de la maladie de Pompe (EMA/OD/098/16).Mots clés: maladies lysosomales, maladie de Pompe, enzymothérapie, récepteur du mannose 6-phosphate / On 53 known rare lysosomal diseases, only 8 can be treated by enzyme replacement therapy with more or less efficiency. There is therefore a need to develop new treatments but also to better characterize these diseases. During this thesis, we focused on Pompe disease which results from the absence or deficiency of the lysosomal enzyme alpha-glucosidase acid (GAA), responsible for the degradation of glycogen in glucose in many tissues. Currently only the infantile form of this disease can be treated while the juvenile/adult form is slightly improved by Myozyme® treatment. This thesis aimed to devel a new enzyme replacement therapy which could prevent the progression of the disease and satisfactorily treat the late onset form of the disease. To do that, we used monosaccharide derivatives “Mannose-6-phosphate analogues (M6P) Functionalized on Aglycone (AMFA)”, which were grafted onto human recombinant GAA (rhGAA) in order to improve its lysosome addressing obtaining the rhGAA-AMFA.A first in vitro study on adult patient fibroblasts showed that the addition of AMFA to rhGAA, produced in Sf9 insect cells, significantly improved its affinity for the M6P receptor (RM6P), its internalization and activity. It was also more efficient on the GAA-/- Pompe mouse model compared to current treatment (Article 1). Then, we demonstrated for the first time the efficiency of rhGAA-AMFA produced in CHO cells in aged mice model. These results suggest the possibility to use this neo-enzyme in the treatment of the adult form that still resists to treatment (Article 2). Finally, the addition of AMFA allows a complete maturation of rhGAA into its active form in myoblasts and myotubes of adult patients and in the quadriceps of aged mice Pompe model. This was not observed for Myozyme® (Article 3). In this thesis we have also demonstrated that novel disaccharide analogues with a better affinity than monosaccharides for RM6P can efficiently target GAA for the treatment of Pompe disease. A patent has been filed on these results (Patent PCT / FR2016 / 052339).In conclusion, this work has led to the development of a new technology more efficient in targeting lysosomal enzymes by mean of new synthetic analogues. An orphan drug designation for the recombinant human acid alpha-glucosidase conjugated with mannose-6-phosphate analogues was obtained on the basis of this work at the European Medicines Agency for the treatment of Pompe disease (EMA/OD/098/16).Key words: lysosomal diseases, Pompe disease, enzyme replacement therapy, mannose 6-phosphate receptor

The effects of laforin, malin, Stbd1, and Ptg deficiencies on heart glycogen levels in Pompe disease mouse models

Indiana University-Purdue University Indianapolis (IUPUI) / Pompe disease (PD) is a rare metabolic myopathy characterized by loss of acid alpha-glucosidase (GAA), the enzyme responsible for breaking down glycogen to glucose within the lysosomes. PD cells accumulate massive quantities of glycogen within their lysosomes, and as such, PD is classified as a “lysosomal storage disease” (LSD). GAA-deficient cells also exhibit accumulation of autophagic debris. Symptoms of severe infantile PD include extreme muscle weakness, hypotonia, and hypertrophic cardiomyopathy, resulting in death before one year of age. Certain LSDs are currently being successfully treated with enzyme replacement therapy (ERT), which involves intravenous infusion of a recombinant enzyme to counteract the endogenous deficiency. ERT has been less successful in PD, however, due to ineffective delivery of the recombinant enzyme. Alternatively, specific genes deletion may reduce lysosomal glycogen load, and could thus be targeted in PD therapy development. Absence of malin (EPM2B) or laforin (EPM2A) has been proposed to impair autophagy, which could reduce lysosomal glycogen levels. Additionally, deficiency of Stbd1 has been postulated to disable lysosomal glycogen import. Furthermore, Ptg deficiency was previously reported to abrogate Lafora body formation and correct neurological abnormalities in Lafora disease mouse models and could have similar effects on PD pathologies. The goal of this study was to characterize the effects of homozygous disruption of Epm2a, Epm2b, Stbd1, and Ptg loci on total glycogen levels in PD mouse model heart tissue, as in severe infantile PD, it is accumulation of glycogen in the heart that results in fatal hypertrophic cardiomyopathy. Gaa-/- mice were intercrossed with Epm2a-/-, Epm2b-/-, Stbd1-/-, and Ptg-/- mice to generate wildtype (WT), single knockout, and double knockout mice. The results indicated that Gaa-/- hearts accumulated up to 100-fold more glycogen than the WT. These mice also displayed cardiac hypertrophy. However, deficiency of Epm2a, Epm2b, Stbd1, or PTG in the Gaa-/- background did not reveal changes of statistical significance in either heart glycogen or cardiac hypertrophy. Nevertheless, since total glycogen was measured, these deficiencies should not be discarded in future discussions of PD therapy, as increasing sample sizes and/or distinguishing cytosolic from lysosomal glycogen content may yet reveal differences of greater significance.

Avaliação do uso de amostras de leucócitos impregnados em papel filtro para o diagnóstico de doenças lisossômicas

Introdução: As Doenças lisossômicas (DLs) são condições genéticas, herdadas na sua maioria de forma autossômica recessiva, caracterizadas usualmente pela deficiência de enzimas lisossômicas específicas, envolvidas na síntese, degradação, armazenamento ou transporte de macromoléculas necessárias para o funcionamento normal do organismo. Nas situações mais típicas, o substrato não degradado acumula-se progressivamente nos lisossomos, com repercussões estruturais e funcionais, levando a sinais e sintomas característicos. Os pacientes apresentam um amplo espectro de manifestações clínicas, que podem incluir disfunção de órgãos, anormalidades esqueléticas, envolvimento neuronal, entre outras. O diagnóstico é usualmente obtido pela identificação da deficiência enzimática específica em leucócitos obtidos do sangue periférico, usualmente realizado em laboratórios de referência. O transporte da amostra pode ser um obstáculo quando o serviço requisitante está situado longe do centro de referência ou em outro país, situação em que a amostra de sangue pode chegar ao laboratório já sem condições de ser analisada. Objetivo: Este estudo teve como objetivo principal, tornar disponível um método mais simples, seguro e acessível que utiliza amostras de leucócitos impregnados em papel filtro (LIPF) como uma nova ferramenta para o diagnóstico bioquímico de pacientes com DLs. Métodos: O estudo envolveu amostras de pacientes com diagnóstico previamente confirmado de DLs (amostra de conveniência, por se tratarem de doenças raras, com incidências individuais ao redor de 1:100.000 recém-nascidos vivos). Foram incluídos no estudo os pacientes com diagnóstico já estabelecido de DLs selecionadas (MPS IVA, Doença de Krabbe, Doença de Gaucher e Doença de Pompe), independente do sexo e/ou idade, atendidos no Serviço de Genética Médica do HCPA, que concordaram em participar do estudo. O grupo de referência negativo foi constituído pelas amostras de 50 indivíduos hígidos, adultos, de ambos os sexos. Resultados: Os resultados obtidos nos ensaios enzimáticos de pacientes com MPS IVA, Doença de Krabbe, Doença de Gaucher e Doença de Pompe, indicaram que as enzimas analisadas em amostras de LIPF permitiram a identificação de todos os pacientes, com sensibilidade de 100%. Os testes de estabilidade realizados nas amostras de LIPF indicaram que as amostras, quando mantidas a 4ºC, se mostram estáveis por pelo menos 30 dias. Conclusões: Nas condições utilizadas, amostras de LIPF se mostraram adequadas para a identificação segura de pacientes com MPS tipo IVA, Doença de Krabbe, Doença de Gaucher e Doença de Pompe. As amostras de leucócitos secos em papel filtro são mais estáveis e seguras para o transporte, indicando que possa ser esta uma importante ferramenta para facilitar a identificação de pacientes com DLDs, especialmente daqueles que vivem em áreas que tem dificuldades para a remessa de amostras líquidas para serviços de referência. / Background: Lysosomal Disorders (LDs) are genetic conditions, mostly inherited in autosomal recessive fashion, usually characterized by a deficiency of specific lysosomal enzymes involved in the synthesis, degradation, storage or transportation of macromolecules necessary for normal functioning of the organism. Typically, the non-degraded substrate is progressively accumulated in lysosomes, with structural and functional repercussions, leading to characteristic signs and symptoms. Affected patients present a wide range of clinical manifestations, which may include organ dysfunction, skeletal anomalies, neuronal involvement, etc. The diagnosis is normally made through identification of the specific enzyme deficiency in white blood cells from a sample of peripheral blood, usually performed in reference laboratories. The transporting of a liquid sample can be a problem when the test orderer is located far from the reference center or in a foreign country, as often the blood sample arrives at the laboratory in poor condition and cannot be properly analyzed. Aim: The main aim of this study was to make available a new technique that is simpler, safer and more accessible, using leukocytes impregnated on filter paper (LIFP) as a new tool for the biochemical diagnosis of patients with LSDs. Methods: This study involved samples of patients with previously confirmed diagnosis of selected LSDs (a convenience sample, as these are rare diseases, with individual incidences around 1:100.000 live newborns). Patients with an established diagnosis of MPS IVA, Krabbe Disease, Gaucher’s disease and Pompe disease regardless of sex and/or age, cared for at the Genetics Service of HCPA and who agreed to participate were included in the study. The negative reference group comprised blood samples from 50 healthy adults of both genders. Results: The results obtained in the enzymatic assays of patients with MPS IVA, Krabbe Disease, Gaucher’s disease, and Pompe disease indicated that the analyzed enzymes in LIFP samples allowed the identification of all patients, with sensitivity of 100%. The stability tests performed in LIFP samples indicated that samples, when maintained at 4ºC, were stable for at least 30 days. Conclusions: In the conditions used, LIFP samples were shown to be adequate for a reliable identification of patients with MPS IVA, Krabbe Disease, Gaucher’s disease, and Pompe disease. Blood samples on filter paper are more stable and reliable for transportation, indicating that this may be an important tool to facilitate the identification of patients with LSDs, particularly those living in areas with difficulties for the shipment of liquid samples to reference cervices.

Conclusões: Nas condições utilizadas, amostras de LIPF se mostraram adequadas para a identificação segura de pacientes com MPS tipo IVA, Doença de Krabbe, Doença de Gaucher e Doença de Pompe. As amostras de leucócitos secos em papel filtro são mais estáveis e seguras para o transporte, indicando que possa ser esta uma importante ferramenta para facilitar a identificação de pacientes com DLDs, especialmente daqueles que vivem em áreas que tem dificuldades para a remessa de amostras líquidas para serviços de referência. / Background: Lysosomal Disorders (LDs) are genetic conditions, mostly inherited in autosomal recessive fashion, usually characterized by a deficiency of specific lysosomal enzymes involved in the synthesis, degradation, storage or transportation of macromolecules necessary for normal functioning of the organism. Typically, the non-degraded substrate is progressively accumulated in lysosomes, with structural and functional repercussions, leading to characteristic signs and symptoms. Affected patients present a wide range of clinical manifestations, which may include organ dysfunction, skeletal anomalies, neuronal involvement, etc. The diagnosis is normally made through identification of the specific enzyme deficiency in white blood cells from a sample of peripheral blood, usually performed in reference laboratories. The transporting of a liquid sample can be a problem when the test orderer is located far from the reference center or in a foreign country, as often the blood sample arrives at the laboratory in poor condition and cannot be properly analyzed. Aim: The main aim of this study was to make available a new technique that is simpler, safer and more accessible, using leukocytes impregnated on filter paper (LIFP) as a new tool for the biochemical diagnosis of patients with LSDs. Methods: This study involved samples of patients with previously confirmed diagnosis of selected LSDs (a convenience sample, as these are rare diseases, with individual incidences around 1:100.000 live newborns). Patients with an established diagnosis of MPS IVA, Krabbe Disease, Gaucher’s disease and Pompe disease regardless of sex and/or age, cared for at the Genetics Service of HCPA and who agreed to participate were included in the study. The negative reference group comprised blood samples from 50 healthy adults of both genders. Results: The results obtained in the enzymatic assays of patients with MPS IVA, Krabbe Disease, Gaucher’s disease, and Pompe disease indicated that the analyzed enzymes in LIFP samples allowed the identification of all patients, with sensitivity of 100%. The stability tests performed in LIFP samples indicated that samples, when maintained at 4ºC, were stable for at least 30 days. Conclusions: In the conditions used, LIFP samples were shown to be adequate for a reliable identification of patients with MPS IVA, Krabbe Disease, Gaucher’s disease, and Pompe disease. Blood samples on filter paper are more stable and reliable for transportation, indicating that this may be an important tool to facilitate the identification of patients with LSDs, particularly those living in areas with difficulties for the shipment of liquid samples to reference cervices.

Avaliação do uso de amostras de leucócitos impregnados em papel filtro para o diagnóstico de doenças lisossômicas

Introdução: As Doenças lisossômicas (DLs) são condições genéticas, herdadas na sua maioria de forma autossômica recessiva, caracterizadas usualmente pela deficiência de enzimas lisossômicas específicas, envolvidas na síntese, degradação, armazenamento ou transporte de macromoléculas necessárias para o funcionamento normal do organismo. Nas situações mais típicas, o substrato não degradado acumula-se progressivamente nos lisossomos, com repercussões estruturais e funcionais, levando a sinais e sintomas característicos. Os pacientes apresentam um amplo espectro de manifestações clínicas, que podem incluir disfunção de órgãos, anormalidades esqueléticas, envolvimento neuronal, entre outras. O diagnóstico é usualmente obtido pela identificação da deficiência enzimática específica em leucócitos obtidos do sangue periférico, usualmente realizado em laboratórios de referência. O transporte da amostra pode ser um obstáculo quando o serviço requisitante está situado longe do centro de referência ou em outro país, situação em que a amostra de sangue pode chegar ao laboratório já sem condições de ser analisada. Objetivo: Este estudo teve como objetivo principal, tornar disponível um método mais simples, seguro e acessível que utiliza amostras de leucócitos impregnados em papel filtro (LIPF) como uma nova ferramenta para o diagnóstico bioquímico de pacientes com DLs. Métodos: O estudo envolveu amostras de pacientes com diagnóstico previamente confirmado de DLs (amostra de conveniência, por se tratarem de doenças raras, com incidências individuais ao redor de 1:100.000 recém-nascidos vivos). Foram incluídos no estudo os pacientes com diagnóstico já estabelecido de DLs selecionadas (MPS IVA, Doença de Krabbe, Doença de Gaucher e Doença de Pompe), independente do sexo e/ou idade, atendidos no Serviço de Genética Médica do HCPA, que concordaram em participar do estudo. O grupo de referência negativo foi constituído pelas amostras de 50 indivíduos hígidos, adultos, de ambos os sexos. Resultados: Os resultados obtidos nos ensaios enzimáticos de pacientes com MPS IVA, Doença de Krabbe, Doença de Gaucher e Doença de Pompe, indicaram que as enzimas analisadas em amostras de LIPF permitiram a identificação de todos os pacientes, com sensibilidade de 100%. Os testes de estabilidade realizados nas amostras de LIPF indicaram que as amostras, quando mantidas a 4ºC, se mostram estáveis por pelo menos 30 dias.
