Cultural dynamics of African management practice

This research study looked at the cultural value preferences in Western management practice for African manager and non-manager employees exemplified by Nigerian cement industries. The study specifically focused on management practice of leadership, motivation, recruitment and promotion around which their cultural values, the meaning of their work-world and their coping strategies are structured. From management and culture theory perspectives, managerial practices are affected both by Western factors, such as education, money, challenging tasks, and by traditional factors, such as family, ethnicity, social connections etc. The theoretical bases for this study drew largely from three streams of literature. The first theoretical base for the study relates to traditional African environment of management, especially the cultural perspectives. The second drew largely from the theoretical discourse on culture, management and organisation perspectives. The mainstream schools of management discourse on management theories and models as proposed by Western management theorists represent the third stream. As a methodology, the study used a quantitative questionnaire survey and qualitative open-ended interviews to collect data on the manager and non-manager employees in the organisations. The quantitative questionnaires and open-ended interviews centered national dimensions of cultures and on these Western and traditional factors of: leadership styles, motivation, dedication, satisfaction, ethnicity, family and social connections. The survey confirms that the dimension of national cultures of Nigeria as measured by the work-values and desires of the employees population are somehow different from those obtained by Hofstede’s study for the West African Region. Nigeria is still more collectivistic, although at least Nigeria has become relatively more individualist since Hofstede’s study. Over the years between Hofstede’ IBM study and the present study, there has been no change in the difference in Power Distance. Power Distance is much higher in Nigeria, like elsewhere in Africa, and this is unlikely to change for the foreseeable future. The large Power Distance in Nigeria means that the ideal manager is benevolent paternalistic. On recruitment and promotion, one major point made is that the traditional factors are generally felt by the respondents as influencing employees’ recruitment and promotion more than the modern (intrinsic) factors. The employees however, generally felt that the modern (intrinsic) factors should or ought to have greater influence. Building on the premises that every society is unique and its trajectory is shaped by its unique historical events, cultural norms and values, it can be argued that since the history of Western management concept in Africa is short, Africa then has a unique opportunity to develop its own unique management values based on its unique traditions. However, the increasing globalisation of market economies suggests that management values in Nigeria in particular and Africa in general can hardly be realised without proactively contributing to the Western management concepts in its unique ways. As the intensity of interaction between Western management practices and African traditional values increases, we can anticipate the increase in the importance of a new form of management concepts and practices in various African countries. Based on others and this study, the study proposes a “management heterogeneity” concept that reflects this new and unique perspective. Management heterogeneity perspective endorses the view that the practice of leadership, motivation, recruitment and promotion are developed differently in different cultural societies and organisations. But it adopts a pragmatic position on the mounting social and economic challenges now facing African organisations and argues further that management techniques, skills and behaviours practiced in different cultures and organisations can be brought together in a positive synergistic blend to address the needs of a given society and organisation and improve its ability to deliver effective and relevant values to its actors. It is the ability to judiciously select and combine the Western and traditional values and practices into new practices that fit the managerial requirements of a given group of organisational members that provides management its competitive edge in a culturally dynamic management environment.

O método é a pesquisaação, apoiada em encontros denominados journalclub, ou clube de leitura, meio empregado para a coleta de dados com a participação de enfermeiros em posição gerencial. Tem a finalidade de fomentar discussões e sensibilização para pesquisa científica em um processo educativo sobre gerenciamento baseado em evidências. A técnica de grupo focal foi utilizada para apreender, na perspectiva dos participantes, o significado do processo educativo e as possibilidades de transformação da prática gerencial. Os encontros foram gravados e os discursos transcritos e codificados com ajuda do NVivo9 software© QRS International. It all started from the idea that managers of healthcare organizations need to apply management tools which enable the assessment of their organizational processes, thus ensuring that the services rendered to the population have the best possible quality. In this sense, the lean healthcare system is presented as a philosophy, a management method with a set of tools which come from lean production and were introduced in healthcare scenarios as a possible alternative to change the way these organizations are managed. Given this context, nurses are presented as central professionals who seek to articulate management and assistance dimensions in healthcare, which makes maintaining the quality of services rendered dependent basically on remaining close to patients. Bringing the nursing management practice and lean methodology into focus as potential tools for process improvement led to the implementation of such methodology practices and tools in this investigation. This is an applied research, exploratory and the case study type. It is divided into three stages: 1) mapping of processes, 2) modeling of processes, and 3) practical application of lean tools and practices. In order to achieve the proposed objectives and confirm the hypothesis, the theoretical and methodological framework of Lean Healthcare was used in all stages of the research, as well as the supporting methods Business Process Modeler Notation and the Bizagi® software in the second stage and the management improvement tool Plan-Do-Check-Act in the third stage. Data analysis proceeded with a qualitative and quantitative approach. The results of the first two stages indicated that the work management process of nurses Management in Nursing Personnel Schedule should be studied in depth in the third stage. Lean principles guided the analysis of the situation encountered and the proposition of the future situation. Its practices and applied tools led to a reduction of 94% in the amount of time nurses spend on activities that were not worthwhile as well as to reducing waste inherent in their management work process. It was concluded that nurses can act effectively to promote best practices and contribute substantially to the change process in healthcare. The results are promising as they indicate a path to implement lean practices in healthcare, starting with small problems. However, the research suggests that the motivation to seek improvements must be systemic and therefore lean healthcare can be adopted as a management strategy focused on creating value for patients, improving healthcare management and assistance essentially. / A dissertação que aqui se apresenta intitula-se Lean Healthcare: estratégia para a qualificação da gestão em saúde e enfermagem e teve como principal objetivo implementar e avaliar a aplicação dos princípios da metodologia lean para a melhoria do processo de trabalho gerencial do enfermeiro e da qualidade da assistência em saúde. Partiu-se da concepção de que os gestores das organizações de saúde necessitam aplicar ferramentas gerenciais que possibilitem a avaliação dos seus processos organizacionais, garantindo que os serviços prestados à população tenham a melhor qualidade possível. Nessa direção, o sistema lean healthcare é apresentado enquanto uma filosofia, um método de gestão com um conjunto de ferramentas provenientes da produção enxuta e introduzidas nos cenários da saúde como um possível caminho para mudar a forma com que essas organizações são gerenciadas. Diante deste contexto, o enfermeiro é apresentado como um profissional central, que busca articular as dimensões gerencial e assistencial do cuidado, o que faz com que a manutenção da qualidade do serviço prestado dependa basicamente da sua permanência junto ao paciente. Focalizando a prática gerencial do enfermeiro e a metodologia lean como ferramenta potencial para a melhoria de processo é que se procurou implementar práticas e ferramentas dessa metodologia nesta investigação. Trata-se de uma pesquisa aplicada, exploratória e do tipo estudo de caso. Encontra-se dividida em três etapas: 1) mapeamento de processos; 2) modelagem de processos e; 3) aplicação prática de ferramentas e práticas lean. Para alcançar os objetivos propostos e confirmar a hipótese levantada utilizou-se do referencial teórico-metodológico Lean Healthcare em todas as etapas da pesquisa, os métodos apoiadores Business Process Modeler Notation e software Bizagi® na segunda etapa, e a ferramenta de gestão de melhoria Plan-Do-Check-Act na terceira etapa. A análise dos dados procedeu com abordagem qualitativa e quantitativa. Os resultados das duas primeiras etapas apontaram para o processo gerencial do trabalho do enfermeiro Gestão da Escala de Pessoal de Enfermagem para ser estudado em profundidade na terceira etapa. Os princípios lean nortearam a análise da situação encontrada e a proposição da situação futura. Suas práticas e ferramentas aplicadas levaram à redução de 94% do tempo do enfermeiro gasto com atividades que não agregavam valor e redução dos desperdícios inerentes ao seu processo de trabalho gerencial. Concluiu-se que o enfermeiro pode agir efetivamente em prol das melhores práticas e contribuir substancialmente para o processo de mudança na saúde. Os resultados são promissores ao indicar um caminho para implementar a prática lean na saúde, começando por pequenos problemas. Contudo, a pesquisa aponta que a motivação para buscar melhorias deve ser sistêmica e, assim, o lean healthcare pode ser adotado como uma estratégia de gestão centrada na criação de valor para o paciente, melhorando essencialmente a gestão e a assistência à saúde.

Processo de trabalho é a transformação de um objeto em produto por meio da intervenção do agente que utiliza os instrumentos pertinentes. O processo de trabalho, embora apresente bases organizadas, sua organização depende das relações pessoais e da interação entre trabalho vivo e trabalho morto. O trabalho vivo acontece na realização do trabalho cotidiano, com o desenvolvimento das potencialidades dos indivíduos e sua autonomia para a construção do trabalho e do cuidado mais efetivo. O enfermeiro é o profissional da equipe que encadeia a construção do trabalho vivo em decorrência de sua inserção nos processos de trabalho e seu potencial articulador na equipe. O processo de trabalho utiliza tecnologias. As tecnologias leves definem as relações e estimulam o desenvolvimento das potencialidades do ser humano e sua autonomia no trabalho e no cuidado. The work process is the transformation of an object into a product through intervention by an agent using pertinent instruments. Although the work process presents organized bases, its organization depends on personal relations and on interaction between live work and dead work. Live work takes place during the performance of routine work, with the development of individuals' potentialities and their autonomy for work construction and more effective care. Nurses are the professionals on the team who concatenate the construction of live work as a result of their insertion in the work processes and their articulating potential on the team. The work process uses technology. Soft technologies define relationships and stimulate the development of human beings' potentialities and their autonomy in work and care provision. Softhard technologies are characterized by the knowledge that composes the work process in health care provision, and hard technologies are characterized by equipment and standards that must support actions in search of the nucleus of live work. Nurses, as professionals who organize care provision, must be capable of promoting effective changes in the care provided. At an ICU, extreme situations between life and death are experienced, and they require that nurses pay a great deal of attention to adversities and that they act promptly to meet various demands. This diploma thesis is focused on the influence of teaching students of the Faculty of Health and Social Studies of the University of South Bohemia in Czech Budejovice on a mentor nurse´s attitude to nursing care provision. In the first chapter of the theoretical section the term nurse is introduced, and it is dealt with her duties, roles, attitudes, it is explained who a mentor nurse is and what topics a pilot certified course contains. The next four chapters are focused on a supervisor´s personality, supervision issues, teachers and their non-traditional roles and the didactic skills of a mentor. The description of a mentor´s didactic skills is divided into education, student diagnostics, teaching goals and classroom practices. The sixth part deals with student clinical practice mentors, their role in clinical practice, assessment and feedback and mentorship as an advancement of nurses´ work. The seventh chapter of the theoretical section introduces the issue of communication as an integral part of nursing practice teaching and in the last section we have focused on the issue of evidence-based nursing. The aim of this thesis was to investigate the impact of nursing practice teaching on a mentor nurse´s attitude to nursing care provision and students´ evaluation of a mentor nurse´s achievements in nursing care provision. The outcomes of the quantitative research, conducted by using a questionnaire method, confirm the hypothesis 1 suggesting that students regard mentor nurses´ nursing procedures as standards for their future practice. We can also confirm the hypothesis 3 suggesting that students believe that a mentor nurse considers the correct information registration into nursing documentation as necessary, and also the hypothesis 2 suggesting that students believe that a mentor nurse can communicate more effectively than other staff nurses was confirmed. Mentor nurses´ attitudes to nursing care provision were identified by using qualitative investigation by asking research questions. Out of the six nurses interviewed, four mentor nurses consider mentorship as knowledge contribution to the field of communication with people, four mentor nurses consider mentorship as stimulation for improving their own expertise, four mentor nurses would be willing to introduce new knowledge into practice and provide nursing care in accordance with the EBN. The knowledge gained form this research work may serve as a suggestion and aid for students planning to carry out clinical nursing practice at a clinic, for nurses who have been working or have been preparing to work as clinic mentor nurses, and certainly for all of those who wish to learn more on this issue and to understand the mentor nurse job.

Exploration of Practice Managers' Decision-Making Strategies in a Managed-Care Paradigm

Practice managers are facing challenging expectations when deploying a managed-care paradigm. The problem addressed in this study was a gap in knowledge regarding practice managers' decision-making strategies that affect, or could be perceived to affect, a climate of excellence with business and client relationships, primary health care, physicians, and patients in a managed-care paradigm. The purpose of the qualitative exploratory study was to explore practice managers' decision-making strategies affecting primary health care, physicians, and patients. Guided by Simon's ideology of decision-making strategies in a management environment, the overarching research question and 3 subquestions centered on how practice managers delineate their decision-making strategies and how those strategies affect primary health care, physicians, and patients. To close the gap in knowledge, the study included (a) a homogeneous purposive sampling of 14 practice managers (n = 2, pilot study; n = 12, main study) as research participants; (b) face-to-face interviews with semistructured, open-ended questions to collect data; and (c) in vivo and pattern coding during data analysis. The study results indicated a need for change agents, interactions, partnerships, and accountability in a managed-care paradigm. Managing health care is complex and practice managers will continue to be challenged. Alliances between practice managers and stakeholders are recommended to meet those challenging expectations. As a result, positive social changes may be observed in improved access to primary health care, better health care treatments, and collaborative interactions in a managed-care paradigm.

The study is aimed at the development of a Practice Model for Private Practice Management in Social work. It is evident from the literature that no practice model for private practice management exists, peculiar to the nature and modus operandi of Social work. A further problem is that the knowledge base of Social work is often derived and adopted from other professions and disciplines to address problem areas in Social work private practice. The problem lies far deeper. The lack of a practice model for private practice management also implies the lack of guidelines on how to employ social workers within the field of private practice. Poor remuneration and socio-economic change in the external environment put financial and emotional pressure on the social worker, which causes him to leave the social work profession. With reference to the above mentioned the researcher developed a practise model for private practice management in Social work to an extend that the content nature and magnitude of private practice is being familiarised and utilised in practice through this practise model. In order to reach this goal, a theoretical frame of reference for private practice management in Social work is created, the profile of existing private practices in South Africa is explored, a practice model for private practice management is developed through the intervention research model and the developed practice model's value is evaluated through focus groups consisting of private practitioners. Interdependent herewith all relevant concepts, principles, processes and elements as components of a practice model is investigated in both the field of social work and economic and management sciences. From the economic and management science a distinction is made between aspects applicable within entrepreneurship and management to private practice management. Based on a comprehensive literature study the practice model is developed and supported by a secondary data analysis from the national databank of the South African Social Workers in Private Practice. The utilisation value of the developed model is evaluated through three separate focus groups consisting of social workers in private practice. The qualitative data analysed through this process is carefully incorporated into a final practice model. Conclusions and recommendations as a result of the research process conclude the study.
