On one hand it is demanded that we build a competent clinical strategy regarding the prevention and treatment, capable of handling the community issues, on the other hand, mental health is still haunted by the figure of the professional focused on individual assistance, in a closed room unit, at the mercy of referrals. Trying to escape from reference s scheme and cross-reference and under or over recovery of mental-health clinic, we sought in this work, based on the construction of clinical narratives, given from the development of the clinical research s method in Fundamental Psychopathology, to understand "how?" to make a consistent clinical strategy, but flexible and open at the same time, before the problems that arise in the mental health field, in this case, bringing as context a unit of the Family Health Program and a Judicial Asylum, both located in cities in the state of Sao Paulo. We worked on the trajectory of these patients and the path followed by the clinician in these cases, focusing primarily a networking and co-responsibility / São muitas as questões que envolvem a clínica em saúde mental, desde a incorporação dos serviços até a maneira como estes devem ser oferecidos. Se por um lado exige-se que se construa uma estratégia clínica competente no que se refere à prevenção e ao tratamento e que saiba manejar as questões comunitárias, por outro, a saúde mental é assombrada pela figura do profissional focado nos atendimentos individuais encerrados numa sala da unidade, à mercê dos encaminhamentos. Tentando fugir dos esquemas de referência e contra-referência e da sub ou supervalorização da clínica dentro da saúde mental, buscou-se nesse trabalho, com base na construção de narrativas clínicas dadas a partir do desenvolvimento do método de pesquisa clínica em Psicopatologia Fundamental, compreender o como? fazer uma estratégia clínica consistente e ao mesmo tempo flexível e aberta diante das problemáticas que se apresentam no campo da saúde mental, nesse caso, trazendo como contexto uma unidade do Programa Saúde da Família e um Manicômio Judiciário, ambos situados em municípios do interior do estado de São Paulo. Our main goal was to analyze the dynamics of the psychic vulnerability in the clinical position and in the patient. Inserted in the field of Fundamental Psychopathology, this research made use of the clinical method to offer a careful thought about the human pathos, highlighting what was peculiar to each case. To achieve such result, hospitality, the capacity to receive those who are in dislocation, was dealt with from the clinical perspective, which considers hospitality itself one of the cornerstone of the clinical position. On the other hand, it was revealed that in this availability to receive the páthico content that comes from the patient lies the vulnerability that accompanies the clinical position. In view of how important it is to support this belief and establish limits to the notion of hospitality, the concept of dietetics, developed in Ancient Greek, was elected, which suggests the fair measure to the use of all pleasures. The art of living proposed by the dietetics can be of great help to think about the regime that is needed to set the laws of hospitality in the psychopathological clinic. When it comes to the patient vulnerability, the journey of young Sara was narrated. In the moment she was hospitalized, Sara received the HIV diagnosis and presented insufficient defensive resources to face the disease. Based on the concept of Psychic Immunological Insufficiency and its articulation with the clinical material, the present study identified some predisposition to this psychopathological manifestation and pointed out some creative strategies that enable defense against the virulent internal and external attack / A partir de atendimentos realizados aos portadores do vírus da aids em um hospital geral, algumas questões surgiram e fomentaram o interesse para a realização da presente dissertação, a qual teve o intuito de analisar a dinâmica da vulnerabilidade psíquica na posição clínica e no paciente. Inserido no campo da Psicopatologia Fundamental e utilizando do método clínico, o presente estudo ofereceu uma reflexão sobre o pathos constituinte do humano, ressaltando o que há de peculiar em cada caso. Para tal feito, primeiramente, a hospitalidade, ou seja, a capacidade de receber aquele que está em deslocamento, foi abordada sob o enfoque clínico, sendo esta, nesta perspectiva, configurada como um dos pilares que fundamenta a posição clínica. Todavia, relevou-se, também, que nesta disponibilidade em receber o conteúdo páthico vindo do paciente está presente a vulnerabilidade que acompanha a posição clínica. Diante da importância em sustentar tal posicionamento e do estabelecimento dos limites à hospitalidade, recorreu-se à noção de dietética desenvolvida na Grécia antiga, a qual propunha a justa medida ao uso dos prazeres. A arte de viver proposta pela dietética serve de reflexão para se pensar o regime necessário para se estabelecer as leis da hospitalidade na clínica psicopatológica. Sobre a vulnerabilidade do paciente, foi narrado o percurso de Sara, uma jovem que, no momento da hospitalização, recebeu o diagnóstico positivo para HIV e manifestou uma insuficiência de recursos defensivos diante do adoecimento. A partir da noção de Insuficiência Imunológica Psíquica articulada ao material clínico, o presente estudo identificou alguns fatores predisponentes desta manifestação psicopatológica e apontou algumas estratégias criativas, as quais possibilitam a defesa diante do ataque virulento interno e externo

Bringing together clinical report and metapsychology-making of the case, a panel is presented following the postulates of the Clincal Method and Fundamental Psychopathology. It delves into the psychic dimensions implied in cases of: acute appendicitis, multiple sclerosis, ghost limb syndrome, and amnesia and blindness due to cranioencephalic post-multitraumatism. We speculate that critical moments are enigmatic forms of reorganization of the body seeking a harmonic. Organic disorders due to illness or accident are possible conditions to reach new balances. We conjecture that reordering in the body is an answer to the paradigm of creative philo-ontogenetic solutions. Introducing into the transference the experience of feeling alive constitutes the element of therapeia, aiming the reestablishment of the vital rhythm of the body, its pulsional harmonics / O presente trabalho deriva da observação clínica com pacientes passando por um momento crítico sob tratamento em instituição hospitalar. Constelando relato clínico e metapsicologização do caso, apresenta-se um painel configurado, seguindo os postulados do Método Clínico e da Psicopatologia Fundamental. Aprofunda-se nas dimensões psíquicas implicadas em casos de: apendicite aguda, esclerose múltipla, dor do membro fantasma, e amnésia e cegueira pós-multitraumatismo cranioencefálico. Especula-se que momentos críticos são formas enigmáticas de reorganização do corpo visando uma harmônica. The patient demands analysis because all her love relationships failed which caused intense suffering. After a few sessions the patient says she knows why her relationships come to an end: She stinks. The stench becomes a recurring theme during her analysis both in her speech as in her dreams, memories, fantasies, hallucinations and also in the transfer with the analyst. A clinical hypothesis is formulated in the terms of the stench being a defense mechanism. As theoretical foundation the following psychoanalytical concepts will be used: symptom, narcissism, defense mechanism, hallucinations, anxiety, femininity among others. Finally, are mapped different functions of the stench in the psychic dynamism and therefore discusses his role as psychic support base / O presente trabalho, inserido no campo de estudo da Psicopatologia Fundamental, tem como base a narrativa de um atendimento clínico. A paciente em questão procura análise, pois todos seus relacionamentos amorosos fracassavam, o que lhe causava intenso sofrimento. Passadas algumas sessões, a paciente diz que sabe o porquê dos seus fracassos nas relações: ela fede. O mau cheiro passa a ser tema recorrente durante sua análise, tanto em seu discurso manifesto como em sonhos, lembranças, fantasias, alucinações e também na transferência. A partir desses elementos é formulada uma hipótese clínica de que o mau cheiro se insere na economia psíquica da paciente como um mecanismo de defesa. Como fundamentação teórica são utilizados os conceitos psicanalíticos de sintoma, narcisismo, mecanismo de defesa, alucinação, angústia, feminilidade, entre outros. Clinical situations with manifestations of erectile dysfunction, which lead one to conceive of a psychopathology of impotence, allow an approach with several different perspectives. This research is being carried out from a psychoanalytic perspective and is being developed within the field of fundamental psychopathology. The text has five chapters. First we present a methodology based on the clinical method, where we describe an investigation which began with masters work and a masters degree, and moved on to the area of doctoral studies. This historical path was possible due to the enigmas that the clinical work produced, permitting articulation between aspects that originally consisted of inhibition in an individual s early childhood and later developed into impotence in adult life. The chapter on psychic and sexual impotence in men is comprised of discussions of clinical cases. The situations related to treatment led to formulations on the nature of inhibition in each case. We thus conceived that there is both a specific and a generalized inhibition, as developed by Freud when dealing with this notion. In terms of specific inhibition, desire is blocked, whereas, in generalized inhibition, the subject's capacity to desire is put into doubt. This distinction would seem to be essential for listening and conducting treatment in cases where the disappearance of sexual desire leads one to recognize the existence of psychic suffering / A proposta deste trabalho é apresentar os desdobramentos de uma pesquisa sobre impotência sexual masculina. Situações clínicas com manifestação de uma disfunção sexual nos homens, as quais nos sugerem pensar numa psicopatologia da impotência, permitem uma abordagem sob diferentes perspectivas. Esta pesquisa se desenvolve desde uma perspectiva psicanalítica e está circunscrita pelo campo da psicopatologia fundamental. O texto está composto por cinco capítulos. No primeiro deles nos dedicamos à apresentação da metodologia baseada no método clínico, onde percorremos os caminhos de uma investigação que teve origem no mestrado, chegando à situação problemática do doutorado. Tal percurso foi possível realizar pelos enigmas que a clínica produziu permitindo uma articulação entre o que, num primeiro momento, a inibição na infância interrogava com o que, num segundo momento, entendemos como impotência no adulto. Os casos clínicos compõem o capítulo sobre impotência psíquica e sexualidade masculina, sendo que tais situações de tratamento conduziram às formulações sobre a natureza da inibição em cada uma delas. Foi assim que pensamos na inibição específica e na inibição generalizada, influenciada pelas formulações freudianas sobre tal noção. Na primeira o desejo se encontra impedido, enquanto na segunda a condição desejante do sujeito fica colocada em questão.
