Do escudo ao escuto : contribuições da supervisão clínico-institucional para a interface psicanálise e saúde mental em CAPS AD

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A presente pesquisa tem por objetivo investigar contribuições da Supervisão Clínico-Institucional para a interface Psicanálise e Saúde Mental em CAPS AD (Centro de Atenção Psicossocial para usuários de Álcool e Drogas) servindo-se, para tal, de entrevistas semi-estruturadas realizadas aos supervisores de CAPS AD da cidade do Recife. Ressaltando o lugar de Supervisão não o único tampouco o principal, mas lugar estratégico nos CAPS AD para articular clínica e instituição a partir da ética da psicanálise, tais depoimentos assumem relevo ao que testemunham de efeitos em sua prática, ao identificar impasses e erigir questões na busca por avançar. Intervir sob a necessidade lógica do lugar de exceção na instituição foi destacada e testemunhada como condição estruturante à construção de um projeto coletivo, sustentando no modelo horizontal dos CAPS AD a instauração de diferenças de lugar na equipe. Tal corte é condição para evitar formação de massa a dois equipe-usuário, para instaurar uma abertura ao não-todo que não se confunda com todo-não (Lebrun, 2009). Poder prescindir do lugar de exceção sob a condição de servir-se deste é operar efeitos de uma transmissão da psicanálise, rumar à assunção do impossível do rapport sexual, castração enquanto motor da clínica e de uma formação continuada. Enquanto limite e desafio, a construção de balizadores clínicos que respeite a diversidade de formações acadêmicas e ainda assim norteie-se pela ética da psicanálise ressaltou uma transferência de trabalho que sustente a construção de um projeto clínico no coletivo, levando em conta a função psíquica da droga para o sujeito, rumando à contramão da démarche segregativa do discurso da ciência e do discurso capitalista. Para além das diferenças de conhecimento, há um saber que habita cada um que se confronta ao Real da clínica. The possibility of psychotherapeutic privilege in South Africa

Privilege is an evidential principle which, on the grounds of public policy, excludes evidence relevant and otherwise admissible. This thesis aims to discover whether privilege should be applied to the psychotherapeutic profession in South Africa. At present, the only profession in South Africa afforded privilege is the legal profession. There are two main theoretical justifications for privilege: the utilitarian and the individual/human rights approach. This thesis considers whether the psychotherapeutic profession wan-ants privilege under either theory, and recommends that the law of privilege integrate both theories rather than adopt one or the other. The impact of the Constitution and the right to privacy receive particular attention. Very little literature or case law on the question of psychotherapeutic privilege was found in South Africa. Consequently, extensive comparative research into the common-law systems of England, Canada and United States of America was done. This research yielded some interesting findings. The first is that case-by-case development of the law of privilege is uncel1ain and fragmented. The next is that psychotherapeutic privilege exists in almost absolute form in the United States of America, but is littered with lacunae causing as much uncertainty as the case-by-case approach to privilege law. The most helpful method of dealing with the problem was found in Canada, where a two-stage approach to protecting personal information, including psychotherapeutic records, has been developed. In light of this research, this thesis reassesses the original viability of psychotherapeutic privilege in South Africa. Privilege, it was found, is not the best solution to protecting psychotherapeutic communications. This thesis recommends legislative adoption of an amended two-stage approach based on the Canadian model for sexual offence trials as the best method of protecting psychotherapeutic communications in both civil and criminal proceedings. The thesis ends by suggesting draft legislative provisions.

The fortigenic exploration of psychotherapists’ experiences in full-time private practice

Psychotherapists in full-time, long-term private practice face a variety of occupational demands. They usually maintain private practices for long periods of their lives often under difficult circumstances and emotional pressures and have come to sustain their practices. From literature it is revealed that various demands, as well as benefits and successes are part and parcel of working in private practice full-time. These various factors can have an impact on the well-being of psychotherapists. Apart from the literature review, the personal experience of the researcher, a psychotherapist in full-time private practice, also contributed to ideas and hypotheses about the study. From a position of exploration and further enquiry, the researcher was interested to explore the experiences of psychotherapists in full-time private practice from a fortigenic perspective. A second objective was to determine if this study could contribute to the development of the theoretical assumptions of positive psychology. The research is grounded in the theoretical perspective of positive psychology and fortigenesis. Both these fields are relatively new in psychology and seem to still be forging a niche within the discipline. This perspective was deliberately chosen due to the applicability to the exploration of strengths and vigour, with regards to the maintenance of the professional context of the psychotherapist. The qualitative research process is presented in a narrative approach by means of narrative synthesis and synergy. The findings of the research conversations are presented in the form of a literary short story. Suggestions are made about the fortigenic qualities of psychotherapists essential for maintaining their work in full-time private practice. It’s applicability and usefulness is discussed. Furthermore, suggestions are made with regards to the field of positive psychology and the way forward for this sub-discipline. Inledning: Känslor spelar en betydande roll i psykisk ohälsa och är ett fokus i terapeutiskt arbete. De kan gömma sig bakom försvar och i förkroppsligade tillstånd och behöva medvetandegöras. Studien syftar till att utforska bild­psyko­terapeuters erfarenheter av att uttrycka känslor i bildterapi, för att se om bild­interventioner kan komplettera det verbala uttrycket i terapi. Frågeställningar: Hur har bildpsykoterapeuter använt bildskapande för att uttrycka och bearbeta egna känslor? Hur upplever de att det hjälpt till respektive försvårat det psykiska arbetet med egna känslor? Metod: Studien baseras på intervjuer med fem kvinnliga bildpsykoterapeuter. Materialet analyserades i en induktiv tematisk analys. Resultat: Studien visar att bildskapande hjälper till att synliggöra känslor. Det bidrar även till att väcka nya, lugna och symbolisera känslor. Vidare framkommer att bildarbetets ram i sig har en påverkan på känslor. Exempelvis kan ledaren/gruppen påverka, men även vilken sorts bildmaterial som används och hur rummet är utformat. Diskussion: Resultatet diskuterades i förhållande till att bildinterventioner både kan verka försvårande i arbetet med känslor och vara hjälpsamt. Terapeutens roll i att handskas med detta genom att bedöma patienten och förebilda känslomässigt hållande lyfts fram. Även vikten av att kontrollera variabler som patientens nuläge, typ av bildmaterial och gruppklimat. / Introduction: Feelings play a significant role in mental illness and are a focus in therapeutic work. They may hide behind defenses and in embodied states and may need to be made aware. The study aims to explore art psychotherapists' experiences of expressing emotions in art therapy, to see if art interventions can complement the verbal expression in therapy. Questions: How have art psychotherapists used image creation to express and process their own emotions? How do they feel that it has helped or hindered the mental work with own emotions? Method: The study is based on interviews with five female art psycho­therapists. The material was analyzed in an inductive thematic analysis. Result: The study shows that image creation helps to make emotions visible. It also helps to evoke new, calm and symbolize emotions. Furthermore, it appears that the framework of image creation itself has an impact on emotions. For example, the leader/group can influence, but also what kind of art material is used and how the room is designed. Discussion: The result shows that art interventions both can hinder and promote helpful work with feelings. The therapist's role in dealing with this by assessing the patient and to model containing of emotions is highlighted. Also the importance of controlling variables such as the patient's current situation, type of art material and group climate.

Inledning: Mäns våld mot kvinnor är ett allvarligt samhällsproblem och ett folkhälsoproblem som måste bekämpas på flera olika sätt. Sammanfattningsvis är forskningsunderlaget för individuell psykoterapi med klienter som utövat våld begränsat och med specifikt psykodynamisk skolade psykoterapeuter ännu outforskat. Syftet: Syftet med den här studien var att undersöka hur psykodynamisk skolade psykoterapeuters erfarenhet av psykoterapi med klienter som utövat våld ser ut. Hur arbetar man? Vad är hjälpsamt? Metod: En kvalitativ metod. Psykodynamiskt skolade legitimerade psykoterapeuter intervjuades. Datan analyserades med tematisk analys. Resultat: Tre huvudteman framkom med tillhörande subteman: 1. Prioritering av klientens egen traumatisering är en nödvändighet 2. Våldet är i fokus i psykoterapin 3. Relationens är av betydelse i psykoterapin. Diskussion: Huvudfynden visade att erfarenhet fanns hos psykodynamisk skolade psykoterapeuter att bedriva psykoterapi med klienter som utövat våld. Både möjligheter och svårigheter framkom. Psykodynamiska interventioner användes i psykoterapin gemensamt med ett eklektiskt förhållningsätt. Relationen i psykoterapin användes som intervention och för att härbärgera klienten. Studiens resultat visar att psykoterapi med psykodynamisk skolade psykoterapeuter kan också vara en potentiell behandling att erbjuda klienter som utövat våld. Mer forskning behövs inom området. / Introduction: Men's violence against women is a major social problem and a public health problem that must be combated in several different ways. In summary, the research about individual psychotherapy with intimate partner violence perpetrators is limited and with specifically psychodynamically trained psychotherapists are still unexplored. Purpose: The purpose of this study was to investigate how psychodynamically trained psychotherapists experience psychotherapy with intimate partner violence perpetrators. How do they work? What is helpful? Method: A qualitative method. Psychodynamically trained licensed psychotherapists were interviewed. The data was analyzed using thematic analysis. Result: Three main themes emerged with associated subthemes: 1.Prioritization of the client's own traumatization is a necessity 2. Violence is in focus during psychotherapy 3. The relationship is important in psychotherapy. Discussion: The result of the study highlights that psychotherapy with psychodynamically trained psychotherapists can also be a treatment for intimate partner violence perpetrators. Both opportunities and difficulties emerged. Psychodynamic interventions are used in psychotherapy together with an eclectic approach. The relationship in psychotherapy is used as an intervention and as a tool to contain the client. The results of the study show that psychotherapy with psychodynamically trained psychotherapists can also be a potential treatment to offer intimate partner violence perpetrators. More research is needed in the area.
