Perceptions of professional nurses on the impact of shortage of resources for quality patient care in a public hospital: Limpopo Province

Text in English / Professional nurses are regarded as the backbone of the health care system and quality patient care is linked to patient safety.The purpose of the study was to describe and explore the perceptions of professional nurses on impact of shortage of resources for quality patient care. The resources include health professionals, equipment and drugs. The study was conducted in Mankweng hospital which is a public hospital in the Capricorn district in Limpopo Province. Qualitative descriptive exploratory design was used which provided the researcher with in-depth information regarding phenomena under study. Data was collected from ten (10) professional nurses who have 5 to 20 years of nursing experience allocated in medical and surgical wards. Unstructured face-to-face interview was conducted using field notes and audio tape. Data was analysed following Creswell (2014) Tesch method. Five themes and eighteen subthemes emerged from data. The findings revealed that the shortage of health professionals and inadequate resources has a negative impact on provision of quality patient care. / Health Studies / M.A. (Health Studies)

To understand better the interconnections between domestic violence and child development, it was necessary to analyze the theme from the beginning of the cultural history path in the world until nowadays, through bibliography references. With the objective to reach an understanding of these professionals about this theme we used a group of problem situations concerning many different forms of domestic violence, and these situations were adapted by a pilot research. The study was done in a Children s Public Hospital in São Paulo City, with 27 professionals from different health departments, which attend in the infirmary and in the emergency room. The analysis of the information allowed observes the understanding and the behavior of these professionals facing the dynamic of the domestic violence phenomenon expressions. In order of an exploratory research, the data reveal how the health professionals faced this phenomenon on a daily basis, highlighting the search of interdisciplinary team resources to break down the relevant factor of the families silence conspiracy . / O presente estudo teve como objetivo descrever e compreender atitudes de profissionais de saúde frente a situações de violência doméstica contra a criança, enfatizando a importância da utilização de instrumentos de proteção à infância, tal como a notificação. Para melhor entender as inter-relações da violência doméstica e do desenvolvimento infantil, buscou-se situar o tema em seu percurso histórico cultural até os dias atuais, por meio de breve revisão bibliográfica. Utilizou-se um conjunto de questões formuladas a partir de situações problemas enfocando as diversas faces da violência doméstica, as quais foram ajustadas a partir de uma aplicação em uma pesquisa-piloto, constituinte de um trabalho mais amplo realizado em Hospital Geral. Conduziu-se o estudo em um Hospital Infantil Público na cidade de São Paulo, com 27 profissionais de diversas áreas da saúde, atuantes na enfermaria geral e no pronto-socorro. A análise das informações possibilitou observar como esse profissionais compreendem e agem diante das dinâmicas de expressão da violência doméstica. Neste sentido, enquanto uma pesquisa exploratória, os dados obtidos permitiram descrever como esses profissionais enfrentam cotidianamente tais situações, destacando-se a busca de recursos em equipe interdisciplinar perante a importância da ruptura do complô do silêncio entre as famílias.

The effect of quality assurance nurse managers on the provision of patient care at selected public hospitals in the Limpopo Province

Patient and public criticism of care in many South African public hospitals has resulted in negative media reports. The purpose of the study was to determine the effect of appointing Quality Assurance Nurse Managers (QANMs) on the provision of quality patient care in selected public hospitals of the Limpopo Province and to make recommendations where appropriate. The study was quantitative, exploratory and descriptive in nature. Data was collected by means of a self-developed questionnaire from 112 respondents, consisting of 10 QANMs and 102 Quality Assurance Team members (QAT). The response rate was 100% for the QANMs and 57% for the QAT members. The study found that most of the ten selected hospitals have a good QA foundation with a vision, mission and goals, QA programme and manual. Moreover, the appointment of the QANMs had a positive impact on the nursing care. Regarding the improvement and change in the provision of nursing care, the majority of the respondents indicated that patients’ complaints about nursing care had declined considerably; the in-service training assisted in improving nursing care, and patients were nursed in totality because of the quality guidelines in the QA manuals. However, the study found that the shortage of equipment, which interferes with the delivery of quality patient care, is a serious problem that hinders the QA programme and needs to be tackled. Purpose: The purpose of this study was to explore the experiences of forensic nurses when providing care to victims of sexual assault in selected public hospitals of Limpopo province. Methods: A qualitative, explorative, descriptive and contextual research design was employed in this study. A sample of 7 registered nurses was purposively selected to partake in in-depth unstructured interviews. Results: Four themes associated with the experience of providing care to victims of sexual assaults emerged during data analysis. These included (1) psychological experiences of nursing victims of sexual assault, (2) physical challenges associated with implementing the forensic nursing specialty in a clinical setting, (3) psychological coping strategies to deal with challenges experienced, and (4) the experience of managerial support. Conclusion: Participants reported negative psychological and physical experiences of providing care to victims of sexual assault, which resulted to occupational stress. La présente étude s’attache à l’absentéisme long pour raison de santé chez les personnels hospitaliers non médicaux. Deux types de congé non ordinaire sont visés : les C.L.M. et C.L.D. Ceux-Ci constituent en effet un enjeu de gestion par les désorganisations et les coûts, principalement cachés, qu’ils suscitent. Ces derniers représentent également un enjeu de santé sociale (voire publique), en raison de la morbidité qu’ils expriment.Afin de les caractériser et de les mesurer, ce travail s’inspire de l’approche socioéconomique des organisations (I.S.E.O.R., Lyon). On s’efforce notamment de procéder à un diagnostic pluriel (social, organisationnel et financier). L’ambition est triple. Il s’agit 1) de faire prendre conscience de l’ampleur des préjudices (effet miroir pour la direction) ; 2) de contribuer au développement d’un référentiel théorique et pratique (évaluation élargie des absences) ; 3) d’encourager la promotion d’un présentéisme-Qualité (préconisations de type R.H.). Les diverses investigations se font sur trois hôpitaux régionaux publics (Auvergne) de taille volontairement différente (C.H.U., C.H. et H.L.). Les résultats laissent à chaque fois apparaître des profils, des fonctionnements, des dépenses et des vécus assez alarmants. Ils témoignent d’une défaillance au niveau des ressources humaines (organisationnelle et managériale). Ces retours négatifs attestent par ailleurs d’une possibilité d’extension de l’analyse de type socio-Économique (application aux interruptions prolongées en structures de soins). Ils autorisent enfin à dégager quelques pistes d’intervention, tantôt transversales(proximité dans les procédures), tantôt spécifiques (particularités de la structure). / The present study focuses on the long absenteeism for health reason at non medicalhospital staff. Two types of non ordinary sick leave are aimed : the C.L.M. and C.L.D. (rulingson salary insurance). Those indeed constitute a challenge of management by thedisorganizations and the costs, mainly hidden, which they cause. They represent also a stakein social health, because of the morbidity they express.In order to characterize and measure them, this work takes as a starting point theorganizations socio-Economic approach (I.S.E.O.R., Lyon). We particularly try to carry out aplural diagnosis (social, organizational and financial). The ambition is threefold. It acts 1) tomake become aware of the scale of the damages (mirror effect for the direction); 2) tocontribute to the development of a theoretical and practical reference frame (widenedevaluation of the absences); 3) to contribute to the promotion of good and really presenteeim(human resources recommendations).The various investigations are done on three publicregional hospitals of voluntarily different size (C.H.U., C.H., H.L. – in the center of France).Each time the results let appear alarming profiles, operations, spending and lived. They giveevidence to a failure in organisational and managerial human resources. These negativereturns also attest an extension possibility of the socio-Economics’ analysis (to the extendedsickness absences in structures of care). They finally allows to identify some tracks ofintervention, sometimes transverse (proximity in the procedures), sometimes specific(peculiarities of the structure).

Η Εργασιακή Ικανοποίηση αποτελεί τον συνδετικό κρίκο ανάμεσα στις θεωρίες των κινήτρων και την εφαρμογή τους στον εργασιακό χώρο και αποτελεί την πιο μελετημένη μεταβλητή στο εργασιακό γίγνεσθαι. Συνοπτικά η επαγγελματική ικανοποίηση μπορεί να περιγραφεί ως ένα σύνολο στάσεων που οι εργαζόμενοι έχουν απέναντι στη δουλειά τους, ή ως μια ψυχολογική προδιάθεση που περιλαμβάνει πολλές επιμέρους στάσεις και συναισθήματα για διάφορες όψεις της εργασίας (Schultz, 1982). Σκοπός της διατριβής είναι η εκτίμηση του βαθμού ικανοποίησης των επαγγελματιών υγείας από την εργασία τους και η ανάδειξη των παραγόντων εκείνων που την επηρεάζουν. Το δείγμα αποτελείται από 572 εργαζομένους των δημόσιων νοσοκομείων της Περιφέρειας Πελοποννήσου όλων των κατηγοριών ιατροί, νοσηλευτές, καθώς και διοικητικό, τεχνικό, βοηθητικό και παραϊατρικό προσωπικό. Τα δεδομένα συλλέχθηκαν με τη μέθοδο του δομημένου ερωτηματολογίου για την περίοδο 2006. Τα αποτελέσματα έδειξαν ότι η ηλικία και το επίπεδο εκπαίδευσης είναι οι πρωταρχικοί παράγοντες οι οποίοι επηρεάζουν την υποκειμενική αντίληψη για την ικανοποίηση των επαγγελματιών υγείας από την εργασία τους. Επιπλέον, περισσότερο συνολικά ικανοποιημένοι είναι οι άνδρες, οι εργαζόμενοι υποχρεωτικής εκπαίδευσης και το βοηθητικό προσωπικό. Μικρότερη ικανοποίηση φαίνεται να βιώνουν οι γυναίκες, οι κάτοχοι μεταπτυχιακού τίτλου και το διοικητικό και νοσηλευτικό προσωπικό. Τα θέματα τα οποία αναδεικνύονται ως μείζονος σημασίας στη διαμόρφωση του επιπέδου ικανοποίησης από την εργασία είναι (α) η αντίληψη των εργαζομένων για τον οργανισμό και τη θέση απασχόλησής του, (β) η αίσθηση επαγγελματικής επάρκειας και προσφοράς αποτελεσματικού έργου και (γ) η έλλειψη επιβράβευσης της Διοίκησης στις περιπτώσεις αποτελεσματικής απόδοσης. / --

Security of electronic personal health information in a public hospital in South Africa

The adoption of digital health technologies has dramatically changed the healthcare sector landscape and thus generates new opportunities to collect, capture, store, access and retrieve electronic personal health information (ePHI). With the introduction of digital health technologies and the digitisation of health data, an increasing number of hospitals and peripheral health facilities across the globe are transitioning from a paper-based environment to an electronic or paper-light environment. However, the growing use of digital health technologies within healthcare facilities has caused ePHI to be exposed to a variety of threats such as cyber security threats, human-related threats, technological threats and environmental threats. These threats have the potential to cause harm to hospital systems and severely compromise the integrity and confidentiality of ePHI. Because of the growing number of security threats, many hospitals, both private and public, are struggling to secure ePHI due to a lack of robust data security plans, systems and security control measures. The purpose of this study was to explore the security of electronic personal health information in a public hospital in South Africa. The study was underpinned by the interpretivism paradigm with qualitative data collected through semi-structured interviews with purposively selected IT technicians, network controllers’, administrative clerks and records management clerks, and triangulated with document and system analysis. Audio-recorded interviews were transcribed verbatim. Data was coded and analysed using ATLAS.ti, version 8 software, to generate themes and codes within the data, from which findings were derived. The key results revealed that the public hospital is witnessing a deluge of sophisticated cyber threats such as worm viruses, Trojan horses and shortcut viruses. This is compounded by technological threats such as power and system failure, network connection failure, obsolete computers and operating systems, and outdated hospital systems. However, defensive security measures such as data encryption, windows firewall, antivirus software and security audit log system exist in the public hospital for securing and protecting ePHI against threats and breaches. The study recommended the need to implement Intrusion Protection System (IPS), and constantly update the Windows firewall and antivirus program to protect hospital computers and networks against newly released viruses and other malicious codes. In addition to the use of password and username to control access to ePHI in the public hospital, the study recommends that the hospital should put in place authentication mechanisms such as biometric system and Radio Frequency Identification (RFID) system restrict access to ePHI, as well as to upgrade hospital computers and the Patient Administration and Billing (PAAB) System. In the absence of security policy, there is a need for the hospital to put in place a clear written security policy aimed at protecting ePHI. The study concluded that healthcare organisations should upgrade the security of their information systems to protect ePHI stored in databases against unauthorised access, malicious codes and other cyber-attacks. / Information Science / M.
