Exploring the construction of quality of life in older people / Lizanlé van Biljon.

Van Biljon, Lizanlé January 2013 (has links)
Ageing populations and the unique challenges they pose are characteristic of the accelerating demographic transition evident in both developed and developing countries. In South Africa the elderly population is also increasing dramatically. There is a disproportionate distribution of older persons per ethnic group, with white older people representing the largest group of older South Africans (21%, proportional to ethnic group). The influx of the baby boomer generation will inevitably lead to an exponential increase in the numbers of white older people within the next two decades. Regardless of integration policies in post-apartheid South Africa, 90% of all residential care facilities are still occupied solely by white older people. Such facilities are described as buildings or other structures used primarily for the purposes of providing accommodation and of providing a 24-hour service to older persons. The increasingly larger segment of white older people holds considerable implications for the future of these facilities since more individuals will turn to this living arrangement. The Older Persons Act of South Africa was inaugurated by the government in 2006 and its key objectives are aligned with the recommendations of the Madrid International Plan of Action on Ageing (2002). Amongst many other objectives, the Older Persons Act emphasises practices that enhance the wellbeing and quality of life (QoL) of all older persons. However, the reigning circumstances in most residential care facilities have been described as challenging. A national audit of residential care facilities in 2010 indicated a need for psychosocial interventions since the QoL of residents was found to be undefined and unspecific. The purpose of the study was to explore the construction of QoL, from the perspective of the older people living in residential care facilities. A purposive sample of 54 participants (male, n=10; female, n=44) with ages ranging between 62 and 95 years was drawn. The participants were able to communicate congruently and understood the research purpose. Participants resided in four similar facilities situated in urban areas in two South African provinces. A multiple-context inquiry was conducted to gather rich data and collateral information. The study made use of interviews, focus groups, journals, and the Mmogo-method® to collect qualitative data. Interpretative Phenomenological Analysis (IPA) and visual analysis methods were used to analyse the data. Interactive Qualitative Analysis (IQA) was conducted with 19 participants, resulting in a conceptual model of QoL. Member-checking was performed by the participants. Ethical approval was granted by the Ethics Committee of the North-West University (Potchefstroom Campus), as part of a larger project, namely “An exploration of enabling contexts (05K14)”. The findings revealed that the nature of QoL is informed by spiritual worldviews, interpersonal contexts and the maintained ability of older people to regulate aspects of their own lives. The nature of QoL was also revealed as transitional throughout the ageing process and that the dimensions of QoL may be found on a continuum. Six domains were elicited in the construction of QoL, namely spirituality, health, relationships, meaningfulness, autonomy and sense of place. Each domain presented with certain contributors and inhibitors influencing the older person’s ability to experience QoL. Findings revealed the strengths of older people to deal with adversities associated with later life. The inhibitors of QoL are emphasised for the attention of policy makers, the managers of residential care facilities, care givers and family members. The study provided insight into the causal influences between the domains of QoL. A conceptual model with systemic properties is proposed. The theoretical implications of this systems model are that QoL domains are mutually informing and exercise a particular influence on the relational states of older people. It is hoped that new knowledge in the area of QoL might direct future research efforts and put resources channeled to residential care facilities to better use. / Thesis (PhD (Psychology))--North-West University, Potchefstroom Campus, 2013.

Veillette-Bourbeau, Ludivine 02 1900 (has links)
In 2009, a multidisciplinary and intersectoral team implemented an innovative research-intervention project: Spot, a community-based rapid HIV testing service for men who have sex with men in Montreal. A case study was undertaken to describe the implementation process of Spot and the contextual factors that affected it. Grounded theory analysis of interviews, participant-observer, and a document analysis allowed the identification of a multistage dynamic implementation process. These stages were influenced by stakeholders’ motivations, the complexity of partnership dynamics, the challenges surrounding coordination and organization of staff members, as well as factors influencing staff members’ daily practices. Motivations like the unique opportunity to contribute to the implementation of an innovative and relevant prevention project, had a constant and positive influence, which united stakeholders in the project and maintained their involvement despite the difficulties. In terms of partnership dynamics, vague definitions of roles and tasks delayed the implementation of the project, although once clarified, everyone felt legitimate and actively participated in the success of the implantation. The management of interinstitutional funds was mentioned as one of the difficulties of coordinating the project and a factor that caused delays in the implementation process. This study allowed learning about the implementation and sustainability of a gay community-based rapid HIV testing service.

Technicalities of ageing in place : a case study of the integration of residential care services through the use of information technology (IT) in the changing context of care

Ibrahim, Rahimah January 2006 (has links)
Through a case study about the impact of IT adoption in a residential aged care organisation, this thesis examines the increasing pressure for service integration as mainstreamed through reform policies. Specifically, the research investigates the role of IT in facilitating the 1997 aged care reform agenda of 'ageing in place' focusing on the levels of transformation from the policy context to the organisational/management context, and to the context of service provision by care staff. A single embedded case study (Yin, 1993) is used in order to meet the general objective to capture the dynamics of the impact of ageing in place in the three social contexts. The research is informed by social constructionism, a theoretical framework that emphasises the significance and effects of language in shaping social realities (Ainsworth, 2001; Hosking, 1999). The framework, therefore, justifies the qualitative analysis of both written (i.e., policy documents) and spoken (i.e., interviews with staff) texts to address meaning in relation to context. Changing technologies can result in altered societal structures (Betz, 2003) at all levels, from the very complex to the very basic. As such, it is important to understand a few basic premises of technology. First, technology is a human invention to improve the well-being of society (Ayres, 1996). Consequently, technological inventions that improve the quality of life are seen by people as a necessity for modern living. In the case of ageing, modernisation and technological advances effectively resulted in people becoming healthier and living longer (Department of Health and Aged Care [DHAC], 2000). Second, technology is a human means to control nature (Betz, 2003). As such, technological advances can be seen as a modernising process of predicting and regulating the effects of the trends existing in the environment, such as ageing. Ageing in the twenty first century presents a challenge to government's development policies because ageing is depicted as a steady force with a long-term economic impact (Johnson, 1999). Third, a technology becomes powerful when it is sponsored by the market (Betz, 2003; Hughes, 1983). Unless a technology is backed by business, it lacks the influence on a large scale. Fourth, technology is used to enable change. By using IT, governments, business and the community are co-operating through a paradigm similar to the business sector. As a result, the service environment is shifting towards more business-like approaches. To sustain the changes brought by a different paradigm and modes of operation, the rhetoric of technology is employed. Therefore, the purpose of the study is to investigate the use of IT in processes of organisational adaptations to reform, which requires the examination of: a) specific meaning of IT as used in long-term care policies for older people since the last structural reform, b) the rationale behind the introduction of a new IT system into a residential care organisation, and c) the meaning of IT as articulated by care staff who have experienced a change in technology. The first paper represents a rhetorical analysis at the macro or policy level. There is a significant influence of a global political actor in developing proactive strategies on ageing, which results in a new, multi-organisational approach in delivering government-subsidised services, such as residential care. Three key institutional texts were selected to represent international to local policy development since the time ageing became a global concern. Since then, ageing is also viewed as a human rights issue. Using Burke's pentad, an analytic framework to analyse rhetoric in texts (Stillar, 1998), these institutional texts are seen to employ the rhetoric of 'technology for sustainability' to justify changes to policy approaches that seek long-term viability. Technology, in the name of sustainable development ensures support for economic growth, which balances the long-term effects of population ageing. The existence of a global force, such as population ageing, allows the intervening powers of the UN in mainstreaming ageing into development policies. Accordingly, it initiates corresponding actions at national (Australian Commonwealth Government) and state (Queensland Government) levels. IT is a medium of communication, knowledge transfer, and standard practice at these levels of actions. The second paper represents a qualitative analysis at the meso or organisational level. This paper explores the cogent rationale in the introduction of a computer-based, care documentation system in a large residential aged care organisation. Twenty two staff, from every level of the organisation, were interviewed to get an insight into the role of IT in substantive changes to organisational structure and modes of service provision. Responses from staff indicate external and internal influence that pressured the organisation to change. In the bid to sustain the future of aged care, the industry is changing through the introduction of new structure of service delivery. The Aged Care Structural Reform instigated a shift towards sustainable service provision that is consumer-driven, with a fixed cost compliance mechanism and performance criteria that are tied to funding. Facing the requirement for evidence to corroborate funding, a residential care organisation changed its structure of service delivery by introducing a new strategic direction. IT is part of this new strategic direction, planning, and operations of a changed service environment. The third paper represents a qualitative analysis at the micro or individual level to examine the impact of IT at frontline service delivery. This study is also based on interviews with twenty-two staff, across the organisational structure; however, this time the focus is more on staff who are involved in providing direct care to older residents at the organisation. The reason behind this is that IT has always been a management tool which handles management priorities such as financial planning and performance monitoring. The themes arising from the interviews indicate discord at the level of service delivery from the introduction of a new technical system. It also points to the idea that staff generally refer to ethical ideas and future promise of the new system. In summary, these three papers attached to this thesis support the notion that the meaning of technology is socially constructed. First, technology in the aged care sector has particular reference to improving or enhancing the well-being of older people, and in this case, the provision of high quality services that fulfil the needs of older people. Second, IT has an important role in meeting the evidence-based requirement, such as in the use of information in manipulating the use of resources required for the ageing population. Third, the meaning of IT is conceived from the context requiring its use such as the need to use resource efficiently to ensure long-term sustainability, which were emphasised in the last reform. Fourth, IT is used to enable structural changes in organisations to implement generic practices originated from the business sector, requiring the use of strong rhetoric such as balance and future. The limit of this case study is that these dimensions of technology can only be applied to the specific context of aged care and is not generalisable to other political contexts. However, the strength of the study rests on the macro-, meso- and micro-analysis of the meaning of technology. Therefore, future studies should investigate and compare the dimensions of technology in other contexts.

Realism, rationalism and revolutionism in Iran's foreign policy : the West, the state and Islam

Gomari-Luksch, Laleh January 2018 (has links)
Iran's foreign policy is consistent and is fundamentally realist with a revolutionist vision while the means are rationalist is the central argument of this dissertation. I make use of the English Schools three traditions of realism, rationalism and revolutionism in analyzing the speeches of Iranian statesmen to identify the ways in which the dynamics of the three traditions have evolved since 1997 and what it means for interpreting the developments of Iran's foreign policy ventures. I utilize both quantitative and qualitative methods of analysis in examining the speeches of the supreme leader, Ayatollah Khamenei, the presidents since 1997. The quantitative method employs a customized software generating figures that represent the recurrence of realist, rationalist and revolutionist terminologies in all the documents downloaded from the official websites of the Iranian statesmen as well as the United Nations and select news agencies and affiliates. The quantitative phase of the analysis, meanwhile, carefully examined selected statements of the supreme leader and the presidents uncovering the foreign policy argumentations and justifications, which were studied alongside foreign policy actions and classified under the three traditions. The findings suggest that Iran's foreign policy is the same as in the other states of international society – it is consistent and dynamic. It is simultaneously realist, rationalist and revolutionist with each tradition serving a specific purpose, which cannot be disentangled from the other two.

Vytváření ideálu Sovětského svazu a jeho mediálního obrazu v československých médiích v 50. letech 20. století / The creation of the Soviet union's ideal and its media image in the Czechoslovak media in 1950's

Cihlářová, Gabriela January 2012 (has links)
The objective of the present thesis entitled The creation of the Soviet union's ideal and its media image in the Czechoslovak media in 1950's is to show the effort of the major daily newspaper of that time - i.e. Rudé právo - to describe Soviet science. Creating a positive image of the Soviet Union through the presentation of scientific breakthroughs represents a significant way of building the ideal image of the USSR. More than fifty years later, it may be well analyzed which scientific fields were glorified by right, wherein the Soviet Union really excelled; and the rest, glorified only for propagandistic purposes. The theoretical part of this paper deals with the political situation in the USSR at the end of 40's and in the 50's as well as with the overall scientific environment during the same period. It characterizes the position of Rudé právo and describes the method of investigation. The paper however centers on the media analysis of ten separate time periods. The basic research method consists in historical and content- related analysis. The areas to be analyzed include the most important military research event of the decade under investigation - the successful test of the Soviet H-bomb, as well as the first successful use of nuclear energy for civilian purposes in the world - the startup...

For instance, this is the case of defects lists generated from the software inspection activity, which defects consist of qualitative data that should be discussed and classified. This scenario was experienced during the Readers Project, where defects lists have token long time to be analyzed, sometimes hours or days, making the decisions about defects difficult. Moreover, feedback questionnaires deserved a careful analysis, since they carried relevant information about the experimental study. Aim: Driven by the described situation this research aimed to support the qualitative analysis conduction in an information-guided way, using visualization and text mining techniques to enable it. This approach allows software inspection defects described similarly be analyzed together, due to the use of these techniques, which may allow the homogeneity of the decisions about them. The same happens in the context of feedback questionnaires. Methodology: Based on the objective, the ways that visualization and text mining could contribute to become qualitative analysis more effective and efficient were investigated and designed. Considering it, the Insight tool was developed to enable the information-guided qualitative analysis. Experimental studies were conducted to validate and evaluate the approach on different contexts. Results: Four experimental studies were conducted: (i) a feasibility study conducted with text documents which gave evidences that the use of visualization and text mining met previous expectations and was feasible to be employed; (ii) a study in the context of software inspection which, although the quantitative data did not present statistical significance, the descriptive statistics and feedback questionnaires analysis gave evidences that visualization and text mining makes the qualitative analysis effective and efficient; (iii) a study in the context of questionnaires, which results were similar to the previous study; (iv) a case study in the context of qualitative analysis of long text documents which gave evidences about the utility and ease of use of the approach. Conclusion: Under the different contexts of evaluation, the Insight, an information-guided qualitative analysis approach that is based on visualization and text mining, gave evidences about the improvements on efficiency and effectiveness. Adding to its relevance in software engineering area, this research also contributes to other scientific fields, which often claim for technological support to conduct their research.

However, the formation of professionals that make up these teams, the overload work with which they live and the own organization of the work process does not always favor the achievement of these practices. This is the problem in which this study focuses. The objective of this study is to analyze the limits, and possibilities of interdisciplinary actions in professional practice of nurses, doctors and dentists in family health units. This is a cross-sectional study, exploratory, descriptive and inferential, developed from the qualitative and quantitative approaches. The sample consisted of Nurses, Physicians and Surgeon dentists of Family Health Teams in João Pessoa city. The questionnaire was constructed, validated and applied to all professionals by lot of the Family Health Teams. The qualitative part was made from participant observation during two months. The results of the questionnaires were submitted to Cluster Analysis. While the observation, thematic analysis was performed and the data were compared with quantitative results. The three professions had limitations on the interdisciplinary practice. The findings of this study show that the exercise of interdisciplinary professional practice depends on the objective order factors related to the labor process, and subjective factors inherent to the worker himself.

The aim of this study is to know the professional training in an interdisciplinary perspective of the following higher-degree courses: Physiotherapy, Speech Therapy, and Occupational Therapy. The study was held in João Pessoa – Paraíba, whose research was based on cross-sectional, exploratory, descriptive, and inferential model, developed from qualitative and quantitative approaches. The sample was composed by scholars enrolled in the last academic year of Physiotherapy, Speech Therapy, and Occupational Therapy higher-degree courses from two Higher Education Institutions (HEI’s) and also by Physiotherapists, Speech Therapists, and Occupational Therapists who compose the rehabilitation team from two specialized services of these HEI’s. For this purpose, the “Assessment of Training to Interdisciplinary” questionnaire adapted for rehabilitation context, aiming to know the profile of scholars and their position about approximations and distancing in training related to interdisciplinarity. For the qualitative phase with professionals, it was utilized a semi-structured interview guideline, aiming to analyze the facilitations and problems of an insertion in a rehabilitation team on professional practice scenario. The results of the questionnaires were submitted to Cluster Analysis by R statistical software; and the interviews’ content were organized and analyzed by Nvivo software, thus enabling the construction of Collective Subject’s Speech. Therefore, there were clear signs, in the higher-degree courses researched, limitations on the training in health professionals for interdisciplinarity, with the restriction of activities which contemplate and favor the join action of students, having a negative effect in professional practice. Professionals’ attitude facing the interdisciplinary actions highlighted the limitation in training, reflected on their difficulties, recognizing other professions’ duties and competencies within rehabilitation team. The discussion was utilized as strategy for overcoming in the search of interdisciplinarity achievement. The findings of this study evinced the changes in training process of future rehabilitation professionals, which requires planning and reorganization of academic practices in the perspective of interprofissional education.
