Avaliação do desempenho e alteração de superfície das limas Wave One e One Shape no decorrer do número de usos / Performance and change surface of files Wave One and One Shape during the number of uses

Os sistemas de lima única constituem a mais recente tecnologia no que se refere ao preparo dos canais radiculares, e vem sendo objeto de estudo sob diversos aspectos. Uma das preocupações referentes aos sistemas automatizados é a manutenção da qualidade e segurança nos preparos quando da reutilização do instrumento. Este estudo teve como objetivo avaliar as alterações de superfície de dois sistemas de lima única - Wave One e One Shape - e seu desempenho através do tempo de preparo no decorrer dos usos. Duzentos condutos de molares inferiores humanos extraídos foram instrumentados com 10 limas Wave One e 10 limas One Shape, acompanhados de irrigação com 10 ml de Hipoclorito de Sódio 2.5%. Cada instrumento foi utilizado por 10 vezes. Os tempos para o preparo completo do canal foram registrados por um cronômetro digital e anotados em tabelas. Para avaliação em microscopia eletrônica de varredura as limas novas foram observadas assim que removidas da embalagem. Após as utilizações, os instrumentos foram limpos, esterilizados em autoclave e observados após 2, 6, 8 e 10 usos. Três avaliadores calibrados observaram as fotomicrografias e registraram de acordo com os scores propostos a presença ou ausência de alterações superficiais. O teste Kappa de concordância entre os avaliadores apresentou maioria de resultados ótimo a moderado (?=5%). Os dados referentes à avaliação das imagens foram submetidos ao teste Wilcoxon-Mann-Whitney com nível de significância de 5% (?=5%). As alterações mais observadas foram aplainamento e microcavidade. O crack foi observado nos dois instrumentos sendo significante a partir do sexto uso para Wave One e do décimo para One Shape. Os dados relativos aos tempos de preparo foram submetidos à análise estatística pelos testes Wilcoxon-Mann-Whitney, ANOVA e Mc-Nemar com nível de significância de 5% (?=5%). Os tempos de preparo registrados para One Shape foram inferiores aos tempos para Wave One. O uso influenciou os tempos de preparo sendo observado um aumento significativo para os dois instrumentos. Um instrumento One Shape fraturou mas a ocorrência não foi estatisticamente significante. / Single-file systems are the most recent technology in regard to the preparation of root canals and have been studied in several ways. Data on preparation times were statistically analysed by the Wilcoxon-Mann-Whitney, ANOVA and Mc-Nemar tests with a significance level of 5% (? = 5%). Regarding the working time taken to shape the canals, One Shape was significantly faster than Wave One. The preparation time was affected by the number of file use increasing significantly on both instruments. A One Shape instrument fractured but the finding was not statistically significant.

Análise da eficácia do instrumento RECIPROC #25 em atingir o forame apical sem glide path / Analysis of the effectiveness of the instrument RECIPROC #25 to achieve the foramen without glide path

O presente estudo avaliou a eficáciado instrumento Reciproc#25 em atingir o forame de canais de molares inferiores sem um glide path manual prévio. Para isso, uma amostra geral de 300 molares inferiores foi radiografada e previamente selecionada quanto ao grau de curvatura segundo critério de Schneider sendo divididos em classes I e II. Após a aplicação dos critérios de inclusão e exclusão, uma amostragem total de 502 canais radiculares foi incluída para formar os grupos experimentais: 253 canais no grupo de molares inferiores classe I e 249 no grupo de molares classe II. Todos os canais foram instrumentados diretamente com a lima 25, sem nenhum glide path prévio, seguindo criteriosamente as diretrizes do fabricante. Os dados foram descritos como a frequência da distribuição do número de canais (%) nos quais foi possível chegar ao forame apical sem a necessidade de glide path, assim como o número de fraturas em cada grupo. Os resultados compilados dos 2 grupos experimentais mostraram que em 93,4% do total dos canais instrumentados, o instrumento R25 foi capaz de ir até o forame apical sem a necessidade de glide path. Em 6,4% do total dos canais, o instrumento R25 não chegou até o forame apical e em somente 0,2% dos casos ocorreu fratura da lima (um caso no grupo classe I, enquanto no grupo classe II não houve nenhuma ocorrência de fratura). O teste Qui-quadrado foi realizado para verificar se uma determinada classe de canais encontra-se mais associada ou não a necessidade de glide path quando o sistema Reciproc é usado. No grupo de molares classe II houve maior número de canais (23) que o instrumento R25 não foi capaz de ir até o forame apical do que no grupo de molares classe I (9), sendo essa diferença estatisticamente significante (Qui-quadrado, p=0,020, X2=5,452). Dentro das condições experimentais do presente estudo, pode-se concluir então quea lima R25 mostrou uma alta eficácia em instrumentar toda a extensão dos canais de molares inferiores classe I e II sem a necessidade de glide path prévio. Além disso, o sistema de instrumentação proposto mostrou-se altamente seguro quanto ao índice de fratura. / The aim of the present studywas to evaluate the effectiveness of the Reciproc #25 instrument in reaching the apical foramen of mandibular molars root canals without any manual glide path. The pooled results of two experimental groups showed that the R25 instrument was able to reach the apical foramen without glide path in 93.4% of the cases. In 6.4%, the instrument R25 was unable to reach the apical foramen and in only 0.2% of the cases occurredinstrument fracture (one fracture in the class I group andnone in the class II group). The chi-square test was accomplished to verify that a certain class of channels is more associated with or without the need for glide path when the Reciproc system is used. . In addition, the instrumentation system was shown to be highly safe in the rate of fracture.

Material and methods: Ninety-six mesiobuccal root canals of mandibular first and second molars were randomly divide into 6 groups (n=16) according to the rotary file used: Group 1- ProTaper Next®; Group 2- ProTaper Gold®; Group 3- Mtwo®; Group 4- BioRaCe®; Group 5- WaveOne Gold®; Group 6- Reciproc®. Root canals were prepared according to manufacturer’s instructions. Cone-beam computed tomography scans were obtained before and after canal preparation. Measurements were made at five different points: 2, 3 and 4 mm from the apex and 2, 3 and 4 mm below furcation. Transportation and centering ability were analyzed following the recommendations made by Gambill et al. (1996). Results: The greatest mesiodistal (MD) transportation (T) was found for Reciproc® files (p<0.05), and the greatest buccolingual (BL) T, for Reciproc®, ProTaper Gold® and ProTaper Next® files (p<0.05). The greatest mesiodistal (MD) centering ability (CA) was found for BioRaCe® files (p<0.05), and the greatest buccolingual (BL) CA, for BioRaCe® and Mtwo® files (p<0.05). Conclusion: All systems produced root canal transportation. No file system achieved perfect CA of root preparation. Reciproc® files had the greatest MD T and BL T. BioRaCe® files had the greatest MD CA, whereas BL CA was similar for BioRaCe® and Mtwo® files. / Objetivo: Avaliar o índice de transporte (IT) e a capacidade de centralização (CC) do preparo do canal radicular com instrumentos endodônticos de níquel-titânio acionados em rotação contínua e reciprocante. Materiais e métodos: Noventa e seis canais mesiovestibulares de molares inferiores (primeiro e segundo) foram aleatoriamente divididos em 06 grupos (n=16), de acordo com o instrumento rotatório utilizado: Grupo 1- ProTaper Next®; Grupo 2- ProTaper Gold®; Grupo 3- Mtwo®; Grupo 4- BioRaCe®; Grupo 5- WaveOne Gold®; Grupo 6- Reciproc®. A técnica de preparo do canal radicular obedeceu a orientação dos fabricantes. Tomografia computadorizada de feixe cônico foi realizada antes e após o preparo dos canais radiculares. Foram estabelecidos 06 níveis para a realização das mensurações nas imagens: 2, 3 e 4 mm aquém do vértice radicular e 2, 3 e 4 mm abaixo da furca. O índice de transporte e a capacidade de centralização foram analizados conforme proposto por Gambill et al. (1996). Resultados: O maior IT no sentido mesiodistal (MD) foi observado com o Reciproc® (p<0,05), enquanto que no sentido vestíbulolingual (VL) foram verificados com Reciproc®, ProTaper Gold® e ProTaper Next® (p<0,05). Os maiores valores de CC(MD) e (VL) foram observados com os sistemas BioRaCe® (p<0,05) e BioRaCe® e Mtwo® (p<0,05), respectivamente. Conclusão: Todos os sistemas promoveram transporte do canal radicular. Nenhum instrumento mostrou capacidade de centralização do preparo perfeita. O Reciproc® obteve os valores maiores de IT(MD) e (VL). O sistema BioRaCe® apresentou a maior CC(MD), enquanto que no sentido (VL) o BioRaCe® e Mtwo® mostraram capacidades similares.

Avaliação microtomográfica de canais mesiais de molares inferiores preparados com sistemas rotatórios e reciprocantes / Computed microtomography analysis of mesial canals of mandibular molars prepared with rotary and reciprocating systems

O objetivo do presente trabalho foi avaliar, em microtomografia computadorizada (CT), o preparo de canais de molares inferiores com diferentes sistemas acionados a motor. Foram selecionadas 58 raízes mesiais patentes, de diâmetro anatômico correspondente a #10. Para a avaliação em TC, um anel de aço inoxidável foi confeccionado nos moldes do suporte para amostra do microtomógrafo, para que servisse de fôrma para a inclusão das raízes em resina Duralay, a fim de padronizar a posição do espécime no escaneamento inicial e final. Os canais foram preparados com os sistemas Reciproc R25 (n=16); WaveOne Primary File (n=16); Twisted File (n=14), e HyFlex (n=12). Após serem escaneados, foram reconstruídos tridimensionalmente e avaliados quantitativamente quanto à variação de volume (mm3), área de superfície (mm2) e structure model index (SMI). Foi, ainda, realizada a avaliação qualitativa das seções transversais por terço e por quadrante (MV, ML, DV, DL), sendo avaliado o toque de paredes. Os dados paramétricos foram analisados estatisticamente pelos testes ANOVA e t para amostras pareadas (&#945;=5%). Não foi observada diferença estatística nos parâmetros quantitativos avaliados para Reciproc (142,77 76,75; 42,22 19,22; e 14,68 17,69, respectivamente); WaveOne (105,09 64,82; 29,54 19,21; 14,81 9,10, respectivamente); Twisted File (111,83 43,09; 33,31 18,40; 9,16 6,57, respectivamente), e HyFlex (151,74 149,37; 43,08 41,44; 10,80 8,52, respectivamente) (p=0,423). Dentro de cada grupo, foi observada diferença significante entre os resultados pré e pós-operatórios. O teste não paramétrico de Kruskal Wallis foi aplicado para a avaliação relativa ao toque de paredes. Foi observado que o sistema HyFlex apresentou a maior porcentagem de toques (82,3 13,1), seguido por Reciproc (81,3 16,9), Twisted File (78,3 14,4) e, por fim, WaveOne (76,9 21,7) (p>0,05). Em relação aos terços não foi observada diferença significativa (p=0,424). Os resultados da avaliação dos quadrantes intergrupo não demonstraram diferenças, porém indicaram tendência do preparo em direção à parede distal no terço cervical. Ao final, pôde-se concluir que os sistemas testados se equivalem quanto ao preparo de canais mesiais de molares inferiores; porém, nenhuma das técnicas foi capaz de tocar completamente em todas as paredes do canal radicular. / The aim of this work was to analyze, in X-ray computed microtomography (CT), the root canal preparation of mandibular molars with different engine driven systems. 58 mesial roots were selected, of anatomic diameter corresponding to #10. For the analysis in TC, a stainless steel ring was manufactured in the same shape as the brace for CT sampling, in order to serve as a mold to insert the roots in Duralay resin, so as to standardize the position of the specimen in the initial and final scanning. The canals were prepared with the Reciproc R25 (n=16), WaveOne Primary File (n=16), Twisted File (n=14) and HyFlex (n=12) systems. After being scanned, they were three-dimensionally reconstructed and quantitatively evaluated as regards the variation of volume (mm3), surface area (mm2) and structure model index (SMI). In addition, the qualitative evaluation of the cross sections by third and by quadrant (MV, ML, DV, DL) was carried out, along with the wall touching evaluation. The parametric data were statistically analyzed by ANOVA and t-tests for paired samples (&#945;=5%). No statistical difference was observed in the qualitative parameters evaluated for Reciproc (142.77 76.75; 42.22 19.22; and 14.68 17.69; respectively), WaveOne (105.09 64.82; 29.54 19.21; 14.81 9.10; respectively), Twisted File (111.83 43.09; 33.31 18.40; 9.16 6.57; respectively) and HyFlex (151.74 149.37; 43.08 41.44; 10.80 8.52) (p=0,423). Significant difference was observed within each group between the pre- and post-results. The Kruskal-Wallis non-parametric test was applied for the evaluation as regards root canal wall touching. At the end, we observed that the HyFlex system presented the highest touching rate (82.3 13.1), followed by the Reciproc (81.3 16.9), Twisted File (78.3 14.4) and, finally, WaveOne (76.9 21.7) systems (p>0.05). As for the thirds, was not observed any significant difference (p=0.424). The results of the evaluation in the intergroup quadrants did not show any difference; however, they showed that there was a preparation trend towards the distal wall in the cervical third. At the end, it can conclude that the systems tested are equivalent as regards the preparation of mesial canals of mandibular molars, but none of the techniques was capable of fully touching all root canal walls.

Avaliação microtomográfica de canais mesiais de molares inferiores preparados com sistemas rotatórios e reciprocantes / Computed microtomography analysis of mesial canals of mandibular molars prepared with rotary and reciprocating systems

O objetivo do presente trabalho foi avaliar, em microtomografia computadorizada (CT), o preparo de canais de molares inferiores com diferentes sistemas acionados a motor. Foram selecionadas 58 raízes mesiais patentes, de diâmetro anatômico correspondente a #10. Para a avaliação em TC, um anel de aço inoxidável foi confeccionado nos moldes do suporte para amostra do microtomógrafo, para que servisse de fôrma para a inclusão das raízes em resina Duralay, a fim de padronizar a posição do espécime no escaneamento inicial e final. Os canais foram preparados com os sistemas Reciproc R25 (n=16); WaveOne Primary File (n=16); Twisted File (n=14), e HyFlex (n=12). Após serem escaneados, foram reconstruídos tridimensionalmente e avaliados quantitativamente quanto à variação de volume (mm3), área de superfície (mm2) e structure model index (SMI). Foi, ainda, realizada a avaliação qualitativa das seções transversais por terço e por quadrante (MV, ML, DV, DL), sendo avaliado o toque de paredes. Os dados paramétricos foram analisados estatisticamente pelos testes ANOVA e t para amostras pareadas (&#945;=5%). Não foi observada diferença estatística nos parâmetros quantitativos avaliados para Reciproc (142,77 76,75; 42,22 19,22; e 14,68 17,69, respectivamente); WaveOne (105,09 64,82; 29,54 19,21; 14,81 9,10, respectivamente); Twisted File (111,83 43,09; 33,31 18,40; 9,16 6,57, respectivamente), e HyFlex (151,74 149,37; 43,08 41,44; 10,80 8,52, respectivamente) (p=0,423). Dentro de cada grupo, foi observada diferença significante entre os resultados pré e pós-operatórios. O teste não paramétrico de Kruskal Wallis foi aplicado para a avaliação relativa ao toque de paredes. Foi observado que o sistema HyFlex apresentou a maior porcentagem de toques (82,3 13,1), seguido por Reciproc (81,3 16,9), Twisted File (78,3 14,4) e, por fim, WaveOne (76,9 21,7) (p>0,05). Em relação aos terços não foi observada diferença significativa (p=0,424). Os resultados da avaliação dos quadrantes intergrupo não demonstraram diferenças, porém indicaram tendência do preparo em direção à parede distal no terço cervical. Ao final, pôde-se concluir que os sistemas testados se equivalem quanto ao preparo de canais mesiais de molares inferiores; porém, nenhuma das técnicas foi capaz de tocar completamente em todas as paredes do canal radicular. / The aim of this work was to analyze, in X-ray computed microtomography (CT), the root canal preparation of mandibular molars with different engine driven systems. 58 mesial roots were selected, of anatomic diameter corresponding to #10. For the analysis in TC, a stainless steel ring was manufactured in the same shape as the brace for CT sampling, in order to serve as a mold to insert the roots in Duralay resin, so as to standardize the position of the specimen in the initial and final scanning. The canals were prepared with the Reciproc R25 (n=16), WaveOne Primary File (n=16), Twisted File (n=14) and HyFlex (n=12) systems. After being scanned, they were three-dimensionally reconstructed and quantitatively evaluated as regards the variation of volume (mm3), surface area (mm2) and structure model index (SMI). In addition, the qualitative evaluation of the cross sections by third and by quadrant (MV, ML, DV, DL) was carried out, along with the wall touching evaluation. The parametric data were statistically analyzed by ANOVA and t-tests for paired samples (&#945;=5%). No statistical difference was observed in the qualitative parameters evaluated for Reciproc (142.77 76.75; 42.22 19.22; and 14.68 17.69; respectively), WaveOne (105.09 64.82; 29.54 19.21; 14.81 9.10; respectively), Twisted File (111.83 43.09; 33.31 18.40; 9.16 6.57; respectively) and HyFlex (151.74 149.37; 43.08 41.44; 10.80 8.52) (p=0,423). Significant difference was observed within each group between the pre- and post-results. The Kruskal-Wallis non-parametric test was applied for the evaluation as regards root canal wall touching. At the end, we observed that the HyFlex system presented the highest touching rate (82.3 13.1), followed by the Reciproc (81.3 16.9), Twisted File (78.3 14.4) and, finally, WaveOne (76.9 21.7) systems (p>0.05). As for the thirds, was not observed any significant difference (p=0.424). The results of the evaluation in the intergroup quadrants did not show any difference; however, they showed that there was a preparation trend towards the distal wall in the cervical third. At the end, it can conclude that the systems tested are equivalent as regards the preparation of mesial canals of mandibular molars, but none of the techniques was capable of fully touching all root canal walls.

Endodontic retreatments are time-consuming and difficult to perform. Removing the root canal filling material of the original treatment with maximum efficiency and safety is a complex task that requires a good technique, patience and perseverance. This filling material is often contaminated and prevents access to areas of the canal harboring bacteria and remnants of necrotic tissue, especially in the isthmus region and in dentinal tubules. The aim of this study was to use micro-computed tomography to evaluate two protocols for the removal of root canal filling material from curved root canals of the mesial roots of mandibular molars using a reciprocating instrument followed by two different rotary instruments manufactured with different nickel-titanium alloys. The reduction in the volume of the filling material inside the root canals was evaluated. Assessments were also made of the influence of the glide path extension on root canal obturation, the extrusion of filling material after the use of the reciprocating instrument, the volume of filling material after retreatment and its presence in isthmus areas. In addition, the use of a solvent and the agitation of a 2% sodium hypochlorite solution using either passive ultrasonic irrigation (PUI) or a plastic instrument in reciprocating motion were evaluated. A total of 40 mesial roots of mandibular molars with a mean curvature of 25 degrees were selected. The teeth were radiographed digitally in the buccolingual and mesiodistal directions, with a digital Xray system (Schick CDR; Schick Technologies, Long Island, NY), using an exposure time of 0.16 s. This was done in order to select the teeth and then compose two groups with similar characteristics to ensure their anatomic homogeneity. Group homogeneity in terms of anatomy, root canal length, curvature angle, and Vertucci classification type II and IV were confirmed and showed that the study groups were well balanced. Two groups were defined according to the retreatment protocol adopted, namely RHPui (Reciproc + Hyflex + PUI) and RMEasy (Reciproc + Mtwo + EasyClean). The teeth were instrumented with the Reciproc R25 instrument and filled using the single-cone technique and AHPlus cement. Prior to the use of this instrument, canal patency was achieved by creating a glide path with PathFile instruments, throughout the entire extension of the canal in Group RHPui, and up to 1 mm short of the total canal length in Group RMEasy. In both groups, the original filling material was removed with the Reciproc R25 instrument. Xylene was used as a solvent in Group RHPui up to the middle third, but not in the apical area, in the later steps of instrumentation with Reciproc R25. In Group RHPui, the Hyflex 40.04 instrument, made with a CM nickeltitanium alloy, was used. After that, xylene solvent was applied for one minute in the pulp chamber, followed by passive ultrasonic irrigation (PUI) with 2% sodium hypochlorite using an irrisonic instrument, applied up to 2 mm short of the working length. In Group RMEasy, the Mtwo 40.04 instrument, made with a conventional nickeltitanium alloy, was used. Xylene solvent was also used in the pulp chamber for one minute, followed by application of a 2% sodium hypochlorite solution agitated mechanically with an EasyClean plastic instrument in reciprocating motion up to the full working length. The two groups were re-obturated using the warm vertical condensation technique, with a gutta-percha cone corresponding to the final instrument, and with AHPus cement. Micro-CT scans were performed after each procedure to evaluate the volume of the remaining material, the extrusion after the use of the reciprocating instruments, the final volume of the obturation at the end of the retreatment and the filling material volume in the isthmus area, both after the original obturation and after re-obturation. The results showed that the initial filling was significantly closer to the apical foramen in Group RHPui than in Group RMEasy, after the initial treatment. After performing the retreatment protocols, the complete removal of filling material from inside the root canals was not achieved in either study group, with no statistically significant difference (P > 0.05) between groups regarding this variable. In the intra-group evaluation, a statistically significant difference (P < 0.05) was found between the procedural steps regarding the reduction in filling material volume, both in the total canal length and at its different levels, except for the middle third after HyFlex, the cervical third after Mtwo, and the middle third after EasyClean. The volume of the final filling material was significantly higher (P < 0.05) than the volume of the original filling material in both study groups. The volume of filling material in the isthmus region was significantly higher after re-obturation in both groups (P < 0.05), and this increase was greater in the RMEasy Group. There was no statistically significant difference (P > 0.05) between groups regarding the total amount of time expended in the removal of filling material (P > 0.05), namely 209.40 s for the RHPui Group and 227.40 s for the RMEasy Group. There was also no statistically significant difference (P > 0.05) between groups regarding the volume of extruded filling material, namely 0.0258 mm3 (0 0.954) in the RHPui Group and 0.0037 mm3 (0 0.565) in the RMEasy Group. The Shapiro-Wilks, Mann-Whitney, Wilcoxon, Friedman and Dunn tests were used in the data analysis. / Os retratamentos endodônticos demandam tempo e são difíceis de serem executados. Remover o material de obturação do tratamento original com a máxima eficiência e segurança é uma tarefa complexa que necessita de técnica, paciência e perseverança. Esse material de obturação muitas vezes está contaminado, e impede o acesso às áreas do canal onde permaneceram restos de tecido necrótico e microrganismos, especialmente na região do istmo e nos túbulos dentinários. O objetivo deste trabalho foi avaliar, por meio da microtomografia computadorizada, dois protocolos de desobturação de canais radiculares curvos de raízes mesiais de molares inferiores utilizando um instrumento reciprocante seguido de dois diferentes instrumentos rotatórios fabricados com diferentes ligas metálicas de níquel-titânio. Além da redução dos volumes de material obturador no interior dos canais, foram avaliados a influência do comprimento da patência do canal radicular (glide path) na obturação, extrusão de material obturador após o uso do instrumento reciprocante Reciproc R25, o volume do material de obturação após o retratamento e a presença deste em áreas de istmo. Também foi avaliado o uso de um solvente seguido da agitação do hipoclorito de sódio a 2% com ultrassom (PUI) e com um instrumento plástico acionado em movimento reciprocante. Foram selecionadas 40 raízes mesiais de molares inferiores com curvatura média de 25 graus que foram divididos em dois grupos de forma homogênea quanto ao comprimento, à curvatura e à anatomia (classes II e IV de acordo com a classificação de Vertucci) dos canais radiculares. Os dentes foram instrumentados com o instrumento Reciproc R25 e obturados com a técnica do cone único e com cimento AHPlus. Previamente ao uso do instrumento Reciproc R25, obteve-se a patência inicial do canal, ou seja, criou-se um glide path, com os instrumentos PathFile em toda a extensão do canal no Grupo RHPui e 1 mm aquém do comprimento total do canal no Grupo RMEasy. Os dois grupos foram definidos de acordo com o protocolo de retratamento adotado, a saber, RHPui (Reciproc + Hyflex + PUI) e RMEasy (Reciproc + Mtwo + EasyClean). Nos dois grupos, a remoção do material obturador original foi realizada com o instrumento Reciproc R25, sendo que no grupo RHPui foi utilizado o solvente xilol até a proximidade do terço médio, não sendo utilizado solvente no terço apical. No Grupo RHPui, utilizouse, então, o instrumento Hyflex 40.04 feito com liga de niquel-titânio CM (control memory ou de memória controlada) e, por último, utilizou-se o solvente xilol por um minuto na câmara pulpar, seguido de irrigação ultrassônica passiva (PUI) com solução de hipoclorito de sódio a 2% utilizando o instrumento Irrisonic, até o limite de 2 mm aquém do comprimento de trabalho. No Grupo RMEasy, foi utilizado o instrumento Mtwo 40.04 fabricado com liga de níquel-titânio convencional e, a seguir, também se utilizou o solvente xilol por um minuto na câmara pulpar seguido de uma agitação mecânica do hipoclorito de sódio a 2% com o instrumento plástico EasyClean em movimento reciprocante até o limite do comprimento total de trabalho. Os dois grupos foram re-obturados com a técnica de condensação vertical aquecida com cone de guta-percha correspondente ao instrumento final e com cimento AHPus. Foram realizadas microtomografias após cada procedimento para avaliar o volume de material remanescente, a extrusão após o uso dos instrumentos reciprocantes, o volume final da obturação ao final do retratamento e o volume na área de istmo, tanto após a obturação original quanto após a re-obturação. Os resultados obtidos mostraram que, no tratamento inicial, a patência do canal (ou glidepath) realizado com instrumentos Pathfile previamente ao uso dos instrumentos Reciproc R25 possibilitou que a obturação dos canais atingisse um limite significativamente (P < 0,05) mais próximo ao forame apical no grupo RHPui, onde o glidepath foi realizado em toda a extensão do canal. Em relação aos diferentes protocolos de retratamento, não houve remoção completa do material obturador do interior dos canais nos dois grupos estudados em todos os níveis avaliados, sem diferença estatisticamente significativa (P > 0,05) entre os grupos com relação a essa variável. Na avaliação intragrupo, houve uma diferença estatisticamente significativa (P < 0,05) entre os passos do procedimento em relação à redução do volume de material obturador, tanto na extensão total do canal, quanto nos seus diferentes níveis, com exceção do terço médio após o Hyflex, no terço cervical após o Mtwo e no terço médio após o EasyClean. O volume do material obturador final foi significativamente maior (P < 0,05) do que o volume do material obturador original, nos dois grupos estudados. O volume de material obturador na região do istmo foi significativamente maior (P < 0,05) após a re-obturação nos dois grupos, sendo significativamente maior (P < 0,05) no grupo RMEasy do que no grupo RHPui. Não houve diferença estatística (P > 0,05) entre os grupos com relação ao tempo total despendido na remoção do material obturador (P > 0,05), sendo 209,40 s para o Grupo RHPui e 227,40 s para o Grupo RMEasy.
