Comparação do transporte, centralização e extrusão apical de detritos após o uso de sistemas de NiTi em canais radiculares curvos

Introdução: Os objetivos do presente estudo foram comparar a extrusão apical de detritos durante o preparo apical #25 com os sistemas Twisted File Adaptive (TFA), WaveOne Gold (WOG) e técnica manual (TM), registrar o tempo de preparo; e comparar WOG com TFA em relação ao transporte do canal radicular (TCR) e centralização (CR) com diâmetros apicais #25 e #35. Metodologia: Quarenta e cinco canais mésio-vestibulares de molares inferiores foram preparados e divididos em 3 grupos (n=15) para avaliar extrusão apical. A extrusão de detritos foi quantificada subtraindo-se o peso final do peso inicial dos tubos Eppendorf. TCR e CR foram avaliados em micro-TC com os sistemas TFA e WOG (n=15). A normalidade dos valores obtidos para cada grupo foi verificada usando teste de Kolmogorov-Smirnov. Para quantificar detritos extruídos e tempo de preparo, os dados foram paramétricos, sendo utilizado o Teste ANOVA de 1 via e post hoc de Tukey’s. Para avaliar TCR e CR, o teste estatístico foi Mann Whitney e Wilcoxon (α=.05). Resultados: Em relação aos detritos, o sistema WOG foi associado com menos extrusão em comparação com TM (P<0,05) ou com o sistema TFA (P> 0,05). O tempo de preparo exigido pela TM foi significativamente maior do que o exigido pelas outras técnicas (P<0,05). Não foram encontradas diferenças estatisticamente significantes entre WOG e TFA em relação à centralização em todos os níveis, quando usado diâmetro apical #25 ou #35 (P> 0,05). Para TCR, diferença significativa foi encontrada entre TFA e WOG a 1 e 7mm com diâmetro apical #25 e #35 (P<0,05). Foi encontrada diferença significante para TFA em 4mm, quando os diâmetros apicais #25 e #35 foram avaliados no mesmo sistema (P <0,05). Conclusões: Todos os sistemas de instrumentação causaram extrusão de detritos apicais em algum grau. Material and methods: Ninety-six mesiobuccal root canals of mandibular first and second molars were randomly divide into 6 groups (n=16) according to the rotary file used: Group 1- ProTaper Next®; Group 2- ProTaper Gold®; Group 3- Mtwo®; Group 4- BioRaCe®; Group 5- WaveOne Gold®; Group 6- Reciproc®. Root canals were prepared according to manufacturer’s instructions. Cone-beam computed tomography scans were obtained before and after canal preparation. Measurements were made at five different points: 2, 3 and 4 mm from the apex and 2, 3 and 4 mm below furcation. Transportation and centering ability were analyzed following the recommendations made by Gambill et al. (1996). Results: The greatest mesiodistal (MD) transportation (T) was found for Reciproc® files (p<0.05), and the greatest buccolingual (BL) T, for Reciproc®, ProTaper Gold® and ProTaper Next® files (p<0.05). The greatest mesiodistal (MD) centering ability (CA) was found for BioRaCe® files (p<0.05), and the greatest buccolingual (BL) CA, for BioRaCe® and Mtwo® files (p<0.05). Conclusion: All systems produced root canal transportation. No file system achieved perfect CA of root preparation. Reciproc® files had the greatest MD T and BL T. BioRaCe® files had the greatest MD CA, whereas BL CA was similar for BioRaCe® and Mtwo® files. / Objetivo: Avaliar o índice de transporte (IT) e a capacidade de centralização (CC) do preparo do canal radicular com instrumentos endodônticos de níquel-titânio acionados em rotação contínua e reciprocante. Materiais e métodos: Noventa e seis canais mesiovestibulares de molares inferiores (primeiro e segundo) foram aleatoriamente divididos em 06 grupos (n=16), de acordo com o instrumento rotatório utilizado: Grupo 1- ProTaper Next®; Grupo 2- ProTaper Gold®; Grupo 3- Mtwo®; Grupo 4- BioRaCe®; Grupo 5- WaveOne Gold®; Grupo 6- Reciproc®. A técnica de preparo do canal radicular obedeceu a orientação dos fabricantes. Tomografia computadorizada de feixe cônico foi realizada antes e após o preparo dos canais radiculares. Foram estabelecidos 06 níveis para a realização das mensurações nas imagens: 2, 3 e 4 mm aquém do vértice radicular e 2, 3 e 4 mm abaixo da furca. O índice de transporte e a capacidade de centralização foram analizados conforme proposto por Gambill et al. (1996). Resultados: O maior IT no sentido mesiodistal (MD) foi observado com o Reciproc® (p<0,05), enquanto que no sentido vestíbulolingual (VL) foram verificados com Reciproc®, ProTaper Gold® e ProTaper Next® (p<0,05). Os maiores valores de CC(MD) e (VL) foram observados com os sistemas BioRaCe® (p<0,05) e BioRaCe® e Mtwo® (p<0,05), respectivamente. Conclusão: Todos os sistemas promoveram transporte do canal radicular. Nenhum instrumento mostrou capacidade de centralização do preparo perfeita. O Reciproc® obteve os valores maiores de IT(MD) e (VL). O sistema BioRaCe® apresentou a maior CC(MD), enquanto que no sentido (VL) o BioRaCe® e Mtwo® mostraram capacidades similares.

Étude comparative de trois systèmes de préparation canalaire en endodontie : Étude in vitro en micro-CT / A comparative study of three canal preparation files systems in endodontics : A Micro-Tomography-based in vitro study

Le but de cette étude est de comparer le respect de la morphologie canalaire après instrumentation à l’aide de trois systèmes de mise en forme canalaire différents : le One Shape NEW Generation®, le Wave One® et le Revo-S® grâce à une étude en Micro-tomographie assistée par ordinateur ou micro-CT (Computed Tomography). La mise en place d’une chaîne méthodologique totalement tridimensionnelle (3D) comprenant la reconstruction, le recalage et la segmentation a permis de traiter les images acquises et d’extraire les images recalées des canaux dentaires avant et après instrumentation. Les artéfacts de segmentation dus aux calcifications et aux débris dentinaires ont été traités. Une méthode d’estimation des zones non instrumentées a également été décrite.Le transport canalaire a été calculé pour chaque coupe de chaque tiers radiculaire, en comparant la position du centroïde avant et après instrumentation. La comparaison des moyennes de transport canalaire ne montre pas de différence significative entre les 3 systèmes d’instrumentation.Cette approche méthodologique en 4 parties a permis de valider un protocole d’imagerie 3D reproductible, qui pourra être appliqué in vitro en recherche endodontique dans l’analyse des effets instrumentaux. / The aim of this study is to compare the respect of the root canal morphology after instrumentation with different shaping systems (One Shape NEW Generation®, Wave One® and Revo-S®), by using Micro-Computed Tomography.We used a fully three-dimensional (3D) methodological process which involved the reconstruction, registration and segmentation. By this methodological process, images have been acquired and processed in order to extract registered canals images before and after the instrumentation. The segmentation artifacts like calcifications and debris have been taken into account. A method to estimate the non-instrumented zones is also described.The canal transportation was calculated for each slice of each root-third by comparing the position of the centroids before and after instrumentation. No significant difference was found between the three instrumentation systems when canal transport means were done.This 4-part methodological approach has enabled the validation of a reproducible 3D imaging protocol. This can be applied in vitro in endodontic research for analysis of the instrumental effects.

La Tomographie Volumique à Faisceau Conique (TVFC) est une technologie d’imagerie tridimensionnelle très pertinente à utiliser en odontologie. Notre travail a pour objectifs de montrer ses intérêts et les applications spécifiques en endodontie. Après avoir redéfini les conséquences délétères possibles per et post opératoires des traitements endodontiques et expliqué les principes de la TVFC, nous explorons dans un premier temps les effets de la préparation canalaire in vitro à l’aide de la microtomographie à haute résolution puis, dans un second temps, la problématique et les intérêts de la création de reconstructions tridimensionnelles fiables et précises. Cette dernière partie aborde les notions de traitement des images issues de TVFC avant d’expliquer la démarche adoptée pour élaborer une classification tridimensionnelle des lésions inflammatoires périapicales d’origine endodontique sous formes numérique et physique. / Cone Beam Computerized Tomography (CBCT) is a highly relevant three-dimensional imaging technology for use in dentistry. Our work aims to show its interests and specific applications in endodontics. After having redefined the possible deleterious per and post-operative consequences of endodontic treatments and explained the principles of CBCT, we first explore the effects of in vitro canal preparation, using high resolution microtomography and then, in a second time, the problematic and the interests of the creation of reliable and precise three-dimensional reconstructions. This last part deals with the notions of CBCT image processing before explaining the approach adopted to develop a three-dimensional classification of endodontic periapical lesions in digital and physical form.
