Determinants of delayed tuberculosis case findings in Makana Local Municipality, Eastern Cape

BACKGROUND: The prevalence of tuberculosis (TB) has been rapidly on the ascendency in the recent years globally due to its co-infection with HIV/AIDS. TB case finding is one of the technical pillars of the Directly Observed Treatment Short course (DOTS) TB strategy and there has been advocacy for early TB case detection to be the new focus of TB control efforts. PURPOSE: The purpose of this non-experimental study was to assess the determinants of TB case finding among pulmonary TB patients registered for treatment in Makana Municipality METHOD: A quantitative, non-experimental, cross sectional descriptive study among PTB patients registered for treatment at the primary health care clinics in Makana Municipality was done. Data collection was by self-administered questionnaires while sampling was by systematic sampling of PTB patients at five systematically selected clinics. RESULTS: Patient–related delay contributed more to total delay, in this study sample, than health system-related delay. Health system delay was found to be significantly associated with poorer finances, passive smoking history, seeking care from multiple health providers, initially visiting a non- NTCP health provider, TB stigma , overcrowding in the household and having difficulty with breathing as an initial symptom (p<0.05). CONCLUSION: There was a significant delay in TB case finding among PTB patients in this local municipality and patient related determinants contributed more than health system related determinants to this delay. There is need for the municipal TB programme to embark on an aggressive health education programme to furnish the populace with accurate information about TB, improve their health seeking behaviour and help address the issue of stigma associated with TB. / Health Studies

Summaries in English and Afrikaans / This research project was undertaken to establish what influence the lack of discipline amongst prisoners and members of Correctional Services has on the rehabilitation of offenders. A complete literature study was done to investigate the current lack of discipline amongst prisoners and members. Discipline is also brought into relationship with community based sentences such as Correctional Supervision as well as the involvement of the community in the release of prisoners. Positive rehabilitation can lead to crime prevention but the Department of Correctional Services and the community will have to take hands to reach this common goal. This subject needs further research in order to make recommendations to promote rehabilitation of off enders and to uplift the standard of discipline within the Department of Correctional Services. / Hierdie ondersoek is onderneem om die invloed van gevangenes en personeeldissipline binne die Departement Korrektiewe Dienste in verband te bring met die invloed wat dit het op die rehabilitasie van oortreders. 'n Volledige literatuurstudie is gedoen om die huidige tendens van swak dissipline onder gevangenes sowel as lede te ondersoek. Die dissipline word ook in verband gebring met gemeenskapgebaseerde vonnisse soos byvoorbeeld Korrektiewe toesig sowel as die betrokkenheid van die gemeenskap by die vrylating van oortreders. Positiewe rehabilitasie kan lei tot misdaadvoorkoming, maar dan sal die Departement Korrektiewe Dienste en die gemeenskap moet hande vat om hierdie gemeenskaplike doel te bereik. In addition, they are afforded with their primary and secondary needs in prisons. Before the lifers are released from prison, pre-release programmes are presented to them in order to facilitate their reintegration process into the community. The professional role of the correctional officer in the rehabilitation of offenders

Corrections has always been conventionally known as an occupational discipline and not a profession. The organization of corrections has been spoken of as a penitentiary, bearing in mind that the models of the buildings were meant for punishment; with correctional officers recognized as guards. The role of the guards was that of the keeper of the keys and their duty was mainly custodial in nature. The occupation was mainly attractive to white males with a record of unemployment and not much education. The occupation was stable, and did not require for any extra skill. Certain researchers believe that the selection methods for prison warders were extremely relaxed with a small amount of empirical validity. There was also thinking that a correctional officer needed a 20/20 vision and an IQ of an imbecile. Conversely, it has been perceived that a correctional officer can be the most significant individual in the offender’s life, having an influence in refining or declining the success of the different treatment programs that an offender undergoes (Josi & Sechrest, 1998, p. 3). Corrections is a human service occupation. Therefore, human service workers need to have knowledge of human behaviour and be able to assess their perspectives on any behaviour wisely and thus formulate reliable estimations. This will give them the opportunity to enjoy suitable decision making powers and formulate a trend on professional behaviour. Their knowledge of human behaviour will also strengthen the capability to foresee behaviour and give special knowledge to prevent any hesitancy(Williamson, 1990, p. 43). The role of the correctional officers (security staff) has intensely changed over the past few decades.The correctional officer is in today's corrections, expected to balance security and still be responsible for changing the behaviour of offenders constructively (Josi & Sechrest, 1998, p. 11). In order to have a comprehensive understanding of the distinctive and contradictory role of the correctional officer, it is very important to appreciate the changes that have transpired in penal philosophy all through the centuries. Throughout the research, the researcher will make an effort to capture the heritage of corrections and the evolving systems of punishment of Europe, the United States of America and South Africa and look at the applications of the rehabilitation concept and how the Correctional Officer has been utilised as a skilled and knowledgeable professional in the whole process. The aim of the research being to determine the effect that correctional and professional officers have on rehabilitation of offenders and determine the process by which the Correctional environment can be transformed to a true profession of highest integrity and competence. Objectives for the study will be: • To analyse the history and development of Corrections internationally and its philosophical background • To examine the impact of the history of rehabilitation in the South African Corrections system, from the development of the first prisons in 1652 to the demilitarisation of prisons system in the 1990s, right through to the actualisation of the South African White Paper on Corrections, 2005 • To critically examine the professional status of the Corrections occupation against other existing professions. The issues on education, training, credentialing, autonomy, code of ethics and special expertise are some of the aspects that will be looked at as the primary criterion for professions • To explain the conception of rehabilitation and its development and application both internationally and nationally • To critically look at the importance of Corrections system while highlighting the mandated role of the Correctional officer and the duality of the role of balancing security and rehabilitation. The study will contribute on the basis of knowledge in particular regard to Education and Training of Correctional Officers. Parental compliance with therapy home programmes within a school for learners with special educational needs : an exploratory study

Thesis (MSc (Rehabilitation))--University of Stellenbosch, 2005. / The school that was studied caters for learners with special educational needs in the Western Cape Province, South Africa. The learners at the school have a variety of physical, intellectual and learning disabilities. They receive rehabilitation inputs aimed at optimising their potential as school learners and as adults. These include occupational and speech therapy, physiotherapy, learning support and educational psychology. The therapists who work with them often utilise home programmes to involve the parents in their child’s therapy and to achieve carry-over from the therapy sessions. Among the professionals at the school there is currently a perception that therapy home programmes are poorly complied with and that the rehabilitation outcomes of the learners are being disadvantaged as a result. A need therefore arose to explore this issue. This study aims to determine the extent of compliance with therapy home programmes by parents and learners of this school. The study also attempted to elicit factors identified by parents and therapists as inhibitors to and/or facilitators for compliance with the home programmes. A descriptive, analytical study design was used. All therapists working at the school, as well as the parents of learners who were expected to comply with a home programme, were invited to participate. Data was collected using two self-compiled, self-administered questionnaires. A parent focus group was added later in an attempt to elicit more depth with regard to some of the issues explored. The data was analysed using quantitative and qualitative methods. Seventy-one percent of parents reported complying at levels adequate for therapeutic benefit to be achieved. However, 25% of the parents reported complying by less than 24% of the time prescribed. The barriers to compliance identified in this study include the quality of teamwork between the parents and the professionals, attitudinal barriers from both the parents and the therapists, the quality of training for the parents and practical difficulties. The family-centred, collaborative model of teamwork was recommended to both the therapists and the parents to facilitate parental input at all levels of the planning and design of the rehabilitation programme.

Investigating teaching and learning support for students with hearing impairment at a university in the Western Cape

Growing numbers of students with hearing impairment are being granted access into higher education in South Africa. However, they still remain under-represented in comparison to students with other impairments, and their needs in the teaching and learning environment in terms of human and technical support as well as communication and access to information remain under-supported. This study came into being because of the absence of research on the academic experiences of students with hearing impairment in higher education in South Africa. The intention of the study was to analyse and describe the teaching and learning (academic) experiences of students with hearing impairment at the case study university. This inquiry focused specifically on educational barriers, coping strategies, assistive technologies, curriculum accessibility as well as support services. Using a qualitative case study design, informed by an interpretive (constructivist) paradigm, purposeful sampling led to the selection of information-rich cases in order to gain insight from the authentic experiences of the students. The context of the case study was a South African university with a relatively large number of registered students with hearing impairment who use the oral method of communication. Data were generated by means of semi-structured interviews with participating students, university lecturers and a staff member from the disability unit. ATLAS.ti was used to code and analyse the data using grounded theory methods, allowing for the discovery of recurring themes. Six major findings emerged from this study, namely that all of the participants identified as belonging to the hearing rather than Deaf identity cultural paradigm, that limited curriculum transformation had taken place, existing support services were largely inadequate, a large number of barriers related to teaching and assessment were experienced, a variety of academic and personal coping strategies were used by the students to support their needs, and some critical factors for success were advocated for by the participants. From the findings and data interpretation and by making use of Bronfenbrenner’s bio-ecological model of human development as a tool, I constructed both an academic learning support framework as well as a set of practical guidelines for teaching students with hearing impairment. Thereafter conclusions were drawn and practical recommendations were made to various stakeholders in the education of students with hearing impairment. Further areas for research are also suggested. The academic learning support framework (as a model of best practice) forms part of my personal contribution to the field of research. / AFRIKAANSE OPSOMMING: Gehoorgestremdheid is wêreldwyd steeds die algemeenste kongenitale afwyking wat by babas gediagnoseer word, en in Suid-Afrika is die meeste mense met gestremdhede dié met gehoorgestremdhede. Alhoewel ’n toenemende aantal studente met gehoorverlies toegang tot hoër onderwys in Suid-Afrika verkry, is hulle steeds onderverteenwoordig in vergelyking met studente met ander gestremdhede en ontvang hulle steeds min ondersteuning, hetsy menslik of tegnies. Hierdie studie het ontstaan as gevolg van die afwesigheid van navorsing oor die persoonlike ervarings van studente met gehoorgestremdheid in hoër onderwys in Suid-Afrika. Die hoofdoel van hierdie gevallestudie was om vas te stel hoe om hierdie studente akademies te ondersteun ten einde hul tersiêre opvoeding en hul kanse om sukses te behaal en grade te verwerf, te verbeter. Die studie het veral gekyk na die opvoedkundige struikelblokke, hanteringstrategieë, ondersteunende tegnologieë, leerplantoeganklikheid, sowel as onder-steuningsdienste. Die metodologie wat vir hierdie studie gebruik is, was kwalitatief van aard. Die gebruik van doelgerigte steekproefneming het gelei tot die keuse van inligtingryke gevalle ten einde insig in die alledaagse ervarings van die studente te verkry. Die agtergrond van die studie was ʼn universiteit met 'n groot aantal geregistreerde gehoorgestremde studente wat van mondelinge/ouditiewe kommunikasiemetodes gebruik maak. Data is deur middel van semi-gestruktureerde onderhoude met hierdie studente, dosente en 'n personeellid van die eenheid vir gestremdhede gegenereer. Die data is met behulp van ATLAS.ti gekodeer en geanaliseer om die herhalendende temas te bepaal. Die ses belangrikste bevindings uit hierdie studie was dat al die deelnemers hulself met horende studente eerder as met gehoorgestremdes in die samelewing geïdentifiseer het, dat beperkte kurrikulum-transformasie plaasgevind het, dat bestaande ondersteuningsdienste grootliks onvoldoende was, dat die deelnemers ʼn groot aantal struikelblokke met betrekking tot onderrig en assessering ervaar het, dat hulle ʼn verskeidenheid akademiese en persoonlike hanteringstrategieë gebruik het om aan hulle behoeftes te voldoen en dat die kritiese faktore vir sukses deur die deelnemers self bepleit moes word. Uit die bevindinge van hierdie navorsing het die navorser 'n holistiese raamwerk, geïnspireer deur Bronfenbrenner se bio-ekologiese model vir menslike ontwikkeling, vir studente met gehoorgestremdheid in hoër onderwys ontwerp. An educational psychologist's perspective in understanding the experiences of residential youth care workers

Thesis (MEdPsych (Educational Psychology)--University of Stellenbosch, 2007. / As primary caregivers (such as residential youth-care workers) are the first teachers of children and spend much more time with the child than any other service provider, educational psychologists need to implement programmes that include primary caregivers in actively working with the youth in their care. This calls for educational psychologists to gain a rich understanding of the experiences of youth-care workers in order to work collaboratively in rehabilitating youth at risk within the youth’s ecosystem. The study attempts to explore the experiences of youth-care workers in residential facilities, using the ecosystemic perspective. A proposed outcome of the research is to contribute to the skills and interventions educational psychologists can use in collaborating with and supporting youth-care workers in residential facilities to rehabilitate youth who are at risk successfully. Qualitative research within the interpretive/constructivist paradigm was employed as the research design. Research was based at two residential facilities in an outlying area of Cape Town, from which four youth-care workers and two youths were drawn as a sample. The data was collected through six semi-structured interviews, observations over a 10-month period at one of the facilities, and photographs of youth-care workers and children. The review of literature and the findings of this research uncovered many experiences that appear to typify residential youth care. These experiences include the multifaceted nature of the role that youth-care workers fulfil in a residential facility; three types of emotional affect experienced by youth-care workers working in facilities; the significance of healthy youth-care worker-child relationships and the importance of these relationships in the successful rehabilitation of youth at risk; and the levels of support and training required in order to perform the task of successfully rehabilitating youth at risk in residential settings.

The primary task of this thesis is to explain the establishment of the 'correctional institution', the Ottery School of Industrues, in Cape Town in 1948 and the programmes of rehabilitation, correctional and vocational training and residential care that the institution developed in the period until 1968. This explanation is located in the wider context of debates about welfare and penal policy in South africa. The overall purpose is to show how modernist discourses in relation to social welfare, delinquency and education came to South Africa and was mediated through a racial lens unique to this country. In doing so the thesis uses a broad range of material and levels from the ethnographic to the documentary and historical. The work seeks to locate itself at the intersection of the fields of education, history, welfare, penalty and race in South Africa.

The aim of the study was to determine the extent to which selected elements of the rehabilitation components of the primary health care service package are currently being implemented. In doing so, the study aimed to identify obstacles within the district management as perceived by rehabilitation therapists that hinder the implementation of the rehabilitation programme, so as to make recommendations for future planning.
