CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / Nos Ãltimos anos, o Programa Nacional de Controle da HansenÃase tem focado suas aÃÃes em Ãreas geogrÃficas definidas com alta detecÃÃo de casos. Este estudo teve o objetivo de caracterizar padrÃes epidemiolÃgicos, espaciais e
temporais da hansenÃase em um agregado de alto risco de transmissÃo, em municÃpios dos estados do MaranhÃo, ParÃ, Tocantins e PiauÃ. Desta forma, foram aplicados diferentes mÃtodos de anÃlise espacial (Descritivo, Abordagem
Bayesiana Local, EstatÃstica Scan Espacial) e quantificada a dependÃncia espacial de diversos indicadores epidemiolÃgicos e operacionais da hansenÃase. Em outro estudo, foram identificados o fluxo de pessoas afetadas e os motivos para migraÃÃo apÃs diagnÃstico. No perÃodo de 2001 a 2009,
82.463 casos novos foram detectados no agregado (coeficiente mÃdio de detecÃÃo: 95,9/100mil habitantes ao ano). No resto do Brasil o coeficiente foi 21,0 (RR=4,56, IC 95%: 4,45-4,66; p<0,0001). Houve fluxo direcionado dos
pacientes com hansenÃase notificados para um municÃpio diferente da sua residÃncia. AraguaÃna, Imperatriz, Marabà e Floriano receberam um nÃmero considerÃvel de casos provenientes dos municÃpios em seu entorno. As capitais SÃo LuÃs, Teresina e BelÃm absorviam tambÃm casos vindos de outros estados. GoiÃnia e BrasÃlia se localizam distante do agregado, mas tÃm destaque pela notificaÃÃo de casos provenientes do agregado. ApÃs o primeiro diagnÃstico, 53,5% dos motivos principais de migraÃÃo foram relacionados a mudanÃas de estilo de vida. AnÃlise Scan Espacial identificou 23 agregados de elevada detecÃÃo de casos novos, a maioria localizados no Parà e MaranhÃo. Estes agregados incluÃram apenas 32% da populaÃÃo, mas 55,4% dos casos novos e 101 (27,1%) municÃpios. TambÃm foram identificados 14 aglomerados significativos para o coeficiente de detecÃÃo em crianÃas e 11 de casos novos com grau 2 de incapacidade/100mil hab. O agregado mais significativo, no centro do MaranhÃo, teve um RR de 2,24 e uma detecÃÃo anual de 10,4 casos com grau 2/100mil hab. O mÃtodo de autocorrelaÃÃo local mostrou superposiÃÃo com Ãreas de alto risco identificadas pelo mÃtodo Bayesiano Local e na anÃlise Scan Espacial. O estudo mostra que a hansenÃase à hiperendÃmica na Ãrea de estudo, sem a perspectiva de exaurir estes casos nos prÃximos anos. AlÃm de diagnÃstico tardio em um nÃmero de casos considerÃvel, houve lacunas na descentralizaÃÃo do atendimento, evidenciado pelo fluxo das pessoas afetadas. A construÃÃo de mapas utilizando outros indicadores, alÃm do coeficiente de detecÃÃo geral, e a sobreposiÃÃo desses mapas pÃde identificar Ãreas desconhecidas em relaÃÃo ao risco de transmissÃo e de detecÃÃo de casos com incapacidades avanÃadas. Essa abordagem poderà ser aplicada em outras Ãreas de risco para assim identificar agregados mais especÃficos de risco elevado para a hansenÃase. / In recent years, the National Leprosy Control has focused its actions on defined geographic areas with high leprosy detection rates. This study aimed to characterize epidemiological, spatial and temporal patterns in a high risk
leprosy cluster in municipalities in the states of MaranhÃo, ParÃ, Piauà and
Tocantins. Different methods of spatial analysis were applied (Descriptive, Local Bayesian Approach, Spatial Scan Statistics), and the spatial dependence of various epidemiological and operational indicators was quantified. In an
additional study, I identified the flow of leprosy-affected individuals, and the
reasons for migration after diagnosis. In the period 2001â2009, 82,463 new
cases were detected in the endemic cluster (mean detection rate: 95.9/100mil
inhabitants per year). In the rest of Brazil, the mean rate was 21.0 (RR=4.56,
95% CI: 4.45 - 4.66; p<0.0001). There was a directed flow of patients who were reported by a municipality other than their residence. AraguaÃna, Imperatriz, Marabà and Floriano notified a considerable number of cases from the municipalities in the proximity. SÃo LuÃs, Teresina and BelÃm received also
cases from other states. GoiÃnia and BrasÃlia are distant from the cluster, but
reported cases from the cluster. After first diagnosis, in 53.5% of cases
migration was related to lifestyle changes. Spatial Scan analysis identified 23
clusters of high detection rates, most of them located in Parà and MaranhÃo.
These clusters included only 32% of the population but 55.4% of new cases,
and 101 (27.1%) municipalities. There were also 14 significant clusters for high detection rates in children, and 11 clusters of new cases with grade 2
disabilities/100.000 population. The most significant cluster, in the centre of
MaranhÃo, had a RR of 2.24 and an annual detection of grade 2 cases of
10.4/100.000 population. The local auto-correlation method showed overlapping with high-risk areas identified by Local Bayesian and Spatial Scan Statistics. The study shows that leprosy is hyperendemic in the study area, without an expected trend of reduced detection rates in the coming years. In addition to late diagnosis in a considerable number of cases, there were shortcomings in the decentralization of the health system, evidenced by the flow of affected people. The use of maps based on other indicators than detection rates and the overlap of these maps highlighted previously unknown risk areas for transmission and for cases with advanced disabilities. This approach can be applied in other endemic areas to identify clusters of high risk for leprosy.

nÃo hà / Os aspectos espaciais e epidemiolÃgicos envolvidos na deficiÃncia ainda nÃo foram abordados de modo expressivo nas pesquisas demogrÃficas e de saÃde no Brasil. Realizar estudos sobre a prevalÃncia das problemÃticas inerentes à real situaÃÃo dessas pessoas à imprescindÃvel por possibilitar conhecer seu perfil de saÃde e condiÃÃes de vida. Teve-se como objetivo geral analisar a distribuiÃÃo espacial das pessoas com deficiÃncia no municÃpio do Crato-CE e, especificamente, identificar as pessoas com pessoas com deficiÃncia, descrever o perfil epidemiolÃgico das pessoas com deficiÃncia e distribuir espacialmente essas pessoas para descriÃÃo dos eventos e localizaÃÃo de Ãreas de sobre-risco. Estudo do tipo transversal com abordagem quantitativa, elaborado de janeiro de 2010 a outubro de 2011. Na primeira fase promoveu-se capacitaÃÃo prÃvia dos ACS mediante aplicaÃÃo do plano de capacitaÃÃo para desenvolvimento de competÃncias relativas à assistÃncia à pessoa com deficiÃncia, passo metodolÃgico de suporte à coleta de dados. Neste momento utilizaram-se instrumentos para a avaliaÃÃo de competÃncias do ACS. Na segunda fase os ACS identificaram as pessoas com deficiÃncia visual, auditiva e fÃsica residentes em suas Ãreas de abrangÃncia utilizando ficha de cadastro. Na terceira fase os domicÃlios das pessoas com deficiÃncia foram georreferenciados. Na quarta os dados nÃo grÃficos foram submetidos à anÃlise estatÃstica atravÃs do programa SPSS versÃo 18. Foram realizados teste Qui-Quadrado e Qui-Quadrado para dados emparelhados nos dados relativos ao perfil do ACS e desenvolvimento de competÃncias. Descreveu-se o perfil das pessoas com deficiÃncia e calculou-se a razÃo de prevalÃncia das variÃveis socioeconÃmicas. Os dados grÃficos foram analisados mediante o software Arcgis versÃo 9.2., empregando-se estimador de intensidade de Kernel. Desse modo, buscou-se verificar a intensidade dos eventos nas Ãreas adstritas a fim de identificar Ãreas de sobre-risco. No decorrer da pesquisa considerou-se a ResoluÃÃo 196/96 inerente à pesquisa envolvendo seres humanos. Nos resultados identificou-se o perfil das PcD como homens em idade produtiva, que moram em zona rural, tÃm baixa instruÃÃo e renda familiar, procedentes do municÃpio de Crato. Na distribuiÃÃo espacial evidenciou-se maior prevalÃncia em zona rural, com maior nÃmero de residentes nos distritos de Dom Quintino, Bela Vista, Campo Alegre, Ponta da Serra e Ãreas urbanas de menor indicador socioeconÃmico. A cobertura dos serviÃos de saÃde à satisfatÃria em relaÃÃo à ESF, mas os serviÃos de reabilitaÃÃo, apoio diagnÃstico, marcaÃÃo de exames e consultas especializadas e os serviÃos ambulatoriais e hospitalares estÃo concentrados na zona urbana. Desse modo, como se supunha, as caracterÃsticas daqueles com deficiÃncia resultam em um perfil que dificulta o acesso e a acessibilidade aos serviÃos de saÃde de reabilitaÃÃo, especialidades mÃdicas e de Ãmbito hospitalar. Ademais, o perfil à sugestivo de pessoas com exacerbada dependÃncia dos serviÃos pÃblicos, portanto, à preciso se criar estratÃgias de mobilidade para promover o acesso aos serviÃos de saÃde. E, ainda: os ACS sÃo sensÃveis a este pleito, mas carecem de medidas adequadas para prover cuidados de promoÃÃo de saÃde na perspectiva do seu papel de mediador. / The spatial and epidemiological aspects involved in disability have not been meaningfully addressed yet in demographic and health research in Brazil. Research on the prevalence of the problems inherent in these peopleâs actual situation is fundamental because this permits knowledge on their health profile and living conditions. The general aim was to analyze the spatial distribution of disabled people in Crato-CE, while the specific aim was to identify disabled people, describe their epidemiological profile and the spatial distribution of these people to describe the events and location of excess risk areas. Cross-sectional study with a quantitative approach, elaborated between January 2010 and October 2011. In the first phase, Community Health Agents (CHA) received previous training through the application of the training plan for competency development in care delivery for disabled people, a methodological phase to support data collection. At that moment, instruments were used to assess the CHAsâ competences. In the second phase, the CHAs identified people with visual, hearing and physical impairments living in their coverage area through the registration forms. In the third phase, the disabled peopleâs homes were georeferenced. In the fourth phase, non-geographical data were submitted to statistical analysis in SPSS version 18. Chi-squared and chi-squared tests for paired data were applied to data on the CHAsâ profile and competency development. The profile of disabled people was described and the prevalence rates of socioeconomic variables were calculated. Graphical data were analyzed in Arcgis version 9.2., using Kernelâs intensity estimator. Thus, the goal was to verify the intensity of events in the areas concerned in order to identify excess risk areas. The research complied with Resolution 196/96, concerning research involving human beings. In the results, the disabled peopleâs profile was identified as men of productive age, living in rural areas, with low education and family income levels, coming from the city of Crato. In the spatial distribution, higher prevalence levels were evidenced in rural areas, with a larger number of residents in the Dom Quintino, Bela Vista, Campo Alegre, Ponta da Serra districts, as well as in urban areas with lower socioeconomic indicators. Health service coverage is satisfactory concerning the FHS, but rehabilitation, diagnostic support, test and specialized appointment scheduling and outpatient and hospital services are concentrated in the urban area. Thus, as supposed, the characteristics of the disabled people result in a profile that hampers the access to and accessibility of rehabilitation services, medical specialties and hospital services. Moreover, the profile suggests people with increased dependence on public services. Therefore, mobility strategies need to be created to enhance health service access. And, also: the CHAs are sensitive to this plea, but lack adequate measures to deliver health promotion care within the perspective of their mediating role.

Poluição, internação e renda: uma abordagem espacial no município de São José dos Campos / Pollution, hospitalization and income: a spatial approach in São José dos CamposMauro, Antonio Carlos Cortez 15 December 2014 (has links)
Introdução - São José dos Campos é um importante centro econômico e polo industrial, situado em uma região de vale e baixa dispersão atmosférica. Essas características são sugestivas de importante poluição do ar local, a qual pode afetar a saúde de populações mais vulneráveis, como a de idosos. Considerando que hospitalização de idosos pode estar relacionada também a fatores de risco, como a exposição à poluição em decorrência do local de moradia e à renda familiar, este trabalho propõe um estudo da distribuição espacial dos locais de habitação de idosos internados por doenças respiratórias e cardiovasculares, que podem ser associadas a processos da poluição do ar, verificando a existência de distribuição aleatória ou formação de agrupamentos e a correlação com a renda familiar predominante nos setores socioeconômicos do Município. Casuística e Métodos - Foi elaborado um banco de dados registrando os endereços, com coordenadas geográficas, de pacientes idosos internados por doenças cardiovasculares e respiratórias no Hospital Municipal de São José dos Campos, no ano de 2010. Foi realizada a espacialização desses endereços sobre o mapa da área urbana, bem como a espacialização da Taxa de Internação desses idosos sobre o mapa dos setores socioeconômicos do Município. Analisou-se a distribuição espacial da Taxa de Internação, utilizando-se estatística descritiva e Índice de Moran. Foi analisada a distribuição geográfica dos locais de habitação dos pacientes internados, aplicando-se o Estimador de Kernel. Resultados - Foram identificadas 1.200 internações de idosos por doenças respiratórias e cardiovasculares. O Índice de Moran, com base na divisão socioeconômica e taxas de internação do Município, não identificou agrupamentos de setores. Porém, a aplicação do Estimador de Kernel na espacialização dos locais de habitação permitiu a visualização de agrupamentos no entorno de importante autoestrada e fonte emissora de poluentes. Conclusões - A existência de agrupamentos de habitações de pacientes internados, próximos à principal rodovia que atravessa o Município, sugere correlação com a poluição por emissões veiculares. Não foram observadas evidências conclusivas sobre a correlação espacial entre taxa de internação e renda. O Inventário de Emissões de Poluentes Atmosféricos do Município, através da caracterização das fontes de poluição, corroborou a identificação de fatores de risco sanitário e ambiental / Introduction - São José dos Campos is an important economic and industrial hub center, located in a valley and region of low atmospheric dispersion. These features are suggestive of important local air pollution, which can affect the health of vulnerable populations such as the aged. Whereas hospitalization of the aged may also be related to risk factors such as exposure to pollution from the place of residence and family income, this thesis proposes a study of the spatial distribution of housing sites for aged patients hospitalized for respiratory and cardiovascular diseases which may be associated to air pollution processes, checking for random distribution or formation of clusters and the correlation with the predominant household income in socioeconomic sectors of the Municipality. Casuistry and Methods - A database was developed registering addresses with geographical coordinates of aged patients hospitalized for cardiovascular and respiratory diseases at São José dos Campos Municipal Hospital, in the year 2010. The spatial distribution of these addresses was performed on the map of the urban area as well as the spatial rate of hospitalization of the aged on the socio-economic sectors map of the Municipality. The spatial distribution of the hospitalization rate was analyzed, using descriptive statistics and the Moran\'s Index. The geographic distribution of housing sites of hospitalized patients was analyzed by applying the Kernel Estimator. Results - 1,200 hospitalizations of aged people were identified for respiratory and cardiovascular diseases. The Moran\'s Index, based on socioeconomic division and admission rates to the municipality, not identified groups of sectors. However, the application of Kernel Estimator for interpolation of local housing allowed the visualization of clusters around the major highway and emission source of pollutants. Conclusions - The existence of clusters of dwellings inpatients, near the main highway that runs through the municipality, suggests correlation with pollution from vehicle emissions. No conclusive evidence on the spatial correlation between hospitalization rate and income were observed. The Emission Inventory of Air Pollutants of the Municipality, through the characterization of pollution sources, corroborated the identification of health and environmental risk factors

Living Arrangements, Intergenerational Dynamics, and Psychological Well-being of Elders: An Examination of Predictors of Elder Depression in Retired Persons in Yancheng, Jiangsu, ChinaWang, Ying 16 November 2009 (has links)
This study explores the relationship between living arrangement and psychological wellbeing in retired elderly individuals living in Yancheng, Jiangsu (PR China). Data on mode of residence, socio-economic background, daily activities, and intergenerational dynamics were collected from 200 subjects, and their potential correlations with depression (assessed via the Geriatric Depression Scale Short Version) were analyzed. Univariate as well as logistic regression confirmed mode of residence as a significant predictor of depression in this group. The following depression odds ratios associated with each mode of residence were derived via logistic regression: 1) nuclear household, i.e. living with a spouse only ¨C 1 [reference category], 2) multigenerational households in which a spouse is not present ¨C 4.341, 3) multigenerational households in which a spouse is present ¨C 0.781, and 4) living alone ¨C 3.018. Based on these ratios, we conclude that the traditional model of intergenerational coresidence is not, in itself, associated with less depression. Rather, it is the presence of a sharing spousal in a household (whether single or multigenerational) that protects against elderly depression. Other predictors of depression identified in backward logistic regression included presence of a chronic illness and self assessed wealth status. Additionally, a number of psychosocial variables were identified as independently correlated with depression, but were subsequently selected out by multivariate analysis. These included: educational background, religious affiliation, membership in an organization, attitude toward aging, and family status. Based on this study, we believe that efforts to promote mental wellbeing among today¡¯s Chinese elders should be directed toward psychosocial factors that are modifiable (education, building supportive social networks etc.) rather than insisting on the traditional ideal of multigenerational living and dependence on filial piety.

The food retail environment surrounding Canadian schools and its impact on overweight and obesitySeliske, Laura Marie 01 October 2007 (has links)
Background: The prevalence of overweight and obesity in Canadian youth has increased considerably over the past few decades. In spite of considerable efforts, individual-level behavioural modification strategies have not been successful at preventing and treating obesity in youth. Research is emerging that has shifted attention to the environmental-level determinants of obesity. There is some evidence that the number and types of food retailers in a given neighbourhood may impact obesity risk in the individuals living in that neighbourhood, but little is known about the impact of the food retail environment on Canadian youth.
Objectives: 1) To describe the food environment surrounding the school neighbourhoods of Canadian youth (grades 6 to 10), and, determine whether access to food retailers around schools differs by area-level SES. 2) To determine whether the number and type of food retailers surrounding schools is related to the overweight status of the students attending those schools. Each of these objectives was explored in a separate manuscript.
Methods: A trend test was performed to determine if exposure to food retailers varied by neighbourhood socioeconomic status (SES) for 188 schools across Canada. Logistic regression was performed using a multilevel modeling approach to determine if an association existed between exposure to food retailers and overweight and obesity in the 7,281 school-aged youth attending the 188 schools, adjusting for individual- and area-level covariates. Types of food retailers considered included: full-service restaurants, fast food restaurants, sub/sandwich shops, donut/coffee shops, convenience stores, and grocery stores.
Results: Objective 1. Nearly 75% of schools had at least one food retailer within 1 km of schools, and over 90% had at least one food retailer within 5 km. Exposure to the different types of food retailers within 1 km of schools did not vary significantly (p<0.05) across schools in low, moderate, and high SES neighbourhoods, with the exception of full-service restaurants. At the 5 km distance, the SES gradient was significant across all food retailer types, with low SES neighbourhoods exposed to the least number of food retailers. This effect disappeared when population density (number of retailers per 10,000 people) was considered, except for sub/sandwich shops and donut/coffee shops.
Objective 2. Increased exposure to all six types of food retailers in the neighbourhoods surrounding schools was associated with a decreased likelihood of overweight and obesity. At the 1 km distance, the total number of food retailers had the strongest protective effect, while individual types of food retailers had a stronger effect at the 5 km distance.
Conclusions: Objective 1. Most students in Canada have at least one food retailer within walking distance of their school. The food retail environment surrounding schools is not significantly impacted by the neighbourhood SES. Objective 2. Increased exposure to different types of food retailers in school neighbourhoods is associated with a decreased likelihood of overweight and obesity in Canadian youth. O objetivo deste trabalho foi determinar a flutuação populacional e a relação com fatores meteorológicos e a distribuição espacial de A. gemmatalis e de C. includens em cultivares de soja com diferentes ciclos de desenvolvimento, visando gerar informações sobre o comportamento dessas lagartas na cultura da soja e elaborar um plano de amostragem sequencial para a cultura da soja. Os experimentos foram conduzidos nos anos agrícolas 2013/14 e 2014/15 na FCAV/UNESP, Jaboticabal, SP. Foram selecionados três campos, e em cada um foi demarcada uma área de 8.000 m2, sendo cada área subdividida em 80 parcelas de 100 m2 (10 m x 10 m). As cultivares de soja utilizadas foram: SYN 1365 RR (precoce), M 7908 RR (média) e BRS Valiosa RR (tardia). As amostragens foram realizadas semanalmente pelo método da batida de pano, em 2 m de linha de plantas, registrando-se o número total de lagartas maiores que 1,5 cm de A. gemmatalis e C. includens. Para o estudo da flutuação populacional das espécies de lagartas da soja os resultados foram submetidos à analise estatística, efetuando-se correlação de Pearson, entre o número de espécies de lagartas e as variáveis meteorológicas, precipitação e temperatura. The objective of this work was to study the population fluctuation and spatial distribution of A. gemmatalis and C. includens in soybean cultivars of different maturing cycles in order to help the development of a sequential sampling plan. The experiments were carried out in seasons 2013/14 and 2014/15 at FCAV / UNESP, Jaboticabal, SP. Three fields were selected, and on each one an area of 8.000 m² was marked and divided into 80 splits of 100 m² (10 m x 10 m). The soybean cultivars of different maturing cycles were: SYN 1365 RR (early), M 7908 RR (middle) and BRS Valiosa RR (late). Samples were taken weekly using the ground-shake method, in two m line of plants, where it was recorded the number of caterpillars A. gemmatalis and C. includens higher than 1.5 cm. Previous issue date: 2007 / Resumo: A compreensão da epidemia do HIV e da Aids em suas diferentes dimensões socioeconômica, cultural, biológica e política entre outras representa, ainda, um grande desafio para a Saúde Pública no Brasil em mais de vinte anos de doença, no país e no mundo. A dinâmica atual da epidemia da Aids atinge de forma diferenciada populações em seus "habitat", e caracteriza-se pelas tendências de "heterossexualização", 'feminização", "interiorização", "pauperização" e, mais recentemente, acometimento de faixa etária mais idosa, concentrando-se nas populações marginalizadas e vulneráveis. Foram georreferenciados os endereços dos indivíduos maiores de 13 anos notificados com Aids no município de Campinas, no período de 1980 a 2005. Utilizou-se o estimador de densidade de pontos kernel para investigar aglomeração de casos. Foram georreferenciados 4.175 casos de Aids, 90% do total notificado pelo Sistema de Vigilância Epidemiológica, segundo as variáveis sexo e faixa etária em diferentes períodos da epidemia. Os mapas indicam uma crescente dispersão dos casos para áreas urbanizadas da periferia da cidade no decorrer dos anos. A distribuição espacial dos casos de Aids em mulheres é diferente da observada entre os homens, evidenciando padrões distintos no tempo e espaço. Embora seja um estudo exploratório que não pondera as ocorrências pela população sob risco, a distribuição dos casos em maiores de 50 anos concentra-se em áreas centralizadas do mapa da cidade. A incorporação de técnicas de geoprocessamento e análise espacial pode contribuir com as ações de prevenção e controle da epidemia por parte do Programa Municipal de DST/Aids e Vigilância Epidemiológica do município de Campinas / Abstract: Understanding the different socio-economic, cultural, biological, and political dimensions of the HIV and Aids epidemic is still a large challenge for the Brazilian Health System after more than twenty years of the disease in the country and the world. The current dynamic of the Aids epidemic touches people in different ways in their "habitats", and is characterized by trends such as heterosexualization, feminization, interiorization, pauperization, and more recently has attacked a much older age band, concentrating in marginalized and vulnerable populations. Addresses were georeferenced for over >13-year-old individuals registered with Aids in Campinas municipal area between 1980 and 2005. A kernel density estimator for points was used to investigate case agglomeration. A total of 4175 Aids cases were georeferenced, 90% of the total notified by the Epidemiological Monitoring System, according to gender and age group in different periods of the epidemic. The maps indicated increasing case dispersion in urbanized peripheral areas of the city as the years elapsed. Spatial distribution of aids cases in women is different to men, showing distinct time and space profiles. Although this was an exploratory study which did not consider occurrences for the at risk population, the distribution of >50-year-old cases is concentrated in centralized areas of the city.
