Consumo de álcool e outras drogas, sintomas depressivos, impulsividade e aspectos dimensionais de personalidade em homens biológicos profissionais do sexo / Alcohol and other drug consumption, depressive symptoms, impulsiveness and dimensional aspects of personality among male sex workers

Introdução: Os homens biológicos profissionais do sexo (male sex workers MSW) constituem uma população heterogênea, de forma que uma classificação tipológica poderá promover o desenvolvimento de políticas públicas mais específicas. Embora existam diversas classificações para o homem que se prostitui nas ruas das cidades (street hustlers), há poucos estudos que avaliam aspectos psicopatológicos específicos desses indivíduos. Este estudo examina aspectos dimensionais da personalidade, impulsividade, uso de álcool e drogas e envolvimento com atividades criminais entre street hustlers da cidade de Santo André/SP. Método: Trata-se de um estudo seccional e retrospectivo, realizado pelo Ambulatório de Transtornos da Sexualidade da Faculdade de Medicina do ABC (ABSex), em associação com o Grupo Interdisciplinar de Estudos de Álcool e Drogas do Instituto de Psiquiatria da Faculdade de Medicina da Universidade de São Paulo (GREA Ipq FMUSP). Oitenta e seis sujeitos foram avaliados por meio de questionários autoresponsivos, sendo as entrevistas realizadas nas ruas de Santo André/SP, no próprio local de trabalho dos entrevistados, entre 2008 e 2010. Resultados: MSW portadores de transtornos de identidade de gênero (TIG) apresentaram maiores níveis de Esquiva ao Dano, mais frequente história de agressão física por parte dos clientes e menor oportunidade de outros empregos quando comparados com MSW sem TIG. MSW com uso inconsistente de preservativos apresentaram mais problemas com uso de álcool e drogas, maiores níveis de Dependência de Gratificação, mais frequente história de tentativas prévias de suicídio e antecedente criminal quando comparados a MSW com uso consistente de preservativos. Conclusões: A prostituição masculina é um fenômeno composto de múltiplos aspectos. Há múltiplas variações em termos de motivações e padrões de vida entre os MSW, de forma ser necessário dividi-los em categorias. A identidade e papel de gênero, uso de substâncias psicoativas e aspectos de personalidade são variáveis importantes a serem consideradas quando do desenvolvimento de políticas de saúde pública mais específicas. A habilidade em avaliar tais características pode ser ferramenta importante no desenvolvimento de propostas mais efetivas para o manejo dessa complexa população / Introduction: Male sex workers (MSW) are a heterogeneous population, and researchers aim to categorize them into typology groups to develop more specific public health policies. Although there are several classifications for MSW who work on streets (street hustlers), there are few studies that evaluate specific psychopathology aspects of these individuals. This study examines the dimensional aspects of personality, impulsiveness, alcohol and drug use, and criminal history among street hustlers in the city of Santo André/SP. Method: This is a cross-sectional and retrospective study, carried out by Ambulatory for the Treatment of Sexual Disorders of ABC Medical School (ABSex), in association with the Interdisciplinary Group of Studies on Alcohol and Drugs, Psychiatry Institute Faculty of Medicine - São Paulo University (GREA - IPQ - FMUSP). Eighty-six subjects were assessed through self-report questionnaires. The interviews were performed on the streets of Santo André/SP, in the workplace of the MSW, from 2008 to 2010. Results: MSW with gender identity disorder (GID) showed higher Harm Avoidance levels, more frequent history of physical aggression by clients and less opportunity of other jobs when compared with street MSW without GID. MSW with inconsistent condom use showed more alcohol and drug use problems, higher levels of Reward Dependence, higher frequency of personal history of suicide attempts and criminal history. Conclusions: Male prostitution is a phenomenon composed of multiple aspects. Variations of personal motivations and life patterns among MSW are huge. So it is necessary to divide them into clusters. Identity and gender roles, substance abuse, personality aspects, like Harm Avoidance and Reward Dependence, are important variables to be considered for the development of more specific public health policies. The ability to assess such characteristics may be an important tool in developing proposals for more effective management of this complex population

Promoting Positive Development: Family Processes and Risk Behavior Among Adolescents

The present study is designed to address the problem of risk behaviors among adolescents, in an effort to promote positive developmental trajectories. Previous studies have resulted in divergent findings pertaining to the predictors of adolescent engagement in risk behaviors. In addition to considering this divergence, the focus of the study is the nature of bidirectional individual ó contextual relationships and their influence on adolescent engagement in risk behaviors. The study tested two models that considered whether parent-adolescent relationship or peer relationship mediated the relation between theory and research-based predictors and the endogenous variable, co-occurring substance use and sexual activity. Participants were 396 demographically diverse multi-problem adolescents from an archived dataset derived from an HIV risk reduction outpatient treatment program for alcohol and other drug use. Participants responded to questions that measured family structure, parent-adolescent relationship quality and communication, religiosity, school connectedness, peer relationship, and engagement in substance use and sexual activity. The study found that the model with peer relationship as the mediator fit the data better than the model with the parent-adolescent relationship mediator, and that the mediated model provided a better fit to the data than direct relations between the exogenous and endogenous variables. The results suggested also that primary caregiver was not a significant predictor of adolescent participation in co-occurring substance use and sexual activity. The present study provides a holistic theoretical and conceptual framework that highlights a constellation of factors determined to contribute significantly to co-occurring substance use and sexual activity, and thereby reshape existing models of risk behavior among adolescents.

Sociodemographic Factors and Health-Risk Behaviors Associated with Recent Utilization of Dental Services Among South-Central Appalachian Adolescents

Background: Oral health is crucial for overall health. Oral health care need is the most prevalent unmet health care need among children and adolescents in the United States. Youth characteristics and socio-demographic factors are known to influence dental health service utilization. While there have been studies listing the factors contributing to the oral health disparities in the Appalachian region, very few studies have inspected the risk behaviours of the youth and their association with dental health care utilization. Methods: A structured questionnaire was administered to high school students in five counties of South-Central Appalachia. The survey included a question on a recent dental visit. The students provided information on socio-demographic characteristics and risk behaviours. Descriptive analysis was performed and the association between recent dental visit as the outcome variable and sociodemographic factors and health risk behaviours as independent variables were analyzed using logistic regression analysis. The analysis was performed using SAS 9.4. Results:80.1% (N= 129) of the adolescents who had admitted to using tobacco products or electronic vapour products reported having visited the dentist in the past 12 months. 87.5% (N= 168) of the females and 80.3% (N=127) of the males reported having visited the dentist in the past 12 months and 81.43% (N=114) of the adolescents who received free or reduced lunch reported having visited the dentist in the past 12 months. Use of tobacco products was the only health risk behaviour significantly associated with dental visit in the past 12 months (p=0.04) after adjusting for covariates with OR=0.5 (95% C I:0.312 -1.01). Conclusion: The findings suggest that health risk behaviours such as using tobacco products may be associated with dental health care utilization and future research should focus on adolescents with health-risk behaviours with the goal of identifying interventions for at-risk populations. Tuberculosis (TBC) is an infectious disease with long history, that goes to the beginnings of humanity. Nowadays, TBC is mainly creating socioeconomic issues connected with overpopulation. In 21st century, TBC remains major concern for public health in terms of infectious diseases in European region. Even though the joint efforts of political, economical and health institution are being successful, there is still need for better health care coordination, diagnosis improvement, and help to the endangered groups of people in lower income countries. The aim of this thesis is to assess the epidemiological situation of TBC incidence in the Central Bohemia region in the years from 2008 to 2017 with a special focus on impact of health and socioeconomic risk factor for developing TBC. Research questions concern population structure characteristics of TBC patients, differences between sexes and factors that affect TBC death. Methods of contingence tables, χ2 -test of independence and logistic regression were applied to analyse of the effect of age, comorbidities, and risk behaviour on the development of TBC. Stratified Multilevel Logistic Regression Modeling for Risk Factors of Adolescent Obesity in Tennessee

Background: US adolescent obesity rates have quadrupled over the past 3 decades. Research examining complex factors associatedwith obesity is limited.Objectives: The purpose of this study was to utilize a representative sample of students (grades 6 - 8) in Tennessee to determine theco-occurrence of risk behaviors with adolescent obesity prevalence and to analyze variations by strata. Methods: The 2010 youth risk behavior survey dataset was used to examine associations of obesity with variables related to sampledemographics, risk and protective behaviors, and region. Hierarchical logistic regression analyses stratified by demographics andregion were conducted to evaluate variation in obesity risk occurring on three hierarchical levels: class, school and district. Results: The sample consisted of 60715 subjects. The overall obesity rate was 22%. High prevalence of obesity existed in males, non-white race, those ever smoked and was positively correlated with age. Across three state regions, race, gender, and specific behaviors (smoking, weight misperception, disordered eating, +3 hours TV viewing, and no sports team participation) persisted as significantpredictors of adolescent obesity, although variations by region and demographics were observed. Multilevel analyses indicate that< 1%, 0 - 1.97% and4.03 - 13.06% of the variation in obesity was associated with district, school and class differences, respectively, whenstratifying the sample by demographic characteristics or region. Conclusions: Uniform school-based prevention efforts targeting adolescent obesity risk may have limited impact if they fail torespond to geographical and demographic nuances that hierarchal modeling can detect. Study results reveal that stratified hi-erarchical analytic approaches to examine adolescent obesity risk have tremendous potential to elucidate significant prevention insights.

Qualitatively directed research outcomes reflect motives involved in young women's final decisions to become an under-age mother, the determinants of the decision making process in the context of sexual reproductive health. They watch as the participants prepare for the role of mother, what helped them solve the situation and what is it difficult to evaluate their experience and maternity and parental competence. The thesis " Alcohol use among adolescents and its causes " explores the phenomenon of alcohol use among adolescents in the target population of students in graduation year and student which are year before graduation of secondary school, gymnasium, or equivalent grades of secondary vocational schools. The topic is first discussed in theory, presents the main concepts, with which is working in the study, as well as researchs from which it was drawn, and the author was inspired by them in his work. There is also discussed research methodolog , sampling, data collection, method which was used (SAQ), there is also description of a methodological tool and its various dimensions, as well as any changes that have occurred on the basis of the pilot study. First, the work describes descriptive results of state alcohol use in the Czech Republic among adolescents. (in English): In my work I deal with the occurrence of risk behavior of students of the 2nd grade of primary schools (in Prague 5). In the first, theoretical part of this work I deal with the explanation of concepts that relate to the issue and form the basis of the work. These theoretical findings are applied in the second, practical part, in which I am interested in whether the students of the 2nd grade are sufficiently informed about the manifestations of risk behavior, whether they are already in some form, whether in the school environment or outsider it, and also which manifestation of risk behavior is currently most pronounced in schools. In my work I also try to point out the short comics of the minimum preventive programs of primary schools. These data are read from the annual reports of primary schools, for a period of 2 years. For my work, I chose quantitative research, which includes a larger number of respondents, specifically online questionnaires. I chose students of the 8th and 9th grades of primary schools in Prague 5, because I find this target group is the most endangered by risk behavior.
