Sjuksköterskors erfarenheter av att delge negativa besked : En integrerad kunskapsöversikt / Nurses’ experiences towards breaking bad news : An integrative review

The event of breaking bad news is a subject that no one present in the practical setting could choose to avoid. It is a subject that the practitioners comes in touch with during a daily basis. Traditionally the role as the breaker of bad news has been settled by the physician, but at the time being, disagreements occur in the clinical setting about who should break bad news to familys and patients. Aim: The aim of this study was to explore the nurses’ experiences of breaking bad news. What are the nurses’ role in the event of breaking bad news, and are there any barriers that can affect the way the bad news is given to the patients and family members? These were two questions of issue that were looked into. Method: The integrative review method based out of Whittemore and Knafl (2005) was used for this review. A systematic search resulted in twelve articles that were deemed fitting for the purpose of this study. Results: This integrative review resulted in five identified roles carried out by the nurse in the event of breaking bad news. Further on, nine factors were identified as being of significance in how the bad news were delivered by nurses and received by patients and the patient’s family members. Nurses expressed a desire and a wish for some kind of education or training in how to break bad news. Conclusion:  This article contributes with five roles experienced by nurses, along with nine different factors to have in mind when carrying out difficult news. The SPIKE model and Gibbs reflective model were discussed to have a great importance in the clinical setting.

Atividades de enfermagem no Centro de Material e Esterilização: subsídios para o dimensionamento de pessoal / Nursing activities in the Sterilization Central Supply: subsidies to dimensioning of nursing staff

Diante da ausência de parâmetros que fundamentem o planejamento e a avaliação do quadro de profissionais de enfermagem nos Centros de Material e Esterilização (CMEs), este estudo, de desenvolvimento metodológico, teve por objetivos identificar e validar as atividades realizadas pela equipe de enfermagem em CMEs de instituições hospitalares, como subsídio para a definição da carga de trabalho da unidade. Para identificar as atividades desenvolvidas pela equipe de enfermagem nos CMEs, foi realizado consulta às indicações formuladas pelos órgãos oficiais e aos manuais técnicos publicados na área. Em seguida, realizou-se levantamento bibliográfico sobre o assunto descrevendo-se, para cada área de trabalho dos CMEs, as atividades consideradas pertinentes e essenciais para o desenvolvimento do trabalho. Para validar as atividades identificadas, optou-se por utilizar o método da Validade de Conteúdo, por meio da aplicação da Técnica Delphi. O instrumento para validação das atividades identificadas ficou constituído por seis áreas de trabalho, 24 subprocessos e 96 atividades, além de 18 atividades específicas da enfermeira. Para compor o grupo de juízes, optou-se por convidar enfermeiras com título de especialista ou com experiência profissional de no mínimo cinco anos na área, que atuassem em unidades de CME de instituições hospitalares da cidade de São Paulo, considerados de bom padrão de desempenho. Ao grupo de juízas, constituído por onze enfermeiras, coube avaliar a pertinência, clareza e objetividade na definição das áreas de trabalho e dos subprocessos relacionados; clareza e objetividade na descrição das atividades indicadas em cada área; representatividade das atividades descritas em cada área; realização das atividades nas áreas apontadas; necessidade de inclusão ou exclusão de atividades em cada área. Foram consideradas validadas as áreas, os subprocessos de trabalho e as atividades que obtiveram índice de concordância mínimo de 70%, em qualquer fase da Técnica Delphi. A análise dos resultados obtidos na primeira fase evidenciou que todos os itens foram validados. Entretanto, a análise dos comentários e das sugestões realizados pelas juízas determinou a realização de alterações no conteúdo do quadro de atividades proposto, que foram submetidas à nova avaliação do grupo, resultando na validação de seis áreas de trabalho, 25 subprocessos, 110 atividades e 25 atividades específicas da enfermeira. Com a realização desta pesquisa evidenciam-se perspectivas para a realização de novas investigações que contribuam para a determinação de parâmetros que subsidiem o processo de dimensionar pessoal de enfermagem nas unidades de CME / Due to the absence of parameters that support the planning and evaluation of the nursing professionals team in the Sterilization Central Supply (SCS), this study, of methodological development, aimed to identify and validate the activities performed by the nursing staff in the SCS of hospitals, as subsidy to determine the workload of the unit. In order to identify the activities developed by the nursing staff in the SCS, indications made by official organs and technical manuals published in the area were consulted. Then, a survey of the literature about the subject was performed, reporting, to each work area of the SCS, the activities considered pertinent and essential to the development of the work. In order to validate the identified activities, the Content Validation method was used, by means of the Delphi Technique. The instrument to validation of the identified activities consisted of six work areas, 24 subprocesses and 96 activities, besides 18 specific activities of the nurse. In order to compose the group of judges, nurses with specialist title or with minimum of five years experience in the area were invited, and who worked in SCS units of hospitals in the city of São Paulo, considered of good standard of performance. The group of judges, consisting of eleven nurses, was in charge of evaluating the pertinence, clarity and objectivity in the definition of the work areas and of the related subprocesses; clarity and objectivity in the description of the indicated activities in each area; representativeness of described activities in each area; performance of the activities in the pointed areas, need of inclusion or exclusion of activities in each area. The areas, subprocesses of work and activities considered validated were those that obtained index of agreement minimum of 70% in any phase of the Delphi Technique. The analysis of the obtained results of the first phase showed that all items were validated. However, the analysis of comments and suggestions performed by the judges determined changes in the content of the proposed activities, which were submitted to new evaluation of the group, resulting in the validation of six work areas, 25 subprocesses, 110 activities and 25 specific activities of the nurse. This research shows perspectives to new investigations that contribute to determination of parameters that subsidize the process of dimensioning of nursing staff in the units of SCS

Det var i första hand viktigt att få en fungerande kommunikation med patienten, men de anhöriga fick inte heller glömmas bort utan spelade en stor roll i omvårdnaden. För att utvecklas i sin roll som sjuksköterska i arbetet med anorexiapatienter var även kommunikationen med kollegor viktig. I bemötande av patienter med anorexia nervosa var det viktigt att se patienten utifrån en helhetssyn, att kunna bemästra svårigheter, att kunna bygga upp ett förtroende mellan sig själv och patienten samt att ha den rätta personligheten. Pasaulyje, taip pat ir Lietuvoje kasmet vis didėja stomuotų pacientų skaičius. Lietuvoje per pastarąjį dešimtmetį pagerėjo chirurginė technika, stomos priežiūros priemonės, tačiau pacientų paruošimas stomos suformavimo operacijai ir pooperacinė priežiūra vis dar nepakankama. Stomos formavimo operacijos ir toliau išlieka aktualia problema fizinėse, socialinėse, finansinėse, seksualinėse ir psichologinėse gyvenimo srityse. Kol kas dar mažai žinoma apie pacientų mokymą, apie slaugytojo vaidmenį, rengiant pacientus stomos formavimo operacijai, nors pastaruoju metu į tai yra atkreipiamas vis didesnis dėmesys. Būtent todėl mes ir nutarėme įvertinti slaugytojo vaidmenį rengiant pacientus stomos formavimui. Tyrimo tikslas - Įvertinti slaugytojo vaidmenį rengiant pacientus gyvenimui su stoma. Tyrimo uždaviniai. Nustatyti slaugytojo vaidmenį parenkant optimalią stomos vietą. Nustatyti slaugytojo vaidmenį informuojant pacientus apie stomą prieš operaciją. Although there has been a considerable progress in surgery techniques and stoma care instruments in Lithuania during the last decade, the preparation of patients for stoma surgery and after-surgery care is still insufficient. Stoma surgery still remains an urgent issue in the physical, social, financial, sexual and psychological aspects. So far the investigation of patients education and the role of medical nurses in preparing patients for stoma surgery was scarce, although lately it is receiving more attention. Role sestry při poskytování péče pacientům - cizincům / Nurse´s role in providing care to patients - foreigners

The theme of this thesis is the role of nurse providing care for patients-foreigners. The thesis was divided into two parts. In the theoretical part it dealt with the issue of providing nursing care to patients-foreigners which sought to highlight the roles of sister, their problem situations and state of readiness to care for these patients. They were set three objectives of this work. The first objective was to determine the roles of nurse which are dominant in their care of the patients- foreigners The second objective was to map the problem situations of nurses in providing care to patients foreigners. The last objective was to map the state of readiness of nurses in patient care of patients-foreigners. In the empirical part were set out five hypotheses. The first hypothesis was that the most common role of nurses in patients-foreigners care is the role of nursing care provider. The second hypothesis claimed that the most problem role of nurse in patients-foreigners care was the role of educator. The third hypothesis was that the greatest obstacle in providing nursing care to patients- foreigners is the language barrier. The fourth hypothesis was that the most common problem in providing nursing care to patients- foreigners is a social isolation. The fifth hypothesis was based on the assumption that nurses are theoretically prepared to care for the patients-foreigners. Based on the evaluation of an anonymous questionnaire the first, third and fourth hypothesis is confirmed. On the contrary, the second and the fifth hypothesis were not confirmed. In the first part of the research investigation was used a quantitative data collection method in the form of an anonymous questionnaire for general nurses to hospital beds stations in Hospital in České Budějovice joint-stock company, Hospital Písek joint-stock company, Hospital Strakonice joint stock company and Hospital Tábor joint-stock company. The questionnaire contained 36 closed, semi-open and open questions. In the second part of the research were used descriptive statistical functions. To evaluate statistically significant relationships were used PivotTable (Crosstabs). In total, 300 questionnaires were distributed of which the final research survey used the 226. Survey results were summarized in graphs and tables. Discussions have been confronted with the literature with the expression of the author's own opinion. At the end of the work were given the confirmed and unconfirmed hypotheses and the overall summary of work with possible suggestions to improve the quality of patients-foreigners care.

Återhämtning Efter Stroke : En Litteraturstudie Om Sjuksköterskans Erfarenheter

ABSTRACT Background: Every year, 30,000 people in Sweden suffer from stroke. The disease leads to disability and patients with stroke are in need of recovery process. Previous research highlighted patients 'and relatives' experiences after stroke. The accumulated knowledge clarifies which nursing measures nurses use to support patients' recovery process. Purpose: to describe the nurse's experience in supporting the patient's recovery after a stroke. Method: Systematic literature study with descriptive synthesis. Results: It emerged that seeing the patient as unique was significant, and to have a functioning recovery process, patients must be given sufficient time and be well informed. Showing honesty also resulted in nurses being able to build trust with patients and relatives. The nurses' support in the form of guidance and individually adapted information as well as physical, emotional, social support was important for patients. A positive communication between patients and nurses contributed to a good care relationship that resulted in success in the recovery process, thus relatives were a resource for patients' recovery process. Conclusion: Patients suffering from stroke may suffer and experience pain after the life change. Patients can also experience well-being in their health, this by having a well-functioning recovery process. Nurses with their unique role and professional knowledge can support the recovery process.

Patienters upplevelse av rehabilitering efter stroke : en litteraturstudie / Patients' experience of rehabilitation after a stroke : a literature review

Bakgrund: Stroke är en av det vanligaste folksjukdomar i Sverige. De som överlever      strokeanfall drabbas av varierande grad av aktivitet och funktionsnedsättningar, som i sin tur kräver rehabilitering. Syfte: Syftet med litteraturstudien var att beskriva patienters upplevelse av rehabilitering efter stroke. Metod: Litteraturstudien genomfördes genom att söka ett antal artiklar i databaserna Cinahl Complete och PubMed.  Resultat: Strokedrabbade hade oftast en önskan om att återgå till sitt gamla liv före insjuknandet. De som drabbas av en omfattande stroke kan påverkas både fysiskt och psykiskt. Tillståndet sänker livskvalitén, självständighet och självkänslan. Entusiasm och positiv attityd från rehabiliteringspersonal beskrevs som grundläggande för en lyckad rehabiliteringsprocess. Med rätt stöd från rehabiliteringspersonal förbättrades patienternas kognitiva och instrumentella funktioner samt att självkänslan stärktes. Rehabilitering behöver startas så snarast som möjligt för att förhindra en långdragen funktionsnedsättning. Den bör anpassas efter individens förutsättningar. Slutsats: Patienter hade delade upplevelser gällande stöd, information, delaktighet och effekten av rehabilitering. Det är betydelsefullt att sjuksköterskan har ett individanpassat förhållningssätt utifrån patientens förutsättningar. Rätt stöd och lyhördhet från vårdpersonalen bidrar till ökad delaktighet samt att den ökar patientens förutsättningar att utföra aktiviteter under rehabiliteringsprocessen. Rehabilitering behövs oavsett boendemiljö och ska vara av hög kvalité för ett positivt resultat. / Background: Stroke is one of the most common diseases in Sweden. Those who survive strokes suffer from varying degrees of activity and disabilities., which in turn require rehabilitation. Aim: The purpose of this literature study was to describe patients' experiences of rehabilitation after stroke. Method: The literature study was carried out by searching for a number of articles in Cinahl Complete and PubMed. Results: Stroke sufferers usually had a desire to return to their old life before falling ill. Those who suffer from an extensive stroke can be affected both physically and mentally. The condition lowers the quality of life, independence, and self-esteem. Enthusiasm and positive attitude from rehabilitation staff was described as fundamental to a successful rehabilitation process. With the right support from rehabilitation staff, the patients' cognitive and instrumental functions were improved, and self-esteem was strengthened. Rehabilitation needs to be started as soon as possible to prevent a long-term disability. It should be adapted to the individual's conditions. Conclusion: Patients had shared experiences regarding support, information, participation, and the effect of rehabilitation. It is important that the nurse has an individualized approach based on the patient's conditions. The right support and sensitivity from the care staff contributes to increased participation and that it increases the patient's ability to perform activities during the rehabilitation process. Sjuksköterskors erfarenheter av omvårdnad i samband med amyotrofisk lateral skleros / Nurses experiences of nursing in connection with amyotrophic lateral sclerosis

Bakgrund: Amyotrofisk lateral skleros (ALS) är ett neurodegenerativt tillstånd som successivt drabbar många olika organsystem. Omvårdnad av patienter med ALS kräver att sjuksköterskor har kunskaper inom respiratorvård, rehabilitering, nutrition, sjukdomens motoriska- och sensoriska symtom, sjukdomens olika sjukdomsstadier samt hur sjukdomen påverkar patientens psykosociala tillstånd. Syfte: Syftet var att lyfta fram sjuksköterskors erfarenheter av omvårdnad i samband med ALS.Metod: Studien genomfördes som en allmän litteraturstudie med en induktiv ansats och baserades på en innehållsanalys av en artikel med mixad metod och åtta kvalitativa artiklar. Resultat: Innehållsanalysen resulterade i följande kategorier: sjuksköterskors erfarenheter kring sociala interaktioner kopplat till omvårdnaden av patienter med ALS sjukdom, sjuksköterskors erfarenheter kring omvårdnaden av patienter kopplat till patofysiologin av ALS sjukdom och sjuksköterskors erfarenheter kring den palliativa omvårdnaden av patienter med ALS sjukdom. Konklusion: Ett positivt förhållningssätt, samverkan i vårdteamet och evidensbaserad omvårdnad utgör en förutsättning för sjuksköterskor för att kunna ge säker- och personcentrerad vård till patienter med ALS. Patientens komplexa omvårdnadsbehov kräver många olika vårdmoment och utmanar sjuksköterskors yrkeskompetens. För att kunna stödja sjuksköterskan krävs mer forskning som ger djupare insyn i sjuksköterskans erfarenhet av omvårdnad i samband med ALS / Background: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative condition that develops gradually affecting many different organ systems. Care of patients with ALS h requires knowledge in respiratory care, rehabilitation, nutrition, the disease's motor and sensory symptoms, the disease's different stages of the disease and how the disease affects the patient's psychosocial condition. Aim: The aim was to highlight nurses' experiences of nursing in connection with ALS.Method: The study was conducted as a general literature study with an inductive approach based on a content analysis of an article with a mixed method and eight qualitative articles. Results: The content analysis resulted in the following categories: nurses' experiences regarding social interactions linked to the care of patients with ALS, nurses' experiences regarding the care of patients linked to the pathophysiology of ALS and nurses' experiences regarding palliative care of patients with ALS. Conclusion: A positive attitude, collaboration in the care team and evidence based nursing are prerequisites for nurses in order to provide safe and person-centered care for patients with ALS. The patient's complex nursing needs challenge nurses' professional competence. In order to support nurses, more research is required that provides a deeper insight into the nurse's experience in connection with ALS.
