Ziel der Doktorarbeit war die Implementierung eines Herzinfarktnetzes im ländlichen Raum mit Hilfe der FITT-STEMI-Studie und damit die Optimierung der Abläufe der Behandlungen und Verbesserung der Prognose der Patienten. Die Ergebnisse wurden quartalsweise mit den beteiligten Berufsgruppen wie Notärzten, Sanitätern, Personal der Intensivstation, den einweisenden Kliniken etc. besprochen und so ein Netzwerk aufgebaut. Dadurch konnte das Einzugsgebiet erweitert werden, die Primärtransporte nahmen von 54 auf 65 % und die telefonischen Infarktankündigungen von 43 auf 80 % zu, zudem konnte die door-to-catheter-Zeit bei Primärtransporten von 40 auf 23 min. gesenkt werden. / The aim of the doctoral thesis was the implementation of a heart attack network in rural areas with the help of the FITT-STEMI study, thus optimizing the treatment processes and improving the prognosis of patients. The results were discussed on a quarterly basis with the occupational groups involved, such as emergency physicians, paramedics, ICU staff, referring clinics, etc., thus creating a network. This allowed the catchment area to be expanded, primary transports increased from 54% to 65% and telephone infarct announcements increased from 43% to 80% Addictionally the door-to-catheter time for primary transports decreased from 40 minutes to 23 minutes.

Symptom Onset to First-Medical-Contact in ST-Segment Elevation Myocardial Infarction Patients

ST-segment elevation myocardial infarctions (STEMI) make up approximately 25% to 40% of total myocardial infarction (MI) presentations. The total occlusion of the coronary artery that results in a STEMI makes timeliness to reperfusion crucial. Previously, the focus has been on decreasing door-to-balloon time (D2B). Although D2B time plays an important role in achieving timely treatment, it is only one component of the route from symptom onset to reperfusion. It has been shown that total ischemic time is a better predictor of clinical outcomes, including mortality and infarct time. Delays between symptom onset to first-medical-contact (FMC) consume the majority of total ischemic time, and remains one of the main reasons that patients do not receive timely care. Factors affecting symptom onset to FMC for STEMI patients receiving primary PCI as a method of reperfusion at the Aswan Heart Center (AHC) in Egypt and the Hamilton General Hospital (HGH) in Canada were examined using the prospectively collected data held in the STEMI registries at these sites and a modified version of the Response to Systems Questionnaire applied in Egypt. Exploring factors linked to early and late presentation in STEMI patients showed that delays were associated with gender, smoking, cardiac history, cardiogenic shock and mortality rate. Furthermore, the type and number of symptoms, presence and actions of bystanders, emotional response and the actions of the patients, as well as transportation time was shown to be different among delay groups. / Thesis / Master of Science (MSc)

Thrombozyteninhibition und Glykoprotein-IIb/IIIa- Rezeptorbesetzung bei intrakoronarer versus intravenöser Bolusgabe von Abciximab bei Patienten mit ST-Hebungs- Infarkt

Bei Patienten mit ST-Strecken-Elevations-Myokardinfarkt (STEMI) ist die direkte intrakoronare Bolusverabreichung des Glykoprotein-IIb/IIIa-Rezeptorantagonist Abciximab, im Gegensatz zur periphervenösen Bolusinjektion, mit einer Reduktion von Infarktgröße und mikrovaskulärer Obstruktion sowie mit einem höheren Anteil geretteten Myokards assoziiert, vermutlich ausgelöst durch eine höhere lokale Arzneimittelkonzentration und der dadurch gesteigerten Hemmung der Plättcheninhibition. Ziel der Arbeit war es herauszufinden, ob es Unterschiede gibt bezüglich der GP-IIb/IIIa Rezeptorbesetzung und der Thrombozyteninhibition im venösen Koronarblut, welches kurz nach intrakoronarer oder periphervenöser Abciximab-Bolusinjektion entnommen wurde. Dafür wurden bei 16 Patienten mit akutem STEMI vor und unmittelbar nach der Gabe eines Abciximab-Bolus sowie nach 30 Minuten Blutproben aus dem Korornarsinus entnommen. Jeweils 8 Patienten erhielten entweder den Bolus intrakoronar oder peripher venös verabreicht. Sofort nach der Bolusapplikation war die Rezeptorbesetzung im venösem Koronarblut signifikant höher bei Patienten, die einen direkten intrakoronaren Bolus erhalten hatten, im Vergleich mit Patienten mit peripherer Bolusadministration (intrakorornarer Bolus: 93.5% [IQR 92.7–95.4], intravenöser Bolus: 74.0% [IQR 17.6–94.0], p = 0.04). Das Ausmaß der Plättcheninhibition war früh nach Bolusgabe ebenso deutlich höher bei intrakoronarer anstatt intravenöser Bolusapplikation. In der späten Blutentnahme 30 Minuten nach der Bolusapplikation konnten keine signifikanten Unterschiede zwischen beiden untersuchten Gruppen, weder bezüglich der GP-IIb/IIIa Rezeptorbesetzung noch der Thrombozytenaggregationshemmung, gefunden werden. Zusammenfassend kann man sagen, dass die direkte intrakoronare Bolusapplikation akut in eine höhere lokale Inhibition der Thrombozytenfunktion und einem größeren Anteil an geblockten GP-IIb/IIIa Rezeptoren im Vergleich zur peripher venösen Bolusinjektion resultiert. Limitierend muss die geringe Fallzahl erwähnt werden. Die Ergebnisse sollten deshalb zurückhaltend interpretiert werden.

Patienters förkunskap om symtom på hjärtinfarkt : en kvantitativ enkätstudie / Patients' knowledge of symptoms of myocardial infarction : a quantitative questionnaire study

Patienter med icke ST-höjningsinfarkt [NSTEMI] och ST-höjningsinfarkt [STEMI] inkommer till sjukhuset för sent för att behandlingen ska kunna leda till bevarande av hjärtmuskelns funktion. Det råder bristfällig information till allmänheten men främst till personer med risk för att utveckla hjärtinfarkt, om vad de kan göra för att förkorta tiden från symtom till vårdkontakt. Primärpreventiv information är viktig och rekommendationen är att informera och bedöma personer primärpreventivt för att förbättra kunskapen. Dessa förkunskaper ger patienterna möjligheten att uppmärksamma symtom och snabbt ta informerade beslut i att söka vård, vilket minskar skadan på hjärtat efter en NSTEMI/STEMI.   Syftet var att undersöka förkunskaper om symtom vid hjärtinfarkt hos patienter som genomgått NSTEMI/STEMI.   En kvantitativ ansats valdes för att svara på studiens syfte, designen är en icke-experimentell deskriptiv, retrospektiv tvärsnittsstudie. En enkätundersökning genomfördes på ett sjukhus i Mellansverige, totalt n= 32 deltagare. Enkäten innehåller till en början demografiska frågor för att sedan inrikta sig på ämnesspecifika frågor med hjälp av likertskalan, flervalsfrågor och öppna fritextfrågor.   Resultatet visade att deltagarnas självskattade förkunskaper om symtom på hjärtinfarkt var bristfälliga, särskilt om atypiska symtom. Deltagarnas symtom vid NSTEMI/STEMI stämde inte överens med deras förväntade symtom, vilket utgjorde en osäkerhet om det verkligen var härledda till hjärtat. Detta ledde till att det tog lång tid från symtom till första vårdkontakt och deltagarna sökte vård på grund av ihållande symtom eller på anhörigas begäran. Fåtalet deltagare hade fått information om symtom eller blivit erbjuden en hälsoundersökning innan insjuknandet och den information de hade fått var från anhöriga eller sjukvården. Majoriteten av deltagarna upplevde inga kvarvarande komplikationer två till tre dagar efter den genomgångna NSTEMI/STEMIN.   Slutsatsen är bristande förkunskap om symtom vid NSTEMI/STEMI, speciellt atypiska symtom och vilka åtgärder som ska vidtas vid symtom. Majoriteten ansåg att symtomen inte stämde överens med den förväntade symtombilden och menade att de inte hade fått information kring hjärt-och kärlsjukdomar innan insjuknandet. Omvårdnadsprofessionen behöver arbeta för att tydliggöra informationen, därefter nå ut till samhället och primärpreventivt inom sjukvården. / Patients with non ST elevation myocardial infarction [NSTEMI] and ST elevation myocardial infarction [STEMI] enter the hospital too late for the treatment to lead to preservation of heart muscle function. There is inadequate information to the public, but mainly to people at risk of developing myocardial infarction, about what they can do to shorten delay, from symptoms to care contact. Primary preventive information is important and the recommendation is to inform and assess people primarily preventively in order to improve knowledge. These prerequisites give patients the opportunity to pay attention to symptoms and quickly make informed decisions in seeking care, which reduces the damage to the heart after an NSTEMI/STEMI.   The aim was to investigate prior knowledge of symptoms of myocardial infarction in patients undergoing NSTEMI/STEMI.   A quantitative approach was chosen to answer the study's purpose, the design is a non-experimental descriptive, retrospective cross-sectional study. A survey was conducted at a hospital in central Sweden, a total of n = 32 participants. The questionnaire initially contains demographic questions to then focus on subject-specific issues with the help of the likert scale, multiple choice questions and open free text questions.   The results showed that the participants' self-rated knowledge of heart attack symptoms was deficient, especially in atypical symptoms. The participants' symptoms at NSTEMI/STEMI didn’t match their expected symptoms, which constituted an uncertainty as to whether it really was derived from the heart. Therefore it took a long time from symptoms to first care contact and the participants sought care because of persistent symptoms or at the relatives' request. Few participants had received information about symptoms or had been offered a health survey before the onset of illness and the information they had received from relatives or health care. The majority of participants experienced no remaining complications two to three days after their NSTEMI/STEMIN.   The conclusion is that there is lack of knowledge of symptoms of NSTEMI/STEMI, especially atypical symptoms and what measures should be taken in the event of symptoms. The majority considered that the symptoms did not match the expected symptom picture and believed that they had not received information about cardiovascular disease before the onset of the disease. The nursing profession needs to work to clarify the information, then reach out to society and primarily preventive in the health care sector.

Does Patient Dementia Limit the Use of Cardiac Catheterization in ST-Elevated Myocardial Infarction?

Regardless of age or mental capacity, percutaneous coronary intervention (PCI) is the first line of treatment for ST-elevated myocardial infarction (STEMI). This study evaluates the disparities in the use of diagnostic cardiac catheterization and PCI in STEMI patients with dementia. A retrospective analysis was performed of Florida‟s comprehensive inpatient surveillance system for the years 2006-2007 with admission diagnosis of STEMI. Logistic regression analysis was used to identify disparities in the use of intervention among all STEMI patients. A total of 8,331 STEMI patients met the inclusion criteria. Of these, 77% were catheterized and of these 67% received PCI. A total of 605 (7.3%) were demented. Patients with dementia were less likely to be catheterized (RR 0.4, 95% CI 0.3-0.5) and less likely to receive PCI within 24 hours (RR 0.5, 95% CI 0.4-0.6). This study concludes that STEMI patients with dementia were much less likely to receive cardiovascular interventions.

Non-Occlusive St-Segment Elevated Myocardial Infarction Following the Administration of Liposomal Amphotericin B in the Treatment of Cryptococcal Meningitis

What is known and objective: Liposomal amphotericin B (L-AmB) is the cornerstone of many serious invasive fungal infections. Despite lower frequencies of commonly reported adverse events in clinical trials compared to conventional formulations, post-marketing complications continue to mount. Case description: We present a case of chest pain following the initial dose of L-AmB for cryptococcal meningitis. Electrocardiogram demonstrated no acute electrocardiogram findings. Upon rechallenge, the chest pain worsened was subsequently accompanied by ST-segment elevation. Emergent coronary angiography found no acute findings. What is new and conclusion: Providers should be aware of cardiac complications with L-AmB, including non-occlusive ST-segment elevation.

Syfte: Att beskriva ambulanssjuksköterskans erfarenhet av att vårda patienter med misstänkt STEMI prehospitalt. Bakgrund: En av det mest frekventa larmet som inkommer till SOS alarm och erhåller ambulans är bröstsmärta, och dessa uppdrag bedöms alltid som livshotande och graderas med de högre prioriteringarna. Den medicinska framfarten sista årtionden har lett till att den prehospitala sjukvården har blivit avancerad och god handläggning kan vara avgörande för patientens fortsatta livskvalitet men även satt större krav på erfarenheten hos ambulanssjuksköterskan. Metod: Studien genomfördes med en empirisk intervjustudie och utgick från kvalitativ design. Reslutat: Analysen av resultatet utmynnade i tre huvudkategorier, Utlarmningsinformation, Vårdandet, Samarbetet.  Som byggdes upp utav 6 underkategorier som beskrev ambulanssjuksköterskans erfarenhet av att vårda patienter med STEMI prehospitalt. Konklusion: I studien framkommer det att ambulanssjuksköterskan hade goda kunskaper i att vårda patienter med STEMI och att det var något de kände sig trygga med på grund av god teoretisk och praktisk erfarenhet. De beskriver att snabbt kunde bedöma en kritisk sjuk patient och att de hade goda handlingsplaner och beredskap för vad som kan hända. De belyser även vikten av ett gott samarbete mellan kollegorna och andra instanser där bristfällande information kunde orsaka stor irritation samt minska patientsäkerheten avsevärt.
