Anestesisjuksköterskans upplevelser av att delta vid omedelbart kejsarsnitt / Nurse anesthetists’ experiences of participating in an immediate caesarean section

Bakgrund: En övergripande indikation för ett omedelbart kejsarsnitt är om mammans, barnets eller bådas hälsa riskeras under en vaginal förlossning. I samband med dessa oförutsedda situationer ska anestesisjuksköterskan ha god omvårdnadskunskap men också kunskap gällande den medicinska utrustningen och assistera anestesiologen i samband med generell anestesi. Syfte: Syftet med studien var att beskriva anestesisjuksköterskans upplevelser av att delta vid omedelbart kejsarsnitt. Metod: En kvalitativ metod tillämpades. Data samlades in genom individuella semistrukturerade intervjuer och den insamlade datan analyserades genom kvalitativ innehållsanalys med manifest ansats. Resultat: Fyra kategorier formades utifrån den data som insamlats. Att hantera en omedelbart livshotande situation där två liv står på spel. Att vara förberedd på det oförutsedda. Att känna ovisshet i informationen till och om modern. Att lära sig hantera stress och otrygghet. Anestesisjuksköterskor beskrev omedelbart kejsarsnitt som en av de viktigaste situationer som de deltog vid i sitt arbete där två liv står på spel. Känslor som ovisshet och stress påverkade hur de hanterade situationen. Slutsats: Omedelbart kejsarsnitt är en akut situation som inte liknar någon annan akut situation. Då simuleringsövningar och erfarenhet är faktorer som bidrar till ett större lugn och fokus i denna akuta situation bör regelbundna övningar vara inplanerade i verksamheten.

Behaviour of continuous concrete T-beams reinforced with hybrid FRP/Steel bars

This work aims to investigate the flexural behaviour of continuous hybrid reinforced concrete T-beams (HRCT). The investigations consist of three parts; the computational part, the experimental part and the finite element analysis. The computational part included two parts, the first one is developing an analytical programme using MATLAB software to investigate the moment-curvature behaviour of HRCT-beams and to design the experimental specimens. This was followed by the experimental part, where six full-scale reinforced concrete continuous T beams were prepared and tested. One beam was reinforced with glass fibre reinforced polymer (GFRP) bars while the other five beams were reinforced with a different combination of GFRP and steel bars. The ratio of GFRP to steel reinforcement at both mid-span and middle-support sections was the main parameter investigated. The results showed that adding steel reinforcement to GFRP reinforced concrete T-beams improves the axial stiffness, ductility and serviceability in terms of crack width and deflection control. However, the moment redistribution at failure was limited because of the early yielding of steel reinforcement at the beam section that did not reach its moment capacity and could still carry more loads due to the presence of FRP reinforcement. The second part of the computational part included the comparison between the experimental results with the ultimate moment prediction of ACI 440.2R-17, and with the existing theoretical equations for moment capacity, load capacity, and deflection prediction. It was found that the ACI 440.2R-17 design code equations reasonably estimated the moment capacity of both mid-span and middle-support sections and consequently predicted the load capacity of the HRCT-beams based on fully ductile behaviour. However, Qu's and Safan's equations underestimated the predicted moment and load-capacity of HRCT-beams. Also, Bischoff's and Yoon's models underestimated the deflection at all stages of the load for both GFRP and HRCT- beams. For the numerical part, a three-dimensional finite element model has been developed using ABAQUS software to examine the behaviour of HRCT-beams. The experimental results were used to validate the accuracy of the FEM, where an acceptable agreement between the simulated and experimental results was observed. Accordingly, the model was used to predict the structural behaviour of continuous HRCT-beams by testing different parameters.

Axial compressive behaviour of stub concrete-filled columns with elliptical stainless steel hollow sections

This paper presents the axial compressive behaviour of stub concrete-filled columns with elliptical stainless steel and carbon steel hollow sections. The finite element method developed via ABAQUS/Standard solver was used to carry out the simulations. The accuracy of the FE modelling and the proposed confined concrete stress-strain model were verified against experimental results. A parametric study on stub concrete-filled columns with various elliptical hollow sections made with stainless steel and carbon steel was conducted. The comparisons and analyses presented in this paper outline the effect of hollow sectional configurations to the axial compressive behaviour of elliptical concrete-filled steel tubular columns, especially the merits of using stainless steel hollow sections is highlighted.

A Direct-Read, A Posteriori Golden Copy Method for Measuring SoC Cache Upsets

A method for measuring system-on-a-chip (SoC) cache upsets is presented and evaluated. In contrast to methods that predict cache contents through analysis or memory access patterns, this method uses system registers to read cache memories directly, thereby creating and checking golden copies to detect individual memory upsets during operation. The test method is driven by the device under test itself and does not require a user to set or know a priori the cache contents. A bare-metal implementation of this “direct golden method” on a Zynq UltraScale+ MPSoC logged upsets in the device’s data cache, data tag, and TLB RAM memories during a neutron radiation beam test. For each of these memories, this direct golden method yields cache upset bit cross sections, such as 7.115 × 10^−16 cm^2 for the data cache. Confidence intervals for these bit cross sections overlap such intervals for three other methods, supporting this method’s validity and candidacy for future use.

Ceramic production in a Roman frontier zone: A comparative Neutron Activation and Petro-Textural analysis of Roman coarse pottery from selected sites on and around the Antonine wall, Scotland.

A series of recent excavations on the 2nd Century AD Antonine frontier forts of the Midland Scottish valley, have produced results which suggest that the army was making its own pottery on an appreciable scale. This was at a time when pottery production was thought to have moved almost exclusively into civilian hands. The possible local ware groups identified by the excavations were largely independent of firm source indicators such as kiln and waster material and the number of available samples was often-small. A program of Neutron Activation and Thin Section petrological analyses was undertaken along with an investigation into Textural Analysis, a facet of the Petrological toolkit. The aim was both to define the site ware groups and a group of specialist vessels thought to be local to Scotland, the Mortaria, and to make statements as to their provenance. Although the Mortaria analysis was limited by problems of sample group size and availability, by improving the objectivity of the statistical handling of the derived data sets and developing methods for the high level study of textural data, the site ware groups were defined successfully at both the "intrall and "inter" site levels. The analyses also furnished interpretations as to the mode and nature of the site production schemes. Through the full analysis of' site Daub samples linked to more traditional provenancing techniques, in all but one case the ware groups could be assigned to the source sites, where contrasting production modes could be identified with military as opposed to civilian production. / SERC

Curve Skeleton and Moments of Area Supported Beam Parametrization in Multi-Objective Compliance Structural Optimization

This work addresses the end-to-end virtual automation of structural optimization up to the derivation of a parametric geometry model that can be used for application areas such as additive manufacturing or the verification of the structural optimization result with the finite element method. A holistic design in structural optimization can be achieved with the weighted sum method, which can be automatically parameterized with curve skeletonization and cross-section regression to virtually verify the result and control the local size for additive manufacturing. is investigated in general. In this paper, a holistic design is understood as a design that considers various compliances as an objective function. This parameterization uses the automated determination of beam parameters by so-called curve skeletonization with subsequent cross-section shape parameter estimation based on moments of area, especially for multi-objective optimized shapes. An essential contribution is the linking of the parameterization with the results of the structural optimization, e.g., to include properties such as boundary conditions, load conditions, sensitivities or even density variables in the curve skeleton parameterization. The parameterization focuses on guiding the skeletonization based on the information provided by the optimization and the finite element model. In addition, the cross-section detection considers circular, elliptical, and tensor product spline cross-sections that can be applied to various shape descriptors such as convolutional surfaces, subdivision surfaces, or constructive solid geometry. The shape parameters of these cross-sections are estimated using stiffness distributions, moments of area of 2D images, and convolutional neural networks with a tailored loss function to moments of area. Each final geometry is designed by extruding the cross-section along the appropriate curve segment of the beam and joining it to other beams by using only unification operations. The focus of multi-objective structural optimization considering 1D, 2D and 3D elements is on cases that can be modeled using equations by the Poisson equation and linear elasticity. This enables the development of designs in application areas such as thermal conduction, electrostatics, magnetostatics, potential flow, linear elasticity and diffusion, which can be optimized in combination or individually. Due to the simplicity of the cases defined by the Poisson equation, no experts are required, so that many conceptual designs can be generated and reconstructed by ordinary users with little effort. Specifically for 1D elements, a element stiffness matrices for tensor product spline cross-sections are derived, which can be used to optimize a variety of lattice structures and automatically convert them into free-form surfaces. For 2D elements, non-local trigonometric interpolation functions are used, which should significantly increase interpretability of the density distribution. To further improve the optimization, a parameter-free mesh deformation is embedded so that the compliances can be further reduced by locally shifting the node positions. Finally, the proposed end-to-end optimization and parameterization is applied to verify a linear elasto-static optimization result for and to satisfy local size constraint for the manufacturing with selective laser melting of a heat transfer optimization result for a heat sink of a CPU. For the elasto-static case, the parameterization is adjusted until a certain criterion (displacement) is satisfied, while for the heat transfer case, the manufacturing constraints are satisfied by automatically changing the local size with the proposed parameterization. Lloyd Ney's "New London Facets:" Abstraction and Rebellion in the Section of Fine Arts

Lloyd Raymond "Bill" Ney's mural New London Facets was commissioned for the New London, Ohio post office through the Treasury Department-run New Deal program, the Section of Fine Arts (the Section), and is the only mural that program officials considered abstract. An examination of the mural today reveals that the label of "abstract" may be a bit extreme; objects in the piece have been abstracted but the mural as a whole is not at all strictly non-representational. This discrepancy and the ensuing controversy over Ney's mural reveal much about the sensitivity of Section officials to abstraction and to subjects outside genre or allegorical scenes typical of Section commissions. Correspondence between Ney and Section officials indicate a fear in the Section that the public would reject and fail to understand or relate to anything outside of the representational norm, a belief against which Ney adamantly and successfully argued. As a result, the Section made its lone exception in the case of Ney and New London Facets. While Ney did not achieve national renown as an artist within his lifetime, his work is still exhibited and auctioned relatively regularly in his hometown of New Hope, Pennsylvania. With the exception of Karal Ann Marling's description of the New London Facets incident in her book Wall to Wall America: A Cultural History of Post-Office Murals in the Great Depression, there is nothing significant published on Ney or his mural. With this thesis I hope to raise awareness of Ney as an artist, provide readers with a complete understanding of the New London Facets commission and approval, and explore the relationship between abstraction and the New Deal art programs. / Art History

The burden of labour and delivery-related complications among pregnant women at Mokopane Hospital of Limpopo Province

Thesis (MPH.) -- University of Limpopo, 2022 / The burden of labour and delivery-related complications are health problems that are life-threatening for the fetus and pregnant women. Mokopane hospital in Waterberg of Limpopo Province reports many maternal health complications. There has not been an investigation into the burden of delivery complications and therefore this study aims to investigate the burden of labour and delivery complication experienced by women giving birth at Mokopane hospital of Limpopo province. Purpose: of this study was to explore the burden of labour and delivery-related complications among pregnant women at Mokopane hospital of Limpopo province. Methods: A cross-sectional, retrospective descriptive study was conducted. The study followed a quantitative approach and the researcher completed a questionnaire using clinical records from all delivery files of mothers delivered at maternity between January 2017 to December 2019 Mokopane hospital. Findings: The major finding of this study was the majority of women were at a low risk of pregnancy (69%) followed by a high risk of pregnancy (24%). The study further revealed that (73.7%) of women at Mokopane hospital were delivered through the normal virginal procedure and (25.8%) delivered through Caesarean section. Moreover, about 86% of the mothers were normal after delivery whilst 14% were sick or had complications. Conclusion: This study, therefore, recommends that educational programs about labour and delivery-related complications and related programs should be prioritised for pregnant women. KEY CONCEPTS The burden: Is the intensity or severity of disease and its possible impact on daily life (Gidron 2013). In the context of this study, the burden will refer to the death and loss of health due to labour and delivery-related complications among pregnant women at Mokopane hospital of Limpopo Province. Labour: This is the process of rhythmic uterine contractions which results in cervical dilatation, a descent of the presenting part; and delivery of the fetus, placenta, and membrane. (Anthony & Van Der Spuy, 2002; Clark, Van de Velde, & Fernando, 2016). In the context of this study, labour will be defined as a physiologic process during which the fetus, membranes, umbilical cord, and placenta are expelled from the uterus. Delivery related complication: Is an acute condition arising from a direct cause of maternal death, such as antepartum or postpartum haemorrhage, obstructed labour, postpartum sepsis, a complication of abortion, pre-eclampsia or eclampsia, ectopic pregnancy and ruptured uterus, or indirect causes such as anaemia, malaria and tuberculosis. (WHO, 2018). In the context of this study, delivery related complications will include amongst others severe antenatal bleeding, Postpartum haemorrhage, nonconvulsive hypertensive disorder of pregnancy (pre-eclampsia), Eclampsia: preeclampsia plus convulsions, Convulsions, Prolonged labour, Premature rupture of the membranes, Retained placenta. Pregnant women: Is a woman who is in the period from conception to birth in which the egg is fertilised by a sperm and then implanted in the lining of the uterus then develops into the placenta and embryo, and later into a foetus (Martin, 2015). In the context of this study, a pregnant woman will be described as a woman who is carrying a developing embryo or fetus within her body.
