Background: Chronic heart failure (CHF) is a complex clinical condition. The higher survival rate after heart attack and other heart diseases, plus aging population, has led to more CHF patients. Symptoms can have negative impact on exercise capacity, ability to perform activities of daily living and quality of life. Heart failure clinics have been established to help better quality of life. Helping to stabilize and maintain patients´ health can reduce the need of hospital admission. Nurses´ role is to promote health, treat and relieve symptoms that can lead to better quality of life and self-care. Aim: To describe specialist nurses´ experiences of promoting health for patients with heart failure. Method: A qualitative inductive approach was used to interview six specialist nurses with competence in heart failure caring. The interviews were analyzed by using qualitative content analysis where the interpretation of the text formed eight categories. Results: Categories revealed that each nurse had her own strategy of caring for, supporting and strengthening both patients and kin, such as through communication, medication management, managing self-care, working interdisciplinary. At times the patients' decisions had to be accepted, even if this led to death. Conclusion: Nurses´ caring, supporting and informing roll along with availability was of great importance to the well being of patients and kin. Creating a good relationship was the foundation of a well functioning co-operation between nurse, patient and next of kin which led to a feeling of security for patients. Most important factor was the patient experiencing health at present.

Bidragande faktorer som påverkar bensårspatienters följsamhet till de egenvårdsråd som ges : en litteraturstudieRyd, Sophie, Sefa, Aferdita January 2011 (has links)
Syftet med föreliggande beskrivande litteraturstudie var att beskriva och sammanställa bidragande faktorer som påverkar patientens följsamhet till de egenvårdsråd som ges av sjuksköterskan gällande bensårsbehandlingen. Syftet var även att sammanställa vilken kvalitet inkluderade artiklar hade samt vilken relevans urvalet hade för inkluderade artiklars resultat. Artikelsökningen ägde rum i databaserna PubMed och Chinal, sökorden som användes var ”leg ulcer”, ”compliance” och ”nursing”. Resultatet baserades på tio artiklar där fyra artiklar hade hög kvalitet, fem artiklar hade medel kvalitet och en artikel hade låg kvalitet. Fyra kvantitativa artiklar samt en kvalitativ artikel använde bekvämlighetsurval, fem kvalitativa artiklarna använde strategiskt urval. Bidragande faktorer som påverkade patientens följsamhet till egenvårdsråd som gavs av sjuksköterskan gällande bensårsbehandlingen var smärta och obehag, patientens och sjuksköterskans bristande kunskaper, inkonsekventa råd, tillit till behandlingen och sjuksköterskan, förlust av identitet och självständighet samt övriga faktorer så som depression, demens och self-efficacy. Bidragande faktorer till bristande följsamhet gällande bensårsbehandling var komplexa. Det är därför viktigt att sjuksköterskan har förståelse för vilka faktorer som påverkar följsamheten för att kunna individanpassa råden och förhoppningsvis öka följsamheten. / The purpose of this descriptive literature review was to describe and summarize the contributing factors that influence patient's compliance to the self-care advice given by nurses regarding leg ulcer treatment. The aim was also to compile the quality of included articles and what relevance the sample had on the included articles results. The search for scientific articles were selected from the databases PubMed and Cinahl with the keyword ”leg ulcer”, ”compliance” and ”nursing”. The result was based on ten articles, of which four articles had high-quality, five articles had medium quality and one article had low quality. Four quantitative articles and one qualitative article used a convenience sample, five qualitative articles used a purposive sample.LyssnaLäs fonetiskt The contributing factors that influenced patient's compliance to the self-care advice given by nurses regarding leg ulcer treatment proved to be pain and discomfort, the patient’s and the nurse’s lack of knowledge, inconsistent advice, trust in the treatment and the nurse, loss of identity and independence as well as other factors such as depression, dementia and self-efficacy. Factors contributing to poor compliance with leg ulcer treatment were complex. It is therefore important that the nurse has an understanding of the factors that influence compliance to personalize the advice and hopefully increase compliance.

Background: Self-care is a multi-dimensional concept and has different definitions. Amongthe definitions, Orem’s definition of self-care is more consistent. Orem (1995) argues that,self-care is a personal activity to take care and maintain of own self health and illness andprevention of disease related complications. Aim: The aim of the paper was to investigate the factors that contribute to self-care behavioramong patients with Type 2 DM as argued in the literature. Method: data was collected from the following electronic databases: CINAHL, PubMed,LibHub, SweMed and Google Scholar-to find full texts. Data was analyzed through CriticalAppraisal Skill Programme. To ensure validity and reliability the author were blinded toreduce study bias and articles were selected according their quality. Result: 31 relevant studies were included in the review, among the major findings of the studywere; Age, Social support/network, high income level, high educational attainment and longType 2 DM diagnosis history had a positive predictor in Type 2 DM patients self-carecontributing factors. Conclusion: To improve a Type 2 DM patients self-care activities the present study concludedthat Demographic, Socio-Economic and Social support factors are among the positivecontributors in patients of Type 2 DM successful Self-Care activities. Syftet med denna systematiska litteraturstudie var att belysa hur sjuksköterskans omvårdnad kan stödja egenvårdsförmågan hos patienter med IBS. Den viktigaste omvårdnadsåtgärden var att tidigt i samband med diagnostiseringen av IBS, ge information och kunskap om sjukdomen, dess prognos samt behandling, vilket visades förbättra upplevd livskvalitet samt ge symtom-lindring. IBS skolor startade av sjuksköterskor förbättrade patienternas hälsobeteende samt minskade symtomen. Genom introduktion av guidebok innehållande råd om kost, motion, stresshantering samt symtomkontroll upplevde patienterna en minskning av symtom, vilket även ledde till en minskning av antalet besök inom sjukvården. När sjuksköterskan använde olika frågeformulär i kontakten med patienter med IBS kunde patienternas problem identifieras och strategier för egenvården planeras samt individuella handlingsplaner skapas. Sjuksköterskans kunskap och förståelse om patientens egen sjukdomsupplevelse samt copingstrategi ledde till att patienten tog en mer aktiv roll i sjukdomshantering. Patienters behandling borde inriktas på livsstilsförändringar, som till exempel psykosocial arbetsmiljö, kost, fysisk aktivitet, samt stresshantering. Genom att patienten förde dagbok över kostintag och sedan gjorde jämförelser med ökade eller minskade symtom kunde faktorer identifieras i matvanor samt kostintag som inverkade på symtomen. Dorotea Orems omvårdnadsmodell har legat till grund för denna studie där tron på människans egna resurser framhålls.
